RESUMO
The paper presents proposals for improving outpatient dental care in the military area. To improve all types of outpatient dental care it is encouraged to use mobile multi-purpose field dental system based on chassis KamAZ, versatile portable dental kit. The construction kit has a modular design that supports rapid deployment of the dental office. For its operation is designed dental kit, which includes modern dental materials, instruments and medicines. Equipping the army medical service data development will help to improve outpatient dental care in the Army.

The article presents results of epidemiological research of dental morbidity among persons of call-up age and indexes of their need in different types of dental aid. Were calculated medical standards for dispensary supply, elaborated propositions in standard of medical survey of persons of call-up age on specialization "stomatology" during registering on military registration and proximately during the call-up on military service in determination of fitness for military service. These data could be used in improvement of organizational structure of dental institutes of Health Ministry of social development of the Russian Federation.

The article presents data of a comparative investigation of volume of ballistic damages of maxillo-facial area, given by different ballistic weapon (from usual PM to newest GSh-18). The article says that was set up a large volume of damages of sort and bone tissues of maxillo-facial area, given by bullet of cartridge of GSh-18 in all searched parameters. All materials, received in investigation, could be used in science substantiation and in elaboration of recommendations in planning of structure of sanitarium casualty, methods of investigations, surgical treatment and rehabilitation of wounded with damages, given by modern ballistic weapon.

Thirty-four patients with foci of chronic rhino- or odontogenic infection were examined. All the patients underwent physical examination, biochemical blood analysis, immunological test for immunoglobulins A, G, and M, circulating immune complexes, leukocyte migration inhibition test with phytohemagglutinin, Con A, with antigens of the retina, vitreous body, iris, and lens, scrapes from the dentogingival pocket, conjunctival and nasal cavities for Chlamydia, followed by direct immunofluorescence, polymerase chain reaction, and culture. Chronic rhino- or odontogenic infection foci impair local ocular immunity in ophthalmologically healthy patients. The foci of chronic rhinogenic infection cause more pronounced changes in systemic and local ocular immunity than those of chronic odontogenic infection. In half the patients with chronic rhino- or odontogenic infection foci, Chlamydia are detectable in the oral, nasal, and ocular mucosae, which suggests that there is generalized infection and there is a need for complex sanitation of the body. Isolated local treatment for Chlamydia infection is not promising.

The article presents the discussions of todays' existing problems of delivery of ambulatory dental care in military-medical institutes of Ministry of Defense. The abovementioned problems are not the single ones in delivery of ambulatory dental care. However, fast solving even of pert of these problems contributes to improvement of delivery of ambulatory dental care in the Armed Forces of RF.

Basing on the clinic's data obtained during the 5-year period (2000-2004) we observed 653 patients with different traumas of maxillofacial area. Among the patients 69.8% were the casualties with mandible fractures, 6.7--with maxilla fractures and 2.5%--with zygomatic bone fractures. The combined injuries of maxillofacial and other areas were observed in 11.6%. The combined injuries of maxillofacial area with extremities constituted 0.6%, with chest--0.5% and adjacent areas of head--9.5%. The analysis of conducted therapeutic work and previous experience allows suggesting the medical tactics concerning the problems of terms, methods and volume of specialized care rendered to the patients with combined maxillofacial trauma. It implies that combined injury of jaws with other body segments requires rendering of specialized care in whole volume, early periods, just immediately after getting the patient from shock.

The epidemiological study of the incidence of stomatologic diseases was conducted in 1030 reserve officers. The high level of stomatologic diseases among them was detected. The high incidence of diseases is characterized by the following indices: dental caries constitutes 100% with its intensity 14.15 +/- 0.22, parodontium diseases--92.87% (gingivitis--17.66%, parodontitis of mild and moderate degree--12.9%, of severe degree--62.30%), oral mucosa injuries--0.49%, not carious dental injuries--32.14%. 40.29% pensioners of RF Defense Ministry required prosthetics and 55.73%--oral sanitation. The results obtained could be used in scientific substantiation of medical standards, which are necessary for providing the reserve officers with dispensary method (active call) and development of recommendations to improve the staff structure of military medical-and-prophylactic institutions and subdivisions of stomatologic profile.

The author describes a method of liquidation of gunshot defects of the hard palate by autotransplantation of a thoracodorsal flap on the vascular pedicle. The method was successfully used in 2 patients.

Free epidermoplasty and skin transplantation is performed on microarterial and microvenous anastomoses. Tissue reactions are studied in 3-21 days. Transplantation of a complex graft with application of microvascular anastomoses possesses definite advantages in comparison with a traditional method of free plasty. They are demonstrated as a complete survival of the transplant tissues, its revascularization, predominance of regenerative processes over the destructive ones in the epidermis, derm and appendages of the skin, preserved viability of tissue components in hair and sebaceous glands. All this is of certain importance for normal functioning of the skin as an organ, as well as an essential cosmetic importance.
