RESUMO

In the introduction the authors mention some statistical aspects of cranial injuries and injuries of the maxillofacial area. Then they pay attention to injuries of the paranasal sinuses, one of the main objects of interest of ENT traumatology. Next the authors deal with their own experience with the diagnosis and treatment of these injuries. Since 1982 they hospitalized 15 patients with slight and 16 with serious injuries of the paranasal sinuses. They describe aetiological factors, clinical manifestations, diagnostic and therapeutic procedures and .da of gunshot wounds are also described. In the conclusion the authors emphasize the importance of accurate and individual diagnosis and the predominantly surgical treatment of injuries of the paranasal sinuses. Finally they emphasize the advantages of interdisciplinary cooperation in these injuries.

RESUMO

In 1983-1987 34 patients with paresis of the recurrent nerve were examined. In 4 patients with left-sided paresis bronchogenic carcinoma was revealed, i. e. in 12% of all examined subjects. In relation to the group of patients with left-sided paresis bronchogenic carcinoma was detected in every fifth patient. As no other symptoms than hoarseness are present in paresis of the recurrent nerve and the results of examinations are frequently negative, the diagnosis is difficult. The authors summarize their experience and draw attention to systematic examination and possible new examination methods which must be indicated. Early diagnosis in two patients made radical therapy possible.

The authors submit their experience with the treatment of chronic otitis media with cholesteatoma and inform on therapeutic results recorded in 1979-1986. Chronic otitis media with cholesteatoma are encountered still relatively frequently. The authors detected it in adults in 40%. In children and adolescents under 20 years the percentage incidence is even higher, 53%. In a group of 38 operated patients the authors investigated the preoperative findings, the type of auditory disorder and its extent. They investigated also the selection of operations and reconstruction operations. The auditory gains were correlated with the percentage occlusion of the cochlear reserve, subjective data on altered hearing, disappearance of tinnitus and discharge from the ear. In children and adolescents a 22% auditory gain was recorded, in the age group of 21-40 years an auditory gain of 19.20%. In more advanced age groups a loss of 16.4%. Subjective improvement of hearing was reported by 41%, no change by 32%, deterioration by 27% of the patients. Constant tinnitus persisted after operation in 7.9% and a constant discharge in 4.5% of the operated patients. In one instance the authors observed a relapse of cholesteatoma in a patient operated by the so-called "closed method".
