RESUMO

Water electrolysis is among the recent alternatives for generating clean fuels (hydrogen). It is an efficient way to produce pure hydrogen at a rapid pace with no unwanted by-products. Effective and cheap water-splitting electrocatalysts with enhanced activity, specificity, and stability are currently widely studied. In this regard, noble metal-free transition metal-based catalysts are of high interest. Iron sulfide (FeS) is one of the essential electrocatalysts for water splitting because of its unique structural and electrochemical features. This article discusses the significance of FeS and its nanocomposites as efficient electrocatalysts for oxygen evolution reaction (OER), hydrogen evolution reaction (HER), oxygen reduction reaction (ORR), and overall water splitting. FeS and its nanocomposites have been studied also for energy storage in the form of electrode materials in supercapacitors and lithium- (LIBs) and sodium-ion batteries (SIBs). The structural and electrochemical characteristics of FeS and its nanocomposites, as well as the synthesis processes, are discussed in this work. This discussion correlates these features with the requirements for electrocatalysts in overall water splitting and its associated reactions. As a result, this study provides a road map for researchers seeking economically viable, environmentally friendly, and efficient electrochemical materials in the fields of green energy production and storage.

RESUMO

Valproate is a commonly prescribed anticonvulsant drug. It has a potential to cause hyperammonemia even in the presence of normal liver function tests. This hyperammonemic state can lead to encephalopathy referred as Valproate induced hyperammonemic encephalopathy (VHE). Here we present a case of valproate induced hyperammonemic encephalopathy in a young male caused by initiation of valproate therapy.

RESUMO

Ciprofloxacin is a commonly used drug in our setup. Neurological disturbances are rare side effects of its use and are reported in older adults, patients with comorbidities, or patients with a background of psychiatric illness and antipsychotic drug use. We report the case of a 21-year-old female who developed delirium after taking ciprofloxacin for a urinary tract infection. She underwent extensive workup and her final diagnosis was based on the exclusion of other causes. Her symptoms completely settled within three weeks of ciprofloxacin discontinuation.

Systemic lupus erythematosus (SLE) usually affects young females and its presentation with haemorrhage is unusual. Intracerebral bleed (ICB) has been described in a few case reports, but massive haematuria has not been reported in the literature so far. Here, we present a case of a 56 year-old-lady who presented with expanding lobar bleed and massive haematuria as the first presentation of SLE secondary to vasculitis. Lupus vasculitis usually presents with thrombosis-related complications and although bleeding in this context is rare, it can lead to catastrophic consequences. Hence, vasculitis-related bleeding should be taken under consideration in a challenging situation like ours.

BACKGROUND: Coronavirus disease-19 has a wide range of clinical presentations and varied outcomes. It is a new disease and researchers are trying to explore its clinical presentation and outcome to know more about the course of the disease. The objective of the present study was to determine the clinical and laboratory characteristics of Coronavirus disease-19 associated with severity of the disease. METHODS: This cross-sectional study was conducted at Rehman Medical Institute, Peshawar from April to August 2020. All patients presented to the hospital and were diagnosed as COVID-19 were enrolled in this study. Disease characteristics and clinical outcomes were noted in both mild and severe cases. Patients were divided into 2 groups based on the disease severity and a comparison was made between these groups in terms of demographics, lab parameters and outcomes. Data were analysed by using SPSS version 24. RESULTS: Out of 227 patients, 80.2% (n=182) were males while 19.8% (n=45) were females. The mean age of the patients was 54.44±14.35 yrs. 61.2% (n=139) had co-morbidities with diabetes being the most common. 26.9% (n=61) had severe disease and 18.1% (n=41) died.20.7% (n=47) had lymphopenia, 48.45% (n=110) had leucocytosis and thrombocytopenia was seen in 11.89% (n=27). CRP, D-dimers, ferritin and LDH were raised in 83.25% (n=189), 80.17% (n=182), 81.05% (n=184), 77.09% (n=175) of the patients respectively. Comparing our designated patient groups revealed that old age, comorbidities, leucocytosis, lymphopenia, raised inflammatory markers were associated with severe disease and that mortality was high in the severe disease group. CONCLUSION: Middle-aged males with comorbidities were the most affected subset of patients. Disease severity was associated with old age, comorbidities and certain lab abnormalities. The outcome was poor in case of severe disease. However, no gender correlation was found with disease severity.

RESUMO

Severe coronavirus disease 2019 (COVID-19) is known to be associated with thrombotic events like ischemic stroke. However, in the case of mild or asymptomatic disease, a thrombotic event like ischemic stroke is rare and has never been reported in our country. We present the case of a 28-year-old male patient with no co-morbidities who was diagnosed to have ischemic stroke involving the basilar artery. No risk factors for ischemic stroke could be found except for post-COVID-19 status, evident by the presence of antibodies against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).

Oromandibular dystonia is defined as a focal dystonia that manifests as forceful contractions of the face, jaw, and/or tongue. Lingual dystonia is a rare subtype of oromandibular dystonia that specifically affects the tongue. Multiple etiologies are thought to attribute to oromandibular dystonia, including brain damage, the use of neuroleptic medications, neurodegenerative disorders, metabolic disorders, neurodevelopmental disorders, and viral infections. Idiopathic cases of isolated lingual dystonia are rare and seldom reported in the literature. This report describes a 35-year-old female patient with lingual dystonia that was present at rest and aggravated during speech. Despite detailed history taking and a thorough examination, along with multiple imaging and laboratory studies, no cause could be established and her case was classified as being that of an idiopathic etiology.

OBJECTIVE: To note ultrasonographic findings used for diagnosing plasma leakage in dengue haemorrhagic fever patients. METHODS: The observational retrospective study was conducted at the Holy Family Hospital, Rawalpindi and comprised records of patients with confirmed dengue infection who were screened for dengue haemorrhagic fever according to Dengue Expert Advisory Group criteria from July 1 to December 31, 2013. Each patient underwent ultrasonography for the detection of ascites, gall bladder wall thickness, pleural and/or pericardial effusion along with their quantification and localisation. RESULTS: Of the 240 patients, 166(69.2%) were men. The overall mean age was 28.9±12.4 years. Of the total, 215(89.5%) had ultrasonographic abnormalities, suggestive of plasma leakage. Quantification and localisation wise, mild abdominal ascites 68(47.2%), right pleural effusion 82(74.5%) and mild pleural effusion 98(89%) were commonly noted. None had pericardial effusion. CONCLUSIONS: Mild ascites and mild right pleural effusion were the commonest pattern of ultrasonographic leak in dengue haemorrhagic fever patients.

RESUMO

Hemophagocytic lymphohistiocytosis (HLH) is a rare disorder. Dysfunction of cytotoxic T and natural killer (NK) cells causes uncontrolled activity of lymphocytes and histiocytes which leads to HLH. Infections, malignancies, and autoimmune disorders are associated with development of HLH. Dengue and Plasmodium vivax are rare causes of HLH. We report the first ever case of a young man who developed fatal HLH that complicated Dengue Hemorrhagic Fever (DHF) and Plasmodium vivax infection.

BACKGROUND: Model for End Stage Liver Disease (MELD) is scoring system used for prioritization of patients waiting liver transplantation. Patients with decompensated chronic liver disease (DCLD) with higher MELD score and hepatic encephalopathy (HE) have poor outcome. We planned to note association between short-term mortality and various cut-off values of at admission MELD score in HCV related DCLD patients presenting with HE. METHODS: In this descriptive case series, 208 Hepatitis C (HCV) related chronic liver disease (CLD) patients with HE were included. At admission serum bilirubin, creatinine and INR were sought to calculate MELD score. Patients who improved were discharged and kept under regular follow up. Outcome was categorized into alive or expired in 3 months after admission. Based on MELD score cut off values of > 15, > 20, > 25 and > 30, association with outcome was determined using Chi-square test. RESULTS: One hundred and six (51%) patients were female. Mean patient age was 55.3 ± 10.6 years. Outcome wise, 128 (61.5%) were alive and 80 (38.5%) expired. Mean MELD score of expired was 22.17 ± 9.14. 55 (51%) patients with MELD score >15, 44 (65.7%) patients with > 20, 30 (73.2%) patients with > 25, and 21 (72.5%) patients with > 30 expired. MELD score > 20 had strongest association with outcome, p value less than 0.01. CONCLUSION: At admission MELD score > 20 are most significantly associated with poor outcome in DCLD patients with HE.

BACKGROUND: Dengue hemorrhagic fever (DHF) is considered to be associated with secondary dengue infection. This study was conducted to note frequency of primary and secondary dengue infection in DHF patients. Additionally these patients were compared in terms of age, gender, laboratory parameter, diseases severity and outcome. MATERIALS AND METHODS: In this cross sectional observational study DHF patients fulfilling DHF criteria of Dengue Expert Advisory Group (DEAG) were included and divided into groups based on dengue specific IgG positivity and ratio of IgM to IgG. Group I, patients with secondary dengue infection were IgG positive or their ratio of IgM to IgG was <1.2. Group II, primary dengue infection patients were IgG negative or their ratio of IgM to IgG was >1.2. The two Groups were compared for statistically significant association in terms of age, gender, laboratory parameter (at admission hematocrit [HCT], platelet, white blood cell [WBC] counts, alanine aminotransferase [ALT] value), severity (DHF or dengue shock syndrome), and outcome (recovered or expired). RESULTS: Two hundred thirty-four DHF patients were included. 66.2% was male and 33.8% female. Mean patient age was 28.8 ± 12.4 years. Based on dengue markers results, 61.5% patients were categorized to Group I, and 38.5% to Group II. Statistically significant association between the two Groups was noted in terms of at admission platelet count, and ALT value, P value <0.05. CONCLUSION: Primary dengue infection is frequently associated with DHF. Patients with DHF caused by secondary dengue infection have lower at admission platelet counts and higher ALT value.
