RESUMO

Objective:The purpose of this study is to explore the clinical characteristics of Coronavirus Disease 2019 (COVID-19) in patients with type 2 diabetes mellitus (T2DM), and analyze the risk factors for adverse outcomes.Methods:2 052 patients diagnosed with COVID-19 who were hospitalized in Shanxi Bethune Hospital between December 1, 2022 and March 20, 2023 were included. They were divided into diabetes group ( n=70) and non-diabetes group ( n=1 982) according to the presence or absence of comorbid T2DM. The two groups were matched at 1:1 via propensity score matching. Clinical characteristics and laboratory examination results of the two groups were compared. According to the outcomes during hospitalization, the two groups were further divided into two subgroups respectively. Univariate analysis and subsequent binary Logistic regression was used to analyze the risk factors of adverse outcomes in patients with COVID-19 and type 2 diabetes. Results:After the propensity score matching, the most common comorbid condition in diabetes group and non-diabetes group was hypertension. The proportion of patients with severe or critical disease in diabetes group was higher compared with non-diabetes group. The levels of hemoglobin A1c (HbA1c), fasting blood glucose (FBG), blood urea, IL-4, IL-6, IL-10, IFN-γ and TNF-α were significantly higher in the diabetes group ( P<0.05). Logistic regression analysis within the diabetes group showed that hypertension ( OR=3.640, 95% CI: 3.156 to 4.290), FBG>11 mmol/L ( OR=3.283, 95% CI: 1.416 to 7.611), HbA1c>10% ( OR=2.718, 95% CI: 1.024 to 7.213) were independent risk factors for adverse outcomes in patients with COVID-19 and type 2 diabetes(all P<0.05). Conclusions:Compared with the non-diabetes group, patients with COVID-19 and T2DM have worse inflammatory response and higher levels of inflammatory cytokines. The elevated levels of FBG and HbA1c are related to the adverse outcome in patients with COVID-19 and T2DM.

RESUMO

Objective To explore the difference of balance ability between patients with chronic nonspecific low back pain ( CNLBP) and healthy individuals, and the correlation between patients’ pain symptoms, lumbar flexibility, abdominal muscle endurance, overall function, quality of life and fear of avoidance with balance ability, so as to guide clinical rehabilitation evaluation. Methods A total of 34 patients with CNLBP were selected as the experimental group, and 34 healthy volunteers without history of low back pain were selected as control group. The plantar pressure measurement system was used to collect the ratio of forefoot to hindfoot pressure, pathlength ( L) of plantar center of pressure ( COP), displacement length in anteroposterior direction ( LAP ), displacement length in mediolateral direction (LML ), mean velocity (v), displacement velocity in anteroposterior direction (vAP ), displacement velocity in mediolateral direction (V-ML) and elliptical swing area (S). In addition, the experimental group was assessed by the visual analogue scale (VAS), the finger floor distance (FFD), the number of sit-ups in 1 minute, the Oswestry disability index (ODI), the 36-item short form survey (SF-36) and the fear avoidance beliefs questionnaire (FABQ), and correlated with plantar pressure parameters. Results All plantar pressure parameters were significantly different between the two groups ( P < 0. 05). The the ratio of forefoot to hindfoot pressure in experimental group was significantly lower than that in control group (P<0. 05), and the parameters L, LAP , LML , v, vAP , vML and S were significantly higher than those of control group (P<0. 05). With eyes open or closed, the VAS score of experimental group was positively correlated with L, LAP , LML(P<0. 05), and FFD and FABQ scores were positively correlated with L and LML , respectively (P< 0. 05). With eyes open, ODI was positively correlated with L, LAP and LML (P< 0. 05), and SF-36 score was negatively correlated with L and LML(P<0. 05). With eyes closed, the number of 1-min sit-ups was negatively correlated with LAP and S (P<0. 05), ODI was positively correlated with L and LML(P<0. 05), and the SF-36 score was negatively correlated with L (P<0. 05). Conclusions The static balance ability of patients with CNLBP is decreased, and it is correlated with pain symptoms, lumbar function, quality of life and psychological status. The result can provide references for the assessment of functional activities.

RESUMO

Objective:To evaluate the effect of dexmedetomidine-based anesthesia on cerebral oxygen saturation in the patients undergoing total laparoscopic hysterectomy. Methods:Fifty American Society of Anesthesiologists Physical Status classificationⅠorⅡpatients, aged 20-64 yr, undergoing elective total laparoscopic hysterectomy, were divided into 2 groups ( n=25 each) using a random number table method: conventional group (C group) and dexmedetomidine group (D group). Bilateral regoinal oxygen saturation (rSO 2) was monitored using a cerebral oxygen saturation monitor. In D group, dexmedetomidine was intravenously infused as a bolus of 0.5 μg/kg before anesthesia induction, and 10 min later dexmedetomidine was then given by an infusion of 0.5 μg·kg -1·h -1 until 30 min before the end of operation. The equal volume of normal saline was given instead in C group. The remaining anesthesia regimen was the same in two groups. The maximum (LrSO 2max, RrSO 2max) and minimum (LrSO 2min, RrSO 2min) of bilateral rSO 2 were recorded from entry to 2 min after induction (D 1), from >2 min after induction to completion of tracheal intubation (D 2), from completion of tracheal intubation to the end of skin incision (D 3), from skin incision to completion of pneumoperitoneum (D 4), from pneumoperitoneum to completion of Trendelenburg position (D 5), within 20 min of Trendelenburg position (D 6), >20-40 min of Trendelenburg position (D 7), >40-60 min of Trendelenburg position (D 8), and from >60 min of Trendelenburg position to 10 min after return to supine position (D 9). The occurrence of 60% baseline value <rSO 2 <75% baseline value and rSO 2≤60% baseline value were also recorded. Results:Compared with D 1 period, LrSO 2min, LrSO 2max, RrSO 2min and RrSO 2max were significantly decreased in D 4-D 8 periods in C group ( P<0.05), and no significant change was found in the parameters mentioned above in the other periods in D group ( P>0.05). Compared with C group, the LrSO 2min, LrSO 2max, RrSO 2min, and RrSO 2max were significantly increased in D 4-D 8 periods in D group ( P<0.05). No bilateral rSO 2 <75% baseline value was found in group D. Conclusions:Dexmetomidine-based anesthesia can elevate intraoperative rSO 2 in the patients undergoing total laparoscopic hysterectomy.

RESUMO

Objective:To analyze and compare the effects of conventional treatment and the dynamic 3C therapy, continuous glucose monitoring system, and on outcomes, length of hospital stay and medical costs in patients with type 2 diabetes mellitus. Methods:A total of 122 type 2 diabetes mellitus patients who visited the Department of Endocrinology of Shanxi Baiqiu'en Hospital between June 2022 and December 2022 were selected and randomized at 1:1 into the control group and the experimental group using the randomized numerical table method. The t-test and non-parametric test were used as appropriate to compare the glycemic control, length of hospital stay and medical costs of the two groups. Results:Before treatment, there was no significant difference in the indicators for blood glucose levels and fluctuations between the two groups ( P>0.05). After treatment, the glycosylated hemoglobin, fasting plasma glucose, and 2-hour postprandial blood glucose decreased in both groups. Moreover, the glycosylated hemoglobin, fasting plasma glucose, and 2-hour postprandial blood glucose showed more significant decreases in the experimental group compared with the control group after treatment. The glycemic variability indicators, including the time out of range, standard deviation of blood glucose, and the largest amplitude of glycemic excursion, decreased after treatment in both groups. The time in range increased after treatment. After treatment, the standard deviation of blood glucose and the largest amplitude of glycemic excursion in the experimental group were lower than those in the control group, and the length of hospital stay in the experimental group was shortened (all P<0.05). Conclusion:3C therapy contributes to better glycemic control and shorter hospital stay in patients with type 2 diabetes mellitus.

RESUMO

With the appearance of direct-acting antivirals (DAAs), breakthroughs have been achieved in the antiviral therapy for hepatitis C and hepatitis C patients can be cured completely. Due to the insidious onset of hepatitis C, most patients do not know their own conditions, and thus expanding diagnosis and treatment through screening is the key to the elimination of hepatitis C. However, hepatitis C virus is distributed widely and unevenly in the world, which results in difficulties in the screening and diagnosis of hepatitis C. This article introduces the current economic research on hepatitis C screening, analyzes the factors affecting the cost-effectiveness of hepatitis C screening, and shares the strategies and advances for hepatitis C elimination in other countries, so as to provide a reference for eliminating hepatitis C in China.

RESUMO

Breast milk, whether the ratio of nutritional ingredients or immunological characteristics, were the most suitable for physiological needs of preterm infants. However, breast milk composition was in dynamic change, causing imbalance of energy, which could not make the premature infant to achieve the desired growth state. Human milk fortifier, retaining advantages of the breast milk, promoted growth of premature infants further. This review primarily focused on two ways of adding human milk fortifiers, standardization fortification and targeted fortification. The former was widely used in neonatal intensive care unit, while the latter was more able to explore the growth of premature infant potentially, and to achieve a more ideal ectopic growth rate. With the development of breast milk bank, storage and component extraction technology brought about donor human milk-based human milk fortifier, which provided new directions of very low birth weight infants and very preterm infants breastfeeding. The progress of human milk fortification of premature infants was closely related to expansion of multi-disciplinary centers, and only the harmonious development of multi-center maked things much more rational and systematic.
