RESUMO

OBJECTIVE: To explore the predictive effect of systemic immune inflammatory index (SII) combined with neutrophil-to-lymphocyte ratio (NLR) on prognosis of acute-on-chronic liver failure (ACLF) patients. METHODS: A retrospective analysis was conducted on the data of 160 patients diagnosed with hepatitis B virus (HBV)-ACLF in Hunan Provincial People's Hospital from June 2022 to June 2023. The patients were divided into a survival group and a death group based on their survival status within 90 days after admission. General data, laboratory indicators, and complications were recorded. The risk factors affecting prognosis of HBV-ACLF patients and the correlation between NLR, SII, and model for end-stage liver disease (MELD) score were analyzed, and the predictive effect of NLR and SII on prognosis was evaluated. RESULTS: Compared to the survival group, the age, MELD score, and incidence of infection of the death group were higher, and the expression levels of total bilirubin, aspartate aminotransferase, serum creatinine, white blood count, neutrophil, NLR, SII, and international normalized ratio were increased, while the expression levels of alanine aminotransferase, platelets and lymphocytes were decreased (all P<0.05). NLR, SII and MELD score were all risk factors affecting the prognosis of HBV-ACLF patients (all P<0.001). There was a positive correlation between NLR, SII and MELD score (P<0.001). SII combined with NLR showed good performance in predicting the prognosis within 90 days after admission in ACLF patients. CONCLUSION: SII combined with NLR has a good prognostic effect on ACLF patients.

Objective: To explore the value of chitinase-3-like protein 1 (CHI3L1) for the diagnosis of patients with chronic liver diseases such as chronic viral hepatitis B, liver cirrhosis and hepatocellular carcinoma (HCC), and analyze the correlation between serum CHI3L1 level and Child-Pugh grading of chronic liver diseases. Methods: The serum CHI3L1 levels of 154 cases of chronic viral hepatitis B, 132 cases of liver cirrhosis and 88 cases of HCC were detected by ELISA, and 92 healthy subjects were taken as the control. Results: The -serum CHI3L1 levels in HCC group, liver cirrhosis group and chronic viral hepatitis B group were higher than in healthy control group (P < 0.001). Serum CHI3L1 level showed a trend of increase in patients with chronic hepatitis to liver cirrhosis and to HCC. The diagnostic efficacy of serum CHI3L1 level on liver cirrhosis showed the sensitivity of 64.4% and the specificity of 96.7%. The diagnostic efficacy of serum CHI3L1 level on HCC showed the sensitivity of 86.8% and the specificity of 97.8%. Serum CHI3L1 level was higher in Child-Pugh grade B and C patients than in Child-Pugh grade A patients, and was positively correlated with Child-Pugh grading of liver function (rs = 0.301, P < 0.001). Conclusion: Serum CHI3L1 level increased in chronic liver diseases and showed an increase trend with the progression of liver diseases. Serum CHI3L1 could be a biomarker for the auxiliary diagnosis of chronic liver diseases.

The technology for stepper drive that can achieve accurate motion in the hydraulic field has always been urgently needed in the industry. This paper proposes a hydraulic stepper drive based on five high speed on/off valves and two miniature plunger cylinders. The stepper drive discretizes the continuous flow medium into fixed small steps through the miniature plunger cylinder and realizes the state control of the drive through the logic action of the high speed on/off valve. This paper improves the current stepper drive and establishes a mathematical model to analyze the error of the drive and calculate the position of the actuator. In addition, through simulation research, the performance parameters such as the single-step step characteristic and pressure characteristic of the stepper drive are studied. The results show that, compared with the technology of current stepper drive, this stepper drive can effectively remove the "post step" phenomenon, greatly improve the stepper accuracy of the stepper drive, and have a more excellent performance.

AIM: To investigate clinical efficacy of transjugular intrahepatic portal shunt (TIPS) for the treatment of cirrhotic portal hypertension. METHODS: 71 cases of patients with cirrhotic portal hypertension and esophageal and gastric variceal bleeding hospitalized from January 2014 to June 2017 were enrolled and treated with TIPS. The change of portal pressure and serum biochemical indexes before and after TIPS were compared, and re-hemorrhage rate, ascites incidence, complications, and survival rate were calculated. RESULTS: 71 patients (male/female 47/24, aged 29-77 years, average 48.9 ± 9.8 years) with cirrhotic portal hypertension received TIPS. The success rate of TIPS was 93% (66/71). During 1-24 months (mean 12.5 ± 7 months) follow-up of 66 patients, 61 cases survived and 5 cases died. The portal pressure decreased significantly from 40.48 ± 3.15 cmH2O to 23.59 ± 4.41 cmH2O after TIPS (P < 0.05). During the follow-up, the incidence of hepatic encephalopathy was 12.1%, the incidence of re-hemorrhage was 18.2%, and there were 4 cases of stent dysfunction, with 1 case of bare stent and 3 cases of dual stent. CONCLUSION: TIPS is an effective procedure for the treatment of cirrhotic portal hypertension complications, since it can reduce portal pressure and significantly alleviate ascites. Liver function is impaired in short-term after TIPS, but TIPS has no significant effect on liver function in middle-term.
