RESUMO

Ceftriaxone was approved in 1997 for the treatment of otitis media despite previous studies that documented an association of ceftriaxone with elevated hepato-biliary enzymes and transient biliary stasis. The case cited here highlights the need for continued awareness education for physicians who may use ceftriaxone to treat common illnesses such as acute exudative tonsillitis and otitis media in children. Specifically, for children with a family history of gallbladder, biliary tract, liver or pancreas dysfunction, ceftriaxone may not be the drug of choice since the likelihood of complications is increased in this population. Additionally, ceftriaxone may cause problems in either adults or children with preexisting disease, who may not be well-nourished, or who may be dehydrated.

RESUMO

Physicians' attitudes are important in dealing with issues of substance abuse in women. The primary care physician is in a unique position to take advantage of the doctor-patient relationship and initiate diagnosis and treatment of substance abuse. When the primary care physician is supportive of the patient, both begin from a position of strength. Patients are no more to blame for contracting the disease state than for getting other medical conditions for which they are at risk. Treatment of substance abuse in women must acknowledge that significant differences exist between men and women patients. Most treatment programs have been based on research studies in men. Current efforts are under way to update the information necessary to deal effectively with the problem of substance abuse among women.

RESUMO

This paper traces the 7-year evolution of a required clerkship in Family Practice from the time of initial grant application to the current academic year. Results of experience in areas of student placement, preceptor recruitment, curriculum development, test construction, grading schema and course evaluation are described. Emphasis on streamlining administrative systems to decrease paperwork of course director is a major focus. Changing needs of department, medical school and student are reflected in the adaptations of the clerkship to these needs.

RESUMO

The Physician Preceptor Program is a preclinical elective integrating human values into medical education. Through an interdepartmental effort, a one-on-one, mentor-type learning model has been established. Home visits, journal keeping, office visits, and round table discussions are integral components of the course. All participants are enthusiastic about the benefits of the program.
