Background: Early feeding practices have a critical role in the future not only in health but also in modulating eating habits. This study aimed to assess breastfeeding and complementary feeding practices and the nutritional status of Portuguese toddlers aged 0-36 months. Methods: EPACI Portugal 2012 is a cross-sectional study of a national representative sample. Trained interviewers collected data about early feeding practices and anthropometrics. Body mass index was classified according to World Health Organization criteria. Frequencies and survival analysis were used to characterize variables. Results: More than 90% of children were initiated breastfeeding, around 20% were exclusively breastfed for six months, and about 20% were breastfed at 12 months while complementary feeding was taking place. Exclusive breastfeeding was determined by maternal prepregnancy body mass index (HR 1.01; 95% CI 1.00, 1.03, P=.03) and low birth weight (HR 1.61; IC 95% 1.21, 2.15, P=.001) of the infants. About 90% were initiated complementary feeding between four and six months, and almost 10% were introduced to cow's milk before 12 months. In the second year of life, 83.2% and 61.6% of toddlers have already consumed nectars and sweet desserts, respectively. About one-third of Portuguese toddlers showed a body mass index z-score >1, and 6.6% were overweight/obese (z-score >2). No association was found between the duration of breastfeeding or timing of complementary feeding and the body mass index z-score in children. Conclusions: Despite the low prevalence of exclusive breastfeeding at six months, Portuguese infants effectively comply with dietary recommendations during the first year of life. The transition to the family diet must be carefully made. There is a high prevalence of Portuguese toddlers at least at overweight risk. The duration of breastfeeding or timing of complementary feeding was not associated with the expression of overweight/obesity.

INTRODUCTION: Nonalcoholic fatty liver disease is the leading cause of pediatric chronic liver disease. Although nonalcoholic fatty liver disease is closely associated with obesity, its relationship with metabolic syndrome in children is not fully understood. The main aim of this study was to evaluate the association between nonalcoholic fatty liver disease and a combination of cardiometabolic risk factors in adolescents with overweight/obesity, using a pediatric metabolic syndrome score (PsiMS) to predict metabolic syndrome. METHODS: A retrospective cohort study was conducted. Subjects with overweight/obesity aged 10 to 17 followed at two clinical centers in Portugal (2018 - 2021) were enrolled. The independent association of nonalcoholic fatty liver disease with PsiMS, and of other potential predictors, was tested through multiple regression analyses. Receiver operator characteristic curve analysis was performed to estimate the optimal cutoff of PsiMS to discriminate metabolic syndrome. RESULTS: Eighty-four subjects were included (median age at baseline 11.5 years). The prevalence rate of nonalcoholic fatty liver disease was 51% and the prevalence rate of metabolic syndrome was 7%. The mean PsiMS was 2.05 ± 0.48 at the first evaluation, and 2.11 ± 0.52 at the last evaluation (mean follow-up time was 15 months). The nonalcoholic fatty liver disease group had significantly (p < 0.05) higher weight and body mass index z-scores, higher rate of severe obesity and higher waist circumference percentile. PsiMS was highly accurate in predicting metabolic syndrome (area under the curve = 0.96), with an optimal cutoff of 2.46 (sensitivity 100%, specificity 89%). In the univariate analysis, no statistically significant association was observed between nonalcoholic fatty liver disease and PsiMS. In the multiple regression analysis, female sex had a negative association with PsiMS (first and last evaluation). Independent predictors of a higher PsiMS at first evaluation were: ≥ 2 metabolic syndrome criteria, body mass index z-score, insulin resistance and dyslipidemia. At the last evaluation, independent predictors of a higher PsiMS were: nonalcoholic fatty liver disease, baseline PsiMS and body mass index increase from baseline. CONCLUSION: The results suggest a good performance of the PsiMS to assess metabolic syndrome and that nonalcoholic fatty liver disease is associated with PsiMS at follow-up.

Mountain ecosystems are important biodiversity hotspots and valuable natural laboratories to study community assembly processes. Here, we analyze the diversity patterns of butterflies and odonates in a mountainous area of high conservation value-Serra da Estrela Natural Park (Portugal)-and we assess the drivers of community change for each of the two insect groups. The butterflies and odonates were sampled along 150 m transects near the margins of three mountain streams, at three elevation levels (500, 1000, and 1500 m). We found no significant differences in odonate species richness between elevations, but marginal differences (p = 0.058) were found for butterflies due to the lower number of species at high altitudes. Both insect groups showed significant differences in beta diversity (ßtotal) between elevations, with species richness differences being the most important component for odonates (ßrich = 55.2%), while species replacement drove the changes between butterfly assemblages (ßrepl = 60.3%). Climatic factors, particularly those depicting harsher conditions of temperature and precipitation, were the best predictors of total beta diversity (ßtotal) and its components (ßrich, ßrepl) for the two study groups. The study of insect biodiversity patterns in mountain ecosystems and of the role played by different predictors contribute to further our understanding on the community assembly processes and may help to better predict environmental change impacts on mountain biodiversity.

Adequate nutritional intake in the first years of life is crucial for future health. The purpose of this study is to assess the adequacy of nutritional intake in Portuguese toddlers. The EPACI Portugal 2012 is a cross-sectional study of a representative sample of toddlers (n = 2230), aged between 12 and 36 months. Data on diets were collected by trained interviewers. The current analysis included 853 children with full data from 3-day food diaries completed by parents/caregivers. Intakes of energy, macro- and micronutrients were estimated through Statistical Program to Assess Dietary Exposure (SPADE). Nutritional adequacy was evaluated using Dietary Reference Values established by the European Food Safety Authority. A large proportion of children exceeded the recommended energy intake. The median daily protein intake was 4.7 g/kg/day, five times more than that recommended. About 9% and 90% of the children consumed a lower proportion of energy than the lower limit of the Reference Intake range for carbohydrates and fat, respectively. Around a third consumed less fibre and magnesium and 100% less vitamin D than the recommended Adequate Intake (AI). Almost a third consumed less vitamin A than the recommended Average Requirement (AR) and 86% of the children showed excessive sodium consumption. Portuguese toddlers consumed a low proportion of energy from fat, had energy and protein intakes above the recommendations and excessive intakes of sodium, and inadequate intakes of vitamin A. Every child consumed less than the recommended AI for vitamin D.

Background: Mountain insect biodiversity is unique, but is menaced by different drivers, particularly climate and land-use changes. In mainland Portugal, the highest mountain - Serra da Estrela - is one of the most important biodiversity hotspots, being classified as Natural Park since 1976. Many lepidopteran and odonate species, including rare and protected species, are known to occur in Serra da Estrela, but basic knowledge on their abundance, distribution and ecology is still lacking. Standardised sampling of these communities is crucial to provide valuable biological information to support short-term decision-making for conservation management, setting simultaneously the standards for mountain biodiversity monitoring aiming to tackle the effects of environmental change in the long-term. New information: This study reports novel information on lepidopteran and odonate species diversity, distribution and abundance from Serra da Estrela Natural Park (Portugal). Seventy-two lepidopteran and 26 odonate species were sampled in this protected area, including the first findings of Apaturailia (Denis & Schiffermüller, 1775), Macromiasplendens (Pictet, 1843) and Vanessavirginiensis (Drury, 1773). New populations of Euphydriasaurinia (Rottemburg, 1775) and Oxygastracurtisii (Dale, 1834), protected species under the Habitats Directive, were found in this Natural Park and novel distribution and ecological data were collected for most species, including several rare species and subspecies [e.g. Aeshnajuncea (Linnaeus, 1758), Coenonymphaglycerioniphioides Staudinger, 1870, Cyanirissemiargus (Rottemburg, 1775) and Sympetrumflaveolum (Linnaeus, 1758)]. All data were collected using standardised sampling allowing its use as a baseline for biodiversity monitoring in Serra da Estrela.

Introduction: A pre-graduate training programme in paediatric surgery was applied to students in four medical schools of Mozambique. In this paper, we evaluate the early results of the programme. Materials and Methods: A pre-graduate training programme was developed and applied in two stages, theoretical education available at an online platform and a face-to-face session. To evaluate the programme, a diagnostic test was applied to all participants before the face-to-face session and, the same test, was applied again at the end of the session. Results: A total of 236 students participated in the programme. Forty-four per cent had a negative score on the diagnostic test. When the test was repeated, 91.9% had a positive score, and the difference between the scores in both tests reached statistical significance (P < 0.05). The participants who completed the first phase of the programme presented a higher median score in both tests (P < 0.05). Conclusions: : The diagnostic tests allowed us to verify there was an increase in knowledge before and after the face-to-face session. There was also a significant difference between those participants who completed the online phase of the programme and those who did only the face-to-face session, which allows us to conclude that the online teaching programme was effective.

Background: Syrphid flies are important ecological indicators and provide crucial ecosystem services, being important pollinators and biological control agents of insect pests. These charismatic insects are conspicuous and, due to their size and colourful patterns, are relatively easy to identify. However, the lack of user-friendly literature (e.g. photographic guides) for most areas may hamper its wider selection as a study group in biodiversity and ecological studies. The syrphid fauna of Madeira Archipelago comprises 26 species, including four endemics (Eumerushispidus Smit, Aguiar & Wakeham-Dawson, 2004; Melanostomawollastoni Wakeham-Dawson, Aguiar, Smit, McCullough & Wyatt, 2004; Myathropausta, Wollaston, 1858 and Xanthandrusbabyssa, Walker, 1849), but, despite the current good taxonomic knowledge on this group, information on species distribution, ecology and conservation is still lacking. Here, we provide a pictorial key to the adult hoverflies of Madeira Archipelago highlighting diagnostic characteristics and present photographs of both males and females (in dorsal and lateral views) in colour plates. The key and plates will help researchers to differentiate these species, thus encouraging the use of this insect group in future bioindication studies. In addition, this study also aims to engage a broader audience of non-experts in improving the knowledge on the distribution and ecology of Madeira syrphids. New information: We provide a checklist for the hoverflies of Madeira Archipelago and a pictorial key to help on species identification.

Hérnia lombar é definida como a extrusão de órgãos intra ou extraperitoneais através da descontinuidade da parede abdominal posterolateral. Corresponde a menos de 1% a 2% de todas as hérnias da parede abdominal, sendo poucos os casos descritos na literatura. Quando a hérnia ocorre no triângulo de Grynfelt, ela é definida como hérnia lombar superior. Uma vez que são incomuns na prática médica, podem ser confundidas com lipomas ou mesmo abscessos. Este artigo apresenta o caso de hérnia gigante de Grynfelt em um paciente do sexo masculino, submetido ao atendimento hospitalar inicial devido à dor lombar associada à massa palpável. Descoberto o diagnóstico, foi realizada incisão transversa no ápice da hérnia onde foi identificado o saco herniário e o trígono de Grynfelt. O reparo foi realizado por meio da colocação de uma tela de polipropileno com resultados satisfatórios. Reforça-se a importância do diagnóstico de uma afecção rara e a experiência do cirurgião para uma abordagem adequada.

OBJECTIVE: To assess the longitudinal association between parental BMI and offspring's BMI, in EPACI Portugal 2012. DESIGN: Longitudinal study with retrospective collection of children's anthropometry data since birth. Children's anthropometric data were gathered from individual child health bulletins, and parents' anthropometrics were self-reported. Children's and parents' BMI were classified according to WHO cut-offs. Linear mixed models with random intercept and slope for age were applied to quantify the association between parental BMI and children BMI Z-score (zBMI). SETTING: EPACI Portugal 2012. PARTICIPANTS: Representative sample from the Portuguese population (n 2230) aged from 12 to 36 months. RESULTS: 58·9 % of the fathers and 35·6 % of the mothers were overweight (OW) or obese. Prevalence of infants who were, at least, at risk of OW increased from 17·0 % to 30·3 % since birth to 12 months. About half of the mothers with pre-pregnancy OW and obesity (OB) gained gestational weight above the recommendations. The children from mothers with gestational weight gain (GWG) below the recommendations showed a -0·15 SD lower zBMI (95 % CI -0·23, -0·06) in early life, comparing with mothers within GWG recommendations. Children of obese mothers were more likely to present a higher zBMI (0·24 SD, 95 % CI 0·13, 0·35) throughout the first months of life. CONCLUSIONS: A high prevalence of OW and OB was observed in Portuguese young adults and toddlers. Mothers' pre-pregnancy BMI and insufficient GWG had a direct effect on offspring BMI. Early effective interventions are needed in order to prevent the transgenerational transmission of OB.

A checklist of the tephritid fauna (Diptera: Tephritidae) from Madeira archipelago is presented, including 17 known species and new distribution records for Porto Santo and Desertas Islands. An identification key to Madeira Tephritidae species with detailed illustrations is also provided. A new species of Oedosphenella Frey 1936, discovered in Madeira Island in 2004, is described as Oedosphenella bob sp. n. This genus is redefined and a key to the known species is provided. Comparative morphological analysis revealed that Bevismyia basuto Munro 1957 from Malawi should be included in the genus Oedosphenella. Therefore, we consider the genus name Oedosphenella Frey 1936 to be a senior synonym of Bevismyia Munro 1957 syn.n.

We examined the effects of a 6-month school-based soccer programme on cardiovascular (CV) and metabolic risk factors in overweight children. Methods: 40 boys [8-12 years; body mass index (BMI) >2 standard deviations of WHO reference values] participated in complementary school-based physical education classes (two sessions per week, 45-90 min each). The participants were divided into a soccer group (SG; n = 20) and a control group (CG; n = 20). The SG intervention involved 3 extra-curricular school-based soccer sessions per week, 60-90 min each. The intervention lasted for 6-months. All measurements were taken at baseline and after 6-months. From baseline to 6-months, the SG significantly improved (p < .05) BMI z-score, waist circumference, waist-to-height ratio, percentage of fat mass, percentage of fat-free mass, diastolic blood pressure, total cholesterol, triglycerides, low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol, but no such improvements were observed for the CG. After the intervention, the prevalence of soccer participants with normal waist-to-height ratio (30 vs. 5%; p = .037), systolic blood pressure (90 vs. 55%; p = .039), total cholesterol (80 vs. 65%; p = .035) and LDL-C (90 vs. 75%; p = .012) were significantly higher than at baseline. The findings suggest that a 6-month school-based soccer intervention program represents an effective strategy to reduce CV and metabolic risk factors in overweight children prepared to take part in a soccer program.

BACKGROUND: Here we present the data obtained from the samples collected as part of a large research project (MACDIV) which aims at understanding the drivers of spider (Araneae) community assembly in Macaronesian islands. To obtain the data, we applied the sampling protocol COBRA (Conservation Oriented Biodiversity Rapid Assessment), in twelve 50 m x 50 m native forest plots and five dry habitat plots on the island of Madeiraand in 5 dry habitat plots on the island of Porto Santo. Through this publication, we contribute to the knowledge of the arachnofauna of the Madeiran archipelago. NEW INFORMATION: From the samples that we collected, we obtained a total of 14,902 specimens, of which 49% were adults (7,263). We identified these specimens to 87 species and 18 morphospecies (undescribed), belonging to 26 families. Species of the family Linyphiidae dominated the samples, with 24 (morpho)species. Out of the 105 recorded (morpho)species, 34 were endemic, 26 native non-endemic, 22 introduced and 23 species of unknown origin. We report seven new records of possibly recently introduced species in the Madeiran archipelago. We also present 21 new records for Madeira island and 32 for Porto Santo (33 for the whole archipelago).

BACKGROUND: The Iberian Peninsula is a diverse region that contains several different bioclimatic areas within one confined space, leading to high biodiversity. Portugal distinguishes itself in this regard by having a high count of spider species (829) and a remarkable number of endemic spider species (42) for its size (approximately 88,890 km2). However, only one non-endemic species (Macrothele calpeiana) is currently protected by the Natura 2000 network and no endemic spider species (aside from Anapistula ataecina) has been assessed according to the IUCN Red List criteria. The objective of this paper is to assess all non-assessed endemic species (41) as well as M. calpeiana. NEW INFORMATION: The 43 assessed species belong to 15 families, the richest being Zodariidae, Dysderidae, Linyphiidae and Gnaphosidae. In general and despite the lack of information on more than half the species, general patterns and trends could be found.Only 18 species (including M. calpeiana and A. ataecina) had enough data to allow their EOO (extent of occurrence) and AOO (area of occurrence) to be quantified. Of these, we modelled the distribution of 14 epigean species, eight of which were found to be widespread. The remaining six fulfilled at least one of the criteria for threatened species. Four species are troglobiont, all of which meet the EOO and AOO thresholds for threatened species. The remaining 25 Portuguese endemics had no reliable information on their range. Only nine species out of the 43 are estimated to be in decline and 11 are stable, with the majority of species having no information on trends (23 species).Forest areas, sand dunes, shrublands and caves host the majority of species. As such, the threats to Portuguese endemics reflect the diversity of habitats they occupy. Urbanisation and climate change seem to be the most important threats to these species, although other factors are also important and represented across the data.A considerable proportion of the currently known Portuguese endemic species can be found in national protected areas, with higher prominence to the Serras de Aire e Candeeiros, Douro Internacional, Vale do Guadiana, Sudoeste Alentejano e Costa Vicentina and Arrábida Natural Parks. These correspond mostly to areas that have been particularly well sampled during the last two decades.

Oceanic islands have been providing important insights on the structuring of ecological communities and, under the context of the present biodiversity crisis, they are paramount to assess the effects of biological invasions on community assembly. In this study we compare the taxonomic and functional diversity of insect herbivore assemblages associated with the dominant tree species of Azorean native forests and investigate the ecological processes that may have originated current patterns of plant-herbivore associations. Five dominant trees-Erica azorica, Ilex perado subsp. azorica, Juniperus brevifolia, Laurus azorica and Vaccinium cylindraceum-were sampled in the remnants of the native forest of Terceira Island (Azores) using a standardised methodology. The taxonomic and functional diversity of insect herbivore assemblages was assessed using complementary metrics and beta diversity partitioning analysis (species replacement and richness differences) aiming to evaluate the variation in insect herbivore assemblages within and between the study plant species. Sixty two insect species, mostly bugs (Hemiptera) and caterpillars (Lepidoptera), were found in the five study plants with indigenous (endemic and native non-endemic) insects occurring with higher species richness and abundance than introduced ones. Species replacement was the most important component of insect herbivore taxonomic beta diversity while differences in trait richness played a major role on functional beta diversity. The endemic E. azorica stands out from the other study plants by having associated a very distinct insect herbivore assemblage with a particular set of functional attributes, mainly composed by large bodied and long shaped species that feed by chewing. Despite the progressive biotic homogenization witnessed in the Azores during the last few decades, several strong associations between the endemic trees and their indigenous insect herbivores remain.

BACKGROUND: The data presented here come from samples collected as part of two recent research projects (NETBIOME - ISLANDBIODIV and FCT - MACDIV) which aimed at understanding the drivers of community assembly in Macaronesian islands. We applied the sampling protocol COBRA (Conservation Oriented Biodiversity Rapid Assessment, Cardoso 2009) in sixteen 50 m x 50 m native forest plots in the Azorean Islands of Pico (6 plots) and Terceira (10 plots) to assess spider diversity. Through this publication, we contribute to the knowledge of the arachnofauna of the Azores and, more specifically, to that of the islands of Pico and Terceira. NEW INFORMATION: The collected samples yielded 8,789 specimens, of which 45% were adults (3,970) belonging to 13 families, 36 species and three morphospecies that have yet to be described. Species of the family Linyphiidae dominated the samples, with 17 species and two morphospecies that have yet to be described (48% of the taxa). Out of the identified (morpho)species, 16 were introduced, 13 Azorean endemic (three of which were undescribed) and seven native (five of them Macaronesian endemics). We report the first record of the introduced species Haplodrassus signifer and Agyneta decora in Pico Island.

BACKGROUND: Azorean volcanic cave biodiversity is under considerable pressure due to ongoing threats of pollution, land use change, touristic activities or climate change. In this contribution, we present the IUCN Red List profiles of 15 cave-adapted arthropod species, endemic to the Azorean archipelago, including species belonging to the speciose genus Trechus (Carabidae), which is represented in Azores by seven species. The objective of this paper is to assess all endemic Azorean cave-adapted species and advise on possible future research and conservation actions critical for the long-term survival of the most endangered species. NEW INFORMATION: Most species have a restricted distribution (i.e. occur in one or two caves), very small extent of occurrence (EOO) and a small area of occupancy (AOO). A continuing decline in the number of mature individuals is inferred from the ongoing cave habitat degradation. The two troglobitic species of the homopteran genus Cixius are in great danger of extinction due to major land-use changes in epigean habitats above their known localities. We suggest, as future measures of conservation, the regular monitoring of the species (every five years), the creation of additional protected caves, the limitation of several aggressive activities around the caves (e.g. decreasing pasture intensification) and in some cases the creation of fences in the entrance of the most important caves.

Children's knowledge about food and nutrition is very important for the develop-ment of appropriate patterns of eating behaviour. Nowadays, because children's input is becoming to get more importance in parents' decisions about nutritional choices it is important to contribute for their knowledge development regarding not only nutrition or food but also healthy habits regarding sleeping and physical activities. The purpose of this study was to contribute for the assessment of nutrition/food preferences in pre-schoolers and the contribution of gaming for nutrition/food knowledge evaluation. It also intended to establish a relation between children's BMI and the knowledge of nutrition/food and good sleep and physical activities habits. An observational, cross-sectional, quantitative study was conducted with 364 children's attending preschool from a metropolitan area of the norther region of Portugal and their parents. Food preferences varied from pasta (13.4%) to meat (8.8%) or tomatoes (6.3%) and undesired foods were, among others, fish (16.2%), soup (13.2%), tomatoes (12.6%) and onions (10.7%). Preschoolers showed high levels of knowledge regarding food categorization (92.3%), good sleeping habits (90.1%) and good physical activities habits (94.0%). Results showed significant differences for sleeping habits knowledge with lower BMI (kg/m2) for children that gave correct answers (M = 16.35, SD = 2.04 vs. M = 17.62, SD = 2.37). Children with increased BMI showed more risk of poor knowledge regarding good sleeping habits (aOR = 1.33, 95% CI = [1.07-1.69]). Parent's low education was found to be a risk factor for children's poor knowledge of good sleeping habits (OR = 8.21, 95% CI = [1.85-35.55])

Breastfeeding is an important strategy to provide all-around care for newborns du-ring the first six months of life. It has the power to prevent early mortality, malnutrition and obesity of the child and also prevent some diseases and contribute for faster recovery of the mother. Moreover it contributes decisively for the establishment of successful bound between the mother and the child. The purpose of this study was to contribute for the study of the relation of breastfeeding with parent's type of family and education. An observational, cross-sectional, quantitative study was conducted with 364 children's attending preschool from a metropolitan area of the norther region of Portugal and their parents. Results showed an overall breastfeeding proportion of 88.9%, regardless of the duration. Considering duration, results showed that only 38.6% of the children were breastfed for a period equal or higher to six months, that is WHO's recommendation. Adjusted logistic regressions showed that the risk of not breastfeeding was associated with non-nuclear families (aOR = 3.19) and other types of families (aOR = 4.36). Regarding education, significant associations were found concerning mothers low education (aOR = 8.39) and both parents low education (aOR = 10.98). No relation was found between breastfeed duration and education on overall, or considering the type of family

During the last few centuries oceanic island biodiversity has been drastically modified by human-mediated activities. These changes have led to the increased homogenization of island biota and to a high number of extinctions lending support to the recognition of oceanic islands as major threatspots worldwide. Here, we investigate the impact of habitat changes on the spider and ground beetle assemblages of the native forests of Madeira (Madeira archipelago) and Terceira (Azores archipelago) and evaluate its effects on the relative contribution of rare endemics and introduced species to island biodiversity patterns. We found that the native laurel forest of Madeira supported higher species richness of spiders and ground beetles compared with Terceira, including a much larger proportion of indigenous species, particularly endemics. In Terceira, introduced species are well-represented in both terrestrial arthropod taxa and seem to thrive in native forests as shown by the analysis of species abundance distributions (SAD) and occupancy frequency distributions (OFD). Low abundance range-restricted species in Terceira are mostly introduced species dispersing from neighbouring man-made habitats while in Madeira a large number of true rare endemic species can still be found in the native laurel forest. Further, our comparative analysis shows striking differences in species richness and composition that are due to the geographical and geological particularities of the two islands, but also seem to reflect the differences in the severity of human-mediated impacts between them. The high proportion of introduced species, the virtual absence of rare native species and the finding that the SADs and OFDs of introduced species match the pattern of native species in Terceira suggest the role of man as an important driver of species diversity in oceanic islands and add evidence for an extensive and severe human-induced species loss in the native forests of Terceira.
