RESUMO

Purpose: To characterize treatments and outcomes in incontinentia pigmenti. Methods: Cases of incontinentia pigmenti were consecutively identified from a retina practice. Inclusion criteria were patients with incontinentia pigmenti with at least 6 months of follow-up. All patients had a full ophthalmic examination, including imaging with widefield fundus photography and widefield fluorescein angiography. Eyes with areas of avascular retina were treated with laser photocoagulation (except for 1 eye with mild changes). Results: Thirty-six eyes of 18 patients with incontinentia pigmenti were included. The median age at presentation was 11 months. On presentation, 7 eyes had a visual acuity (VA) of 20/40 or better and 3 eyes had VA of 20/50 to 20/100. The remaining 26 eyes could fix and follow or had at least light perception (LP) VA given the patients' young age. Of the 36 eyes, 20 (56%) had retinal involvement. The mean follow-up for treated patients was 6.9 years. Seventy-four percent of treated eyes required 1 laser session only. No eye that received laser treatment subsequently developed a retinal detachment. Of the 26 eyes with initial fix-and-follow or LP VA, 12 had Snellen or Allen VA testing at follow-up. Nine of these eyes had a follow-up VA of 20/40 or better. Of 10 eyes with a Snellen or Allen VA recorded at the initial visit, 9 had a final VA that was the same or improved. Conclusions: Laser photocoagulation was effective in treating patients with retinal manifestations of incontinentia pigmenti. Except for 1 eye, VA remained stable at the final follow-up.

Globe ruptures, while uncommon, must be promptly recognized to optimize the possibility of preserving vision. Differentiating open globe injuries from corneal abrasions, hyphema, and other ocular injuries is critical. When a globe rupture is identified, prompt ophthalmology consultation, placement of a rigid eye shield, administration of prophylactic systemic antibiotics (and antifungals with an appropriate history), and a tetanus vaccine update (if needed) are key to a successful outcome, so long as the visual prognosis is not dismal. We describe a case of ocular trauma presenting as a blood blister adherent to the cornea, which was initially assessed (by the triage nurse) to be a blood clot but later identified as a globe rupture. We include a discussion of globe rupture recognition and its management by the emergency department.

We report the case of a 64-year-old male who developed sudden onset of bilateral no light perception vision and bilateral total internal ophthalmoplegia after pituitary apoplexy. He underwent transsphenoidal pituitary decompression. Four months after the surgery, the patient recovered excellent functional vision (20/25) in one eye, though with significant residual visual field loss. He regained full extraocular motility bilaterally.

Timely ophthalmologic consultation is important to ensure patients receive high quality ophthalmologic care in the Emergency Department (ED). Tele-ophthalmology may prove useful in safely and efficiently managing ED eye-related complaints. Prior to implementing such a solution, current consultation patterns must be understood. We aimed to assess case-mix acuity and consultation workflow patterns in the ophthalmology consult service at a tertiary emergency department in New York City. The medical records of patients with eye-related complaints who presented to the ED between January 1, 2015 and December 31, 2015 were reviewed. Visits were retrospectively assigned acuities and the ophthalmologic subspecialty involved in the case was recorded. The number of ophthalmologic consultations ordered and consultant response times were analyzed. Patients who were transferred to the ED for eye-related complaints were included. The ED received 1090 eye-related complaints in this period. 60% were retrospectively assigned low acuity, 27% were assigned medium acuity, and 13% were assigned high acuity. Ophthalmology was consulted on 19% of low, 18% of medium, and 48% of high acuity cases. 44% of complaints involved the anterior segment and 30% involved oculoplastics. 2/3 of transfer patients initially assigned high acuity were downgraded to low acuity upon examination. On average, the consult note was created and signed within 109 and 153 min, respectively, after consult order. ED consults are heavily weighted towards pathology of low-to-medium acuity affecting the anterior segment and ocular adnexa. Currently available tele-ophthalmology technology can potentially address a large volume of eye-related visits.

RESUMO

The authors describe the case of a 19-year-old female who suffered posttraumatic emphysema of the optic nerve sheath. She suffered massive head trauma requiring emergent neurosurgery and was incidentally found to have air in her optic nerve sheath on CT scan. At 6 weeks follow up, her visual acuity (20/25 uncorrected) and color perception in the affected eye were excellent. Her examination was notable for an afferent pupillary defect, mild disc pallor, and optic nerve atrophy on optical coherence tomography. This is a case of a patient with posttraumatic optic nerve sheath emphysema who recovered excellent visual function and received follow-up ophthalmic imaging.

Optical coherence tomography (OCT) images of the retina are a powerful tool for diagnosing and monitoring eye disease. However, they are plagued by speckle noise, which reduces image quality and reliability of assessment. This paper introduces a novel speckle reduction method inspired by the recent successes of deep learning in medical imaging. We present two versions of the network to reflect the needs and preferences of different end-users. Specifically, we train a convolution neural network to denoise cross-sections from OCT volumes of healthy eyes using either (1) mean-squared error, or (2) a generative adversarial network (GAN) with Wasserstein distance and perceptual similarity. We then interrogate the success of both methods with extensive quantitative and qualitative metrics on cross-sections from both healthy and glaucomatous eyes. The results show that the former approach provides state-of-the-art improvement in quantitative metrics such as PSNR and SSIM, and aids layer segmentation. However, the latter approach, which puts more weight on visual perception, outperformed for qualitative comparisons based on accuracy, clarity, and personal preference. Overall, our results demonstrate the effectiveness and efficiency of a deep learning approach to denoising OCT images, while maintaining subtle details in the images.
