RESUMO

Epithelioid hemangioendothelioma is a rare bone tumor. The authors report a case of a grade 1 multifocal epithelioid hemangioendothelioma of the foot in a 24-year-old man treated by large surgical resection of the two lesions.

RESUMO

Miliary tuberculosis is rare and requires rapid diagnosis. Outcome is fatal in 25% of the cases. Since radiography and laboratory tests contribute little to early diagnosis, clinical findings are primordial. Antituberculosis antibiotic therapy is frequently started before microbiological confirmation of the diagnosis.

RESUMO

Hydatid disease is the most common human disease caused by helminths, but primary skeletal involvement is uncommon. Diagnosis is usually obtained late after considerable extension, half of which concerns the spine. The dissemination mode leads to local malignancy with severe prognosis. Medical imaging is essential. Plain x-ray findings are polymorphous and nonspecific, but computed tomography and magnetic resonance imaging are particularly useful for exhaustive pretherapeutic assessment of extension and for patient follow-up.

RESUMO

Bone and joint localizations of treponematosis vary greatly although there are many common features. Osteal and periosteal lesions are common. We report radiographic descriptions of bone and joint treponematosis, which must not go unrecognized due to the increasing incidence of these diseases.

RESUMO

We report a case of dual parathyroid adenoma associated with an ectopic gland in the right latero-esophageal region of the mediastinum revealed by asymptomatic hypercalcemia. Because of this dual localization and the lack of MBI uptake on the mediastinal scintigram, thoracotomy was used as the first line approach instead of cervicotomy.

RESUMO

The authors report a case of american pulmonary histoplasmosis discovered by chance on a chest radiograph in a non immunocompromised patient, back from a stay in French Guyana. Confronted with the negativity of usual mycotic research, diagnosis has been made thanks to wedge excision by video-assisted thoracic surgery. The authors briefly sum up the recent facts regarding this imported infection, stressing the interest of a direct approach by surgical practices not very invasive that allow to eliminate with certainty neoplasm or tuberculosis.

RESUMO

We describe the contribution of computed tomography in the determination of the renal damage after using tiaprofenic acid. Computed tomography realized six hours after injection of contrast medium revealed corticomedullary enhancement secondary to vascular and tubular stasis. This stasis is compatible with interstitial edema in relation with usually reversible lesions of tubulo-interstitial nephritis induced by of nonsteroidal anti-inflammatory drugs.

RESUMO

One hundred and ninety duodenal ulcer patients who had relief of pain and endoscopically proven ulcer healing after a short treatment period are allocated at random to double-blind maintenance treatment with a synthetic dehydroprostaglandin-E1, rioprostil, 300 micrograms, or ranitidine, 150 mg, at bedtime for 6 months. Patients are monitored every two months and examined by endoscopy after six months of treatment, or more often if warranted. The cumulative relapse rate in the rioprostil group at six months is 32% (25/78) vs. 28% (20/72) in the ranitidine group. This difference is not significant. The percentage of side effects observed is 17% in the rioprostil group vs. 5% in the ranitidine group, but discontinuation of treatment is observed with the same frequency in the two groups.

A total of 156 patients with endoscopically proven duodenal ulcers are randomized in a double-blind, multicentre trial comparing rioprostil, 300 micrograms b.d., with ranitidine, 150 mg b.d. With rioprostil, the cumulative healing rate by endoscopy is 73% at 4 weeks, and 87% at 6 weeks. With ranitidine it is 79% and 92%, respectively. There is no difference in the occurrence of pain after the two types of treatment, and the side effects are comparable in the two groups. These results show that rioprostil is probably as effective and safe as ranitidine in the treatment of duodenal ulcer.

RESUMO

Diagnosis of annular pancreas is possible by endoscopic retrograde cholangiopancreatography (ERCP) only when the branch encircling the duodenum is connected to the pancreatic duct system. The authors report on a case of annular pancreas characterized by pain and relapsing acute pancreatitis secondary to the malformation. Attention is focused on both the importance of endoscopic retrograde cholangiopancreatography for diagnosing the congenital duct anomaly and the exceptional association of acute pancreatitis and an annular pancreas.
