RESUMO

Transvaginal small bowel evisceration is a rare surgical emergency that requires urgent surgery to prevent bowel necrosis, sepsis, and death. It was first reported in 1864 by Hyernaux with less than 100 cases reported since the original publication. The overall mortality rate is reported as 5.6 percent. We present the case of a 49-year-old woman who presented to the emergency department with a chief complaint of moderate abdominal pain and vaginal bleeding for 1 hour. The patient reported that she underwent a robotic-assisted laparoscopic hysterectomy 11 weeks prior for uterine fibroids. Visual examination revealed a loop of the small bowel coming from the superior aspect of her vagina. Literature reviews have noted a higher incidence of dehiscence following robotic-assisted total laparoscopic hysterectomy. It is important for the emergency physician to make the diagnosis, initiate prompt consultation with departments of obstetrics and gynecology and general surgery, and treat for potential infection.

Neurodegenerative diseases are a devastating group of disorders that can be difficult to accurately diagnose. Although these disorders are difficult to manage owing to relatively limited treatment options, an early and correct diagnosis can help with managing symptoms and coping with the later stages of these disease processes. Both anatomic structural imaging and physiologic molecular imaging have evolved to a state in which these neurodegenerative processes can be identified relatively early with high accuracy. To determine the underlying disease, the radiologist should understand the different distributions and pathophysiologic processes involved. High-spatial-resolution MRI allows detection of subtle morphologic changes, as well as potential complications and alternate diagnoses, while molecular imaging allows visualization of altered function or abnormal increased or decreased concentration of disease-specific markers. These methodologies are complementary. Appropriate workup and interpretation of diagnostic studies require an integrated, multimodality, multidisciplinary approach. This article reviews the protocols and findings at MRI and nuclear medicine imaging, including with the use of flurodeoxyglucose, amyloid tracers, and dopaminergic transporter imaging (ioflupane). The pathophysiology of some of the major neurodegenerative processes and their clinical presentations are also reviewed; this information is critical to understand how these imaging modalities work, and it aids in the integration of clinical data to help synthesize a final diagnosis. Radiologists and nuclear medicine physicians aiming to include the evaluation of neurodegenerative diseases in their practice should be aware of and familiar with the multiple imaging modalities available and how using these modalities is essential in the multidisciplinary management of patients with neurodegenerative diseases.©RSNA, 2020.

Transthyretin amyloid deposition is present in 17% of autopsies of patients with heart failure and a preserved ejection fraction (HFpEF). Technetium-pyrophosphate scintigraphy (Tc-PYP) is sensitive and specific to diagnose cardiac transthyretin amyloid deposition (ATTR). The prevalence of ATTR by Tc-PYP was evaluated along with echocardiographic parameters in patients with HFpEF. One-hundred consecutive patients with HFpEF, who had Tc-PYP, were retrospectively evaluated. Echocardiographic variables were analyzed to compare patients with positive versus negative ATTR infiltration. Myocardial ATTR was present in 19% of patients. Individuals with ATTR were older with a mean age of 82 ± 7 versus 75 ± 13 years (P = 0.03), had increased left ventricular hypertrophy with the interventricular septum measuring 1.6 (IQR, 1.4-2.0) versus 1.4 (IQR, 1.3-1.6) cm (P = 0.002), had a greater mean left ventricular mass index of 160 ± 50 g/m versus 131 ± 44 g/m (P = 0.01), and a reduced global longitudinal strain measuring -11% (IQR, -9 to -12) versus -12% (IQR, -10 to -16), P = 0.04. The prevalence of ATTR myocardial deposition demonstrated by Tc-PYP in patients with HFpEF is comparable to that of autopsy studies. It is more common in older patients, with increased left ventricular hypertrophy and reduced global longitudinal strain.

BACKGROUND: An intramedullary spinal cord abscess is a rare, albeit widely publicized entity. Classically, patients have an acute onset of symptoms with fevers and leukocytosis supporting the diagnosis. We present a case of intramedullary spinal cord without classic history or imaging characteristics in which the diagnosis was made with diffusion weighted magnetic resonance imaging (MRI). CASE DESCRIPTION: A 57-year-old physician presented with severe neck and shoulder pain, which progressed over several days to right-sided hemiparesis with dysesthesias. There was no history of fevers, rigors, or illness. A contrast enhanced MRI of the cervical spine revealed an intramedullary lesion centered around C6-T1 that showed peripheral enhancement with gadolinium and edema extending rostrally and caudally. He was then transferred to our institution where the novel application of diffusion weighted MRI of the spinal cord was performed, suggesting an abscess. He then underwent focal laminectomies and biopsy of this lesion with drainage of the necrotic cavity. Intraoperative Gram stain revealed gram-positive cocci, and cultures were sent to the laboratory. After draining the purulent material and completing a course of tailored antibiotics, the patient showed improvement of his neurologic deficit. CONCLUSIONS: The use of diffusion weighted imaging in the spine is a novel application of technology that provided an accurate preoperative diagnosis and allowed us to tailor our surgical approach and provide a rapid focal decompression.

RESUMO

Aneurysmal bone cyst is an uncommon nonneoplastic lesion that occurs predominantly during the first 2 decades of life. Angiographic- and tissue-phase imaging provided more striking evidence of this unexpected lesion than skeletal-phase imaging in a 13-year-old girl with pain in the right buttock and posterior thigh.
