RESUMO

Currently, the methods used to represent loads do not differ between the characteristics that compose them or the nature of these. Therefore, the purpose of this research is to develop digital twins mirroring load models that can be used for more precise studies on power-flows and stability within the National Transmission Grid (NTG). Off-line sampling data of different electric measurements have been used in six substations of the Guayaquil (Ecuador) area. These values were organized by statistical methods and by time periods, to determine the parameters that make up the static load model. Dynamic models are also constructed for the same six substations using the analysis of current and voltage signals obtained from the substations. All data is organized to show a digital twin mirroring visual representation of the disturbances that may occur in the substation buses. A more accurate description of the static and dynamic responses can be obtained by replacing the general model that is currently used by engineers and planners with off-line sampling data. Digital twins help the electric utility businesses gather, visualise, and contextualise data from different sources, and enable to act on data, and to understand what-if modelling stability scenarios.

RESUMO

Screening of serum and fecal samples from huemul (Hippocamelus bisulcus) and pudu (Pudu puda) from southern Chile for Mycobacterium bovis and Mycobacterium avium paratuberculosis (MAP) found all but four samples Mycobacterium-negative. The positive sequences showed only 92-93% similarity with MAP and were from remote Isla Riesco populations.

The HLA compatibility continues to be the main limitation when finding compatible donors, especially if an identical match is not found within the patient's family group. The creation of bone marrow registries allowed a therapeutic option by identifying 10/10 compatible unrelated donors (URD). However, the availability and frequency of haplotypes and HLA alleles are different among ethnic groups and geographical areas, increasing the difficulty of finding identical matches in international registries. In this study, the HLA-A, -B, -C, -DRB1, and -DQB1 loci of 1763 donors registered in the Colombian Bone Marrow Registry were typed by next-generation sequencing. A total of 52 HLA-A, 111 HLA-B, 41 HLA-C, 47 HLA-DRB1, and 20 HLA-DQB1 alleles were identified. The 3 most frequent alleles for each loci were A*24:02g (20,8%), A*02:01g (16,1%), A*01:01g (7.06%); B*35:43g (7.69%), B*40:02g (7.18%), B*44:03g (6.07%); C*04:01g (15.40%), C*01:02g (10.49%), C*07:02g (10.44%); DRB1*04:07g (11.03%), DRB1*07:01g (9.78%), DRB1*08:02g (6.72%); DQB1*03:02g (20.96%), DQB1*03:01g (17.78%) and DQB1*02:01g (16.05%). A total of 497 HLA-A-C-B-DRB1-DQB1 haplotypes were observed with a frequency greater than or equal to 0.05% (> 0.05%); the haplotypes with the highest frequency were A*24:02g~B*35:43g~C*01:02g~DQB1*03:02g~DRB1*04:07g (3.34%), A*29:02g~B*44:03g~C*16:01g~DQB1*02:01g~DRB1*07:01g (2.04%), and A*01:01g~B*08:01g~C*07:01g~DQB1*02:01g~DRB1*03:01g (1.83%). This data will allow the new Colombian Bone Marrow Donor Registry to assess the genetic heterogeneity of the Colombian population and serve as a tool of interest for future searches of unrelated donors in the country.

Resumen: EXIT (tratamiento ex útero intraparto). El manejo anestésico para esta cirugía es significativamente diferente del manejo anestésico de una cesárea convencional, involucra anestesia general profunda con halogenados, administración de narcóticos, relajación uterina adecuada y preservación del flujo útero placentario hacia el feto durante la anestesia y al mismo tiempo tiene el beneficio de la anestesia general para que el producto obtenga anestesia y facilite el acceso a la vía aérea del neonato antes del pinzamiento del cordón umbilical. El fin de mantener la oxigenación a través de la placenta es efectuar la maniobra de intubación sin el riesgo de hipoxia. En el Hospital Infantil de México «Federico Gómez¼ se lleva a cabo este tipo de cirugías desde junio de 2007; sin embargo, aún no se cuenta con un manejo homogéneo. Material y métodos: A través de un reporte de casos se hizo una revisión perioperatoria de las pacientes embarazadas, a las cuales se les dio manejo anestésico para cirugía EXIT en el período comprendido entre junio de 2007 y mayo de 2018. Resultados: De los 43 casos manejados, la información anestésica perioperatoria obtenida permitió homologar el manejo anestésico del binomio materno fetal para poder realizar un protocolo intrahospitalario. Conclusión: El poder realizar un protocolo para el manejo anestésico del binomio materno fetal en el proyecto EXIT permite disminuir las complicaciones y la morbimortalidad ofreciendo mejor calidad en la atención.


Abstract: The anesthetic management for this procedure is quite different from the anesthetic management of a conventional caesarean section. It includes deep general anesthesia with halogenated and narcotic administration, to get adecuate uterine relaxation and preservation of best uteroplacental flow to the fetus during anesthesia and the benefit of general anesthesia to aim the neonate show effects of of anesthetic medication by this way facilitate airway access before umbilical cord clamping. The aim to mantain placental oxigenation to neonate is avoid hypoxia risk during intubation technique. In the Hospital Infantil de Mexico «Federico Gómez¼ this procedures have been carried out since June 2007, however, there is still no homogeneous management. Material and methods: Through a case report, a perioperative files review was made of pregnant patients who were given anesthetic management for EXIT procedure in June 2007 to May 2018 period. Results: Of the 43 cases, the perioperative anesthetic information obtained allowed to standardize the anesthetic management of the maternal fetal binomial in order to perform an intrahospital protocol. Conclusion: The ability to perform a protocol for the anesthetic management of the maternal fetal binomial in the exit project allows to reduce complications and morbidity and mortality in the binomial, offering better quality of care.
