RESUMO

In 2005, odontogenic cyst was classified as keratocyst odontogenic tumor due to being aggressive and recurrent. The keratocyst odontogenic tumor has characteristics, with slow development, does not cause metastases and provides great bone destruction. The aim of this study was to discuss the aspects regarding the diagnosis, prognosis and treatment of odontogenic keratocyst tumor, through the report of two cases. Both initially underwent decompression of the lesion present proximity of anatomical structures to be great and noble, aiming to prevent pathological fractures. We carried out the clinical-radiographic and after regression of the lesion, patients underwent enucleation total.


El año 2005 quiste odontogénico fue clasificado como un tumor queratoquiste odontogénico (TQO) debido a su agresividad y recurrencia. El TQO tiene las características del tumor: crecimiento lento, no causa metástasis y proporciona una gran destrucción ósea. El objetivo de este estudio fue examinar los aspectos relacionados con el diagnóstico, pronóstico y tratamiento del TQO mediante la presentación de dos casos. Ambos casos fueron inicialmente sometidos a descompresión debido al gran tamaño de la lesión y la proximidad de ésta con estructuras anatómicas importantes, con el objetivo de prevenir las fracturas patológicas. Se llevó a cabo un seguimiento clínico-radiográfico y después de tener una regresión de la lesión postdescompresión, los pacientes fueron sometidos la enucleación total.

AIM: To evaluate the influence of deproteinized bovine bone mineral (DBBM) particles concomitant with the placement of a collagen membrane on alveolar ridge preservation and on osseointegration of implants placed into alveolar sockets immediately after tooth extraction. MATERIAL AND METHODS: The pulp tissue of the mesial roots of (3) P(3) was removed in six Labrador dogs and the root canals were filled. Flaps were elevated in the right side of the mandible, and the buccal and lingual alveolar bony plates were exposed. The third premolar was hemi-sectioned and the distal root was removed. A recipient site was prepared and an implant was placed lingually. After implant installation, defects of about 0.6 mm wide and 3.1 mm depth resulted at the buccal aspects of the implant, both at the test and at the control sites. The same surgical procedures and measurements were performed on the left side of the mandible. However, DBBM particles with a size of 0.25-1 mm were placed into the remaining defect concomitant with the placement of a collagen membrane. RESULTS: All implants were integrated into mature bone. No residual DBBM particles were detected at the test sites after 4 months of healing. Both the test and the control sites showed buccal alveolar bone resorption, 1.8 ± 1.1 and 2.1 ± 1 mm, respectively. The most coronal bone-to-implant contact at the buccal aspect was 2 ± 1.1 an 2.8 ± 1.3 mm, at the test and the control sites, respectively. This difference in the distance was statistically significant. CONCLUSION: The application of DBBM concomitant with a collagen membrane to fill the marginal defects around implants placed into the alveolus immediately after tooth extraction contributed to improved bone regeneration in the defects. However, with regard to buccal bony crest preservation, a limited contribution of DBBM particles was achieved.

AIM: To describe the early healing within the void obtained after the elevation of the sinus mucosa and simultaneous implant installation without the use of any grafting material in monkeys. MATERIAL AND METHODS: Implants were installed simultaneously with the elevation of the maxillary sinus using the lateral approach in eight monkeys without the use of grafting material. The healing after 4, 10, 20 and 30 days was evaluated in the area distal to the implants. Paraffin sections were prepared and analyzed using qualitative histological methods. RESULTS: The healing process was initiated by the formation of a coagulum and followed by a provisional matrix and woven bone. Subsequently, a parallel-fiber bone replaced woven bone. The dimension of the elevated area shrank during the healing process. Sprouts of woven bone, present to a moderate extent after 4 days, were more numerous after 10 and 20 days. Newly formed bone originated from the sinus walls and septa, while there was no evidence of participation of the Schneiderian membrane in this process. After 30 days, the window access appeared to be closed by a layer of newly formed trabecular bone. CONCLUSIONS: The coagulum that filled the void distal to the implant after simultaneous elevation of the sinus floor gave rise to newly formed bone. However, the void occupied by the coagulum shrank substantially. The Schneiderian membrane did not provide a basis for new bone formation in the early phase of healing.
