RESUMO

Between 1980 and 1996, 17 children who were prepubescent have had a combined intraarticular and extraarticular reconstruction of the anterior cruciate ligament using the iliotibial band that does not violate the physes. The average chronological age of the patients was 11 years (range, 2-14 years) and the average skeletal age of the patients was 10 years (range, 2-13 years). Eight of the 10 patients who had attained skeletal maturity were evaluated at an average of 66.5 months postoperatively (range, 25-168 months). All knees were stable subjectively by history and objectively by KT1000 testing. The average Lysholm score at assessment was 97.4. No child with a traumatic disruption had leg length discrepancy develop.

One hundred and seven hips (ninety-four patients) that had risk factors associated with the development of heterotopic ossification after total hip arthroplasty were treated with a single dose of radiation after the operation in an attempt to prevent the formation of heterotopic bone. A study was conducted to compare the efficacy of a single dose of 550 centigray (nineteen hips) with that of a single dose of 700 centigray (eighty-eight hips). Heterotopic ossification developed in twelve (63 per cent) of the nineteen hips that were treated with 550 centigray; grades 1, 2, and 3, according to the classification of Brooker et al., developed in four hips each. Two of the patients who received 550 centigray were symptomatic. Heterotopic ossification developed in nine (10 per cent) of the eighty-eight hips that were treated with 700 centigray; the lesion was grade 1 in six, grade 2 in one, and grade 3 in two. None of the patients who received 700 centigray were symptomatic. We concluded that single-dose irradiation consisting of 550 centigray is inadequate for the prevention of heterotopic ossification in high-risk patients after total hip arthroplasty. We recommend a dose of 700 centigray as effective prophylaxis for these patients.

RESUMO

The use of an optical local oscillator for coherent detection with a photodiode can significantly reduce the responsivity of the detector because of saturation effects. Consequently, local-oscillator shot-noise-limited operation of the detector may not be possible. This effect is analyzed and formulations are developed for the optimum optical local-oscillator power level and the resultant maximum possible signal-to-noise ratio in terms of parameters derived from the photodiode current versus the optical power response curve. An effective heterodyne responsivity that can be used as a part of the specifications when one is procuring photodiodes for use in coherent detection systems is defined.

Using an in-shoe plantar pressure sensor, the pressure under the first metatarsophalangeal (MTP) joint was measured in 20 asymptomatic control subjects during their normal gait. A 7-micron in-shoe pressure sensor recorded the pressure under the first MTP joint in the 20 volunteers while they were wearing their normal footwear (athletic footwear), a wooden postoperative shoe, a fiberglass short leg walking cast, and a postoperative shoe with a first MTP joint cutout orthotic device. The results showed both casting, and the postoperative shoe with the first MTP joint cutout orthotic device significantly reduced pressure under the first MTP joint compared with normal footwear, with an average decrease of 31% and 43%, respectively. However, the standard postoperative shoe did not significantly reduce first MTP pressure compared with normal footwear. Certain surgeries performed on the distal first metatarsal may benefit from a diminution of loading forces encountered during normal gait. The results of this study indicate that a reduction of first MTP pressures can best be accomplished with either a postoperative shoe with a cutout orthotic device or a short leg walking cast. A standard postoperative shoe showed inconsistent results and had no statistically significant effect on decreasing the pressure under the first MTP joint.

Forty-five patients with low back pain (LBP) of longer than six months' duration and for whom all conservative therapies failed were placed in a pantaloon cast for a two- to four-week trial period. Significant pain relief occurred in 31 (69%) of these patients. Of these 31, 23 patients were treated with spinal arthrodesis using lateral, intertransverse process fusions with iliac bone grafts. Before fusion, the painful lumbar segments were determined by facet analgesic blocks or the normal saline acceptance test. After an average follow-up period of 14 months, 17 (74%) had significant pain relief. The pantaloon cast may be an effective tool for identifying those chronic LBP patients who might benefit from spinal stabilization procedure.
