Los Grupos de Diagnóstico Relacionados (GDRs) constituyen un sistema de clasificación de pacientes diseñados con una lógica clínica y con un consumo de recursos similares. La utilización de las diferentes versiones de este sistema de clasificación de pacientes y la aparición de nuevos hace relevante la comparación, de tres de estas versiones HCFA-GDR, AP-GDR e IR-GDR, en relación con la explicación en la variabilidad de la estancia y su utilidad en el SNS (AU)

OBJECTIVE: Oral lichen planus (OLP) presents with various clinical forms that can be classified into 2 major types: reticular OLP and atrophic-erosive OLP. Our objective was to investigate immunologic differences between these 2 types. STUDY DESIGN: We investigated possible immunologic differences between 26 patients with reticular OLP and 26 patients with atrophic-erosive OLP. RESULTS: No differences were detected in serum Ig levels or complement levels. However, the mean proportions of CD4+CD45RO+ and DR+ lymphocytes were significantly higher in patients with atrophic-erosive OLP than in patients with reticular OLP, whereas the mean proportion of CD8+CD45RA+ lymphocytes was significantly lower in patients with atrophic-erosive OLP. CONCLUSION: These findings suggest that the 2 clinical types of OLP might have different immunopathogenic mechanisms.

We present a study of 166 extractions of impacted lower third molars, all vertical and all extracted by the same surgeon. Each tooth was classified according to the Pell-Gregory scales of position for the occlusal plane (scale A-C) and the ascending ram us of the mandible (scale 1-3). The extraction was subsequently rated as 'easy' or 'difficult'.Taking Pell-Gregory class C as a predictor of a 'difficult' extraction, specificity was 88% but sensitivity was low at 15%. Taking Pell-Gregory class 3 as an indicator of 'difficult', sensitivity was somewhat better (50%), but at the expense of specificity (62%). Likelihood ratios for the individual classes also indicated that the scales are of little value for predicting a difficult extraction.
