RESUMO

The experience of surgical treatment of 1532 patients with the complicated forms of diabetic foot syndrome (DFS) was analyzed. The original clinical classification of the complicated DFS was suggested. The classification considered the clinical form (symbol "C"), anatomic localization (symbol "Z") and etiology (symbol "E"). The classification allows to define surgical tactics depending on concrete complications of the DFS, frames conditions for the unification and uniform registration of the form and severity of the disease and volume of the surgical treatment.

RESUMO

Original clinical classification of a complicated diabetic foot syndrome (DFS), based on summarizing of experience, concerning surgical treatment of 1532 patients, suffering complicated DFS, was proposed, necessitating determination of clinical form (symbol C), anatomic localization (symbol Z) and etiological factor (symbol E) of the lower extremity tissues affection. The classification applied permits to choose a surgical tactics depending on presence of specific complication of DFS, and this creates conditions for unification and universal registration of form and severity of a complicated DFS, as well as the volume of a surgical aid

RESUMO

Results of pancreatic computer tomography (CT) were studied in 162 patients, suffering an acute pancreatitis (AP). There were conducted 293 investigations in total. Basing on the obtained results analysis there were established the optimal terms of the CT conduction, the indications were-substantiated for performance of the investigation in dynamics in patients, who were or were not operated on. After the first AP attack the CT performance was indicated on the 7-10th day, after reattack--on 2-3d day of the disease. There was substantiated the necessity to conduct CT at the end of the disease first month and in the absence of pathological changes in pancreas--reinvestigation at the end of the 6th month. In revealing pathological changes of the pancreatic gland structure, it is expedient to perform dynamical observation using ultrasonographic investigation. The standard protocol for monitoring of the pancreatic gland state was elaborated, taking into account the CT role and significance.

RESUMO

In 49 patients with an acute pancreatitis and 91 - with chronic pancreatitis were studied external and internal secretory pancreatic function in remote terms after the treatment. In 48 (52.7%) patients, suffering chronic pancreatitis, the investigated indexes were studied in dynamic before and after the operation. There was established the disorders of external secretory pancreatic function in an acute pancreatitis of severe stage and noninfected pancreonecrosis in patients, to whom direct operative pancreatic interventions were not performed. Indications for conduction of internal drainage of the pancreatic ducts, taking into account data of preoperative instrumental examination and initial activity of fecal elastase-1, were substantiated. The advantages of Beger and Frey operations, in comparison with resecting procedures, were proved because of lesser occurrence of the pancreatic functions disorders.

RESUMO

The influence of preparation Octrestatin, manufactured by Russian company "Pharm-Synthesis", on activity of amylase in the blood and urine in dynamic among 38 operated persons and 79 patients with an acute pancreatitis, who were not operated, was analyzed. There was established high efficacy of Octrestatin application as an inhibitor of pancreatic secretion, possible faster normalization of the amylase activity in the blood and urine, and prophylaxis of the wave-like course of the disease also.
