RESUMO

Background: Open Bankart repair plus inferior capsular shift (OBICS) and Latarjet procedure (LA) are considered appropriate treatment alternatives for high-performance athletes. The purpose of this study was to evaluate the functional outcomes and recurrence rate of each surgery. Our hypothesis: there were no differences between the two treatments. Methods: A prospective cohort study was conducted with n=90 contact athletes divided into two groups of 45 patients. One group was treated with OBICS, and the other one with LA. The mean follow-up period was 25 (24-32) months for the OBICS group and 26 (24-31) months for the LA group. Primary functional outcomes of each group were assessed at baseline, six months, one year, and two years after surgery. The functional outcomes were also compared between the groups. The evaluation tools used were the Western Ontario Shoulder Instability score (WOSI) and the American Shoulder and Elbow Surgeons scale (ASES). In addition, recurrent instability and range of motion (ROM) were also evaluated. Results: In each group, significant changes were found in the WOSI score and ASES scale from pre-op to postop. However, there were no significant differences between the functional outcomes of the groups at the final follow-up (P-values 0.73 and 0.19). Three dislocations and one subluxation (8.8%) were reported in the OBICS group, and three subluxations were reported in the LA group (6.6%), revealing no significant differences between the groups (P=0.37). Moreover, there were no significant differences between preoperative and postoperative ROM in each group or in terms of external rotation (ER) and ER in 90º abduction between the groups. Conclusion: No differences were found between OBICS and LA surgery. Both procedures can be indicated according to the surgeon's preference to reduce recurrence rates in contact athletes with recurrent anterior shoulder instability.

INTRODUCTION: Aneurysms and pseudoaneurysms of the arteries of the foot and ankle are rare clinical entities. In most cases occur within months or years of the initial trauma, complications in the aforementioned situations are very uncommon, however, they should be ruled out at medical check-ups. We will present a clinical case in which pseudoaneurysm of the Distal Peroneal Artery occurred as a result of a fracture- dislocation of the ankle. CLINICAL CASE: Patient of 60 years of age with trauma in right ankle, his radiographic study of the right ankle evidencing fracture- dislocation of the same. Surgical treatment is decided four days after the injury. In the postoperative course with edema, circulatory changes of abnormal form that merited imaging complement and arteriography was performed which indicates the presence of pseudoaneurysm of approximately 28 × 30 mm in distal peroneal artery meriting specific treatment with adequate control of symptoms. CONCLUSION: We believe that it is of the utmost importance the correct assessment and physical examination of patients undergoing ankle surgery in successive post-surgical controls to detect these types of complications early and treat them in time.

En la mayoría de los casos ocurren a los meses o años del trauma inicial, las complicaciones en las situaciones antes mencionadas son muy infrecuentes; sin embargo, deben descartarse en los controles médicos. Presentaremos un caso clínico en el cual se produjo seudoaneurisma de la arteria peronea distal como consecuencia de una luxofractura de tobillo. CASO CLÍNICO: Paciente de 60 años de edad con trauma en tobillo derecho, su estudio radiográfico de tobillo derecho evidenció luxofractura del mismo. Se decide tratamiento quirúrgico cuatro días después de la lesión. En el postoperatorio padeció edema, ambos circulatorios de forma anormal que ameritó complemento imagenológico y se realizó arteriografía, la cual indica presencia de seudoaneurisma de 28 × 30 mm aproximadamente en arteria peronea distal ameritando tratamiento específico con adecuado control de síntomas. CONCLUSIÓN: Creemos que es de suma importancia la correcta valoración y examen físico de los pacientes sometidos a cirugía de tobillo en los controles postquirúrgicos sucesivos para detectar de manera precoz este tipo de complicaciones y tratarlos a tiempo.

En la mayoría de los casos ocurren a los meses o años del trauma inicial, las complicaciones en las situaciones antes mencionadas son muy infrecuentes; sin embargo, deben descartarse en los controles médicos. Presentaremos un caso clínico en el cual se produjo seudoaneurisma de la arteria peronea distal como consecuencia de una luxofractura de tobillo. Caso clínico: Paciente de 60 años de edad con trauma en tobillo derecho, su estudio radiográfico de tobillo derecho evidenció luxofractura del mismo. Introducción: Aún se discuten ampliamente las indicaciones terapéuticas (tratamiento quirúrgico y no quirúrgico) para las lesiones del manguito rotador. El objetivo de este estudio fue evaluar la variabilidad entre cirujanos ortopédicos argentinos respecto de las indicaciones de tratamiento para diferentes lesiones del manguito rotador. Materiales y Métodos: Se diseñó un cuestionario basado en el estudio de Dunn y cols. para recopilar las opiniones de cirujanos ortopedistas sobre la decisión del tratamiento para lesiones del manguito rotador. El cuestionario se distribuyó a 2 grupos, cirujanos traumatólogos y especialistas en hombro, tomando las indicaciones dadas durante 2017, tanto de tratamiento quirúrgico como de no quirúrgico. El cuestionario constaba de 2 secciones: 4 casos clínicos hipotéticos y 11 preguntas sobre factores que pueden influir en la toma de decisiones por parte del cirujano. Resultados: Se enviaron 556 cuestionarios, 117 traumatólogos y 45 especialistas lo respondieron. Los especialistas comunicaron realizar, en su mayoría, reparaciones artroscópicas (60%) y los traumatólogos, reparaciones por técnica abierta con pequeños abordajes (49%) y método abierto convencional (22%). El porcentaje de fracaso estimado en pacientes sometidos a reparación del manguito rotador es del 20%, sin diferencias entre los profesionales (p = 0,42). Ambos grupos reportaron opiniones semejantes (p = 0,74) en relación con la contraindicación del uso de corticoides en posibles candidatos quirúrgicos. Conclusión: Se observó una variabilidad considerable en las indicaciones de tratamiento y manejo de estos pacientes en gran parte de los casos presentados de lesiones potencialmente controvertidas, esto dificulta establecer protocolos o consensuar guías que ayuden a estandarizar tratamientos de elección para estas lesiones. Introduction: Indications in the operative and nonoperative treatment of rotator cuff tears are widely discussed and not standardized. The aim of this study is to evaluate the variability among Argentinean orthopedic surgeons regarding the indications of treatment for different rotator cuff injuries. Materials and Methods: A questionnaire was developed based on the study carried out by Dunn et al. Our aim was to gather the opinions of Argentinean orthopedic surgeons on decision-making concerning the treatment of different rotator cuff lesions. The questionnaire was sent to 2 groups: general orthopedic surgeons and shoulder specialist surgeons, included according to the number of shoulders treated surgically and non-surgically in 2017. The questionnaire has two sections: 4 hypothetical clinical cases and 11 questions on factors that could influence treatment selection. Results: Out of 556 questionnaires sent, 162 were completed, 117 by general orthopedic surgeons and 45 by shoulder specialists. The specialists reported mostly arthroscopic repairs (60%), general orthopedic surgeons reported repairing these injuries mainly by mini-open or by standard open technique (49% and 22% respectively). The failure rate was considered to be 20%, not evidencing differences between professionals (P=0.42). There was also agreement in the contraindication of the use of corticosteroids before surgery (P=0.74). Conclusion: This study evidences significant variability in the management of these patients (for which we selected potentially controversial injuries), making it difficult to establish protocols or consensus guidelines to help standardize treatments for these injuries. Migrants in transit through Mexico to the USA are at risk of violence, including sexual violence, during the immigration process. This study sought to identify the socio-demographic factors, migration experiences and health conditions associated with the likelihood of sexual violence. A mixed methods study was conducted between 2012 and 2015. The quantitative phase of the work involved a non-random sample (n = 3539) of migrants who were the users of migrant shelters in Mexico. A probit regression model was used to identify the variables associated with the experience of sexual violence by participants. A total of 58 semi-structured interviews took place with migrants who had either experienced sexual violence or who were acquainted with the sexual violence experienced by other migrants. Of those who experienced any kind of violence, 5.7% reported having experienced sexual violence, with statistically significant differences by gender. According to male migrants, women in transit had the advantage of having an 'entry ticket [to the USA] between their legs'. The dynamics of undocumented transit migration provide multiple opportunities for gender-based inequality and sexual violence. We consider the major underreporting of sexual violence due to the stigma and normalisation of violence, in a social context marked by impunity.

Objetivo: Determinar si los resultados clínicos y radiográficos difieren después del tratamiento conservador y el tratamiento quirúrgico para las fracturas de la estiloides cubital asociadas a fracturas inestables del radio distal tratadas mediante placa volar bloqueada. Materiales y Métodos: Estudio multicéntrico, retrospectivo y descriptivo que incluye pacientes operados en cuatro instituciones, entre 2009 y 2012. Todos tenían fracturas de la estiloides cubital en el contexto de una fractura del radio distal. Las fracturas de la estiloides cubital fueron tratadas de forma conservadora (grupo I) o con cirugía (grupo II). Resultados: El seguimiento promedio fue de 56 meses. Se evaluó a 57 pacientes; grupo I (29 casos) y grupo II (28 casos). Los pacientes del grupo II tuvieron 2,76 veces (IC95% 1,086; 8,80) más posibilidades de lograr una consolidación que aquellos del grupo I. El puntaje DASH y el dolor, tanto en reposo como en actividad, no presentaron diferencias significativas (p = 0,276 y p = 0,877). La desviación cubital y la fuerza obtuvieron mejores resultados en el grupo I (p = 0,0194 y p = 0,024). Conclusiones: Aunque los pacientes con estabilización quirúrgica de la estiloides cubital tuvieron 2,76 más posibilidades de lograr la consolidación que aquellos del grupo I, no hubo diferencias significativas en la evaluación subjetiva (DASH y dolor) entre ambos grupos. Tampoco hubo diferencias significativas cuando se consideró el grado de compromiso de la estiloides cubital, pero la fuerza y la desviación cubital presentaron mejores resultados en el grupo sin fijación de la estiloides cubital. Objectives: To evaluate potential differences in clinical and radiological outcomes after surgical versus conservative management of ulnar styloid fractures associated with unstable distal radius fractures treated by locked volar plating. Materials and Methods: This was a multicenter, retrospective and descriptive study including surgical patients treated at four different institutions between 2009 and 2012 for ulnar styloid fractures associated with unstable distal radius fractures. Ulnar styloid fractures were treated conservatively in group I and surgically in group II. Results: The average follow-up was 56 months. The study included 57 patients divided into two groups (group I [29 cases] and group II [28 cases]). Patients in group II had 2.76 times (95% CI: 1.086; 8.80) more chances of achieving bone union than those in group I. DASH and pain scores, both at rest and during activity, did not show significant differences between the two groups (p = 0.276 and p = 0.877). Group I presented milder ulnar deviation and better strength (p = 0.0194 and p = 0.024). Conclusions: Although patients who underwent surgery for ulnar styloid fractures had 2.76 more chances of achieving bone union than those who received conservative management, there were no significant differences between both groups in subjective evaluations (DASH and pain scores) or when considering the degree of ulnar styloid involvement. However, the parameters of strength and ulnar deviation were better in the conservative management group. Level of Evidence: III

OBJECTIVES: The objectives of the study are to 1) estimate the burden of physical, sexual, and psychological violence among migrants in transit through Mexico to the US; and 2) examine the associations between experiencing violence and sociodemographic characteristics, migratory background, and health status in this vulnerable population. METHOD: A cross-sectional study combining qualitative and quantitative methods was carried out from 2009 to 2015 with a sample of 12,023 migrants in transit through Mexico to the US. Information on gender (male, female, and transsexual, transgender and transvestite -TTTs-); nationality; health status; migratory background; and experiences with violence was obtained. Fifty-eight migrants participated in in-depth interviews to explore any experiences of violence during their journey. A descriptive analysis was performed and a probit regression model was applied to analyze the factors associated with violence. Qualitative information was analyzed to understand experiences, meanings and responses to violence. RESULTS: The overall prevalence of suffering from any form of violence was 29.4%. Nearly 24% reported physical violence, 19.5% experienced psychological violence, and approximately 2% reported sexual violence. TTTs experienced a significantly greater burden of violence compared to men and women. Violence occurred more frequently among migrants from Central American (30.6%) and other countries (40.0%) than it did among Mexican migrants (20.5%). Experiences involving sexual, physical and psychological violence as well as theft and even kidnapping were described by interviewees. Migrants mistrust the police, migration authorities, and armed forces, and therefore commonly refrain from revealing their experiences. CONCLUSION: Migrants are subjected to a high level of violence while in transit to the US. Those traveling under irregular migratory conditions are targets of even greater violence, a condition exacerbated by gender inequality. Migrants transiting through Mexico from Central American and other countries undergo violence more frequently than do Mexican migrants. Protective measures are urgently needed to ensure the human rights of these populations.

A cocrystal, C15H22O3·C15H22O3, (I), obtained from Drimys winteri, is composed of two isomeric drimane sesquiterpene lactones, namely valdiviolide, (Ia), and 11-epivaldiviolide, (Ib), neither of which has been reported in the crystal form. Both diastereoisomers present three chiral centres at sites 5, 10 and 11, with an SSR sequence in (Ia) and an SSS sequence in (Ib). O-H···O hydrogen bonds bind molecules into chains running along [120] and the chains are in turn linked by π-π stacking interactions to define planar weakly interacting arrays parallel to (001).

RESUMO

The natural compound dendocarbin A, C15H22O3, is a sesquiterpene lactone isolated for the first time from Drimys winteri for var chilensis. The compound crystallizes in the orthorhombic space group P212121 and its X-ray crystal structure confirmed the S/R character of the chiral centres at C-5/C-10 and C-9/C-11, respectively. The α-OH group at C-11 was found to be involved in intermolecular hydrogen bonding, defining chains along the <100> 21 screw axis.

RESUMO

The natural compound hobartine {systematic name: (1R)-3-[(1S,5S)-(4,4,8-trimethylbicyclo[3.3.1]non-7-en-2-yl)methyl]-2,3-dihydro-1H-indole}, C20H26N2, (I), is an indole alkaloid isolated from Aristotelia chilensis as part of a study of secondary metabolites from Chilean flora. The colourless compound has a tetracyclic structure closely related to the strongly coloured polymorphic structures discussed in Paz et al. [Acta Cryst. (2013), C69, 1509-1512] and Watson et al. [Acta Cryst. (1989), C45, 1322-1324]. The main differences reside in the absence of a keto group in (I) compared with the previous structures, as well as an endo double bond in (I) contrasting with the exo double bond found in the previous structures. The supramolecular structure of (I) in strongly related to the twofold screw axis, around which isolated chains build up, internally linked by an N-H···N hydrogen bond which is the only significant intermolecular interaction present in the structure.

OBJECTIVE: Document the transnational utilization of health resources and services by Mexican immigrants in the United States. MATERIALS AND METHODS: Between December 2009-February 2011, Interviews and focus groups were conducted in California and four states of México. Data were collected from 135 individuals, including return migrants, allopathic physicians and traditional healers. RESULTS: Faced with obstacles to accessing US health care and some health services within the Mexican system, many immigrants within the US make use of Mexican health resources and services, either from a distance or during visits to Mexico. These resources and services include allopathic medicine, traditional medicine, and home remedies and medicines. CONCLUSIONS: The legal status of immigrants and their access to health insurance in the US are related to whether their transnational use of Mexican health resources and services is formal or informal; immigrants who are undocumented and without health insurance are the most vulnerable.

Objetivo. Documentar la utilización transnacional de recursos y servicios de salud de los migrantes mexicanos en Estados Unidos de América (EUA). Material y métodos. Entre diciembre de 2009 y febrero de 2011, a través de entrevistas y grupos focales, se platicó en cuatro estados mexicanos y en California con 135 personas: migrantes de retorno, médicos alópatas y médicos tradicionales. Resultados. Ante los obstáculos para acceder al sistema de salud de EUA y ciertas facilidades del sistema de salud mexicano, muchos migrantes utilizan en EUA, a la distancia y en sus visitas a México, recursos y servicios de salud mexicanos: médicos alópatas y tradicionales, medicamentos y remedios caseros. Conclusiones. La situación legal de los migrantes y su acceso a un seguro de salud en EUA están relacionados con una utilización transnacional más formal o informal de recursos y servicios de salud mexicanos, siendo los migrantes indocumentados y sin seguro los más vulnerables.

Introducción: la medicina folclórica ecuatoriana, utiliza las cocciones de las hojas de Vernonanthura pathens (Kunth H. Rob), para combatir entre otras, paludismo, dolores pre y posparto, estomacales, erupciones de piel, diarreas y parásitos. Objetivos: examinar la presencia de actividad antifúngica en fracciones obtenidas de un extracto metanólico de las hojas de V. patens. Métodos: el extracto metanólico de hojas se fraccionó por columna cromatográfica empleando los siguientes sistemas de disolventes: hexano, hexano/acetato de etilo, acetato de etilo y acetato de etilo/metanol. La actividad antifúngica se midió mediante la técnica de difusión en agar con medio (potato dextrosa agar) PDA, en pocillos de 5 mm de diámetro, adicionando 20 µL de las fracciones en concentraciones de 100 y 200 µg/mL disueltas en dimetilsulfóxido. La fracción activa fue analizada para su identificación estructural por cromatografía gaseosa-espectrometría de masas. Resultados: de las fracciones obtenidas solo presentó actividad antifúngica la fracción de hexano al 100 por ciento, con porcentajes de inhibición del 57,6 y 80,2 por ciento frente a Penicillium notatum, y 64,8 y 81,5 porciento frente a Fusaryum oxysporum, a los quince días de incubación. Se propusieron las estructuras de 29 compuestos como constituyentes de la fracción hexánica, los que en su mayoría, son hidrocarburos. Conclusiones: se comprobó la presencia de actividad antifúngica en la fracción de hexano al 100 por ciento, lo que puede incrementar el arsenal de usos medicinales de la planta estudiada y enriquecer la medicina folclórica ecuatoriana


Introduction: the Ecuadorian folk medicine uses the coction from Vernonanthura pathens (Rob H. Kunth) leaves to treat, malaria, pain before and after childbirth, stomachache, skin rashes, diarrheas and parasitism. Objectives: to examine the presence of antifungal activity in fractions obtained from a methanol extract from V. patens leaves. Methods: the methanol extract from the leaves was fractionated by column chromatography using the following solvent systems: hexane, hexane/ethyl acetate, ethyl acetate and ethyl acetate/methanol. Antifungal activity was measured by the agar diffusion technique with PDA medium in 5 mm wells, by adding 20 µL fractions at concentrations of 100 and 200 µg/mL dissolved in dimethylsulfoxide. The active fraction was analyzed for structural identification by gas chromatography-mass spectrometry. Results: of the fractions obtained, only the 100 percent hexane fraction showed antifungal activity, since its inhibitory percentages were 57.6 and 80.2 percent against Penicillium notatum and 64.8 and 81.5 percent vs. Fusaryum oxysporum on the fifteenth day of incubation. The structures of 29 compounds were suggested to be the constituents of the hexane fraction, most of them hydrocarbons. Conclusions: the presence of antifungal activity in the 100 percent hexane fraction was proved, which can extend the medicinal uses of the studied plant and to enrich the Ecuadorian folk medicine

Introducción: la medicina folclórica ecuatoriana, utiliza las cocciones de las hojas de Vernonanthura pathens (Kunth H. Rob), para combatir entre otras, paludismo, dolores pre y posparto, estomacales, erupciones de piel, diarreas y parásitos. Objetivos: examinar la presencia de actividad antifúngica en fracciones obtenidas de un extracto metanólico de las hojas de V. patens. Métodos: el extracto metanólico de hojas se fraccionó por columna cromatográfica empleando los siguientes sistemas de disolventes: hexano, hexano/acetato de etilo, acetato de etilo y acetato de etilo/metanol. La actividad antifúngica se midió mediante la técnica de difusión en agar con medio (potato dextrosa agar) PDA, en pocillos de 5 mm de diámetro, adicionando 20 µL de las fracciones en concentraciones de 100 y 200 µg/mL disueltas en dimetilsulfóxido. La fracción activa fue analizada para su identificación estructural por cromatografía gaseosa-espectrometría de masas.
