INTRODUCTION: Osteosarcomas of the jaw (OSJs) are rare tumors with distinct characteristics from osteosarcomas affecting other bones. This study aims to analyze the clinical, pathological, and therapeutic characteristics of OSJs. METHODS: A retrospective, descriptive cross-sectional study including patients diagnosed with OSJ registered at the "La Paz" University Hospital, Madrid, was performed. RESULTS: Data of eight patients with a diagnosis of OSJ were obtained during the study period of 22 years (2002-2024). The mean age of the patients was 41 years. The distribution was 1:1 between the maxilla and mandible. Painful inflammation was the most frequent clinical manifestation. Conventional osteoblastic osteosarcoma was the most predominant histological type. Survival rate at 5 years was 50%, which decreased to 25% at 10 years. CONCLUSIONS: OSJs differ from conventional osteosarcomas of long tubular bones. Surgery continues to be the mainstay of treatment. However, more studies are needed through which more standardized protocols can be proposed for adjuvant therapeutic management.

The objectives of the present research were to analyze adherence to the Mediterranean diet (AMD), to observe which variables most affect AMD, and to analyze whether AMD affects physical fitness and anthropometric parameters in high-level adolescent athletes. A total of 96 adolescent athletes in the under-16, under-18, and under-20 categories selected by the Athletics Federation of the Region of Murcia, of whom 47 were male (age = 18.31 ± 2.31 years old) and 49 female (age = 17.27 ± 1.44 years old), participated in this study. They completed the KIDMED questionnaire to discover their AMD, as well as an anthropometric and physical condition assessment. Results: The findings show that 61.45% had an excellent degree of AMD, 31.25% a moderate one, and 7.30% a poor one. The parameters that most conditioned AMD were the consumption of fruit, vegetables, nuts, legumes, fish, breakfast cereals, and dairy products (p = 0.011-0.000). AMD did not show significant differences in anthropometric characteristics and physical fitness (p = 0.057-0.996). Conclusions: The majority of high-level adolescent athletes have a moderate or excellent AMD. The degree of AMD seems to have no influence on physical fitness and anthropometric parameters in this population.

BACKGROUND AND AIM: Severe hemodynamic fluctuations during dental treatment can trigger highly undesirable physical reactions. A study was made to determine whether the administration of propofol and sevoflurane contributes to the stabilization of hemodynamic parameters during dental treatment in pediatric patients versus the use of local anesthesia alone. MATERIALS AND METHODS: Forty pediatric patients needing dental treatment were assigned to either general anesthesia with local anesthesia (study group [SG]) or local anesthesia alone (control group [CG]). Two percent sevoflurane in oxygen (100% oxygen, 5 L/min) and continuous propofol infusion (target-controlled infusion [TCI], 2 µg/mL) were used as general anesthesia agents in SG; and 2% lidocaine with 1:80,000 adrenaline was used as local anesthesia in both groups. Heart rate, blood pressure and oxygen saturation were measured before starting dental treatment (baseline) and every 10 min during dental treatment. RESULTS: Blood pressure (p < .001), heart rate (p = .021) and oxygen saturation (p = .007) decreased substantially after the administration of general anesthesia. The levels of these parameters subsequently remained low and then recovered at the end of the procedure. On the other hand, the oxygen saturation values remained closer to baseline in SG versus CG. In contrast, the hemodynamic parameters experienced lesser fluctuations in CG than in SG. CONCLUSIONS: General anesthesia affords more favorable cardiovascular parameters during the entire dental treatment in comparison to local anesthesia alone (blood pressure and heart rate decrease significantly and oxygen saturation proves more stable and with values closer to baseline), and allows dental treatment to be performed on healthy, lacking cooperative ability children who otherwise could not be treated with local anesthesia alone. No side effects were observed in either group.

Kinanthropometric and physical fitness variables have been habitually used for the detection of sports talent in adolescent populations. Considering these variables, players who obtained better scores than their peers in kinanthropometric and physical fitness variables have been traditionally selected for promotion in their teams, without taking into account the possible effect of the maturation process. The aim of the present study was to analyse the differences between adolescent volleyball players in terms of success assessment and the age category, as well as to identify variables that may predict success in volleyball. A total of 101 volleyball players in the U14 age category (28 boys and 73 girls; age = 13.28 ± 0.49 years) and 99 players in the U16 age category (20 boys and 79 girls; age = 15.24 ± 0.59 years) participated in the study. Significant differences were observed in biological maturation in male groups (p < 0.001-0.041), kinanthropometric variables related to bone structure and muscle mass in male groups (p < 0.001-0.048), in variables related to adipose tissue in the U16 female group (p = 0.012-0.032) and in physical fitness tests related to muscle strength and power (p < 0.001-0.049), indicating that more successful (MS) players showed a more advanced biological maturation process, higher values of kinanthropometric variables related to bone structure and muscle mass, and lower values in variables related to adipose tissue, as well as higher levels of physical fitness. The difference in biological maturation and its relationship with structural and muscular anthropometric variables in boys, and physical fitness tests related to muscle strength and power in both boys and girls, are of great importance in the selection process of sports talent in volleyball. These findings should be considered by clubs and coaches, who could be discarding players who could improve their sports performance in later stages when their maturational development is equalised.

BACKGROUND: We aimed to describe infective endocarditis cases from noncardiac surgery centers, as current knowledge on infective endocarditis is derived mostly from cardiac surgery hospitals. METHODS: An observational retrospective study (2009-2018) was conducted in 9 noncardiac surgery hospitals in Central Catalonia. All adult patients diagnosed with definitive infective endocarditis were included. Transferred and nontransferred cohorts were compared, and a logistic regression model was used to ascertain the prognostic factors. RESULTS: Overall, 502 infective endocarditis episodes were included: 183 (36.5%) were transferred to the cardiac surgery center, whereas 319 were not, with (18.7%) and without (45%) surgical indications. Cardiac surgery was performed in 83% of transferred patients. In-hospital (14% vs 23%) and 1-year (20% vs 35%) mortality rates were significantly lower in transferred patients (P < .001). Among the patients not undergoing cardiac surgery despite an indication, 55 (54%) died within 1 year. The multivariate analysis identified the following independent predictive factors for in-hospital mortality: Staphylococcus aureus infective endocarditis (odds ratio: 1.93 [1.08, 3.47]), heart failure (odds ratio: 3.87 [2.28, 6.57]), central nervous system embolism (odds ratio: 2.95 [1.41, 5.14]), and Charlson score (odds ratio: 1.19 [1.09, 1.30]), whereas community acquisition (odds ratio: 0.52 [0.29, 0.93]), cardiac surgery (odds ratio: 0.42 [0.20, 0.87]), but not transfer (odds ratio: 1.23 [0.84, 3.95]) were identified as protective factors. One-year mortality was associated with S. aureus infective endocarditis (odds ratio: 1.82 [1.04, 3.18]), heart failure (odds ratio: 3.74 [2.27, 6.16]), and Charlson score (odds ratio: 1.23 [1.13, 1.33]), whereas cardiac surgery (odds ratio: 0.41 [0.21, 0.79]) was identified as a protective factor. CONCLUSION: Patients not transferred to a referral cardiac surgery center have a worse prognosis compared to those ultimately transferred, as cardiac surgery is associated with lower mortality rates.

In recent years, there has been an increase in knowledge about the barriers experienced by people with disabilities in the education system or sports. However, no studies have analyzed the barriers for those who try to succeed in both disciplines (dual career). The purpose of this study was to examine the barriers faced by student-athletes with/out disability to a dual career combining studies and sport. Two groups were involved in the study (n = 162): student-athletes with disabilities (n = 79) and student-athletes without disabilities (n = 83). Data collected included: (a) socio-demographic aspects; and (b) barriers towards achieving a good balance between sport and academics during the dual career, through the "Perceptions of dual career student-athletes" (ESTPORT) questionnaire. The results showed that student-athletes with disabilities were more likely to perceive in a greater extent the barriers, the university is far from my home (p = 0.007) and the university is far from my training site (p = 0.006), I find myself unable to balance study and training time (p = 0.030), I have to take care of my family (p<0.001), and my current job does not allow me to study enough (p<0.001). The MANOVA analysis showed that the factors gender, competitive level, and employment status had an influence on the perception of some barriers between groups. In conclusion, student-athletes with disabilities perceived barriers more strongly than those without disabilities, and measures are needed to ensure their inclusion in the education system.

Actualmente las infecciones de transmisión sexual (ITS) son un problema de salud pública importante debido a su elevada prevalencia y a que precisan de un diagnóstico y un tratamiento precoces para evitar complicaciones. En los últimos años se está observando un aumento exponencial de los casos de infecciones causadas por Chlamydia trachomatis y gonococo en población menor de 25años. También se ha detectado un aumento de la incidencia de sífilis y de hepatitisC (VHC), sobre todo en hombres que tienen sexo con hombres (HSH). El herpes genital sigue siendo la segunda ITS más frecuente en el mundo, por detrás del condiloma acuminado, y la primera causa de úlcera genital en España en la población sexualmente activa. Durante el año 2020 se observó un descenso de los casos notificados de VIH, pero casi la mitad de estos nuevos casos presentaban un diagnóstico tardío (<350CD4cel/μl). Las guías actuales recomiendan ofrecer anualmente el cribado de ITS a las poblaciones de riesgo y más frecuentemente en función de dicho riesgo. Las ITS pueden presentarse, entre otras, en forma de síndromes, como son el síndrome secretor (uretritis, proctitis, cervicitis) o el síndrome ulcerado (úlceras). Las ITS que pueden cursar con síndrome secretor están causadas principalmente por Neisseria gonorrhoeae y C.trachomatis, que infectan conjuntamente hasta en el 40% de los casos, y que producen uretritis, cervicitis o proctitis según el lugar en que se localizan. El gonococo tiene un periodo de incubación de 2 a 7días y la clamidia de 2 a 6semanas, y se diagnostican a través de PCR y/o cultivo (este último solo válido para gonococo) de las muestras recogidas según prácticas sexuales...(AU)

Empirical treatment to cover both germs will be accomplished with ceftriaxone, 1g single intramuscular dose plus doxycycline 100mg every 12h orally for 7days, or azithromycin 1g single dose orally (we will use azithromycin only if we suspect a poor compliance with treatment, difficulty in going to the control or in pregnancy). Likewise, whenever we diagnose an STI firstly, we must offer advice and health education in order to promote the adoption of safe sexual behaviours and the correct use of barrier methods. Secondly, we must also screen for other STIs (HIV, syphilis, hepatitisB, and hepatitisA andC depending on the risk), offer HBV and HAV vaccination if it is appropriate, and finally study and treat all sexual partners from the previous 3months.

Background: Very limited data are available on the hemodynamic and ventilatory changes during sedation and general anesthesia using bispectral index (BIS) monitoring in intellectually disabled children. The purpose was to determine the hemodynamic and ventilatory changes after propofol and sevoflurane administration in children with special needs (CSN) versus healthy children (HC) during dental treatment. Material and Methods: Forty pediatric patients needing dental treatment were allocated into two groups: children without systemic disease (healthy children [HC]) and mentally disabled children (children with special needs [CSN]). Sevoflurane in oxygen (100% oxygen, 5 l/min) and continuous propofol infusion (target-controlled infusion [TCI], 2 µg/ml) were used as sedative agents, and 2% lidocaine with 1:80,000 adrenaline was used as local anesthesia in both groups. Heart rate (HR), oxygen saturation (SaO2), respiratory rate (RR), exhaled carbon dioxide (CO2), blood pressure (BP) and bispectral monitoring (BIS) values were recorded during the entire dental treatment procedure. Results: A statistically significant decrease in systolic BP, diastolic BP and RR was observed, with no significant differences between the healthy and disabled groups. In contrast, the HR and BIS values were lower in the CSN group than in the healthy patients (p ≤ 0.05). Conclusions: Patients with special needs had lower HR and BIS values than healthy patients, while BP, SaO2 and exhaled CO2 showed similar results in both groups. INTRODUCTION: Oral lichen planus is a chronic autoimmune inflammatory disease of unknown origin, characterized by various clinical forms of which the atrophic-erosive causes patients the greatest symptomatology. For this reason, there are different treatments that improve the associated signs and symptoms. One of these therapies is photobiomodulation (PBM), which, although new, has a high level of acceptance in dentistry based on evidence. However, there are inconsistent results in its application against lichen planus. The aim of this review was to evaluate the effect of photobiomodulation and its effectiveness as a therapeutic alternative for atrophic-erosive lesions. MATERIAL AND METHODS: The databases PubMed, Google Scholar and Cochrane Library were searched to identify studies investigating the photobiomodulation treatment in atrophic-erosive lesions of oral lichen planus. A total of 294 articles were identified, published between 2017 and 2022, and then evaluated; 7 articles that met all the inclusion criteria were included in this study. RESULTS: The type of laser light source used in PBM was the diode laser (four cases), the Nd-YAG laser at the same wavelength of 1064 nm (two cases) and the He-Ne laser (one case). The minimum and maximum wavelengths used were 630 nm and 1064 nm, respectively. Most studies used lesions treated with topical corticosteroids as a control group. The follow-up times of the studies were highly variable. CONCLUSIONS: Photobiomodulation is a treatment that competently combats oral lichen planus lesions by improving signs and symptoms, with no known adverse reactions so far, which makes it more beneficial compared to more conventional therapies, such as corticosteroids, for which side effects have been found.

INTRODUCTION: This paper examines the convergence of the opinions of European higher education institution (HEI) experts on possible implementation of dual career policies and provision areas at university level. METHODS: An online 32-item questionnaire encompassing 26 dual career aspects collected the opinion of European HEI experts as the last phase of a Delphi method preceded by i) focus groups with student-athletes who aimed to identify needs for dual careers and ii) a workshop with 21 dual career experts to generate the statements to be included in the survey. Seventy-one HEI experts from 12 EU member states participated in the survey, rating each dual career aspect identified in previous phases. RESULTS: The relative position of each aspect has been plotted based on relevance (x-axis) and feasibility (y-axis). The Quadrant IV of the resulting scatterplots identified the following nine aspects rated as highly relevant and highly feasible for implementation: tutorship/mentorship, psychological support, programmes based on integration of academic departments and sports services, and adaptable programmes to the needs of each student-athletes (assistance/tutorship area), individual study plans and distance learning (curricula requirements area), publicity for student-athletes and initiatives for increasing the awareness of student-athletes and knowledge of dual career issues (social support area), and access to educational facilities (logistic support area). DISCUSSION AND CONCLUSIONS: The HEI experts' views represent a coherent and useful starting point to develop a deep understanding of the considered 26 aspects founded on a phenomenological lifeworld-led approach and emphasizes the need for a minimum standard for dual career policies and provisions.

Atherosclerotic cardiovascular diseases (ASCVD) are the leading cause of morbidity and mortality in Western societies. Statins are the first-choice therapy for dislipidemias and are considered the cornerstone of ASCVD. Statin-associated muscle symptoms are the main reason for dropout of this treatment. There is an urgent need to identify new biomarkers with discriminative precision for diagnosing intolerance to statins (SI) in patients. MicroRNAs (miRNAs) have emerged as evolutionarily conserved molecules that serve as reliable biomarkers and regulators of multiple cellular events in cardiovascular diseases. In the current study, we evaluated plasma miRNAs as potential biomarkers to discriminate between the SI vs. non-statin intolerant (NSI) population. It is a multicenter, prospective, case-control study. A total of 179 differentially expressed circulating miRNAs were screened in two cardiovascular risk patient cohorts (high and very high risk): (i) NSI (n = 10); (ii) SI (n = 10). Ten miRNAs were identified as being overexpressed in plasma and validated in the plasma of NSI (n = 45) and SI (n = 39). Let-7c-5p, let-7d-5p, let-7f-5p, miR-376a-3p and miR-376c-3p were overexpressed in the plasma of SI patients. The receiver operating characteristic curve analysis supported the discriminative potential of the diagnosis. We propose a three-miRNA predictive fingerprint (let-7f, miR-376a-3p and miR-376c-3p) and several clinical variables (non-HDLc and years of dyslipidemia) for SI discrimination; this model achieves sensitivity, specificity and area under the receiver operating characteristic curve (AUC) of 83.67%, 88.57 and 89.10, respectively. In clinical practice, this set of miRNAs combined with clinical variables may discriminate between SI vs. NSI subjects. This multiparametric model may arise as a potential diagnostic biomarker with clinical value.

The dual career allows elite athletes to attain their maximum competitive and academic performance, but the COVID-19 pandemic hindered their development and changed their perception of the importance given to the sporting and educational environment. For this reason, the aim of the present study was to determine the differences in the motivations and perceived barriers, the importance given to academic qualifications, and the perception of the dual career from a multifactorial perspective, of elite athletes according to sex, type of sport practiced, job performance, time of sports career, type of athlete, and type of scholarship received. A total of 100 student-athletes participated in the research study by completing the "Perceptions of dual career student-athletes" questionnaire. The results showed that athletes from individual modalities (p = 0.012) and those who did not receive any scholarships described more barriers (p < 0.001). In addition, women studied more because they enjoyed it (p = 0.007); athletes from individual modalities studied to work later (p = 0.008); athletes who do not work perceived a greater influence between study and sports performance (p = 0.029); at the beginning and at the best stage of their sports career, a greater influence of academics on performance was perceived (p = 0.016); and athletes who considered themselves professionals, and athletes who did not receive any scholarships (p = 0.025), reported that the conciliation between sports and academic life was difficult (p = 0.034). The results obtained point to the importance of dual career scholarships for student-athletes, as well as the need for the programs implemented for these athletes to consider sex, sport modality or type of scholarship granted.

Background: The identification of sport talent among adolescent athletes is a topic that in recent years has been a major focus of interest for both the scientific community and sport managers. Both anthropometry and physical performance through fitness tests have demonstrated to be key elements. Biological maturation, due to its influence on anthropometric variables and physical fitness, has also been studied in relation to sport talent identification. Objective: To analyse differences according to biological maturation status in anthropometric characteristics and performance in physical fitness tests, and to determine which variables predict better performance in physical fitness tests in adolescent volleyball players. Methods: A cross-sectional design was followed to collect the data. A total of 48 male sub-elite volleyball players (14.17 ± 0.73 years) completed a socio-demographic and sports ad hoc questionnaire. Anthropometric variables were measured following the guidelines of the International Society for the Advancement in Kinanthropometry (ISAK) including four basic measurements (body mass, height, sitting height and arm span); eight skinfolds (triceps, biceps, subscapular, iliac crest, supraspinale, abdominal, thigh and calf); four girths (arm relaxed, flexed and tensed arm, middle thigh and calf); five breadths (biacromial, biileocrestal, humerus, femur and bi-styloid); three lengths (acromiale-radiale, radiale-stylion and stylion-medio dactilion); and a height (ilioespinale). Physical fitness was assessed, including the sit-and-reach, back scratch, long jump, medicine ball throw, counter movement jump (CMJ), 20 meters sprint, and agility tests. Furthermore, maturity offset and age at peak height velocity (APHV) was calculated. Results: Significant differences were found in the body mass (Mean Difference, MD = 20.86-30.75), height (MD = 11.72-19.09), sitting height (MD = 4.27-10.27), arm span (MD = 12.91-20.78), body mass index (MD = 3.72-5.63), upper limb length (MD = 7.76), corrected muscle girths (MD = 2.06-9.31), ∑6 and 8 skinfolds (MD = 3.67-50.21) fat mass and percentage (MD = 0.30-11.58), muscle (MD = 4.13-10.64) and bone mass (MD = 1.61-3.54) (p < 0.001-0.030), showing higher values the early maturers. In the physical fitness tests, significant differences were observed in the medicine ball throw (MD = 1.26-2.80) and in CMJ power (MD = 156.71-379.85) (p < 0.001). Regression models identified fat mass percentage predicted worse physical test performance (p < 0.001), while age, maturation offset, muscle and bone variables were predictors of better physical performance (p < 0.001). Conclusions: Significant differences based upon the stages of biological maturation were found in the anthropometric and physical condition variables in favor of the players whose maturation process was more advanced, with the variables related to fat and adipose, muscle and bone development conditioning their performance in the physical condition tests.

Previous studies have found that student-athletes (S-As) have difficulties in achieving dual career (DC) success. However, no studies have analysed the opinion of the S-As on the functioning of DC with a qualitative methodology. The aim of the present work was to collect the opinions of elite university S-As in relation to DC policy adopted by their academic institutions in different European countries. In total, 77 athletes (F = 35, M = 42; age range: 20-25 years) participated in 15 national face-to-face focus groups in five different countries, to discuss aspects that higher education institutes should implement in relation to: 1) the athletes' needs; 2) assistance/tutorship: 2) curricula requirements; 3) financial support; 4) logistic support; 5) social support; and 6) dual career policies. Fifty of the athletes competed in individual sports and twenty-seven team sports. Of them, 57 was enrolled at undergraduate, 17 was enrolled in a master and 3 in a PhD. The athletes were presented with 13 open-ended questions one by one, and were ensured freedom to interact. All the discussions were recorded. After this, a general discussion took place in which the participants identified and agreed on a final list of statements from their focus group deemed to be relevant to DC athletes as university students. Then, at a consensus meeting, the findings were combined, repetitions were eliminated, and fragmented statements were condensed into broader ones. A final list of 31 statements, organized in six related content units, were identified in relation to the athletes' needs (n = 5), assistance/tutorship (n = 5), curricula requirements (n = 4), financial support (n = 4), logistic support (n = 4), social support (n = 6), and DC policies (n = 3), respectively. In conclusion, this cross-national qualitative research study synthesized the S-As views about their needs and the most relevant DC policies and provisions that higher education institutes should provide to ensure them with positive academic experiences towards the achievement of a degree.

BACKGROUND: Differences in kinanthropometric and physical fitness performance between boys and girls usually start during adolescence, as a result of the changes in the hormonal environment that occur with the advance of age and biological maturation; Methods: A total of 96 1st Regional Division players adolescent volleyball players, 48 males, (age = 14.17 ± 1.00 years-old) and 48 females (age = 14.41 ± 1.21 years-old) underwent a kinanthropometric assessment, were asked to perform different physical fitness test and to complete a questionnaire. Chronological age, maturity offset, age at peak height velocity (APHV), and birth quartile were calculated; Results: Statistical differences were observed between male and female players in the APHV (p < 0.001). Male players showed higher values in the bone and muscle-related variables (p < 0.001-0.040), as well as in the strength and power production-related physical tests (p < 0.001-0.012), while the female showed higher values in the fat-related variables (p = 0.003-0.013), and performed better in the flexibility tests. Age, maturity offset, and birth quartile showed to have statistical influence in the differences found between sex groups; Conclusions: There is a clear influence of age and biological maturation on the differences found between sexes in adolescent volleyball players that could be taken into account regarding grouping in early stages.

Spain is one of the many countries highly affected by the COVID-19 crisis, establishing very restrictive measures with a complete lockdown for more than 3 months. This situation forced the complete closure of sport practice and national or international competitions, leading to a negative impact on physical and psychological health of high-performance athletes. Therefore, the objectives of this study were (a) to determine the effects of the COVID-19 health crisis on Spanish high-performance athletes in terms of sports practice, life quality, and emotional state and (b) to identify the profile with the greatest difficulties during and after the lockdown. A sample of 130 high-performance athletes aged between 18 and 34 years (67 women and 63 men) participated in this study (83.1% achieved a medal in National-International elite competitions; 86.9% were considered student-athletes). Measures included socio-demographic data through a 5-dimension ad hoc survey: physical activity and exercise using an adapted version from the International Physical Activity Questionnaire (IPAQ); health status and limitations using an adapted version of SF-12 Health Questionnaire; Perceived stress (Short-PSS); and Mood States (29-item POMS). All participants have shown a significant decrease pre-post-lockdown in both health and performance perception, especially in women, individual athletes, medalists, and student-athletes. Strong limitations of training, attention, and motivation as well as a moderate negative emotional state during lockdown were reported, in women, individual athletes, medalists, and student-athletes. Even with an improved emotional state and energy level in the post-lockdown period, moderate-to-high stress scores were reported by women and medalists. Our findings highlight the importance of paying attention to the physical and psychological health of elite athletes on three profiles: team athletes (due to social distance), student-athletes (dual-career issues), and women athletes (prevalence of implicit gender inequalities in sport).
