[Local hemostasis during the carotid endarterectomy under condition of antithrombocytic therapy].

Comparative studying of preparation TachoComb application efficacy and the convenient tampon procedure for stopping of bleeding from the suture line, fixing the expanding flap, made of polytetrafluoroethylene, in arteriotomy zone was studied in the patients, operated for carotid arteries stenosis and obtaining Plavix preparation protractedly in 75 mg dosage. The preparation TachoComb application have had promoted the operative time reduction as well as the bleeding duration and the blood loss volume.

[Application of preparation endotelon in complex treatment of chronic arterial insufficiency of lower extremities]. / Primenenie preparata éndotelon v kompleksnom lechyenii khronicheskoi arterial'noi nedostatochnosti nizhnikh konechnostei.

The results of treatment of 43 patients with chronic ischemia of extremities with application of preparation endotelon were summarized. Stable positive effect, preserved during 2-4 month was noted in 34 patients. Lowering of regional venous pressure, at average for the first week by (1.3 +/- 0.31) kPa, or (10 +/- 2.3) mm Hg, was observed after the first course of treatment conduction.

[Yucaipa-like viruses isolated in Kazakhstan in 1987-1989]. / Izuchenie iukeipapodobnykh virusov, vydelennykh v Kazakhstane v 1987--1989 gg.

From domestic birds 13 strains of avian paramyxoviruses, serotype 1, and 14 strains of serotype 2 were isolated. Avian paramyxoviruses, serotype 2, differ antigenically and biologically from each other and from the prototype variant chicken/Yucaipa/California/56. The virus was also detected experimentally in birds having contact with the infected specimens. Examinations of avian blood sera revealed wide dissemination of viruses related to the chicken/Yucaipa/56 strain in domestic bird breeding farms (43.6% to 50.0% of positive birds). The detected variability of the antigenic structure of the isolates attests to the potential emergence of a pathogenic variant.

In the experiment, a sluggish wound of an extremity against the background of chronic ischemia was modelled in white rats. Then, a scapular muscle was transferred onto a wound defect. Free transfer of the latissimus dorsi flap (LDF) aimed at closure of the extensive wound defect and revascularization of the tissue ischemia area was performed in 29 patients: for high-voltage electric burn (12 cases), frostbite (5), radiation ulcer (4) and obliterative diseases of the lower extremities (8). Complications--partial, or total necrosis of a flap--occurred in 11 patients. Risk of free LDF transfer is justified because of absence of the other methods for prevention of amputation of the extremities.

[Surgical treatment of combined occlusions of the aorto-femoral and femoro-popliteal vascular segments in patients with severe ischemia of the lower extremities]. / Khirurgicheskoe lechenie sochetannoi okkliuzii sosudov aorto- bedrennogo i bedrenno-podkolennogo segmentov u bol'nykh s tiazheloi ishemiei nizhnikh konechnostei.

The experience with surgical treatment of 107 patients with severe ischemia of the lower extremities, including 52 with associated lesion of the vessels of the aorto-femoral and femoro-popliteal segments, is summarized. A complex of preoperative and intraoperative assessment of regional hemodynamics was used. This permitted to avoid the amputation of an extremity in 86.6% of the patients.
