La prevalencia de hipolactasia tipo adulto está influenciada por la etnicidad y la geografía. Los genotipos CC y GG, de los SNPs C/T-13910 y G/A-22018, respectivamente determinan hipolactasia en ciertos grupos étnicos y países del mundo. El objetivo de este estudio fue analizar estos SNPs en muestras de los tres grupos étnicos que habitan el Caribe Colombiano. Trescientos sesenta y un sujetos, agrupados como afrodescendientes, indígenas y mestizos, fueron genotipificados usando PCR/RFLP. El análisis genético se hizo mediante Arlequin 3.11 y las frecuencias genotípicas fueron comparadas con Statgraphics Centurion XVI. Solamente el SNP C/T-13910 mostró equilibrio de Hardy- Weinberg y no hubo desequilibrio de ligamiento entre los SNPs estudiados. La frecuencia del genotipo CC-13910 fue 90% en afrodescendientes, 95% en indígenas y 80% en mestizos. En indígenas la frecuencia de GG-22018 fue 23% pero dicho genotipo no se halló en afrodescendientes y mestizos. El genotipo AA-22018 no se halló en indígenas. Ningún grupo presentó el genotipo TT-13910. Las frecuencias genotípicas fueron estadísticamente diferentes entre los grupos estudiados y las de los genotipos CC-13910 y GG-22018 no concordaron con las frecuencias fenotípicas reportadas en otros estudios. Los resultados sugieren que la posibilidad diagnóstica de hipolactasia mediante genotipificación de estos polimorfismos es escasa en el Caribe Colombiano.(AU)
The prevalence of adult-type hypolactasia is influenced by ethnicity and geography. The CC and GG genotypes of the SNPs C/T-13910 and G/A-22018, respectively indicate hypolactasia in certain ethnic groups worldwide. The aim of this study was to analyse these SNPs in samples of the three ethnic groups that inhabit the Colombian Caribbean. Three hundred and sixty-one subjects were genotyped using PCR/RFLP. These subjects were grouped as being of African descent, Indigenous and Mestizo. The genetic analysis was performed through Arlequin 3.11 and genotype frequencies were compared with Statgraphics Centurion XVI. Only SNP C/T-13910 showed Hardy-Weinberg equilibrium and there was no linkage disequilibrium between the SNPs. The frequency of CC-13910 was 90% in Afro-descendant, 95% in indigenous people and 80% in mestizos.The frequency of GG-22018 was 23% in indigenous people, but this genotype was not present in afro-descendants and Mestizos. The indigenous people did not have AA-22018, and none of the groups had TT-13910. The genotype frequencies were statistically different among the groups studied and the frequencies of CC-13910 and GG-22018 were not in concordance with the phenotype frequencies reported in other papers. The results suggest that diagnostic possibility of hypolactasia by genotyping of those polymorphisms in the Colombian Caribbean population is scarce.(AU)

A frequÛncia do genótipo CC-13910 foi de 90% para afrodescendentes, 95% para indígenas e 80% em mestiþos. Nos indígenas a frequÛncia de GG-22018 foi de 23% mas tal genótipo nÒo esteve presente na populaþÒo de afrodescendentes e mestiþos. O genótipo AA-22018 nÒo foi encontrado em indígenas. Nenhum grupo apresentou o genótipo TT-13910. As frequÛncias genotípicas foram estatisticamente diferentes entre os grupos avaliados e as dos genótipos CC-13910 e GG-22018, nÒo concordaram com as frequÛncias fenotípicas relatados em outros estudos. Objetivo: Objetivo: Determinar la asociación entre el crecimiento bacteriano de la flora intestinal y la presión arterial de hombres adultos mayores hipertensos después de cinco semanas de consumo de soya (Glicine máx). Materiales y métodos: Estudio cuasiexperimental para evaluar la asociación entre cambios en la presión arterial y el crecimiento bacteriano en 24 hombres hipertensos con edades entre 61 y 82 años residentes en el barrio Las Malvinas (suroccidente de Barranquilla), quienes adicionaron a su dieta habitual "leche de soya" de grano entero sin lactosa (18 gramos de proteína de soya por día) durante 6 semanas. En el análisis microbiológico de las heces se determinaron los géneros y la cantidad de bacterias mediante técnicas tradicionales. Se midió la presión arterial al inicio y final del tratamiento. Resultados: Los recuentos de bacterias aeróbicas al inicio fueron 10 5 a 10(9) y al final 10(6)a 10(9) UFC/gramo de muestra; en bacterias anaeróbicas se encontraron los recuentos más altos: 10 5 a 10 9 antes y 10 6 a 1010 UFC/g al final. Después del tratamiento con soya la presión sistólica disminuyó en 37.5 % de los participantes, la diastólica en 16.67 %, la media en 45.83 % y la de pulso en 45.83 %; sin embargo, no hubo significancia estadística en ningún caso. El coeficiente de correlación de Pearson entre presión arterial y crecimiento bacteriano fue - 0.14. Conclusiones: Se observó una asociación débil e inversa entre el incremento de la microbiota intestinal y la disminución de la presión arterial.
Objective: To determine the association between bacterial growth of intestinal flora and arterial pressure of hypertensive elderly men after five weeks of consumption of Soybean (Glycine max). Materials and methods: Quasi-experimental design to evaluate the association between changes in blood pressure and bacterial growth in 24 hypertensive men aged between 61 and 82 years living in Las Malvinas (southwest of Barranquilla), who added to the usual diet "Soy milk study whole" grain lactose (18 g. soy protein per day) for 6 weeks. In the microbiological analysis of stool gender and amount of bacteria were determined by traditional techniques. Blood pressure at the beginning and end of treatment was measured. Results: The counts of aerobic the start bacteria was 10 5 to 10 9, and the end 10(6) to 10 9 CFU / gram of sample, in anaerobic bacteria are the highest counts found: 10 5 to 10 9 before and 10 6 to 1010 CFU /g at the end. After treatment with soy systolic pressure decreased by 37.5 % of participants, diastolic in 16.67 %, 45.83 % on average and 45.83 % in pulse, but there was no statistical significance in any case. The Pearson correlation coefficient between arterial pressure and bacterial growth was - 0.14. Conclusions: Aweak inverse association between increased intestinal microbiota and decrease in arterial pressure was observed.
RESUMO
OBJECTIVE: To assess the feasibility and effectiveness of the International Nosocomial Infection Control Consortium (INICC) multidimensional hand hygiene (HH) approach in Colombia, and analyze predictors of poor HH compliance. METHODS: An observational, prospective, interventional, before-and-after study was conducted from May 2003 through September 2010 in 10 intensive care units (ICUs) of six hospitals in three cities. The study was divided into two periods: a baseline and a follow-up period. Observations for HH compliance were done in each ICU during randomly selected 30-min periods. The multidimensional HH approach included: (1) administrative support, (2) supplies availability, (3) education and training, (4) reminders in the workplace, (5) process surveillance, and (6) performance feedback. RESULTS: A total of 13 187 opportunities for HH were observed. Overall HH compliance increased from 50% to 77% (relative risk 1.55, 95% confidence interval 1.43-1.68; p=0.0001). Multivariate and univariate analyses showed that several variables were significantly associated with poor HH compliance: males vs. females (67% vs. 77%; p=0.0001), physicians vs. nurses (59% vs. 78%; p<0.0001), and adult vs. pediatric ICUs (76% vs. 42%; p<0.001), among others. CONCLUSIONS: Adherence to HH was increased by 55% with the INICC approach. Programs targeted at improving HH in variables found to be predictors of poor compliance should be implemented.

DESIGN: A before-after prospective surveillance study to assess the impact of a multidimensional infection control approach for the reduction of catheter-associated urinary tract infection (CAUTI) rates. SETTING: Pediatric intensive care units (PICUs) of hospital members of the International Nosocomial Infection Control Consortium (INICC) from 10 cities of the following 6 developing countries: Colombia, El Salvador, India, Mexico, Philippines, and Turkey. PATIENTS: PICU inpatients. METHODS: We performed a prospective active surveillance to determine rates of CAUTI among 3,877 patients hospitalized in 10 PICUs for a total of 27,345 bed-days. The study was divided into a baseline period (phase 1) and an intervention period (phase 2). In phase 1, surveillance was performed without the implementation of the multidimensional approach. In phase 2, we implemented a multidimensional infection control approach that included outcome surveillance, process surveillance, feedback on CAUTI rates, feedback on performance, education, and a bundle of preventive measures. The rates of CAUTI obtained in phase 1 were compared with the rates obtained in phase 2, after interventions were implemented. RESULTS: During the study period, we recorded 8,513 urinary catheter (UC) days, including 1,513 UC-days in phase 1 and 7,000 UC-days in phase 2. In phase 1, the CAUTI rate was 5.9 cases per 1,000 UC-days, and in phase 2, after implementing the multidimensional infection control approach for CAUTI prevention, the rate of CAUTI decreased to 2.6 cases per 1,000 UC-days (relative risk, 0.43 [95% confidence interval, 0.21-1.0]), indicating a rate reduction of 57%. CONCLUSIONS: Our findings demonstrated that implementing a multidimensional infection control approach is associated with a significant reduction in the CAUTI rate of PICUs in developing countries.

Design. Before-after prospective surveillance study to assess the efficacy of the International Nosocomial Infection Control Consortium (INICC) multidimensional infection control program to reduce the rate of occurrence of ventilator-associated pneumonia (VAP). Setting. Neonatal intensive care units (NICUs) of INICC member hospitals from 15 cities in the following 10 developing countries: Argentina, Colombia, El Salvador, India, Mexico, Morocco, Peru, Philippines, Tunisia, and Turkey. Patients. NICU inpatients. Methods. VAP rates were determined during a first period of active surveillance without the implementation of the multidimensional approach (phase 1) to be then compared with VAP rates after implementation of the INICC multidimensional infection control program (phase 2), which included the following practices: a bundle of infection control interventions, education, outcome surveillance, process surveillance, feedback on VAP rates, and performance feedback on infection control practices. This study was conducted by infection control professionals who applied National Health Safety Network (NHSN) definitions for healthcare-associated infections and INICC surveillance methodology. Results. During phase 1, we recorded 3,153 mechanical ventilation (MV)-days, and during phase 2, after the implementation of the bundle of interventions, we recorded 15,981 MV-days. The VAP rate was 17.8 cases per 1,000 MV-days during phase 1 and 12.0 cases per 1,000 MV-days during phase 2 (relative risk, 0.67 [95% confidence interval, 0.50-0.91]; [Formula: see text]), indicating a 33% reduction in VAP rate. Conclusions. Our results demonstrate that an implementation of the INICC multidimensional infection control program was associated with a significant reduction in VAP rate in NICUs in developing countries.
