BACKGROUND: Cataract is the leading cause of blindness in developing countries and identification of the barriers to accessing treatment is essential for developing appropriate public healthcare interventions. To evaluate the barriers to cataract surgery after diagnosis and assess the postoperative outcomes in Sao Paolo State, Brazil. METHODS: This prospective study evaluated cataract patients from 13 counties in São Paulo State in 2014. Cataract was diagnosed in the community by a mobile ophthalmic unit and patients were referred to a hospital for management. Gender, age, distance to the hospital and local municipal health structure were evaluated as possible barriers. Data were analyzed for postoperative outcomes and the impact on blindness and visual impairment. RESULTS: Six hundred patients were diagnosed with cataract with a mean age of 68.8±10.3 years and 374 (62.3%) were females. Two hundred and fifty-four (42.3%) patients presented to the referral hospital. One hundred forty-four (56.7%) underwent surgery, 56 (22.0%) decided not to undergo surgery, 40 (15.7%) required only YAG-Laser and 14 (5.5%) required a spectacle prescription only. Visual acuity increased statistically significantly from 1.07±0.73 logMAR at presentation to 0.25±0.41 logMAR at the final visit after intraocular lens implantation (p=0.000). There was a statistically significantly decrease from 17 (11.8%) blind patients and 55 (38.2%) visually impaired patients at presentation to 2 (1.4%) and 5 (3.5%) patients respectively after treatment (p=0.000). CONCLUSION: Less than half of the individuals with cataract presented to the hospital for surgery. Among the patients who underwent treatment, there was an overall decrease in the number of blind individuals and visually impaired individuals. The barriers to cataract surgery were older age, greater distance to the hospital, municipalities with fewer inhabitants and less ophthalmic services.

PURPOSE: To evaluate the ocular axial length (AL) and keratometry (K) in Brazilian children with congenital/developmental cataract, assess the differences and evolution of AL and K according to age, and establish functional models of AL and K as function of age. METHODS: Children with congenital/developmental cataract aged 1.5 months old to 8 years old and no other ocular diseases were included. All eyes with unilateral cataract, the left eyes from children with bilateral cataracts, and healthy eyes from children with unilateral cataract were analyzed. After the administration of anesthesia, K was measured with a portable automatic keratometer, and AL was measured with a contact biometer. Cataract surgery was performed immediately after the measurements were taken. The data were statistically analyzed, and a linear regression with an age logarithm was used to model the relationship. RESULTS: Forty-four eyes with cataract were included in this analysis, comprising 15 eyes with unilateral cataract and 29 left eyes from children with bilateral cataracts. The mean age was 27.3 months with a mean AL of 20.63 ± 2.11 mm and a mean K of 44.94 ± 2.44 D. The K value was significantly steeper and the AL value was significantly shorter in younger children (P< 0.001). No significant differences were found neither between eyes with unilateral and bilateral cataracts nor between eyes with unilateral cataract and their corresponding healthy eyes (P >0.05). CONCLUSION: The values of K and AL significantly change with age, especially during the first 6 months of life. A linear functional relationship between K and AL with the logarithm of age and between K and AL was established.

RESUMO Objetivo: Avaliar o comprimento axial (AL) e a ceratometria (K) de olhos de crianças brasileiras com catarata congênita/desenvolvimento, analisar diferenças e evoluções de acordo com a idade e estabelecer modelos funcionais de comprimento axial e ceratometria em função da idade e entre eles. Métodos: Crianças com catarata congênita/desenvolvimento com idade de 1,5 meses a 8 anos de idade e sem outras doenças oculares foram incluídas. Todos os olhos com catarata unilateral, o olho esquerdo de crianças com catarata bilateral e o olho sadio de crianças com catarata unilateral foram analisados. Após a administração de anestesia, a ceratometria foi obtida com um ceratômetro automático portátil e o comprimento axial medido com um biômetro de contato. Em seguida, a cirurgia de catarata foi realizada. Os dados foram analisados estatisticamente, a regressão linear com o logaritmo da idade foi utilizado para modelar os relacionamentos. Resultados: Todos os olhos com catarata unilateral (n=15) e um olho selecionados aleatoriamente a partir dos casos bilaterais (n=29) foram incluídos na análise (total= 44 olhos). A idade média foi de 27,3 meses, as médias do comprimento axial e da ceratometria foram respectivamente 20,63 ± 2,11 mm e 44,94 ± 2,44 dioptrias. A ceratometria foi significativamente mais curvo e comprimento axial significantemente mais curto em crianças mais jovens (P<0,001). Não foram encontradas diferenças significativas na comparação entre os olhos com cataratas unilaterais e bilaterais e comparando os olhos com catarata unilateral a correspondentes olhos saudáveis (P>0,05). Conclusão: Os valores de ceratometria e comprimento axial mudam significativamente com a idade, principalmente nos primeiros seis meses de vida. Foi estabelecida uma relação funcional linear entre comprimento axial e ceratometria com o logaritmo da idade e entre ceratometria e comprimento axial.

OBJETIVO: Descrever a implantação do teste de reflexo vermelho nas 30 cidades de inserção do Hospital das Clínicas da Faculdade de Medicina de Botucatu, HC/FMB/UNESP (480.337 habitantes), a criação de um centro para referência de crianças com reflexo vermelho alterado ou duvidoso, a Triagem do reflexo vermelho e outro para o tratamento da catarata infantil, o Centro de tratamento da catarata infantil. MÉTODOS: O exame do reflexo vermelho foi divulgado em 30 cidades da região de Botucatu. Foram realizadas palestras aos municípios, convocados pelo Departamento Regional de Saúde VI do estado de São Paulo (DRS VI). Foram distribuídos 109 "pen torch ophthalmoscope", às maternidades e Unidades Básicas de Saúde (UBSs) das cidades. A Triagem do reflexo vermelho recebeu os casos de reflexo vermelho alterado ou duvidoso e estabeleceu o diagnóstico oftalmológico. O Centro de tratamento da catarata infantil realizou o exame pré-operatório, o tratamento cirúrgico e o acompanhamento das crianças com catarata. RESULTADOS: Após um ano de funcionamento a Triagem do reflexo vermelho atendeu 29 crianças, 17 do sexo masculino e 12 do feminino, com idade média e desvio padrão (dp) de 10,09 ± 20,35 meses (7 dias - 98 meses). 16 pacientes foram encaminhados com reflexo vermelho alterado, idade média e dp de 13,17 ± 24,14 meses (7 dias - 98 meses), a alteração foi confirmada em todos os casos, 13 deles apresentavam catarata. Em 13 encaminhamentos com reflexo duvidoso, idade média e dp de 6,29 ± 14,46 meses (7 dias - 98 meses), a alteração não se confirmou. A incidência de alterações do reflexo vermelho encontradas foi de 9,2/10.000 nascidos vivos e a incidência the catarata foi de 7,9/10.000 nascidos vivos. CONCLUSÃO: Descrevemos a implantação do Teste do reflexo vermelho na Região de Botucatu, a criação da Triagem do reflexo vermelho e do Centro de tratamento da catarata infantil e dificuldades encontradas.

METHODS: Thirty Wistar rats were divided into 3 groups: control (GC), diabetic (DG), and treated diabetic (TG) and the observations were made 1 month (M1) and 12 months (M2) after diabetes induction. Diabetes was induced by intravenous alloxan (42 mg/kg). The treated group received acarbose orally and insulin by subcutaneous injection. Eyes were prepared for transmission electron microscopy, specifically for ultrastructure of the Bruch membrane and choroidal vessels. RESULTS: Ultrastructural examination of the diabetic rat choroid showed deposits in the Bruch membrane and accumulation of vesicles, glycogen and dense bodies in endothelial cell cytoplasm. The most affected group was that of the diabetics on month 12 (GDM2). The treated diabetics showed the least alterations on month 12 (GTM2). CONCLUSION: Diabetic rats develop degenerative alterations in the Bruch membrane and choroidal vessels. These alterations are more evident in animals submitted to chronic disease, but they are also present in acute disease. Degenerative processes were not avoided with short-term treatment. Long-term treatment inhibited the progress of these processes.

PURPOSE: To evaluate the effect of phacoemulsification through clear cornea incision in the steepest meridian (ISM), on the magnitude of preoperative keratometric astigmatism. To map the magnitude of surgically induced astigmatism (SIA) by clear cornea incision in the positions: nasal (N), temporal (T), superior temporal (ST) and inferior temporal (IT). METHODS: A prospective keratometric study was performed in 48 eyes of 48 patients, submitted to phacoemulsification by incision in the steepest meridian. Keratometric measures were taken preoperatively and one month after surgery. Surgically induced astigmatism was determined by the rectangular coordinate by the modified ten-step method. RESULTS: There were 21 right eyes and 27 left eyes submitted to surgery, 16 eyes being N, 4 eyes T, 22 eyes TS, and 6 eyes TI. The average preoperative and postoperative cylindrical ametropias were, respectively, 1.06 +/- 0.65 D and 0.89 +/- 0.80 D. We found a statistical difference regarding decrease in the preoperative keratometric astigmatism (p=0.016). The total average of the surgically induced astigmatism was 0.94 +/- 0.56 D. In relation to the groups, surgically induced astigmatism was 1.06 +/- 0.66 D in N, 0.87 +/- 0.20 D in T, 0.95 +/- 0.55 D in TS and 0.61 +/- 0.25 D in TI. OBJETIVO: Conhecer os efeitos do diabetes e o impacto de seu tratamento medicamentoso em curto e longo prazo sobre os vasos da coróide e membrana de Bruch. MÉTODOS: Foram estudados 30 ratos Wistar, divididos em 3 grupos experimentais: grupo controle (GC), grupo diabético (GD) e grupo diabético tratado (GT), estudados 1 mês (momento M1) e 12 meses (momento M2) após o início do experimento. O diabetes foi induzido por aloxana endovenosa, na dose de 42 mg/kg. O GT foi tratado com hipoglicemiante oral (acarbose) e insulina subcutânea. Após o sacrifício, os olhos foram preparados para exame ao microscópio eletrônico de transmissão, interessando a ultra-estrutura da membrana de Bruch e os vasos da coróide. RESULTADOS: O exame ultra-estrutural da coróide dos ratos diabéticos mostrou depósitos na membrana de Bruch, acúmulo de vesículas, glicogênio e corpos densos no citoplasma das células endoteliais. O grupo mais afetado foi de ratos diabéticos de 12 meses (GDM2). Os animais com menor intensidade de alterações foram os ratos tratados por 12 meses (GTM2). CONCLUSÃO: Os ratos diabéticos desenvolveram alterações degenerativas na membrana de Bruch e vasos da coróide. Estas alterações foram mais evidentes nos animais submetidos à doença crônica, mas também ocorreram agudamente. O tratamento a curto prazo não foi capaz de evitar os processos degenerativos. A longo prazo, o tratamento inibiu a progressão destes processos.

Mapear a magnitude do astigmatismo cirurgicamente induzido por incisões nas posições nasal (N), temporal (T), temporal superior (TS) e temporal inferior (TI). MÉTODOS: Foi realizado estudo ceratométrico prospectivo em 48 olhos de 48 pacientes, submetidos a facoemulsificação com incisão do meridiano mais curvo. As medidas ceratométricas foram realizadas no pré-operatório e um mês após a realização da cirurgia. O astigmatismo cirurgicamente induzido foi determinado pelo método das coordenadas retangulares em 10 passos, modificado. RESULTADOS: Foram operados 21 olhos direitos e 27 olhos esquerdos e, de acordo com a posição das incisões em córnea clara, foram divididos em: 16 olhos N, 4 olhos T, 22 olhos TS, 6 olhos TI. A média da ametropia cilíndrica pré-operatória foi de 1,06D ± 0,65 e a pós-operatória de 0,89D ± 0,80. Houve diminuição estatisticamente significativa no astigmatismo corneal preexistente (p=0,016). A média total de astigmatismo cirurgicamente induzido foi de 0,94D ± 0,56. Em relação aos grupos o astigmatismo cirurgicamente induzido foi de 1,06D ± 0,66 em N, 0,87D ± 0,20 em T, 0,95D ± 0,55 em TS e 0,61D ± 0,25 em TI. Não houve diferença estatística com relação ao astigmatismo cirurgicamente induzido nos grupos N, T, TS e TI (p=0,426). CONCLUSÃO: A técnica se mostrou efetiva na redução do astigmatismo ceratométrico pré-operatório. A média do astigmatismo cirurgicamente induzido foi de 1,06D ± 0,66 em N, 0,95D ± 0,55 em TS, 0,61D ± 0,25 em T e 0,87D ± 0,20 em TI.
