BACKGROUND: Hepatocellular carcinoma (HCC) is an aggressive malignant neoplasm that requires liver transplantation (LT). Despite patients with HCC being prioritized by most organ allocation systems worldwide, they still have to wait for long periods. Locoregional therapies (LRTs) are employed as bridging therapies in patients with HCC awaiting LT. Although largely used in the past, transarterial embolization (TAE) has been replaced by transarterial chemoembolization (TACE). However, the superiority of TACE over TAE has not been consistently shown in the literature. AIM: To compare the outcomes of TACE and TAE in patients with HCC awaiting LT. METHODS: All consecutive patients with HCC awaiting LT between 2011 and 2020 at a single center were included. All patients underwent LRT with either TACE or TAE. Some patients also underwent percutaneous ethanol injection (PEI), concomitantly or in different treatment sessions. The choice of LRT for each HCC nodule was determined by a multidisciplinary consensus. The primary outcome was waitlist dropout due to tumor progression, and the secondary outcome was the occurrence of adverse events. In the subset of patients who underwent LT, complete pathological response and post-transplant recurrence-free survival were also assessed. RESULTS: Twelve (18.5%) patients in the TACE group (only TACE and TACE + PEI; n = 65) and 3 (7.9%) patients in the TAE group (only TAE and TAE + PEI; n = 38) dropped out of the waitlist due to tumor progression (P log-rank test = 0.29). Adverse events occurred in 8 (12.3%) and 2 (5.3%) patients in the TACE and TAE groups, respectively (P = 0.316). Forty-eight (73.8%) of the 65 patients in the TACE group and 29 (76.3%) of the 38 patients in the TAE group underwent LT (P = 0.818). Among these patients, complete pathological response was detected in 7 (14.6%) and 9 (31%) patients in the TACE and TAE groups, respectively (P = 0.145). Post-LT, HCC recurred in 9 (18.8%) and 4 (13.8%) patients in the TACE and TAE groups, respectively (P = 0.756). Posttransplant recurrence-free survival was similar between the groups (P log-rank test = 0.71). CONCLUSION: Dropout rates and posttransplant recurrence-free survival of TAE were similar to those of TACE in patients with HCC. Our study reinforces the hypothesis that TACE is not superior to TAE as a bridging therapy to LT in patients with HCC.

RESUMO

In this review, we presented the principles of radial keratotomy (RK), its evolution, enhancement, and complications, and strategies to manage the consequences of RK in the present day. It is essential to understand the RK procedure f, the theoretical background that supported this surgery, the current effect on the cornea, and how to approach patients needing vision improvement. These patients are developing cataracts that need to be handled well, from the IOL calculation to the surgical procedure. Guided keratorefractive surgery is the most accurate procedure to improve these patient's vision and life. Nevertheless, some patients may need other approaches, such as sutures, penetrating keratoplasty, corneal rings, and pinhole implants, depending on the degree of irregularity of the cornea, ablation depth for guided surgery or if the sutures are open.

PURPOSE: The aim of this study was to evaluate the quality of life (QOL) after intrastromal ring implantation in patients with keratoconus. METHODS: This was a prospective, randomized, interventional study. We analyzed 60 eyes of 30 patients aged 16 to 35 years who were treated at the Department of Ophthalmology and Visual Sciences, Federal University of São Paulo, Brazil. The Visual Function Questionnaire (VFQ-25) and Short-Form 36 Questionnaire (SF-36) were used before intracorneal ring segment (ICRS) implantation and at 3, 6, and 12 months after surgical intervention. RESULTS: The mean corrected visual acuity improved from a mean of 0.32 ± 0.2 logMAR (20/40) preoperatively to 0.14 ± 0.11 logMAR (20/25) 1 year postoperatively ( P = 0.001). The mean spherical equivalent varied from -7.24 ± 3.47 preoperatively to -4.13 ± 2.41 postoperatively ( P = 0.001). The overall composite score for the VFQ-25 improved from 55.1 preoperatively to 80.4 1 postoperatively ( P = 0.001). SF-36 showed statistically significant improvement in all scores. When analyzing the correlation between visual acuity and VFQ composite score, a significant correlation was found between both variables (Pearson correlation coefficient of -0.40, P = 0.001). CONCLUSIONS: Patients with keratoconus had increased psychological symptoms and lower QOL and improved psychosocial criteria associated with corneal remodeling and decreased visual dependence on others after surgery. Extrapolation of these data to the whole keratoconus population suggests that ICRS implantation could improve QOL in these patients.

ABSTRACT Purpose: To compare the differences between the apparent and actual chord μ. Methods: In this prospective, comparative, non-randomized, and non-interventional study, imaging examinations using Pentacam and the HD Analyzer were performed in the same room under the same scotopic conditions. The inclusion criteria were patients aged 21-71 years, able to provide informed consent, myopia up to 4D, and anterior topographic astigmatism up to 1D. Patients using contact lenses, those with previous eye diseases or surgeries, corneal opacities, corneal tomographic changes, or suspected keratoconus were excluded. Results: Altogether, 116 eyes of 58 patients were analyzed. The patients' mean age was 30.69 (±7.85) years. In the correlation analyses, Pearson's correlation coefficient of 0.647 indicates a moderate positive linear relationship between apparent and actual chord μ. The mean actual and apparent chord μ were 226.21 ± 128.53 and 278.66 ± 123.90 μm, respectively, with a mean difference of 52.45 μm (p=0.01). The analysis of mean pupillary diameter resulted in 5.76 mm using the HD Analyzer and 3.31 mm using the Pentacam. Conclusions: We found a correlation between the two measurement devices, and even though we found considerable differences, both can be used in daily practice. Given their differences, we should respect their peculiarities as well.


RESUMO Objetivo: Comparar as diferenças entre a chord aparente μ e o chord real μ. Métodos: Estudo prospectivo, comparativo, não randomizado e não intervencionista. Os exames de imagem (Pentacam e HD Analyzer) foram realizados na mesma sala e nas mesmas condições escotópicas. Os critérios de inclusão foram idade de 21 a 71 anos; compreensão do termo de consentimento; miopia até 4D e astigmatismo topográfico anterior até 1D. Os critérios de exclusão foram usuários de lentes de contato; pacientes com doenças oculares prévias ou cirurgias; opacidades da córnea; a presença de alterações tomográficas da córnea ou suspeita de ceratocone. Resultados: Em nosso estudo foram analisados 116 olhos de 58 pacientes. A média de idade foi de 30,69 anos (± 7,85). Análises de correlação foram desenvolvidas e o coeficiente de correlação de Pearson (0,647) indica uma relação linear positiva moderada entre as variáveis. A média do chord μ real foi 226,21 ± 128,53 μm e a média do chord μ média foi 278,66 ± 123,90 μm, com diferença média de 52,45 μm (p=0,01). A análise do diâmetro pupilar médio apresentou: 5,76mm no HD Analyzer e 3,31mm no Pentacam. Conclusões: Entendemos a existência de uma diferença significativa entre os métodos e assim a medida de ambos os dispositivos com base em princípios diferentes devemos respeitar suas peculiaridades. Como encontramos correlação entre as duas medidas, acreditamos que ambas podem ser utilizadas na prática diária.

ABSTRACT Objective To evaluate intraocular lens power calculation and postoperative refractive errors in patients with high myopia undergoing cataract surgery, comparing predicted target refraction and actual postoperative refraction measured 30 days after surgery with SRK/T formula. Methods This retrospective analysis comprised 39 eyes of 31 patients undergoing cataract surgery through phacoemulsification with in-the-bag IOL implantation. Axial length was measured by partial coherence interferometry or immersion ultrasound biometry, with measurements greater than 26 mm and preoperative myopia greater than -6.0 D Manifest refraction was performed at the 1-month postoperative visit, and the spherical equivalent was analyzed. Results After analysis of 39 eyes of 31 patients undergoing cataract surgery with a mean axial length of 30.4 (standard deviation of 2.2) mm, the mean preoperative refractive spherical equivalent was -15.6 (standard deviation of 7.6) D, ranging from -24.0 to -13.4 D. At 30 days postoperatively, the mean spherical equivalent was -0.35 (standard deviation of 1.1) D, ranging from -2.4 to 2.50 D. Conclusion We encountered a correlation between the absolute refractive error and the dioptric power of the intraocular lens. Against expectations, in our study, ultrasound biometry yielded better results than the optical biometer device, probably due to the small number of patients undergoing optical biometry, suggesting that well-performed immersion biometry can still produce satisfactory results.


RESUMO Objetivo Avaliar os cálculos de potência da lente intraocular e os erros refrativos pós-operatórios em pacientes com alta miopia submetidos à cirurgia de catarata, comparando a refração-alvo prevista e a refração pós-operatória real medida 30 dias após a cirurgia com a fórmula SRK/T. Métodos Esta análise retrospectiva incluiu 39 olhos de 31 pacientes com cirurgia de catarata de facoemulsificação não complicada com implantação de lente intraocular na bolsa. Os comprimentos axiais foram medidos por biometria de coerência óptica ou ultrassônica (imersão), com medidas de axial length (AL) maiores que 26 mm em pacientes com miopia maior que -6.0 D. A refração manifesta foi realizada na consulta pós-operatória de 1 mês, e o equivalente esférico foi analisado. Resultados Após análise de 39 olhos de 31 pacientes submetidos à cirurgia de catarata com AL médio de 30,4 (desvio-padrão de 2,2) mm, o equivalente esférico refrativo médio pré-operatório foi de -15,6 (desvio-padrão de 7,6) D, variando de -24,0 a -13,4 D. Aos 30 dias de pós-operatório, o equivalente esférico médio foi de -0,35 (desvio-padrão de 1,1) D, variando de -2,4 a 2,50 D. Conclusão Encontramos uma correlação entre o erro refrativo absoluto e o poder dióptrico da lente intraocular. Contrariando as expectativas, em nosso estudo, a biometria ultrassônica apresentou melhores resultados que o biômetro óptico, provavelmente devido ao pequeno número de pacientes submetidos à biometria óptica, sugerindo que a biometria de imersão bem executada ainda pode produzir resultados satisfatórios.

Purpose: Compare the safety and efficacy of wavefront-guided photorefractive keratotomy (PRK) 6 months after cross-linking (CXL) to wavefront-guided PRK alone for refractive correction in patients with bilateral asymmetric corneal topography. Methods: Prospective randomized clinical trial with 16 patients (32 eyes). CXL with subsequent PRK after 6 months in one eye, and PRK alone was performed in contralateral eyes. The follow-up was 10 years. We analyzed visual outcomes, Scheimpflug topography, and corneal haze evaluation. Results: Eyes in the PRK group showed better results than in the CXL + PRK group. Mean postoperative CDVA was 0.044 logmar (SD, 0.073) in the PRK group and 0.1 logmar (SD, 0.21) in the CXL + PRK group, the mean sphere was + 0.21 (SD, 0.6) D in the PRK group and 0.87 (SD, 2.3) D in the CXL + PRK group, and mean SE was -0.35 (SD, 0.65) D in the PRK group and 0.62 (SD, 2.32) D in the CXL + PRK group. In one patient, a steepening of 2.5 D and a thinning of 17 µm occurred in PRK alone group. Two patients in the CXL + PRK group presented corneal haze. The overall complication rate was 18,75% (haze and ectasia). Conclusion: Non-simultaneous CXL and PRK procedures yielded good refractive results, but worse than those obtained with PRK alone. Although one patient in the PRK group developed corneal ectasia, the CXL + PRK group had a higher loss of vision lines, indicating less safety.

INTRODUCTION: hepatocellular adenoma - AHC - is a rare benign neoplasm of the liver more prevalent in women at reproductive age and its main complication is hemorrhage. In the literature, case series addressing this complication are limited. METHODS: between 2010 and 2022, 12 cases of bleeding AHC were attended in a high-complexity university hospital in southern Brazil, whose medical records were retrospectively evaluated. RESULTS: all patients were female, with a mean age of 32 years and a BMI of 33kg/m2. The use of oral contraceptives was identified in half of the sample and also half of the patients had a single lesion. The mean diameter of the largest lesion was 9.60cm and the largest lesion was responsible for bleeding in all cases. The presence of hemoperitoneum was documented in 33% of the patients and their age was significantly higher than the patients who did not have hemoperitoneum - 38 vs 30 years, respectively. Surgical resection of the bleeding lesion was performed in 50% of the patients and the median number of days between bleeding and resection was 27 days. In only one case, embolization was used. The relation between ingrowth of the lesions and the time, in months, was not obtained in this study. CONCLUSION: it is concluded that the bleeding AHC of the present series shows epidemiological agreement with the literature and may suggest that older patients trend to have hemoperitoneum more frequently, a fact that should be investigated in further studies.

PURPOSE: This study evaluated the luminous behavior applied to materials used in intraocular surgeries. METHODS: Discs of the different products were delivered in 19.00 mm × 3.00 mm. Each sample was fixed on support keeping it perpendicular to the spectrophotometer beam. Later, their analyses were carried out in the air/PMMA ratio. The graphs of individual profiles of the measurements along the length were constructed according to each of the filters from the spectrophotometric analysis. In addition, descriptive statistics of transmittance and absorbance for each wavelength presented were correlated for each filter. RESULTS: It is possible to observe that the minimum absorption measure was found in the Red Filter, especially in the blue and green light spectrum. CONCLUSION: Using filters in PMMA materials appears to improve visual quality in corneal implants, especially the red filter, due to greater absorbance of light leading to fewer light scattering phenomena through corneal rings. However, further studies comparing the effects of different filters on Intracorneal rings should be carried out to elucidate this field of study.

RESUMO Introdução: o Adenoma Hepatocelular - AHC - é uma neoplasia benigna rara do fígado associada a mulheres com idade reprodutiva. Sua principal complicação é o sangramento,mas as séries de casos direcionadas a esta intercorrência são limitadas. Métodos: entre os anos de 2010 e 2022 foram registrados 12 casos de AHC sangrante em um hospital universitário de alta complexidade no sul do Brasil, cujos prontuários foram retrospectivamente avaliados. Resultados: todos os pacientes eram do sexo feminino, com média de idade de 32 anos e de IMC de 33kg/m2. O uso de anticoncepcionais foi identificado em metade da amostra e também metade das pacientes apresentava lesão única. O diâmetro médio da maior lesão foi de 9,60cm e a maior lesão foi responsável pelo sangramento em todos os casos. Hemoperitôneo foi documentado em 33% das pacientes e a idade destas foi significativamente maior do que as pacientes que não apresentaram hemoperitôneo - 38 vs 30 anos, respectivamente. A ressecção cirúrgica da lesão sangrante foi realizada em 50% das pacientes e a mediana de dias entre o sangramento e a ressecção foi de 27 dias. Em apenas um caso foi lançado mão da embolização da lesão. Não se demonstrou correlação entre a taxa de redução das lesões e o passar dos meses, nos casos em que foi adotado tratamento conservador. Conclusão: conclui-se que aos AHC sangrantes da presente série apresentam concordância epidemiológica com a literatura e pode sugerir que as pacientes com maior idade tendem a apresentar hemoperitôneo mais frequentemente, fato que deve ser investigado em maiores estudos.

RESUMO

Keratoconus is an ectatic corneal disease that affects both men and women, usually at a young age. Risk factors such as eye rubbing, environmental and genetic factors contribute to its pathogenesis. Keratoconus presents a variable prevalence, being as high as 5% of the population in the Middle East. In this review, an English-language search for keratoconus epidemiology was undertaken using the PubMed database. We present and discuss the findings of the most relevant epidemiological studies on keratoconus and describe its characteristics and risk factors, correlating them with its pathogenesis.

PURPOSE: Report a new technique for improvement of pterygium surgery: donor graft harvesting with air. CASE REPORTS: We describe a technique that was used with 138 patients who underwent surgery that included a conjunctival autograft and fibrin glue with air injection to dissect the Tenon capsule from the conjunctiva. RESULTS: No complications occurred with this technique and all patients achieved good results. DISCUSSION: The surgical time with this new technique was similar to that reported in the literature. In addition, the graft thickness was close to that reported with femtosecond laser dissection and it was thinner than that reported with aesthetic/saline dissection (unable to determine statistical significance). CONCLUSIONS AND IMPORTANCE: With the injection of an air bubble, we could easily dissect the conjunctiva from the Tenon capsule and quickly acquire thin grafts without any additional costs or complications.

OBJECTIVE: Evaluation of central corneal densitometry changes following Ferrara corneal ring segment implantation in patients with keratoconus, especially the correlation between corneal densitometry and keratometry. METHODS: Retrospective, non-comparative, interventional study based on the review of medical records of patients diagnosed with keratoconus who underwent Ferrara corneal ring segment implantation. Pre and post-operative corneal densitometry measurements obtained with Pentacam HR (Oculus, Wetzlar, Germany) were analyzed. The follow-up time was 3 months, and data comparison was made, using specific statistical analysis, with the data of 3 months postoperatively. RESULTS: The study sample consisted of 43 eyes of 36 patients. The mean corrected visual acuity improved from 0.82 LogMAR preoperatively (SD ± 0.33) to 0.19 LogMAR (SD ± 0.13) postoperatively. The mean spherical equivalent varied from -4.63 (SD ± 3.94) preoperatively to -2.16 (SD ± 2.63) postoperatively. Asphericity varied from -0.69 (SD ± 0.32) preoperatively to -0.27 (SD ± 0.31) postoperatively. The mean maximum K was 54.01D (SD ± 3.38) preoperatively and 51.50D (SD ± 2.90) postoperatively. The mean anterior densitometric value was 18.26 (SD ± 2.03) preoperatively and 17.66 (SD ± 1.84) postoperatively. CONCLUSION: Corneal densitometry is an interesting technology that should be studied in keratoconus patients. Our results suggest that the corneal densitometry in the cornea's anterior layer reduces after ICRS implantation and correlates with corneal keratometry. Further studies should be performed to increase the knowledge in this field.

OBJECTIVE: To investigate the role of ICRS in halting keratoconus progression in a large sample of patients. METHODS: A retrospective, non-comparative, interventional study based on the review of medical records of patients diagnosed with keratoconus who underwent Ferrara corneal ring segment implantation. A retrospective chart analysis study of 123 operated eyes with follow-up ranging from 3 to 16 years (mean 5.3 ± 3.6 years) was performed. This study was carried out at Ocular Surgery Unit, São Paulo, Brazil. All topographic data were obtained from Pentacam (Oculus, Arlington, USA). The same surgeon performed all surgeries, and the Ferrara ICRS nomogram was used for ICRS selection in all cases. RESULTS: Corrected distance visual acuity, keratometry, and topographic astigmatism improved in most cases, with statistical significance. In 42 eyes (53.8%), there was an increase in K1 or K2, and in 36 (46.2%), there was a reduction or maintenance in K1. Considering a difference higher than 1 D, between 3-month post-surgery and final visit (group 3), 32 eyes (41%) showed an increase, and 46 eyes (59%) ended equal or below this value. CONCLUSION: The implantation of ICRS showed improvement in visual and keratometric indexes. The majority of patients did not increase more than one diopter in keratometric values after ICRS implantation. ICRS may be effective for slowing disease progression, especially in older patients.

BACKGROUND: Strongly associated with obesity, non-alcoholic fatty liver disease is considered the hepatic manifestation of the metabolic syndrome. It presents as simple steatosis and steatohepatitis, which can progress to cirrhosis and its complications. Among the therapeutic alternatives is bariatric surgery. AIM: To compare the effect of the two most frequent bariatric procedures (sleeve and bypass) on liver disease regarding to epidemiological, demographic, clinical and laboratory parameters. METHODS: The results of intraoperative and 12 months after surgery liver biopsies were used. The NAFLD activity score (NAS) was used to assess and compare the stages of liver disease. RESULTS: Sixteen (66.7%) patients underwent Bypass procedure and eight (33.3%) Sleeve. It was observed that the variation in the NAFLD activity score was significantly greater in the Bypass group than in Sleeve (p=0.028) and there was a trend regarding the variation in fibrosis (p=0.054). CONCLUSION: Both surgical techniques were effective in improving the hepatic histology of most operated patients. When comparing sleeve and bypass groups, bypass showed better results, according to the NAS score.

Abstract Aim: This study aimed to compare the running strategy between men and women in the 10,000 m race. Methods: Data from one-hundred and twenty-nine athletes who competed the 10,000 m final of the 2015, 2017 and 2019 world championship were grouped according to gender: 1) male group (n= 63; age= 25.9±4.4), and 2) female group (n= 66; age= 27.3±4.8). The running strategy was determined by assessing velocity over each kilometer. The velocity was calculated from the times available on the official website of the International Athletics Federation. Results: Men decreased running velocity after the first kilometer and kept the speed stable until km 9 and the kilometer 10 was greater when compared with 6-9 kilometer, while women increase running velocity until km 2 and kept the speed stable until km 9. The kilometer 10 velocity was greater when compared with 1-9 kilometer. (p< 0.001). In addition, women showed a greater average velocity variation when compared to men (p< 0.001). Conclusion: Our findings show a sex differences in the 10,000 m race strategy between world-class.

AIM: To evaluate the clinical results after implantation of a new intrastromal corneal ring segment (ICRS) associated with photorefractive keratectomy (PRK) to correct high myopia (HM) patients with thin corneas. METHODS: We evaluated 42 eyes of 23 HM patients that had ICRS implantation followed by PRK. The mean age of patients was 29.1±7.12y (range 18 to 40 years old). Uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), keratometry, spherical equivalent, pachymetry, and aberrometry were compared using ANOVA with repeated measurements evaluated preoperatively and at last follow-up visit after the procedures. The refractive predictability and simulated/real corneal ablation were also assessed. RESULTS: The mean follow-up time after PRK was 6.8±1.6mo. The mean preoperative UCVA improved from 20/800 preoperative to 20/100 after ICRS and 20/35 after PRK. The mean preoperative BCVA was 20/25 (range from 20/30 to 20/20) and remained unchanged after ICRS implantation. Following the PRK the mean BCVA was 20/25 (range from 20/30 to 20/20). The mean spherical equivalent decreased from -7.25±1.12 (range -5.00 to -9.00) preoperatively to -3.32±1.0 (range -2.00 to -5.00) postoperatively (P<0.001) after ICRS implantation and decreased from -2.44±1.51 preoperatively to 0.32±0.45 (range -0.625 to 0.875) postoperatively (P<0.001) after PRK. The change in BCVA and topographic astigmatism was statistically significant (P<0.0001). CONCLUSION: ICRS in HM associated with PRK can be a tissue saving procedure and an alternative surgical option for correction of moderate to high myopia.

RESUMO O ponto de centralização da cirurgia refrativa é tema de grande importância e gera muita discussão entre especialistas e cirurgiões da área. Afinal, qualquer alteração na luz pode alterar o tamanho da pupila, além disso, o eixo visual da linha de fixação para a fóvea é particular em cada paciente. Existem opções para centralização em cirurgia refrativa com resultados diferentes na literatura. Ainda não há consenso sobre a melhor técnica em cirurgia refrativa que avalie cada caso específico visando um resultado cirúrgico final satisfatório.

RESUMO Racional: Fortemente associada à obesidade, a doença hepática gordura não alcoólica é considerada a manifestação hepática da síndrome metabólica. Ela apresenta-se como esteatose simples e esteato-hepatite, podendo evoluir para cirrose e suas complicações. Entre as alternativas terapêuticas está a cirurgia bariátrica. Objetivo: Comparar o efeito sobre a doença hepática dos dois procedimentos bariátricos mais frequentes - sleeve e bypass - e comparar dados epidemiológicos, demográficos, parâmetros clínicos e laboratoriais. Métodos: Utilizou-se o resultado das biópsias hepáticas realizadas no intra-operatório e 12 meses após a operação. O NAFLD activity score foi utilizado para avaliar e comparar os estágios da doença hepática. Resultados: Dezesseis (66,7%) pacientes foram submetidos ao bypass e oito (33,3%) ao sleeve. Observou-se melhora significativa no IMC e glicemia nas duas técnicas cirúrgicas enquanto que os níveis de fosfatase alcalina, ferritina, Gama-GT e TGP reduziram com significância apenas no grupo bypass. A redução no NAFLD activity score foi significativamente maior no grupo bypass que no sleeve (p=0,040). Conclusão: Ambas as técnicas foram eficazes em promover a melhora da histologia hepática da maior parte dos pacientes operados. Quando comparadas o bypass apresentou melhores resultados.
