Background: How the prefrontal cortex (PFC) recovers its functionality following lesions remains a conundrum. Recent work has uncovered the importance of transient low-frequency oscillatory activity (LFO; < 4 Hz) for the recovery of an injured brain. We aimed to determine whether persistent cortical oscillatory dynamics contribute to brain capability to support 'normal life' following injury. Methods: In this 9-year prospective longitudinal study (08/2012-2021), we collected data from the patient E.L., a modern-day Phineas Gage, who suffered from lesions, impacting 11% of his total brain mass, to his right PFC and supplementary motor area after his skull was transfixed by an iron rod. A systematic evaluation of clinical, electrophysiologic, brain imaging, neuropsychological and behavioural testing were used to clarify the clinical significance of relationship between LFO discharge and executive dysfunctions and compare E.L.´s disorders to that attributed to Gage (1848), a landmark in the history of neurology and neuroscience. Findings: Selective recruitment of the non-injured left hemisphere during execution of unimanual right-hand movements resulted in the emergence of robust LFO, an EEG-detected marker for disconnection of brain areas, in the damaged right hemisphere. In contrast, recruitment of the damaged right hemisphere during contralateral hand movement, resulted in the co-activation of the left hemisphere and decreased right hemisphere LFO to levels of controls enabling performance, suggesting a target for neuromodulation. Similarly, transcranial magnetic stimulation (TMS), used to create a temporary virtual-lesion over E.L.'s healthy hemisphere, disrupted the modulation of contralateral LFO, disturbing behaviour and impairing executive function tasks. In contrast to Gage, reasoning, planning, working memory, social, sexual and family behaviours eluded clinical inspection by decreasing LFO in the delta frequency range during motor and executive functioning. Interpretation: Our study suggests that modulation of LFO dynamics is an important mechanism by which PFC accommodates neurological injuries, supporting the reports of Gage´s recovery, and represents an attractive target for therapeutic interventions. BACKGROUND: Oral mucositis (OM) is a complication of chemoradiotherapy treatment of head and neck squamous cell carcinoma (HNSCC) patients with no effective therapy. This study was designed to assess the efficacy of preventive low-level laser therapy (LLLT) in reducing the incidence of grade 3-4 OM. MATERIAL AND METHODS: From June 2007 to December 2010, 94 HNSCC patients entered a prospective, randomized, double-blind, placebo-controlled phase III trial. Chemoradiotherapy consisted of conventional radiotherapy plus concurrent cisplatin every 3weeks. A diode InGaAlP (660nm-100mW-1J-4J/cm(2)) was used. OM evaluation was performed by WHO and OMAS scales and quality of life by EORTC questionnaires (QLQ). RESULTS: A six-fold decrease in the incidence of grades 3-4 OM was detected in the LLLT group compared to the placebo; (6.4% versus 40.5%). LLLT impacted the incidence of grades 3-4 OM to a relative risk ratio of 0.158 (CI 95% 0.050-0.498). After treatment QLQ-C30 showed, differences favoring LLLT in physical, emotional functioning, fatigue, and pain; while the QLQ-H&N35 showed improvements in LLLT arm for pain, swallowing, and trouble with social eating. CONCLUSION: Preventive LLLT in HNSCC patients receiving chemoradiotherapy is an effective tool for reducing the incidence of grade 3-4 OM. Efficacy data were corroborated by improvements seen in quality of life.

Phase diagrams for potassium and sodium chlorides are determined by molecular dynamics and free energy calculations. Two rigid-ion interaction models, namely, the Born-Mayer-Huggins (BMH) and Michielsen-Woerlee-Graaf (MWG) effective pair potentials, have been used. The critical and triple point properties are discussed and compared with available experimental and simulation data. The MWG model reproduces the experimental liquid-gas equilibria better than the BMH model, being the accordance very good in the lowest temperature region of the coexistent liquids, particularly for NaCl. However, both models underestimate the critical temperatures of KCl and NaCl. Relatively to the solid-gas equilibria, the models do not reproduce well the experimental data. As for the solid-liquid coexistences either the BMH or the MWG models appear unrealistic.

RESUMO

Analisaram-se 46 casos de carcinoma de células renais do INCa, no período de janeiro/76 a dezembro/85, sendo os aspectos clínicos e patológicos comparados com a literatura. Houve mais homens que mulheres, e a incidência foi maior na 6 década de vida (um caso ocorreu numa criança de 5 anos). Sinais mais comuns: hematúria, dor abdominal e perda de peso. A triade hematúria, dor e massa palpável ocorreu em quatro casos (8,6 porcento). As metástases acometeram mais frequentemente pulmöes, ossos e linfonodos. Concluiu-se que o estadiamento clínico é um fator prognóstico importante, havendo diferneça estatísticamente significante entre a sobrevida na doença locorregional e na doença avançada.
