Introduction. Salmonella spp. is a zoonotic pathogen transmitted to humans through contaminated water or food. The presence of extended-spectrum ß-lactamases is a growing public health problem because these enzymes are resistant to third and fourth generation cephalosporins. Objective. To characterize extended-spectrum ß-lactamases in Salmonella spp. isolates received by the acute diarrheal disease/foodborne disease surveillance program of the Grupo de Microbiología of the Instituto Nacional de Salud. Materials and methods. A total of 444 Salmonella spp. isolates, resistant to at least one of the cephalosporins, were obtained between January 1997 and June 2022. The extendedspectrum ß-lactamases phenotype was identified by the double disk test. DNA extraction was carried out by the boiling method, and the blaCTX-M, blaSHV, and blaTEM genes were amplified by PCR. Results. All the isolates were positive for the extended-spectrum ß-lactamases test. The genes identified were: blaCTX-M + blaTEM (n=200), blaCTX-M (n=177), blaSHV (n=16), blaSHV + blaCTX-M (n=6), blaTEM (n=13) and blaSHV + blaCTX-M + blaTEM (n=3). Twenty-six isolates were negative for the evaluated genes. Positive extended-spectrum ß-lactamases isolates were identified in Bogotá and 21 departments: Chocó, Magdalena, Meta, Bolívar, Casanare, Cesar, Córdoba, Quindío, Atlántico, Tolima, Cauca, Cundinamarca, Huila, Boyacá, Caldas, Norte de Santander, Risaralda, Antioquia, Nariño, Santander y Valle del Cauca. Conclusion. Resistance to third generation cephalosporins in Salmonella spp. isolates was mainly caused by blaCTX-M. Isolates were resistant to ampicillin, tetracycline, chloramphenicol, and trimethoprim-sulfamethoxazole (44 %; 197/444). The most frequent extended-spectrum ß-lactamases-expressing serotypes were Salmonella Typhimurium and Salmonella Infantis.

La presencia de ß-lactamasas de espectro extendido es un creciente problema para la salud pública debido a que estas enzimas confieren resistencia contra las cefalosporinas de tercera y cuarta generación. Objetivo. Caracterizar las ß-lactamasas de espectro extendido en aislamientos de Salmonella spp. recibidos por el programa de vigilancia de enfermedad diarreica aguda o enfermedad transmitida por alimentos del Grupo de Microbiología del Instituto Nacional de Salud. Materiales y métodos. Entre enero de 1997 y junio de 2022, se recibieron 444 aislamientos de Salmonella spp. resistentes, por lo menos, a una de las cefalosporinas de tercera generación. El fenotipo de las ß-lactamasas de espectro extendido se identificó con la prueba de doble disco. El ADN se extrajo por ebullición y mediante PCR se amplificaron los genes blaCTX-M, blaSHV y blaTEM. Resultados. Todos los aislamientos fueron positivos para la prueba de ß-lactamasas de espectro extendido. Los resultados de la amplificación por PCR fueron: blaCTX-M + blaTEM (n=200), blaCTX-M (n=177), blaSHV (n=16), blaSHV + blaCTX-M (n=6), blaTEM (n=13) y blaSHV + blaCTX-M + blaTEM (n=3). Del total, 26 aislamientos fueron negativos para los genes evaluados. Los aislamientos positivos para ß-lactamasas de espectro extendido se identificaron en Bogotá y en 21 departamentos: Chocó, Magdalena, Meta, Bolívar, Casanare, Cesar, Córdoba, Quindío, Atlántico, Tolima, Cauca, Cundinamarca, Huila, Boyacá, Caldas, Norte de Santander, Risaralda, Antioquia, Nariño, Santander y Valle del Cauca. Conclusión. La resistencia a las cefalosporinas de tercera generación en aislamientos de Salmonella spp. fue generada principalmente por blaCTX-M. El 44 % (197/444) de los aislamientos presentó resistencia a ampicilina, tetraciclina, cloranfenicol y trimetoprimsulfametoxazol Los serotipos portadores de ß-lactamasas de espectro extendido más frecuentes fueron S. Typhimurium y S. Infantis.

Objetivo. Caracterizar las ß-lactamasas de espectro extendido en aislamientos de Salmonella spp. recibidos por el programa de vigilancia de enfermedad diarreica aguda o enfermedad transmitida por alimentos del Grupo de Microbiología del Instituto Nacional de Salud. This study aimed to analyze perceptions and recommendations from stakeholders on the effectiveness of fall detection systems for older adults, aside from any additional technological solutions they may use within their activities of daily living (ADLs). This study performed a mixed-method approach to explore the views and recommendations of stakeholders concerning the implementation of wearable fall detection systems. Semi-structured online interviews and surveys were conducted on 25 Colombian adults classified into four stakeholder groups: older adults, informal caregivers, healthcare professionals, and researchers. A total of 25 individuals were interviewed or surveyed, comprising 12 females (48%) and 13 males (52%). The four groups cited the importance of wearable fall detection systems in ADLs monitoring of older adults. They did not consider them stigmatizing nor discriminatory but some raised potential privacy issues. The groups also communicated that the apparatus could be small, lightweight, and easy to handle with a help message sent to a relative or caregiver. All stakeholders interviewed perceived assistive technology as potentially useful for opportune healthcare, as well as for promoting independent living for the end user and their family members. For this reason, this study assessed the perceptions and recommendations received concerning fall detectors depending on the needs of stakeholders and the settings in which they are used.

Resumen | Introducción. Listeria monocytogenes es un patógeno transmitido por alimentos que causa infecciones en humanos, entre ellas, meningitis, meningoencefalitis y septicemias, así como abortos. Con la tipificación serológica se han identificado 13 serotipos, siendo el 4b el causante de la mayoría de los brotes en el mundo. Objetivo. Determinar la frecuencia y la distribución de los serotipos y subtipos moleculares de L. monocytogenes aislados de alimentos en Colombia entre el 2010 y el 2018. Materiales y métodos. Se hizo un estudio descriptivo y retrospectivo a partir del análisis de 2.420 aislamientos que fueron identificados como L. monocytogenes y otras especies, por medio de pruebas bioquímicas, serológicas y de subtipificación molecular mediante electroforesis en gel de campo pulsado (PFGE). Resultados. De los 2.420 aislamientos recibidos, 2.326 fueron confirmados como L. monocytogenes. Los serotipos encontrados fueron: 4b (52%), 4d-4e (14,5%), 1/2a (11%), 1/2c (9,4%), 1/2b (9 %), y 3a, 3b, 3c, 4c, 4d, 4e y 7 (menos de 2%). Procedían de Bogotá (43%), Antioquia (25%), Valle (10%), Nariño (9%) y otros departamentos (7%). La caracterización genotípica agrupó los aislamientos evaluados en 167 patrones de PFGE; los perfiles más frecuentes se presentaron en productos lácteos, cárnicos y alimentos preparados. Conclusión. El 96,1 % de los aislamientos correspondieron a L. monocytogenes, con una buena concordancia entre el aislamiento y la identificación; el serotipo 4b, extremadamente virulento, fue el más frecuente. El análisis molecular evidenció la posible diseminación y permanencia en el tiempo de varios serotipos, lo que resalta la importancia de incluir este patógeno en los programas de vigilancia epidemiológica en alimentos.


Abstract | Introduction: Listeria monocytogenes is a food-borne pathogen that may cause infections in humans such as meningitis, meningoencephalitis, and septicemia, as well as abortions. By serological typing 13 serotypes have been identified of which 4b is responsible for most of the outbreaks in the world. Objective: To determine the frequency and distribution of serotypes and molecular subtypes of L. monocytogenes isolated in Colombia from food from 2010 to 2018. Materials and methods: We conducted a retrospective and descriptive study based on the analysis of 2,420 isolates confirmed as L. monocytogenes and other species using biochemical and serological tests, and pulsed-field gel electrophoresis (PFGE) for molecular subtyping. Results: Of the 2,420 isolates received, 2,326 were confirmed as L. monocytogenes. The serotypes found were 4b (52%), 4d-4e (14.5%), 1/2a (11%), 1/2c (9.4%), 1/2b (9%), and 3a, 3b, 3c, 4c, 4d, 4e and 7 (less than 2%). The isolates came from Bogotá (43%), Antioquia (25%), Valle (10%), Nariño (9%), and other departments (7%). The genotypic characterization grouped the isolates in 167 PFGE patterns. The most frequent patterns were identified in various dairy and meat products, and in prepared foods. Conclusion: A 96.1% of the isolates corresponded to L. monocytogenes showing good agreement between isolates and identification. Serotype 4b, highly virulent, was the most frequent. The molecular analysis showed the possible dissemination and permanence over time of several serotypes, which highlights the importance of including this pathogen in epidemiological food surveillance programs.

Little is known about the genetic diversity of Salmonella enterica serovar Typhi (S. Typhi) circulating in Latin America. It has been observed that typhoid fever is still endemic in this part of the world; however, a lack of standardized blood culture surveillance across Latin American makes estimating the true disease burden problematic. The Colombian National Health Service established a surveillance system for tracking bacterial pathogens, including S. Typhi, in 2006. Here, we characterized 77 representative Colombian S. Typhi isolates collected between 1997 and 2018 using pulse field gel electrophoresis (PFGE; the accepted genotyping method in Latin America) and whole genome sequencing (WGS). We found that the main S. Typhi clades circulating in Colombia were clades 2.5 and 3.5. Notably, the sequenced S. Typhi isolates from Colombia were closely related in a global phylogeny. Consequently, these data suggest that these are endemic clades circulating in Colombia. We found that AMR in S. Typhi in Colombia was uncommon, with a small subset of organisms exhibiting mutations associated with reduced susceptibility to fluoroquinolones. This is the first time that S. Typhi isolated from Colombia have been characterized by WGS, and after comparing these data with those generated using PFGE, we conclude that PFGE is unsuitable for tracking S. Typhi clones and mapping transmission. The genetic diversity of pathogens such as S. Typhi is limited in Latin America and should be targeted for future surveillance studies incorporating WGS.

Introduction: Salmonella enterica subsp. enterica serovar Give is found in ruminants, pigs, poultry, and aquatic environments, but rarely in humans. In Colombia, this serotype was ranked 11th. in the laboratory surveillance of acute diarrheal disease between 2000 and 2013. Objective: To characterize phenotypic and genotypic isolates of Salmonella related to an outbreak of foodborne Illness in the department Vichada in the fifth epidemiological week of 2015. Materials and methods: Following the Instituto Nacional de Salud method, we tested 37 fecal samples for Salmonella spp. while the sample of canned sardines was processed according to the ISO 6579:2002 Cor.1:2004 standard. The isolates were confirmed by serology and/or real-time PCR, antimicrobial susceptibility tests, and pulsed-field gel electrophoresis with the XbaI and BlnI enzymes. Results: All human isolates (11) and that from food (1) were identified as S. Give. The food isolate exhibited tetracycline resistance. PFGE analysis with XbaI grouped ten isolates from samples of human origin in pattern COIN15JEXX01.0005 and the remaining isolates in COIN15JEXX01.0006 with 96.3% similarity. All isolates were confirmed with the BlnI enzyme, and four (three human isolates and the one from food) were matched to the pattern COIN15JEXA26.002 with 95.65% similarity. Conclusion: Our study confirmed that canned sardines were related to the transmission of S. Give in the outbreak, which is the third one caused by this serotype in Colombia.

Resultados. Todos los aislamientos de origen humano (11) y el aislamiento del alimento (1), se identificaron como S. Give y este último presentó resistencia a la tetraciclina. El análisis por PFGE-XbaI agrupó bajo el patrón COIN15JEXX01.0005 diez aislamientos de origen humano y a los restantes bajo el COIN15JEXX01.0006, con un 96,3 % de similitud. Los resultados de todos los aislamientos se confirmaron con la enzima BlnI; cuatro de ellos (tres humanos y el del alimento) se agruparon bajo el patrón COIN15JEXA26.002, con un porcentaje de similitud del 95,65 %. Conclusión. El estudio confirmó que las sardinas enlatadas se relacionaron con la transmisión de S. Give en el brote, que es el tercero ocasionado por este serotipo en Colombia.

INTRODUCTION: Shigellosis is endemic in low-and middle-income countries, causing approximately 125 million episodes of diarrhea and leading to approximately 160 .000 deaths annually one-third of which is associated with children. OBJECTIVE: To describe the characteristics and antimicrobial resistance profiles of Shigella species recovered in Colombia from 1997 to 2018. MATERIALS AND METHODS: We received isolates from laboratories in 29 Colombian departments. We serotyped with specific antiserum and determined antimicrobial resistance and minimal inhibitory concentrations for ten antibiotics with Kirby-Bauer tests following the Clinical and Laboratory Standards Institute recommendations. RESULTS: We analyzed 5,251 isolates of Shigella spp., most of them obtained from stools (96.4%); 2,511 (47.8%) were from children under five years of age. The two most common species were S. sonnei (55.1%) and S. flexneri (41.7%). The highest resistance rate was that of tetracycline (88.1%) followed by trimethoprim-sulfamethoxazole (79.3%) and ampicillin (65.5%); 50.8% of isolates were resistant to chloramphenicol, 43.6% to amoxicillin/clavulanic acid, and less than 1% to cefotaxime, ceftazidime, gentamicin, and ciprofloxacin. In S. sonnei, the most common resistance profile corresponded to trimethoprim-sulfamethoxazole (92%) whereas in S. flexneri the most common antibiotic profiles were multidrug resistance. CONCLUSIONS: In Colombia, children under five years are affected by all Shigella species. These findings should guide funders and public health officials to make evidence based decisions for protection and prevention measures. The antimicrobial resistance characteristics found in this study underline the importance of combating the dissemination of the most frequently isolated species, S. sonnei and S. flexneri.

La shigelosis es endémica en los países de ingresos bajos y medios y ocasiona aproximadamente 125 millones de episodios de diarrea y 160.000 muertes al año, un tercio de los cuales se presenta en niños. Objetivo. Describir las características y los perfiles de resistencia antimicrobiana en aislamientos de Shigella spp. recuperados en Colombia entre 1997 y 2018. Materiales y métodos. Los aislamientos provenían de laboratorios en 29 departamentos de Colombia. La serotipificación se hizo con antisueros específicos de Shigella spp. y, la determinación de los perfiles de resistencia y la concentración inhibitoria mínima de diez antibióticos, por Kirby-Bauer. Resultados. Se estudiaron 5.251 aislamientos de Shigella spp. obtenidos de materia fecal (96,4 %); el 47,8 % de ellos correspondía a niños menores de cinco años. Las especies más frecuentes fueron S. sonnei (55,1 %) y S. flexneri (41,7 %). Se presentó resistencia a tetraciclina (88,1 %), trimetoprim-sulfametoxasol (79,3 %), ampicilina (65,5 %), cloranfenicol (50,8 %) y amoxicilina-acido clavulánico (43,6 %). La resistencia no superó el 1 % contra cefotaxime, ceftazidima, gentamicina y ciprofloxacina. Para S. sonnei, el perfil de resistencia más frecuente correspondió a trimetoprim-sulfametoxasol, en contraste con S. flexneri, cuyos perfiles fueron todos multirresistentes. Conclusiones. Los niños menores de cinco años se vieron afectados por todas las especies de Shigella spp., aspecto que los legisladores en salud pública deben considerar a la hora de tomar decisiones en torno a las medidas de prevención y protección frente a esta enfermedad. Las características de resistencia antimicrobiana de los aislamientos de Shigella spp. en Colombia ponen de manifiesto la importancia de combatir la diseminación de las dos especies más frecuentes en casos clínicos, S. sonnei y S. flexneri.

Los resultados de todos los aislamientos se confirmaron con la enzima BlnI; cuatro de ellos (tres humanos y el del alimento) se agruparon bajo el patrón COIN15JEXA26.002, con un porcentaje de similitud del 95,65 %. Conclusión. El estudio confirmó que las sardinas enlatadas se relacionaron con la transmisión de S. Give en el brote, que es el tercero ocasionado por este serotipo en Colombia.


Abstract | Introduction: Salmonella enterica subsp. enterica serovar Give is found in ruminants, pigs, poultry, and aquatic environments, but rarely in humans. In Colombia, this serotype was ranked 11th. in the laboratory surveillance of acute diarrheal disease between 2000 and 2013. Objective: To characterize phenotypic and genotypic isolates of Salmonella related to an outbreak of foodborne Illness in the department Vichada in the fifth epidemiological week of 2015. Materials and methods: Following the Instituto Nacional de Salud method, we tested 37 fecal samples for Salmonella spp. while the sample of canned sardines was processed according to the ISO 6579:2002 Cor.1:2004 standard. The isolates were confirmed by serology and/or real-time PCR, antimicrobial susceptibility tests, and pulsed-field gel electrophoresis with the XbaI and BlnI enzymes. Results: All human isolates (11) and that from food (1) were identified as S. Give. The food isolate exhibited tetracycline resistance. PFGE analysis with XbaI grouped ten isolates from samples of human origin in pattern COIN15JEXX01.0005 and the remaining isolates in COIN15JEXX01.0006 with 96.3% similarity. All isolates were confirmed with the BlnI enzyme, and four (three human isolates and the one from food) were matched to the pattern COIN15JEXA26.002 with 95.65% similarity. Conclusion: Our study confirmed that canned sardines were related to the transmission of S. Give in the outbreak, which is the third one caused by this serotype in Colombia.

Abstract | Introduction: Shigellosis is endemic in low-and middle-income countries, causing approximately 125 million episodes of diarrhea and leading to approximately 160.000 deaths annually one-third of which is associated with children. Objective: To describe the characteristics and antimicrobial resistance profiles of Shigella species recovered in Colombia from 1997 to 2018. Materials and methods: We received isolates from laboratories in 29 Colombian departments. We serotyped with specific antiserum and determined antimicrobial resistance and minimal inhibitory concentrations for ten antibiotics with Kirby-Bauer tests following the Clinical and Laboratory Standards Institute recommendations. Results: We analyzed 5,251 isolates of Shigella spp., most of them obtained from stools (96.4%); 2,511 (47.8%) were from children under five years of age. The two most common species were S. sonnei (55.1%) and S. fbxneri (41.7%). The highest resistance rate was that of tetracycline (88.1%) followed by trimethoprim-sulfamethoxazole (79.3%) and ampicillin (65.5%); 50.8% of isolates were resistant to chloramphenicol, 43.6% to amoxicillin/clavulanic acid, and less than 1% to cefotaxime, ceftazidime, gentamicin, and ciprofloxacin. In S. sonnei, the most common resistance profile corresponded to trimethoprim-sulfamethoxazole (92%) whereas in S. fbxneri the most common antibiotic profiles were multidrug resistance. Conclusions. In Colombia, children under five years are affected by all Shigella species. These findings should guide funders and public health officials to make evidence-based decisions for protection and prevention measures. The antimicrobial resistance characteristics found in this study underline the importance of combating the dissemination of the most frequently isolated species, S. sonnei and S. ftexneri.


Resumen | Introducción. La shigelosis es endémica en los países de ingresos bajos y medios y ocasiona aproximadamente 125 millones de episodios de diarrea y 160.000 muertes al año, un tercio de los cuales se presenta en niños. Objetivo. Describir las características y los perfiles de resistencia antimicrobiana en aislamientos de Shigella spp. recuperados en Colombia entre 1997 y 2018. Materiales y métodos. Los aislamientos provenían de laboratorios en 29 departamentos de Colombia. La serotipificación se hizo con antisueros específicos de Shigella spp. y, la determinación de los perfiles de resistencia y la concentración inhibitoria mínima de diez antibióticos, por Kirby-Bauer. Resultados. Se estudiaron 5.251 aislamientos de Shigella spp. obtenidos de materia fecal (96,4 %); el 47,8 % de ellos correspondía a niños menores de cinco años. Las especies más frecuentes fueron S. sonnei (55,1 %) y S. ftexneri (41,7 %). Se presentó resistencia a tetraciclina (88,1 %), trimetoprim-sulfametoxasol (79,3 %), ampicilina (65,5 %), cloranfenicol (50,8 %) y amoxicilina-acido clavulánico (43,6 %). La resistencia no superó el 1 % contra cefotaxime, ceftazidima, gentamicina y ciprofloxacina. Para S. sonnei, el perfil de resistencia más frecuente correspondió a trimetoprim-sulfametoxasol, en contraste con S. ftexneri, cuyos perfiles fueron todos multirresistentes. Conclusiones. Los niños menores de cinco años se vieron afectados por todas las especies de Shigella spp., aspecto que los legisladores en salud pública deben considerar a la hora de tomar decisiones en torno a las medidas de prevención y protección frente a esta enfermedad. Las características de resistencia antimicrobiana de los aislamientos de Shigella spp. en Colombia ponen de manifiesto la importancia de combatir la diseminación de las dos especies más frecuentes en casos clínicos, S. sonnei y S. ftexneri.

INTRODUCTION: Malnutrition is common in haemodialysis patients and closely related to morbidity and mortality. We evaluated the effect of twelve weeks of supplementation with resveratrol and curcumin on recovery of bone and muscle mass and protein oxidation, lipid peroxidation on patients with chronic kidney disease and iron overload undergoing hemodialysis, we performed a randomized, double-blind, placebo-controlled trial. METHODS: We included a total of 40 patients, were randomly assigned to two groups, 20 to the group with antioxidant supplementation (Resveratrol + Curcumin) (Group A), treated with a daily oral dose of 500 mg of Resveratrol and 500 mg of Curcumin, and 20 to the control group treated with placebo (Group B). RESULTS: Significant differences were found in the body composition of the patients between both groups. There was a significant difference in Body Mass Index (BMI) values (p = 0.002), fat percentage (p = 0.007), muscle mass (p = 0.01) bone mass (p = 0.01), as well as in the score of the subjective global evaluation (p = 0.03). Also differences were found between the basal and final serum levels of Triglycerides (TG) (p = 0.01), VLDL (p = 0.003). A significant decrease in the levels of serum ferritin (2003.69 ± 518.73 vs 1795.65 ± 519.00 ng/mL; p = 0.04). Nor were significant differences observed between the baseline and the final Thiobarbituric Acid Reactive Substances (TBARS) values (70.45 ± 69.21 vs 50.19 ± 32.62, p = 0.24). The same results was obtained for carbonyl values (2.67 ± 0.75 vs 2.50 ± 0.85; p = 0.50). DISCUSSION: The present study is the first assay on patients with chronic kidney disease and iron overload that demonstrates the beneficial effects of combined supplementation with Curcumin and Resveratrol on muscle and bone mass. There was a significant decrease in circulating levels of ferritin, to finding that remarkably novel.

Prostate cancer is initially treated via androgen deprivation therapy (ADT), a highly successful treatment in the initial pursuit of tumor regression, but commonly restricted by the eventual emergence of a more lethal 'castrate resistant' form of the disease. Intracrine pathways that utilize dehydroepiandrosterone (DHEA) or other circulatory precursor steroids, are thought to generate relevant levels of growth-stimulating androgens such as testosterone (T) and dihydrotestosterone (DHT). In this study, we explored the capacity of the active vitamin D hormone to interact and elicit changes upon this prostatic intracrine pathway at a metabolic level. We used androgen dependent LNCaP cells cultured under steroid-depleted conditions and assessed the impact of vitamin D-based compounds upon intracrine pathways that convert exogenously added DHEA to relevant metabolites, through Mass Spectrometry (MS). Changes in relevant metabolism-related gene targets were also assessed. Our findings confirm that exposure to vitamin D based compounds, within LNCaP cells, elicits measurable and significant reduction in the intracrine conversion of DHEA to T, DHT and other intermediate metabolites within the androgenic pathway. The aassessment and validation of the biological model and analytical platforms were performed by pharmacological manipulations of the SRD5α and HSD-17ß enzymes. The data provides further confirmation for how a vitamin D-based regime may be used to counter intracrine mechanisms contributing to the emergence of castrate-resistant tumors.

Introducción: Salmonella spp. es un enteropatógeno que se transmite a los humanos a través de alimentos o agua contaminada. En 1997, el Grupo de Microbiología del Instituto Nacional de Salud de Colombia inició el programa de vigilancia de enfermedad diarreica aguda y fiebre tifoidea, que incluye Salmonella spp. Este informe presenta los resultados fenotípicos y genotípicos de los aislamientos recuperados de 2005 a 2011 como parte de la vigilancia. Métodos: Un total de 4.010 aislamientos de Salmonella spp. fueron analizados por serotipificación con el esquema Kauffmann-White-LeMinor, patrones de sensibilidad antimicrobiana y de electroforesis en gel de campos pulsados (PFGE). Resultados: Se identificaron un total de 93 serovares, con 9 predominantes, Typhimurium, Enteritidis, Typhi, Dublin, Panama, Derby, Braenderup, Saintpaul y Uganda. Salmonella spp. presentó altos porcentajes de resistencia a tetraciclina y ácido nalidíxico. El 52,4% (2.101/4.010) de los aislamientos fueron sensibles a todos los antibióticos. La multirresistencia se observó en el 54,9% de los aislamientos de Typhimurium, representada por 81 combinaciones. Por PFGE se analizaron 51,9% aislamientos (2.083/4.010) de 34 serovares, generando 828 patrones electroforéticos XbaI. De estos, 8 se reportaron en al menos 2 países en Latinoamérica. Conclusión: La vigilancia de Salmonella spp. permite conocer la distribución de los serovares, su resistencia y la identificación de clones endémicos en Colombia, aportando bases para un tratamiento óptimo en las infecciones generadas por este patógeno y en el diseño de programas para disminuir la dispersión de aislamientos multirresistentes (AU)

AIM: To assess the relationship of brain-derived neurotrophic factor (BDNF) with cognitive impairment in patients with type 2 diabetes. METHODS: The study included 40 patients with diabetes mellitus type 2 (DM2), 37 patients with chronic kidney disease in hem dialysis hemodialysis therapy (HD) and 40 healthy subjects. BDNF in serum was quantified by ELISA. The Folstein Mini-Mental State Examination was used to evaluate cognitive impairment. RESULTS: The patients with DM2 and the patients in HD were categorized into two groups, with cognitive impairment and without cognitive impairment. The levels of BDNF showed significant differences between patients with DM2 (43.78 ± 9.05 vs 31.55 ± 10.24, P = 0.005). There were no differences between patients in HD (11.39 ± 8.87 vs 11.11 ± 10.64 P = 0.77); interestingly, ferritin levels were higher in patients with cognitive impairment (1564 ± 1335 vs 664 ± 484 P = 0.001). The comparison of BDNF values, using a Kruskal Wallis test, between patients with DM2, in HD and healthy controls showed statistical differences (P < 0.001). CONCLUSION: Low levels of BDNF are associated with cognitive impairment in patients with DM2. The decrease of BDNF occurs early and progressively in patients in HD.

Background. Increased oxidative stress is a well described feature of patients in hemodialysis. Their need for multiple blood transfusions and supplemental iron causes a significant iron overload that has recently been associated with increased oxidation of polyunsaturated lipids and accelerated aging due to DNA damage caused by telomere shortening. Methods. A total of 70 patients were evaluated concomitantly, 35 volunteers with ferritin levels below 500 ng/mL (Group A) and 35 volunteers with ferritin levels higher than 500 ng/mL (Group B). A sample of venous blood was taken to extract DNA from leukocytes and to measure relative telomere length by real-time PCR. Results. Patients in Group B had significantly higher plasma TBARS (p = 0.008), carbonyls (p = 0.0004), and urea (p = 0.02) compared with those in Group A. Telomeres were significantly shorter in Group B, 0.66 (SD, 0.051), compared with 0.75 (SD, 0.155) in Group A (p = 0.0017). We observed a statistically significant association between relative telomere length and ferritin levels (r = -0.37, p = 0.001). Relative telomere length was inversely related to time on hemodialysis (r = -0.27, p = 0.02). Conclusions. Our findings demonstrate that iron overload was associated with increased levels of oxidative stress and shorter relative telomere length.

UNLABELLED: Introduction : One of the major worldwide public health problems today are the infections caused by carbapenem-resistant Enterobacteriaceae (CRE), among which carbapenem-resistant Klebsiella pneumoniae (CRKP), constitutes one of the most common pathogens causing nosocomial infection. OBJECTIVE: This study was aimed at describing the dissemination of KPC-3 enzyme-producing Klebsiella pneumoniae in clinical isolates from hospitals in Bogotá. MATERIALS AND METHODS: Eighty-two CRKP isolates collected from 10 hospitals in Bogotá from 2008-2010 were analysed; disk diffusion and microdilution were used for phenotypic detection of enzymes and PCR for genotyping. Automated and manual methods were used for determining profiles for antimicrobial susceptibility testing (AST) with 13 agents. PFGE was used for obtaining the isolates´ genetic relationship. RESULTS: This study gives an overview of CRKP patterns in 10 hospitals in Bogota which were found to present resistance to multiple antibiotic families. The CRKPs were grouped in different clones, each having different subtypes, and were spread in the 10 hospitals over the three-year period (2008-2010). CONCLUSIONS: The dissemination of KPC-3-producing Klebsiella pneumoniae nosocomial isolates in Bogota highlights the need for strengthening epidemiological surveillance against this type of microorganism and the development of specific priority activities for preventing and controlling such infection.

BACKGROUND: A contributing factor to the emergence of castrate resistant prostate cancer (CRPC) is the ability of the tumor to circumvent low circulating levels of testosterone during androgen deprivation therapy (ADT), through the production of "intracrine" tumoral androgens from precursors including cholesterol and dehydroepiandrosterone (DHEA). As these processes promote AR signaling and prostate cancer progression their modulation is required for disease prevention and treatment. METHODS: We evaluated the involvement of the vitamin D receptor ligand EB1089 in the regulation of genes with a role in androgen metabolism using the androgen dependent cell lines LNCaP and LAPC-4. EB1089 regulation of androgen metabolism was assessed using QRT-PCR, luciferase promoter assays, western blotting, enzyme activity assays, and LC-MS analyses. RESULTS: EB1089 induced significant expression of genes involved in androgen metabolism in prostate cancer cells. Real-Time PCR analysis revealed that VDR mediated significant regulation of CYP3A4, CYP3A5, CYP3A43, AKR1C1-3, UGT2B15/17, and HSD17B2. Data revealed potent regulation of CYP3A4 at the level of mRNA, protein expression and enzymatic activity, with VDR identified as the predominant regulator. Inhibition of CYP3A activity using the specific inhibitor ritonavir resulted in alleviation of the anti-proliferative response of VDR ligands in prostate cancer cells. Mass spectrometry revealed that overexpression of CYP3A protein in prostate cancer cells resulted in a significant increase in the oxidative inactivation of testosterone and DHEA to their 6-ß-hydroxy-testosterone and 16-α-hydroxy-DHEA metabolites, respectively. CONCLUSIONS: These data highlight a potential application of VDR-based therapies for the reduction of growth-promoting androgens within the tumor micro-environment.

Introducción. Las enfermedades transmitidas por alimentos constituyen un problema sanitario de alto impacto. Listeria monocytogenes se asocia frecuentemente con contaminación de alimentos lácteos y cárnicos. Vigilar su presencia en varios puntos de esta producción, es vital para controlar la diseminación del microorganismo. Objetivo. Determinar la prevalencia de L. monocytogenes en manipuladores de alimentos de lácteos y cárnicos en 10 departamentos de Colombia y buscar la asociación entre la presencia del microorganismo y posibles factores de riesgo. Materiales y métodos. Se llevó a cabo un estudio descriptivo de corte transversal, en el cual se determinó la presencia de L. monocytogenes en muestras de materia fecal y frotis de manos, provenientes de 1.322 manipuladores de alimentos. Se aplicó un cuestionario para conocer posibles factores de riesgo, y se llevó a cabo un análisis estadístico para determinar frecuencias y establecer relaciones entre los posibles factores de riesgo y el estado del portador. Resultados. Hubo 138 (10,4 %) manipuladores de alimentos que resultaron positivos para L. monocytogenes y se encontró asociación estadísticamente significativa entre la presencia del microorganismo y "no conocer el concepto de contaminación cruzada" (OR (IC95%=1,518; p=0,004), así como con "no practicar procedimientos de limpieza y desinfección adecuados" (OR (IC95%)=1,292; p=0,005). Conclusión. Se estableció una prevalencia de portadores de L. monocytogenes entre manipuladores de alimentos de derivados lácteos y cárnicos, y se logró asociar las prácticas higiénicas de los manipuladores de alimentos como factores de riesgo. El estudio se convierte en una herramienta útil para la vigilancia, la caracterización epidemiológica y el planteamiento de estrategias de control de este microorganismo.


Introduction: Foodborne diseases are a high-impact health problem. Listeria monocytogenes is frequently associated with contaminated meat and dairy foods. Monitoring their presence at various stages during production is important to control the spread of this microorganism. Objective: To determine the prevalence of L. monocytogenes in meat and dairy food handlers in ten departments of Colombia and to find an association between the presence of this microorganism and possible risk factors. Materials and methods: A cross sectional study was carried out to determine the presence of L. monocytogenes in fecal samples and swabs taken from the hands of 1322 food handlers. A questionnaire was applied to determine possible risk factors and a statistical analysis was carried out to determine frequencies and relationships between potential risk factors and the presence of L. monocytogenes. Results: 138 (10.4%) food handlers were positive for L. monocytogenes and a statistically significant association was found between the presence of this microorganism and the following risk factors: "Lack of knowledge about the concept of cross-contamination" (OR ( CI 95%) = 1.518 P = 0.004), and "Not practicing cleaning and disinfection procedures appropriately" (OR ( CI 95%) = 1.292 P = 0.005) Conclusion: Prevalence of L. monocytogenes carriers was established among meat and dairy food handlers and food handlers practices were associated as risk factors for carrier condition. This study is a useful tool for the monitoring, epidemiological characterization and definition of strategies to control this pathogen.

INTRODUCTION: Salmonella Enteritidis is recognized worldwide as one of the main agents of human gastrointestinal infection. Several reports indicate the presence of isolates with decreased sensitivity to ciprofloxacin that can lead to a delayed response or the development of resistance during treatment. OBJECTIVE: To describe and characterize isolates of Salmonella Enteritidis associated to an outbreak of food-borne diseases in Popayán, Cauca. MATERIALS AND METHODS: Ten Salmonella Enteritidis isolates from nine patients and one food sample (chicken sandwich) were analyzed by biochemical tests, serotyping and antimicrobial sensitivity. The minimum inhibitory concentration to ciprofloxacin was determined by E-test and the genetic profile of the isolates was tested by pulsed field gel electrophoresis (PFGE) with XbaI and Blnl enzymes. RESULTS: Salmonella Enteritidis was identified in all isolates. They were resistant to nalidixic acid and had a decreased sensitivity to ciprofloxaxin between 0.25 and 0.5 µg/ml; all isolates were sensitive to all the other antimicrobials we tested. Ten isolates were grouped by PFGE with the XbaI enzyme in the COIN11.JEG.X01.0038 pattern, and seven isolates were confirmed with the BlnI enzyme using the COIN11.JEG.A26.0009 pattern. CONCLUSION: We report for the first time an outbreak of nalidixic acid-resistant Salmonella Enteritidis in Colombia and confirmed by phenotypic and genotypic analysis the association between the isolates from patients and the chicken sandwich as the source of infection.

Introducción. Salmonella Enteritidis es reconocida a nivel mundial como uno de los principales agentes de infección gastrointestinal. Varios reportes indican la presencia de aislamientos con sensibilidad disminuida a la ciprofloxacina que puede conllevar a una respuesta retardada o al desarrollo de resistencia durante el tratamiento. Objetivo. Describir y caracterizar los aislamientos de Salmonella Enteritidis asociados a un brote de enfermedad transmitida por alimentos en Popayán, Cauca. Materiales y métodos. Se analizaron 10 aislamientos de Salmonella Enteritidis, nueve de pacientes y uno de alimentos (emparedado de pollo), por pruebas bioquímicas, serotipificación y sensibilidad antimicrobiana. La concentración inhibitoria mínima a la ciprofloxacina se determinó por E-test y el perfil genético de los aislamientos se evaluó por electroforesis en gel de campo pulsado (Pulsed Field Gel Electrophoresis, PFGE) con las enzimas Xbal y Blnl. Resultados. En todos los aislamientos se identificó Salmonella Enteritidis con resistencia al ácido nalidíxico y sensibilidad disminuida a la ciprofloxaxina entre 0,25 y 0,5 µg/ml; todos fueron sensibles a los demás antimicrobianos ensayados. La PFGE agrupó los 10 aislamientos con la enzima Xbal en el patrón COIN11.JEG.X01.0038 y siete aislamientos se confirmaron con la enzima BlnI con el patrón COIN11.JEG.A26.0009. Conclusión. Se reporta por primera vez en Colombia un brote de Salmonella Enteritidis con resistencia al ácido nalidíxico y se confirma por análisis fenotípico y genotípico la asociación entre los aislamientos de los pacientes con el del emparedado de pollo como la fuente de infección.
