RESUMO

Stereotactic body radiotherapy has experienced exponential development in recent years, as its ablative capacity has demonstrated a benefit in certain patients including oligometastatic and oligoprogressive patients. We present a case of a 59-year-old woman with a history of infiltrating metastatic ductal breast carcinoma with vertebral bone oligoprogression. After radical surgical treatment and re-irradiation SBRT, a complete response was achieved. After more than two years of follow-up the patient remains with neither local nor distant recurrence. Patients with oligometastatic or oligoprogressive breast cancer are candidates for radical treatment modalities. Stereotactic body radiotherapy has demonstrated promising local control, progression free survival and overall survival in these patients. In this context, there are currently ongoing phase III studies in order to provide stronger evidence. (AU)


La radioterapia estereotáctica corporal ha sufrido un desarrollo exponencial en los últimos años. Debido a su capacidad ablativa, esta técnica ha demostrado un beneficio en determinados pacientes incluyendo los pacientes oligometastásicos o con oligoprogresión. Se presenta el caso de una mujer de 59 años con antecedentes de carcinoma ductal de mama infiltrante con oligoprogresión ósea vertebral. Tras tratamiento quirúrgico y reirradiación del lecho con SBRT se obtuvo una respuesta completa a nivel local. Tras más de dos años de seguimiento, la paciente permanece sin recidiva local ni a distancia. Las pacientes con cáncer de mama oligometastásico o con oligoprogresión son candidatas a recibir un tratamiento radical. La radioterapia corporal estereotáctica ha demostrado unas cifras de control local, supervivencia libre de progresión y supervivencia global prometedoras en estas pacientes. En este contexto, se están desarrollando ensayos fase III que aportarán una evidencia más sólida en el tratamiento de estas pacientes. (AU)

Background: Several immunotherapy (IT) agents are FDA approved for treatment of melanoma and non-small-cell lung cancer (NSCLC). The addition of stereotactic radiosurgery (SRS) or stereotactic body radiation therapy (SBRT) to immunotherapy looks promising. A systematic review was conducted to evaluate the possible synergistic effects of immune checkpoints inhibitors (ICIs) and stereotactic radiation therapy in melanoma and NSCLC. Materials and methods: Pubmed databases from January 2010 to December 2020 were reviewed to identify English language studies reporting control of local and abscopal effect of the combination of ICI-SBRT/SRS in metastatic NSCLC and melanoma cancer. The inclusion criteria were followed according to PICO criteria. Results: Thirty-nine articles were included of the 2141 initial results. The reported rates for local control were 16.5-100% and 40-94% in brain and extracerebral metastases, respectively. Distant/abscopal response rates were 1-45% in extracerebral metastases. Abscopal effect could not be evaluated in brain metastases because it was not reported in studies. Treatments were well tolerated with few grade 4 toxicities and no grade 5. Conclusions: The combined treatment of ICI-SBRT/SRS achieves high local control and non-negligible abscopal response in patients with extracerebral metastases, with its benefit in cerebral metastases being more controversial. Clinical trials are needed to better characterize the potential synergism.

The aims of this study were to investigate changes in energy balance-associated metabolites associated with radiotherapy in patients with breast cancer, and to relate these changes to the clinical and pathological response-to-treatment. We studied 151 women with breast cancer who received radiotherapy following surgical excision of the tumor. Blood was obtained before and after the irradiation procedure. The control group was composed of 44 healthy women with a similar age distribution to that of the patients. We analyzed the concentrations of metabolites involved in glycolysis, citric acid cycle and amino acid metabolism using targeted quantitative metabolomics. Post-surgery, pre-radiotherapy, patients had major alterations in the serum concentrations of products of glycolysis, citric acid cycle and amino acid metabolism. The strongest alterations were decreases in serine, leucine and isoleucine concentrations. Alterations in metabolite levels were partially, or totally, reversed after irradiation; the concentrations of serine, leucine and isoleucine approached equivalence to those of the control group. Estrogen receptor-positive patients were those with lower concentrations, while triple negative patients had higher concentrations of these amino acids. The normalization of the amino acids serine, leucine and isoleucine concentrations could be clinically relevant because the normalization of these energy-balance metabolites would suggest that residual micro-metastatic disease had been effectively diminished by the radiotherapy, and may be an indicator of its efficacy.

The study of cryptic species allows to describe and to understand biodiversity, and the evolutionary processes shaping it. Mites of the family Rhinonyssidae are permanent parasites of the nasal cavities of birds, currently including about 500 described species and 12 genera. Here, we tested the hypothesis that mites from five populations of the genus Tinaminyssus-three isolated from European turtle doves (Streptopelia turtur), and two from Eurasian collared doves (Streptopelia decaocto; Aves: Columbiformes)-are, in fact, two cryptic species inhabiting different hosts. First, we performed a morphometrical study on 16 traits. Then, we used the ITS1-5.8S rDNA-ITS2 nuclear region (ITS region), and a fragment of the mitochondrial cytochrome c-oxidase 1 (COI) to carry out phylogenetic and species delimitation analyses on Tinaminyssus species. Morphological analyses revealed a lack of biometric differentiation among Tinaminyssus populations from the two host species. However, molecular analyses indicated a high degree of genetic differentiation between populations of Tinaminyssus sp. from S. turtur and S. decaocto. Overall, results show that they can be considered as different cryptic species, suggesting a case of evolutionary stasis, likely because of the anatomical similarity between closely-related bird host species.
