Ethnopharmacological relevance: Serpylli herba extract (SHE), composed of the aerial parts of wild thyme (Thymus serpyllum L.) (Lamiaceae family), is traditionally used in Europe and North Africa to treat diarrhea, gastric ulcers, intestinal parasites and upper respiratory tract infections. Recently, SHE has generated a great interest for irritable bowel syndrome (IBS) management, probably due to its intestinal anti-inflammatory properties shown in experimental colitis and the fact that its active components could preserve the intestinal barrier integrity, which is altered in patients with IBS. Aim of study: We aimed to test the effects of a SHE in a rat experimental model resembling human IBS. Materials and methods: IBS was provoked by deoxycholic acid (DCA). Rats were then treated with SHE (100 mg/kg) or gabapentin (70 mg/kg) and different inflammatory and gut barrier integrity markers were evaluated. Moreover, several gut hypersensitivity and hyperalgesia determinations were performed. Results: SHE improved referred pain and visceral hypersensitivity. Additionally, SHE enhanced immune status by downregulating of the expression of the pro-inflammatory mediators Il-1ß, Il-6, Ifn-γ, Tlr-4, and the inducible enzyme Cox-2, thus inducing visceral analgesia, and promoting the restore of the gut barrier function by upregulating the mucins Muc-2 and Muc-3. These anti-inflammatory effects could be related to its action on mast cells since it significantly inhibited the ß-Hexosaminidase production in RBL-2H3 cells. Lastly, SHE also seems to modulate the serotonin pathway by restoring the altered expression of the 5-HT receptors Htr-3 and Htr-4. Conclusion: SHE could be considered a potential new treatment for IBS, since it ameliorates hypersensitivity, visceral hyperalgesia, and inflammation. These beneficial effects may be due to the inhibition of mast cells degranulation and serotonin pathway.

INTRODUCTION: Cerebral swelling often occurs during craniotomy for cerebral tumors. Poor brain relaxation can increase the risk of cerebral ischemia, possibly worsening the outcome. The surgical team should identify any risk factors that could cause perioperative brain swelling and decide which therapies are indicated for improving it. The present investigation aimed to elucidate the risk factors associated with brain swelling during elective craniotomy for supratentorial brain tumors. METHODS: This prospective, nonrandomized, observational study included 52 patients scheduled for elective supratentorial tumor surgery. The degree of brain relaxation was classified upon the opening of the dura according to a four-point scale (brain relaxation score: 1, perfectly relaxed; 2, satisfactorily relaxed; 3, firm brain; and 4, bulging brain). Moreover, hemodynamic and respiratory parameters, arterial blood gas, and plasma osmolality were recorded after the removal of the bone flap. RESULTS: This study showed that the use of preoperative dexamethasone was associated with a brain relaxation score of ≤2 (p = 0.005). The median midline shift of 6 (3-0) mm and median hemoglobin level of >13 g/dL were associated with a brain relaxation score of ≥3 (p = 0.02 and p = 0.01, respectively). The dosage of mannitol (0.25 g/kg versus 0.5 g/kg), physical status, intraoperative position, tumor diameter and volume, peritumoral edema and mass effect, World Health Organization (WHO) grading, mean arterial pressure, PaCO2, osmolality, and core temperature were not identified as risk factors associated with poor relaxation. CONCLUSION: The use of preoperative dexamethasone was associated with improved brain relaxation, whereas the presence of a preoperative midline shift and a higher level of hemoglobin were associated with poor brain relaxation.

Introduction:Statins are used with the understanding that a slightly increased risk of diabetes is outweighed by their cardiovascular benefits. However, it may be necessary to reconsider whether statin therapy really increase this risk mainly in the population with prediabetes.Methods:A multicenter, cross-sectional, observational study was conducted to assess the relationship between statin therapy and glucose metabolism in 407 patients aged 63.1 years (11SD) diagnosed with dyslipidemia and prediabetes treated in specialized lipid clinics in Spain.Results:Significant differences were found in HbA1c values among treatment groups (p=0.015). Patients treated with pitavastatin (1–4mg/day) showed the lowest HbA1c levels, with significant differences compared to patients treated with atorvastatin 40–80mg/day (p=0.016) and simvastatin 10–40mg/day (p=0.036). By contrast, patients treated with atorvastatin 40–80mg/day showed the highest HbA1c levels compared to those receiving atorvastatin 10–20mg/day (p=0.003), pitavastatin 1–4mg/day (p=0.016), pravastatin 20–40mg/day (p=0.027), rosuvastatin 5–10mg/day (p=0.043), and no statin treatment (p=0.004). Patients treated with simvastatin 10–40mg/day also had higher values than those treated with atorvastatin 10–20mg/day (p=0.016) and pitavastatin 1–4mg/day (p=0.036) or with no statin treatment (p=0.018).Conclusions:This study suggests that there are differences in the diabetogenic effect of statins. Introducción:Las estatinas son utilizadas de acuerdo con el entendimiento de que el pequeño riesgo de incremento de diabetes se ve compensado por sus beneficios cardiovasculares. Sin embargo, puede resultar necesario reconsiderar si la terapia con estatinas incrementa realmente el riesgo, principalmente en la población con prediabetes.Métodos:Se realizó un estudio multicéntrico, transversal y observacional para evaluar la relación entre la terapia con estatinas y el metabolismo de la glucosa en 407 pacientes de 63,1 años (11 DE) diagnosticados de dislipidemia y prediabetes tratados en clínicas especializadas en lípidos en España.Resultados:Se encontraron diferencias significativas en los valores de HbA1c entre los grupos de tratamiento (p=0,015). Los pacientes tratados con pitavastatina (1-4mg/día) reflejaron los menores niveles de HbA1c, con diferencias significativas en comparación con los pacientes tratados con atorvastatina 40-80mg/día (p=0,016) y simvastatina 10-40mg/día (p=0,036). Por contra, los pacientes tratados con atorvastatina 40-80mg/día reflejaron los mayores niveles de HbA1c en comparación con los pacientes que recibieron atorvastatina 10-20mg/día (p=0,003), pitavastatina 1-4mg/día (p=0,016), pravastatina 20-40mg/día (p=0,027), rosuvastatina 5-10mg/día (p=0,043) y los que no recibieron estatinas (p=0,004). Los pacientes tratados con simvastatina 10-40mg/día tuvieron también valores más elevados que aquellos pacientes tratados con atorvastatina 10-20mg/día (p=0,016) y pitavastatina 1-4mg/día (p=0,036) que no recibieron estatinas (p=0,018).Conclusiones:El presente estudio sugiere que existen diferencias en cuanto al efecto diabetógeno de las estatinas. Simvastatina y las altas dosis de atorvastatina pueden guardar relación con un mayor deterioro del metabolismo de la glucosa que pitavastatina y demás estatinas con menor potencia de reducción de lípidos, tales como pravastatina. Limosilactobacillus fermentum CECT5716, a probiotic strain isolated from human milk, has reported beneficial effects on different gastrointestinal disorders. Moreover, it has shown its ability to restore altered immune responses, in association with microbiome modulation in different pathological conditions. Therefore, our aim was to assess the effects of a Limosilacbacillus fermentum CECT5716 in a rat experimental model of irritable bowel syndrome (IBS) that resembles human IBS. The experimental IBS was induced by deoxycholic acid (DCA) in rats and then, Limosilactobacillus fermentum CECT5716 (109 CFU/day/rat) was administered. Behavioral studies, hyperalgesia and intestinal hypersensitivity determinations were performed and the impact of the probiotic on the inflammatory and intestinal barrier integrity was evaluated. Additionally, the gut microbiota composition was analyzed. Limosilactobacillus fermentum CECT5716 attenuated the anxiety-like behavior as well as the visceral hypersensitivity and referred pain. Moreover, this probiotic ameliorated the gut inflammatory status, re-establishing the altered intestinal permeability, reducing the mast cell degranulation and re-establishing the gut dysbiosis in experimental IBS. Therefore, our results suggest a potential use of Limosilactobacillus fermentum CECT5716 in clinical practice for the management of IBS patients.

Objetivo: Analizar la evidencia científica en torno a la efectividad del modelo de cuidados sin sujeciones dirigido a las personas mayores que viven en residencias. Método: revisión narrativa a través de la búsqueda bibliográfica en bases de datos de ciencias de la salud: PubMed, Cuiden, la Biblioteca Cochrane Plus y Cochrane Library, CINAHL, la Biblioteca Virtual en Salud y en el buscador temático Dialnet. Resultados: Se seleccionaron 20 artículos que generaron cuatro categorías de análisis: justificación del modelo sin sujeciones físicas, percepción de los profesionales sobre el modelo de cuidados sin sujeciones físicas, influencia de los programas educativos dirigidos a profesionales para facilitar la reducción de sujeciones y valoración de los beneficios de un programa de eliminación de sujeciones físicas. Conclusiones: Las sujeciones físicas tienen importantes consecuencias negativas tanto físicas como psíquicas, repercutiendo en el bienestar, la autonomía y la calidad de vida de las personas mayores. Su uso se puede reducir o incluso eliminar con razonables niveles de seguridad en los centros residenciales, ya que su empleo no tiene cabida en la sociedad del siglo xxi (AU)


Objective: To analyse the scientific evidence regarding the effectiveness of a physical restraints free care model on institutionalized elderly. Method: this narrative review has been carried out in 6 databases related to health sciences (PubMed, Cuiden, the Cochrane Plus Library and the Cochrane Library, CINAHL, Biblioteca Virtual en Salud and Dialnet). Results: 20 studies were selected and 4 categories were established: justification of the physical restraints free care model, professional’s perception about the physical restraints free care model, educational intervention programs to professionals to reduce restraints, assessment of benefits of a physical restraints reduction program. Conclusions: physical restraints have important adverse effects on physical and mental health. They also have implications on welfare, autonomy and quality of life of residents. Its use can be reduced or even eliminated with reasonable levels of safety; therefore, the use of physical restraints is not acceptable nowadays in our society (AU)

Objetivo: Determinar a prevalência de parestesia do nervo alveolar inferior em exodontias dos terceiros molares mandibulares realizadas por estudantes de Odontologia. Material e métodos: O presente estudo retrospectivo incluiu os prontuários de 226 pacientes submetidos à exodontia dos terceiros molares mandibulares durante o período de julho de 2015 a agosto de 2017 por estudantes de Odontologia da Liga Acadêmica de Cirurgia da Universidade Federal de Campina Grande, Campus-Patos, Paraíba, Brasil. Resultados: Foram removidos 238 terceiros molares mandibulares de pacientes com faixa etária entre 16 a 42 anos, sendo a maioria dos pacientes do gênero feminino (71,68%). Nove dos pacientes (3,9%) relataram algum grau de comprometimento no território de inervação do nervo alveolar inferior com remissão total do sintoma no período de 3,7 meses. A prevalência da parestesia em relação ao número de terceiros molares inferiores extraídos foi de 1:25. Conclusão: A prevalência de parestesia decorrente da exodontia de terceiros molares mandibulares realizadas por estudantes de Odontologia é de 3,9%... (AU)


Objective: To define the prevalence of inferior alveolar nerve damage in mandibular third molar extractions performed by dentistry students. Materials and methods: The present retrospective study included the medical records of 226 patients submitted to mandibular third molar extraction during the period from July 2015 to August 2017 by students of Dentistry of the Academic League of Surgery of the Federal University of Campina Grande, Campus- Patos, Paraíba, Brazil. Results: 238 mandibular third molars were extracted from patients aged 16 to 42 years, with the majority of patients being females (71.68%). Nine of the patients (3.9%) reported some degree of involvement in the territory of innervation of the inferior alveolar nerve with total suspension of the symptom in the period of 3.7 months. The prevalence of damage in relation to the number of lower third molars extracted was 1:25. Haspin, a highly conserved kinase in eukaryotes, has been shown to be responsible for phosphorylation of histone H3 at threonine 3 (H3T3ph) during mitosis, in mammals and yeast. Here we report that haspin is the kinase that phosphorylates H3T3 in Drosophila melanogaster and it is involved in sister chromatid cohesion during mitosis. Our data reveal that haspin also phosphorylates H3T3 in interphase. H3T3ph localizes in broad silenced domains at heterochromatin and lamin-enriched euchromatic regions. Loss of haspin compromises insulator activity in enhancer-blocking assays and triggers a decrease in nuclear size that is accompanied by changes in nuclear envelope morphology. We show that haspin is a suppressor of position-effect variegation involved in heterochromatin organization. Our results also demonstrate that haspin is necessary for pairing-sensitive silencing and it is required for robust Polycomb-dependent homeotic gene silencing. Haspin associates with the cohesin complex in interphase, mediates Pds5 binding to chromatin and cooperates with Pds5-cohesin to modify Polycomb-dependent homeotic transformations. Therefore, this study uncovers an unanticipated role for haspin kinase in genome organization of interphase cells and demonstrates that haspin is required for homeotic gene regulation.

Dietary polyphenol intake is associated with improvement of metabolic disturbances. The aims of the present study are to describe dietary polyphenol intake in a population with metabolic syndrome (MetS) and to examine the association between polyphenol intake and the components of MetS. This cross-sectional analysis involved 6633 men and women included in the PREDIMED (PREvención con DIeta MEDiterranea-Plus) study. The polyphenol content of foods was estimated from the Phenol-Explorer 3.6 database. The mean of total polyphenol intake was 846 ± 318 mg/day. Except for stilbenes, women had higher polyphenol intake than men. Total polyphenol intake was higher in older participants (>70 years of age) compared to their younger counterparts. Participants with body mass index (BMI) >35 kg/m2 reported lower total polyphenol, flavonoid, and stilbene intake than those with lower BMI. Total polyphenol intake was not associated with a better profile concerning MetS components, except for high-density lipoprotein cholesterol (HDL-c), although stilbenes, lignans, and other polyphenols showed an inverse association with blood pressure, fasting plasma glucose, and triglycerides. A direct association with HDL-c was found for all subclasses except lignans and phenolic acids. To conclude, in participants with MetS, higher intake of several polyphenol subclasses was associated with a better profile of MetS components, especially HDL-c.

Introduction: Bisphosphonates consist of a range of drugs used in the treatment of osteopathy or some osteotropic malignancies. Medication-related osteonecrosis of the jaw (MRONJ) is a serious adverse consequence of their use. Conventional treatment is not always effective, so it is necessary to resort to the use of adjuvant therapies. This study aimed to evaluate the effectiveness of the association of surgery, antibiotic therapy and low-level laser (LLL) therapy as a treatment strategy for MRONJ through the presentation of a clinical case. Case Presentation: A 49-year-old female patient presented for the diagnosis and treatment of bone lesions in the maxillae. The patient reported that she had used zoledronate for 1 year. Five years later there were 2 painless bone lesions on both sides of the maxilla, following the extraction of the first upper molars. Clinical, tomographic and histopathologic examination established the diagnosis of MRONJ. The treatment consisted of the curettage of the necrotic bone, antibiotic and thirteen LLL therapy sessions. Integral mucosal healing was observed after a two-month follow-up and no symptoms were detected. The patient was evaluated at 6 and 12 postoperative months without complications. Conclusion: The combination of surgery, antibiotic therapy, and LLL therapy has shown to be effective in the treatment of MRONJ.

RESUMEN La evolución favorable de los pacientes afectados con dermatitis atópica está muy relacionada con un diagnóstico y orientación precoz en la atención primaria, para un seguimiento más especializado en las consultas de dermatología y alergología, por ser una entidad que ofrece dificultades en su identificación. Es necesario incrementar el nivel de información en los médicos de las áreas de salud, por ser estos escenarios donde ocurre el primer contacto con el paciente. El objetivo es ofrecer una visión actualizada sobre el diagnóstico y tratamiento de la dermatitis atópica que contribuya a la formación de los médicos en la atención primaria. Se realiza una revisión bibliográfica de los últimos 5 años, principalmente en las bases de datos PubMed y Scielo sobre el tema. Se abordan aspectos de interés relacionados con las manifestaciones clínicas, criterios diagnósticos y tratamiento. Los resultados que se ofrecen en este trabajo contribuirán a la formación profesional para una mejor promoción, prevención, diagnóstico precoz y tratamiento de esta enfermedad, cuya prevalencia es mayor en la infancia (AU).


ABSTRACT The favorable evolution of the patients affected with Atopic Dermatitis is quite related with a diagnosis and precocious orientation in the primary attention, for a more specialized follow up in the dermatology and alergology consultations, as it is an entity that offers difficulties in its identification. It is necessary to increase the level of knowledge of doctors in these medical areas as these are the scenarios where it occurs the first contact patient doctor. Offering an updated vision about Atopic Dermatitis that contributes to the continuous formation of the professionals of the health sector in the primary attention. A revision of articles in the PubMed and Scielo database is carried out principally during the last 5 years. Aspects of interest related with the clinical manifestations, diagnoses criteria and treatment are considered. The results that are offered in this work will contribute to the professional formation for a better promotion, prevention, precocious diagnosis and treatment of this illness that is more likely to be found in the childhood (AU).

ABSTRACT The favorable evolution of the patients affected with Atopic Dermatitis is quite related with a diagnosis and precocious orientation in the primary attention, for a more specialized follow up in the dermatology and alergology consultations, as it is an entity that offers difficulties in its identification. It is necessary to increase the level of knowledge of doctors in these medical areas as these are the scenarios where it occurs the first contact patient doctor. Offering an updated vision about Atopic Dermatitis that contributes to the continuous formation of the professionals of the health sector in the primary attention. A revision of articles in the PubMed and Scielo database is carried out principally during the last 5 years. Aspects of interest related with the clinical manifestations, diagnoses criteria and treatment are considered. The results that are offered in this work will contribute to the professional formation for a better promotion, prevention, precocious diagnosis and treatment of this illness that is more likely to be found in the childhood.

The aim of this study was to evaluate the anaesthetic failure rate of the pterygomandibular technique with an inferior alveolar nerve block in lower third molar extraction procedures. Materials and Methods: A total of 67 healthy patients (men and women, aged between 18­35 years) requiring third molar extractions were selected from the Discipline of Surgery, of the Araçatuba Dental School. The anaesthetic technique utilised was a pterygomandibular block using 2% mepivacaine hydrochloride with epinephrine 1:100,000 (Mepivalem®) in a maximum standardised amount of three tubes (5.4 ml) per procedure (performed by the same operator). Surgical procedures were initiated after confirming the sensation of anaesthesia in the area anesthetised at the inferior alveolar nerve. Painful symptoms were recorded during surgery. Results: In total, 117 surgical procedures, i.e., 114 exodontias and three coronectomies, were performed. During surgical procedures, four patients reported pain during manoeuvres of odontosection, representing an anaesthetic failure rate of 3.41%. Alternative techniques utilised in these four patients involved milohioideo nerve, intraligamentaria, and intrapulpal anaesthesia. Conclusion: The success of inferior alveolar nerve block anaesthesia is difficult to predict due to large anatomical variations, and it is difficult to observe via diagnostic means. The relatively low percentage of anaesthetic failure reported in this study may be related to the number of procedures performed(AU)


O objetivo deste estudo foi avaliar a taxa de falha anestésica da técnica pterigomandibular com bloqueio do nervo alveolar inferior em procedimentos de extração do terceiro molar inferior. Material e Método: um total de 67 pacientes saudáveis (homens e mulheres, com idade entre 18-35 anos) que requeriam extrações do terceiro molar foram selecionados da Disciplina de Cirutgia da Faculdade de Odontologia de Araçatuba. A técnica anestésica utilizada foi um bloqueio pterigomandibular usando cloridrato de mepivacaína a 2% com epinefrina 1: 100,000 (Mepivalem®) em uma quantidade máxima padronizada de três tubos (5,4 ml) por procedimento (realizado pelo mesmo operador). Os procedimentos cirúrgicos foram iniciados após confirmação da sensação de anestesia na área anestesiada no nervo alveolar inferior. Sintomas dolorosos foram registrados durante a cirurgia. Resultados: No total, foram realizados 117 procedimentos cirúrgicos, isto é, 114 exodontias e três coronectomias. Durante os procedimentos cirúrgicos, quatro pacientes relataram dor durante as manobras de odontossecção, representando uma taxa de falha anestésica de 3,41%. As técnicas alternativas utilizadas nesses quatro pacientes envolveram nervo milohioideo, intraligamentar e anestesia intrapulpar. Conclusão: o sucesso da anestesia com bloqueio do nervo alveolar inferior é difícil de prever devido a grandes variações anatômicas e é difícil de observar por meio de meios diagnósticos. A porcentagem relativamente baixa de falha anestésica relatada neste estudo pode estar relacionada ao número de procedimentos realizados(AU)

Los síntomas dolorosos se registraron durante la cirugía. Resultados: En total, se realizaron 117 procedimientos quirúrgicos, es decir, 114 exodontias y tres coronectomías. Durante los procedimientos quirúrgicos, cuatro pacientes relataron dolor durante las maniobras de odontosección, representando una tasa de falla anestésica del 3,41%. Las técnicas alternativas utilizadas en estos cuatro pacientes involucraron nervio milohioideo, intraligamentar y anestesia intrapulpar. Conclusión: el éxito de la anestesia con bloqueo del nervio alveolar inferior es difícil de predecir debido a grandes variaciones anatómicas y es difícil de observar por medio de medios diagnósticos. El objetivo fue ofrecer una revisión actualizada sobre la etiopatogenia y fisiopatología de la dermatitis atópica que contribuya al incremento y actualización de la información sobre este tema. Se realizó una revisión en artículos de la base de datos PubMed y Scielo principalmente de los últimos 5 años y se sistematizan los resultados para de esta forma contribuir a la superación de los profesionales en la atención primaria de salud (AU).


The atopic dermatitis (DA by its acronym in Spanish) is a chronic inflammatory disease and its etiopathogenesis is multifactorial. The diversity of risk factors described in the bibliography evidences the complicatedness of the research with these aims and the interrelation among them offers a problem for the precise approach in each case, given the overlapped effects found. The aim is to give an up-dated review on atopic dermatitis etiopathogenesis and physiopathology helping to increase and up-date information on this topic. A review was carried out in articles appeared mainly in PubMed and Scielo databases during the last five years, and the results were systematized to contribute to the professional growth of health professionals working in the primary health care (AU).

RESUMO

ABSTRACT: Objective: This study was reduction of a bilateral mandibular fracture through the installation of dental implants with immediate loading, re-establishing the stomatognathic function of the patient. Case Report: A 58-year-old male patient seek the emergency room from hospital after suffering a motorcycle accident. After physical and imaging examinations was diagnosed with bilateral mandibular fracture. The treatment of choice, once the patient was edentulous, was the reduction and stabilization of the fracture with the installation of 5 dental implants with immediate loading. After 1 year of postoperative follow-up, the facial contour along with the occlusion were reestablished. Radiographically, it was found the proper placement of plates and screws with correct baseline realignment and maintenance of implant-supported prosthesis. Conclusion: In this case report with follow-up of 18 months, the functional prosthetic rehabilitation and immediate aesthetic with immediate loading system, after reduction and fixation of mandibular fractures proved to be a good treatment option.

El objetivo de este estudio fue realizar una revisión sistemática de la literatura con relación al uso de piezocirugia comparado con instrumentos rotatorios convencionales en las exodoncias de los terceros molares inferiores. Material y método. Se realizó una búsqueda electrónica en las bases de datos: PubMed, ISI Web of Knowledge, Science Direct en los últimos 8 años, utilizando las palabras claves; tercer molar, osteotomía, instrumentos quirúrgicos, cirugía maxilofacial. Resultados. se seleccionaron veintiseis artículos después de aplicar los criterios de inclusión y exclusión. Se seleccionaron artículos que evaluaron el tiempo transoperatório, edema facial, dolor posoperatorio y cantidad de analgésico ingerida, una variable común en los estudios fue el mayor tiempo de trabajo durante los procedimientos realizados con dispositivos piezoeléctricos. Conclusión. de acuerdo con las evidencias encontradas, la mayoría de los estudios relacionaron el uso de piezocirugia para exodoncias de terceros molares inferiores con menor edema facial posoperatorio, sin embargo los procedimientos realizados con piezocirugia representan mayor tiempo transoperatório, lo que puede aumentar el dolor posoperatorio y la presencia de trismus


The aim of this study was to conduct a systematic review of the literature regarding the use of Piezosurgery instead of conventional rotary instruments in the extractions of lower third molars. Material and method. An electronic databases search was performed in PubMed, ISI Web of Knowledge, Science Direct in the last eight years, using the keywords; third molar, osteotomy, surgical instruments, maxillofacial surgery. Results. Twenty six articles were selected after applying inclusion and exclusion criteria. Articles that evaluated the perioperative time, facial edema, postoperative pain and amount of analgesics used, were selected, and a common variable in the studies was the longest working time during the procedures performed with piezoelectric devices. Conclusion. According to the evidence found, most studies have associated the use of Piezosurgery for extractions of third molars with less facial post-operative oedema. However piezosurgery has a longer greater intra-operative time, which can increase post-operative pain and presence of trismus

INTRODUCTION: The objective of the present study is to assess the safety and feasibility of the use of telemedicine-based services for surgical wound care and to measure patient satisfaction with telemedicine-based follow-up. MATERIAL AND METHODS: 24 patients were included, they were provided with a corporate mail address. On day 7 after surgery patients sent, via email, an image of their surgical wound together with a completed questionnaire in order to obtain an early diagnosis. Two independent physicians studied this information and the histologic analysis of the specimen. On day 8, all patients underwent face-to-face office examination by a third physician and all of them completed a satisfaction questionnaire at the end of the study. RESULTS: The use of telemedicine-based services showed a sensitivity of 100%, a specificity of 91.6%, a positive predictive value of 75% and a negative predictive value of 100%. Degree of concordance between the two physicians, as regards the necessity of face-to-face follow-up yielded a kappa coefficient of 0.42 (standard error 0.25 and confidence interval 95% (0.92-0.08), which means a moderate agreement between the two evaluations. 94% of patients were satisfied with telemedicine-based follow-up and 93% showed their preference for this procedure over conventional methods. CONCLUSIONS: The telemedicine-based follow-up, has proven to be feasible and safe for the evaluation of early postoperative complications. Patients reported high levels of satisfaction with the procedure. Telemedicine-based follow-up could become standard practice with the development of a specific mobile application.

RESUMO

Bone reconstruction to reestablish bone width and height is required in case of bone atrophy. A 45-year-old white woman was referred to the oral clinical surgery of Aracatuba Dental School, University Estadual Paulista-UNESP, in 2013 with the main complain of bone exposure at the superior right site of the maxilla after 1-year reconstruction with bone block allograft. Bone block was removed besides the dental implant next to the bone allograft. Complication of bone allograft could jeopardize treatment planning.
