RESUMO

The paper analyzes left ventricular structure-fraction relationships in the development of postinfarct aneurysm. The altered internal architectonics induces to systemic hemodynamic changes, drastically elevated intraventricular pressure. This is caused by to the dysfunction of the papillary-trabecular complex in the left ventricular cavity, which in turn. This leads to the fact that the heart work as a positive-displacement pump, by losing its capacity as a centrifugal component, by making the myocardium require additional energy expenditures, which in turn appears as varying heart failure. This investigation is of definite practical value in developing adequate correction methods for postinfarct aneurysm which can occur with the retained or formed certain ratios of cardiac structures.

RESUMO

The paper presents evidence for and results of development of a new hydrodynamic optimized design of the automyoneoventricle (AMNV) for extracorporeal circulation in critical myocardial lesions. The chosen geometry of externally spirally braided AMNV forms a curled flow that excludes the presence of eddy and stagnant zones which present a risk for thrombogenesis. Moreover, the paper details the original process for manufacturing the prototypes of the AMNV having spiral latex ribs, which has been modified to give its thromboresistant and antibiotic properties. The proposed biomechanical system has been put through tests yielding satisfactory results and it is to undergo clinical trials.

RESUMO

An analysis of the range of postoperative complications following cavapulmonary anastomosis reveals a number of medical engineering problems in surgery techniques. The severity of the surgery due to the nonphysiological rearrangement of blood circulation makes it necessary to search for the most optimal types of anastomosis in terms of the hemo- and hydrodynamics of circulation. To optimize the performance of cavapulmonary anastomosis, namely to define the mutual localization and orientation of the implanted anastomoses against the axes of inflow and outflow, hemo- and hydrodynamic studies were made by Doppler laser anemometry. From the hydrodynamic point of view, the findings indicate the most optimal type of cavapulmonary anastomosis-cruciform anastomosis by displacing 0.5 d from the outflow axis at an angle of 60ë5 degrees.

RESUMO

The paper is concerned with the present-day problem of modern medicine, namely with surgical correction of the diseases of the initial part of the pulmonary circulation. The authors suggest hemohydrodynamic simulation of the right heart complicated by extracardial conduit. The simulation was performed at the high theoretical, practical and methodological level to obtain highly significant information which is likely to have paramount importance for clinical practice.

RESUMO

An original morphological study has been conducted to identify the mechanisms underlying the morphofunctional transformations of the left-ventricular structures taking place at late terms after implantation of the apico-aortal conduit. Analysis of the moulds of the left-ventricular cavity has convincingly demonstrated the peculiar changes in its structures bringing about alterations in the blood ejection mechanism on creation of a new outflow tract from the left ventricle of the heart. The conclusions derived from this work are essential not only for implantation of the apico-aortal conduit but for understanding the mechanisms of blood ejection in some congenital heart diseases.

RESUMO

In 76 experiments on mongrel dogs the authors tried to solve principal problems interfering with a broader clinical application of extracardiac conduit for left ventricular-aortic shunting. They present an original conduit design whose prolonged trials in chronic experiments on dogs yielded favourable results. A new, relatively atraumatic and easy-to-perform method was used for conduit implantation. Functional condition of conduits and systemic and intraventricular haemodynamics were investigated shortly after operation and in the late postoperative period. At various intervals after surgery the conduits corresponded haemodynamically and functionally to systemic blood flow. Use of conduit to construct a new left ventricular outflow tract proved to be an effective alternative in surgical treatment of many hitherto inoperable aortic root defects.

RESUMO

Studies of the hemodynamic features of the blood flow in extracardiac conduits allow for a grounded and aimed approach to the prosthesis design. The lack of interrelationship between the duct geometry, the flow pattern, and the heart anatomy is one of the reasons for the existing conduits inadequacy. Here is proposed a confuser conduit model providing a stable uninterrupted flow. Development of a hemodynamically optimal type-size series of conduits would allow one to reduce the incidence of complications resulting from inadequate prosthesis design.
