RESUMO

Glomus tumours are benign neoplasms that usually arise in the skin of the extremities but have infrequently been found to occur in other sites including the stomach. We report on a 71-year-old female with non-specific epigastric pain who was diagnosed as having a small, intramural gastric tumour in addition to a cholecystolithiasis. Intraoperatively, the tumour was investigated by frozen section, but the diagnosis remained inconclusive. The ultimate histological examination showed clusters of uniform epithelioid cells surrounding wide vascular spaces. This led to the diagnosis of a glomus tumour. In a review of the recent literature,we discuss the methods and limitations of preoperative diagnostic measures.

In our mycological laboratory we saw an increase in the occurrence of Candida glabrata isolates from 0.5% in 1990 to 22.2% in 1996, mainly isolates of high-risk patients of the intensive care unit and the oncological ward. We were interested to analyse whether this increase in C. glabrata was also demonstrable in a corresponding number of deep-seated candidosis. Eighteen patients were autopsied and examined microscopically. In all patients, repeated occurrence of C. glabrata, with or without C. albicans during the hospital treatment, was found. Serology and fungal cultures of these intensive care/oncological patients were indicative of invasive candidosis. In 12 of these 18 patients, there was no evidence of deep-seated candidosis at autopsy. Three patients had a typical aspergillosis of the lungs, which was not diagnosed during their hospital treatment. In five patients (two of them with an aspergillosis of the lungs), we found sporadic yeast cells in kidney or lung tissue, characterized by size and missing pseudomycelium as C. glabrata. In no case was a cellular intravital defensive reaction against these yeast cells found. Therefore, we interpreted the demonstration of C. glabrata in tissues as a transitory fungaemia and not as a deep-seated C. glabrata mycosis.

We were interested to know whether this increase of C. glabrata is also corresponding to the number of endomycoses. Eighteen patients have been autopsied and examined histologically. During the hospital course we found repeatedly in all patients C. glabrata with or without C. albicans. Serological and culture results of this intensive care/oncological patients were indicative of an invasive endomycosis. In 12 of 18 patients there was no evidence of an invasive mycosis in autopsy tissues. Three patients had a typical aspergillosis of the lungs, which was not diagnosed during hospital course. In 5 patients, two of them with aspergillosis of the lungs, we found in kidney or lung tissue sporadic yeast cells, typified by size and missing pseudomycelium as C. glabrata. In no case a tissue reaction as an intravital defense reaction against these yeast cells was observed. Therefore, we interpreted the findings of C. glabrata in tissues as representing a transient fungemia and not as an endomycosis due to C. glabrata.

RESUMO

One case of isolated tumorlike amyloidosis of the lung is reported. The patient was found by mass radiographic survey. Before operation the histological diagnosis was unknown. The specimen of lung from thoracotomia showed tumorlike amyloidosis and independent from amyloidosis sarcoid granulomatosis of uncertain cause. Isolated tumorlike amyloidosis of the lung is a very rear diagnosis. Thus diagnosis before operation is very difficult. In case of circular focus of undetermined etiology surgical intervention is indicated to avoid delayed resection of bronchial carcinoma.

Reported on tumourlike circular focus opacities in the thoracic X-ray photogramms of 34 patients, 27 of them were discovered by mass x-ray surveys. They include: 13 organoid tumours, 1 bronchial adenoma, 2 mucodermoid, 1 cystic adenocarcinoma of bronchus, 1 fibroma, 1 reticulo-cytoma, 1 bronchial papilloma, 7 cystic tumours, partially located in the mediastinum, 3 intrapulmonary malformations of blood vessels, 1 mediastinal neurinoma, and 3 cases with suspicion of organoid tumours. 29 patients had to undergo an operation. Finally the authors give a review of literature of indication for surgical treatment, and they represent guidelines for the medical expert assessment fitness for the National Service.

Starting from three own cases of an epithelioid sarcoma, the literature about this type of tumor is discussed. It occurs in males nearly three times as often as in females, with the maximum frequency between 20 and 29 years of age. Finger, hand, wrist and forearm are preferably affected with the epitheloid sarcoma. The microscopic features are: nodular growth, necroses and hemorrhages within the nodules, the tumor tissue consisting of spindle-type and epitheloid cells with spaces in between, and lymphocyte infiltrates and siderin deposits at the periphery. The neoplasm's cytogenesis is not known. With regard to the tumor, the differential diagnosis should make a clear-cut distinction particularly between inflammatory granulomas and benignant neoplasms. The tumor trends to reactivation to a large extent. Metastases are both lymphogenous and hematogenous. A radical surgical removal is strongly recommended.

RESUMO

Report is given of cryobiological principles, technique and results of cryotherapy for eyelid or conjunctival tumors in 181 cases. In the majority of cases they were basaliomas. In all cases the therapy was successful. No eyelid contractions or tear duct stenosis were noted. The follow-up of 4,5 years showed only 2 recidives. Histologically, cryonecrosis showed capillary ectasis with hyperemia.

RESUMO

In a 69 years old female patient a paraplegia originated after a sudden fall on the back. By autopsy a fibrocartilaginous embolia in the arteries of the thoracolumbar cord and an extensive myelomalacia were detected.
