Introducción: las enfermedades pulmonares pueden generar hospitalizaciones, estancias prolongadas, complicaciones e inclusive la muerte. Estas enfermedades repercuten negativamente en la capacidad inspiratoria y en la calidad de vida. Por lo tanto, el presente artículo pretende analizar los efectos del Método JaPer sobre la capacidad inspiratoria de los pacientes hospitalizados. Material y métodos: ensayo clínico aleatorizado inicialmente con 653 pacientes hospitalizados que pasó a 587 distribuidos en 2 grupos (Grupo experimental-GE: Método JaPer vs Grupo control-GC: uso convencional de inspirómetro volumétrico). Se determinó la capacidad inspiratoria máxima, caminata de los 6 minutos, antropometría y un cuestionario creado por los autores. Se aplicó un programa de entrenamiento de 2 semanas de 3 sesiones por día. Al GE se le aplicó el Método Japer con protocolo estandarizado para hacer ejercicios entre el 50 y el 80 % de la capacidad inspiratoria máxima del paciente y a los pacientes del GC se les realizó incentivo inspiratorio a inspiración máxima. Resultados: 587 pacientes (F:300 vs M:287) con edad promedio de 53.61±14.24 años, entre los cuales el 9.88 % tenía bajo peso, 17.89 % peso normal y el 27.26 % y 44.97 % sobrepeso y obesidad, respectivamente. Todos los participantes realizaron una caminata de 6 minutos para determinar los metros recorridos (GE: 387.70±47.59 vs GC: 371.30±49.10), velocidad (GE: 64.62±7.93 vs GC: 61.88±8.18) y el Vo2 estimado (GE: 9.96±0.79 vs GC: 9.69±0.82). Todas las variables y la capacidad inspiratoria máxima (GE: 1708.54±707.84 vs GC: 1448.83±692.79) determinaron pre y post entrenamiento. Conclusiones: el Método JaPer obtuvó mejores resultados en todas las variables evaluadas con diferencia significativa (p= <0.05) frente al grupo control, resaltando que la capacidad inspiratoria máxima aumentó en ambos grupos (GE:44 % vs GC:28 %; p= <0.05).


Introduction: Pulmonary diseases can generate hospitalizations, prolonged stays, complications and even death. These diseases have a negative impact on inspiratory capacity and quality of life. Therefore, this paper tries to analyze the effects of the JaPer method on the inspiratory capacity of hospitalized patients. Materials and methods: Randomized clinical trial initially with 653 patients, which went on to 587 hospitalized patients distributed in 2 groups (Experimental group: JaPer Method vs. Control group: Conventional use of volumetric inspirometer). Maximum inspiratory capacity, 6-minute walk, anthropometry, and a questionnaire created by the authors were determined. A 2-week training program of 3 sessions per day was applied. The GE applied the Japer Method with a standardized protocol to exercise between 50 and 80% of the patient's maximum inspiratory capacity, and the CG underwent inspiratory incentive at maximum inspiration. Results: 587 patients (F:300 vs M:287) with a mean age of 53.61±14.24 years and 9.88% of the population were underweight, 17.89% normal weight and 27.26% and 44.97% overweight and obese, respectively. All participants performed a 6-minute walk to determine meters traveled (GE: 387.70±47.59 vs CG: 371.30±49.10), speed (GE: 64.62±7.93 vs CG: 61.88±8.18) and estimated VO2 (GE: 9.96 ±0.79 vs CG: 9.69±0.82). All variables and maximal inspiratory capacity (GE: 1708.54±707.84 vs CG: 1448.83±692.79) were determined before and after training. Conclusions: The JaPer method obtained better results in all the variables evaluated with a significant difference (p=<0.05) compared to the control group. Highlighting that the maximum inspiratory capacity increased in both groups (GE:44% vs CG:28%; p=<0.05).

RESUMEN Introducción. El hígado es el mayor órgano sólido, desempeña muchas funciones en el cuerpo. El ejercicio físico es valorado como una intervención no farmacológica eficiente en el fomento del bienestar físico, mental y funcional de pacientes con cáncer. Objetivos. Determinar los efectos del ejercicio físico en el cáncer hepático y sintetizar estudios prospectivos sobre la asociación de la actividad física y el ejercicio físico con el riesgo de cáncer de hígado. Metodología. Revisión sistemática con análisis retrospectivo y descriptivo de artículos científicos publicados en bases de datos indexadas de enero del año 2008 a junio del 2019. Así mismo, se llevó a cabo la selección y evaluación de los archivos por medio de la Declaración de PRISMA, PICO, Cochrane y escala de PEDro. Resultados. Se encontró que la actividad física se asocia con un riesgo reducido de desarrollar cánceres de hígado (IC95%: 0,38-0,80). El ejercicio aeróbico redujo los lípidos hepáticos (grasa hepática 8.9±3.2 a 5.6±1.8%; p <0.05, grasa visceral 54,7±6,0 a 49,6±5,5cm2; p <0,05) y el ejercicio de resistencia aumentó la sensibilidad a la insulina (5.9±5.9 a 4.6±4.6 vs 4.7±2.1 a 5.1±2.5; p<0.05), mejoró la flexibilidad metabólica (-0.0206±0.010 vs 0.004±0.003; p<0.05). Conclusiones. La actividad física y el ejercicio representan un papel importante dentro de la enfermedad del cáncer, ya que se asocia con un menor riesgo de desarrollarlo, así como aminorar la sintomatología que conlleva en especial el cáncer hepático.


ABSTRACT Introduction. The liver is the largest solid organ; it performs many functions in the body. Physical exercise is valued as an efficient non-pharmacological intervention in the promotion of physical, mental and functional well-being of cancer patients. Objectives: To determine the effects of physical exercise on liver cancer and synthesize prospective studies on the association of physical activity and physical exercise with the risk of liver cancer. Methodology. Systematic review with retrospective and descriptive analysis of scientific articles published in indexed databases from January 2008 to June 2019. Likewise, the selection and evaluation of the archives was carried out through the Declaration of PRISMA, PICO, Cochrane and PEDro´s scale. Results. It was found that physical activity is associated with a reduced risk of developing liver cancers (95%CI 0.38-0.80). Aerobic exercise reduced liver lipids (liver fat 8.9±3.2 to 5.6±1.8%; p<0.05, visceral fat 54.7±6.0 to 49.6±5.5cm2; p<0.05) and resistance exercise increased insulin sensitivity (5.9 ± 5.9 to 4.6±4.6 vs. 4.7±2.1 to 5.1±2.5; p <0.05), improved metabolic flexibility (-0.0206± 0.010 vs. 0.004±0.003; p <0.05). Conclusions. Physical activity and exercise represents an important role in cancer disease, since it is associated with a lower risk of developing it, as well as reduce the symptoms which is especially associated with liver cancer.

RESUMO

We report four cases of Taenia saginata taeniasis in different urban communities of Aragua state, Venezuela. After subsequent treatment with praziquantel and a saline purge, adult tapeworms were collected from all four patients and demonstrated to be T. saginata by morphological and molecular characterization. The finding of T. saginata in four distinct and separate urban municipalities of the Aragua state indicates the pertinence of rigorous meat inspection, and the importance of establishing parasite prevalence in human and bovine Venezuelan populations.

Background: Previously we reported the use of a monoclonal antibody-based (HP10) antigen (Ag) detection lateral flow assay (LFA) for the diagnosis of extraparenchymal neurocysticercosis (EP-NCC). The assay performed well when used with cerebrospinal fluid (CSF) samples but not with their paired serum samples, due to false-positive reactions in some known negative control cases. Methods: Our novel modification involves pretreatment of serum samples using a combination of sodium deoxycholate and dithiothreitol. Results: The modification overcomes the problem of false positives when using negative serum samples from clinically characterized cases of EP-NCC and bovine cysticercosis. In general, there was good agreement between HP10 Ag enzyme-linked immunosorbent assay (ELISA) and the HP10 Ag-LFA, but the HP10 Ag-ELISA was marginally more sensitive than the modified HP10 Ag-LFA. Conclusions: The modified HP10 Ag-LFA provides a field test for the rapid identification of endemic human and bovine cysticercosis.

To evaluate diagnosis of active neurocysticercosis, paired cerebral spinal fluid (CSF) and serum samples from 24 neurocysticercosis (NCC) patients and 17 control neurological patients were assayed in the HP10 Taenia antigen (Ag) ELISA. The CSF samples were also tested with an HP10 Lateral Flow Assay (LFA). The HP10 Ag was detected by ELISA in the CSF of 5/5 patients with Definitive extraparenchymal NCC, and in 4/5 of the corresponding sera. In the Definitive parenchymal group, on the other hand, the HP10 Ag was absent in 2/3 CSF (with a very low value in the one positive sample) and all the corresponding serum samples. Samples of CSF from 4/7 patients in the Probable parenchymal group, were also significantly HP10 Ag positive, suggesting the presence of extraparenchymal cysts not identified by the imaging studies. With the possible exception of one patient, the corresponding serum samples of the Probable parenchymal NCC group, were all HP10 Ag negative. Samples of CSF from 9 NCC patients diagnosed with Mixed parenchymal and extraparenchymal NCC were all significantly HP10 Ag positive, confirming the presence of extraparenchymal cysts, with only 7/9 of the corresponding serum samples being HP10 positive. Thus detection of the HP10 Ag indicates extraparenchymal and not parenchymal cyst localization and is more sensitive with CSF than serum. Three neurological patients clinically diagnosed as subarachnoid cyst, hydrocephalus and tuberculoma, respectively, were clearly positive for HP10 Ag. Of these, two were confirmed as NCC by subsequent imaging; the third died prior to further examination. Thus, a total of 8 patients had their clinical diagnosis questioned. Finally, there was good agreement between the HP10 Ag ELISA and LFA with CSF samples giving an optical density ≥0.4 in the ELISA assay. In conclusion, the HP10 Ag assay should provide a valuable and reciprocal tool in the clinical diagnosis and follow up of extraparenchymal NCC.

Diagnosis of Taenia solium cysticercosis in endemic rural communities depends on serological tests, as typically there is no access to imaging facilities. The HP10 antigen ELISA (HP10 Ag ELISA), which detects a high molecular weight secreted protein of viable metacestodes, has been employed for the diagnosis of both human and porcine cysticercosis in such communities. In this communication, we formally demonstrate that the HP10 Ag ELISA, already known to function for the detection of T. saginata and T. solium cysticercosis, also detects a similar high molecular weight antigen of T. hydatigena. Thus, the HP10 Ag assay, while specific for human cysticercosis, may not be recommended for the diagnosis of porcine cysticercosis where there is co-infection of pigs with T. solium and T. hydatigena.

Se presenta un caso inusual de infección por Hymenolepis diminuta en infante de un año de edad residente en Maracay, estado Aragua. El hallazgo se realizó de manera fortuita por el personal del Laboratorio de Parasitosis Intestinales y Serología de Esquistosomiasis de la Dirección General de Salud Ambiental del Ministerio del Poder Popular para la Salud, a donde fueron remitidos ejemplares adultos del parásito eliminado de forma espontánea por el paciente. Antes de la expulsión del parásito, el paciente presentó sintomatología digestiva, fiebre y manifestaciones alérgicas. La representante del infante refirió la presencia de ratas y ratones en el jardín de la vivienda. Este caso se constituye en el primer informe de infección por H. diminuta en el estado Aragua. Se sugiere realizar investigación epidemiológica más rigurosa para determinar la prevalencia de himenolepiosis.


Presentation of an unusual case of Hymenolepis diminuta infection in a one-year old infant residing at Maracay, Aragua State. The finding occurred fortuitously by the staff of the Intestinal Parasites and Schistosomiasis Serology Laboratory of the General Direction of Environmental Health of the Ministerio del Poder Popular para La Salud, who received two adult forms of the parasite eliminated spontaneously by the patient. Before the expulsion of the parasite, the patient had digestive symptoms, fever and allergic reactions. The infant’s guardian referred the presence of rats and mice in the garden of the house. This case is the first report of this type of infection by H. diminuta in Aragua State. A more rigorous epidemiological investigation is suggested to determine the prevalence of himenolepiosis.

This study evaluates five synthetic peptides derived from four, potentially protective, Taenia saginata oncosphere molecules for the serodiagnosis of T. solium cysticercosis/neurocysticercosis in three distinct Venezuelan endemic regions. The peptides, all of which have been described previously, are designated HP6-3, Ts45W-1, Ts45W-5, Ts45S-10 and TEG-1. In clinically verified and seropositive hospital cases, combining the results of three of the individual peptide-based ELISAs (HP6-3, Ts45W-1 and Ts45W-5) afforded the best balance between sensitivity (85%) and specificity (83.5%), a significant improvement on the 63.6% specificity obtained with the routinely employed T. solium cyst-fluid-based ELISA. Similarly, in the seropositive Venezuelan endemic zone samples, 89.09% of Amerindians, 77.27% of symptomatic rural subjects and 67.83% of non-symptomatic rural subjects were also classed as seropositive by the combined peptide-based ELISAs. The profile of antibody recognition to individual peptides varied between the different groups of samples examined. The relevance of the above findings for the serology and prognosis of T. solium cysticercosis/neurocysticercosis in hospital- and field-based situations is discussed.

A serological study was undertaken in 1998 to evaluate levels of Taenia solium cysticercosis in 3 rural Venezuelan communities. Infection with viable metacestodes was diagnosed with a trapping enzyme-linked immunosorbent assay (ELISA) that detects a secreted product of viable parasites. Anti-metacestode antibodies were assayed by ELISA using T. solium vesicular fluid as antigen. A total of 1254 sera was collected from 3 communities (Canoabo, Sanare, and Rio Tocuyo) where previous studies had suggested the presence of T. solium. Our results demonstrate an unusually high seroprevalence of cysticercosis, indicating an attendant risk of transmitting the disease to other areas. The seroprevalence of infection with viable cysts, as indicated by detection of circulating parasite antigen, was 9.1% in Canoabo, 6.1% in Sanare, and 5.7% in Rio Tocuyo. The corresponding frequency of antibodies to T. solium cyst antigens was 36.5% in Canoabo, 36.5% in Sanare, and 4% in Rio Tocuyo. As these communities are probably representative of many others in Venezuela, T. solium cysticercosis may be a significant public health problem and more work is certainly indicated. An important finding was that local knowledge of the disease and its transmission do not necessarily guarantee diminished disease prevalence, indicating a lack of appropriate vigilance towards disease control.
