RESUMO

The solitary fibrous nodule is a rare clinical disease that mainly affects the pleura, but has been occasionally described in other anatomical sites. This type of tumors can have malignant components and therefore it is important to differentiate them from other retroperitoneal masses. We describe the case of a patient with ectopic pregnancy in whom a solitary fibrous nodule with laparoscopy was found. A peritoneal pelvic tumor with smooth surface, 20,2 g, firm was detected. The mass was independent of colon, uterus, ovaries or salpinx and was very near to the iliac vessels on the right side. A small fragment was biopsied and sent to trans surgical histopathology study and the rest of the mass was removed completely without complications. The histopathologic report described that the tumor contained sclerosed cells with collagenous bands and sings of hemorrhage and calcification, compatible with a benign pelvic solitary fibrous tumor. The solitary fibrous nodule is a rare, benign disease, but with malignant potential. These tumors must be resected when they are incidentally found during other surgical procedures or if diagnosed preoperatively. The laparoscopic approach has advantages in the identification and resection these tumors.

About 90 cases of platelet satellitism (PS) have been published, most of them involving neutrophils, a few comprising monocytes, and one case involving basophils. The case of a 30-year-old female patient with cutaneous vasculitis who developed asymptomatic severe neutropenia is reported. All blood smears (BS) prepared from peripheral blood samples collected with either ethylenediaminetetraacetic acid, trisodium citrate, and heparin showed PS involving neutrophils, eosinophils, and monocytes. Immunohistochemistry analysis of her skin biopsy and BS, employing peroxidase stain for the detection of antibodies directed against IgG, IgA, IgM, and C3, revealed an intense positive reaction only for IgG in the endothelium and leukocyte clumps within the microvasculature, as well as in peripheral blood neutrophils displaying the PS phenomenon. Transfer of the PS phenomenon was demonstrated by incubating the patient's plasma with leukocytes from an ABO-compatible healthy donor. In our patient, PS did not disappear after incubation at 37 degrees C, suggesting that this might be a different or "atypical" PS phenomenon probably mediated by an autoimmune process involving an IgG-class antibody responsible for both conditions, PS, and cutaneous vasculitis.

Antecedentes. Algunos estudios realizados en países desarrollados con población caucásica han revelado un incremento en la frecuencia de linfoma gástrico primario (LGP) durante las últimas dos décadas. Objetivo. Establecer la frecuencia de LGP en dos institutos nacionales de salud de la Ciudad de México en un intervalo de 18 años. Métodos. Revisamos 1,854 neoplasias gástricas malignas diagnosticadas en el periodo 1979-1986 en pacientes mexicanos estudiados en dos instituciones. Una de ellas (INC) es un hospital oncológico, mientras que el otro (INN) es un hospital general que atiende un alto número de enfermedades gastrointestinales. Resultados. Ciento sesenta y tres (8.8 por ciento) linfomas gástricos fueron identificados entre las 1,985 neoplasias gástricas malignas. La incidencia de LGP se incrementó de dos a tres veces en los últimos años del estudio en ambas instituciones. El incremento no pareció deberse a la selección de pacientes ni al número de tumores gástricos malignos estudiados. Conclusiones. Nuestros resultados revelan un incremento de LGP en la población mexicana estudiada en dos centros hospitalarios de alto nivel en años recientes. Se requieren estudios adicionales en otros países y grupos étnicos para definir si existe o no en ellos incremento de LGP.
