[Therapeutic attitude in choledocoduodenal fistulas complicating duodenal ulcer]. / L'attitude thérapeutique dans les fistules cholédocoduodénales compliquant l'ulcère duodénal.

The authors present 5 observations of choledocoduodenal fistula complicating chronic duodenal ulcer disease. Four cases were observed in Central Africa, the other in France. After a study of the frequency, and especially of the higher incidence in Africa, the authors review the problems associated with the pathology, clinical and diagnostic aspects of this complication. Moreover, they evoke the essential problems of management, as based on their experience and a review of the literature: medical vs. surgical treatment, should and how they treat the biliary lesions. Their individual results are criticized. A policy for the management of this complication is considered.

[Pneumatosis cystoides intestinalis manifested by pneumoperitoneum]. / Pneumatose kystique intestinale révélée par un pneumopéritoine.

We report the case of a female patient, suffering from chronic respiratory insufficiency and treated with steroids, in whom the discovery of pneumoperitoneum led to exploratory laparotomy. The plain abdominal X-rays demonstrated gas in the walls of the stomach as well as in the peritoneal cavity. Intraoperative exploration revealed the existence of pneumatosis cystoides of the stomach and small intestine. This observation is a reminder that not all cases of pneumoperitoneum are surgical, and notably this is the case for cystic intestinal pneumatosis.

[Splenic rupture in chronic pancreatitis. Study of 2 cases and review of the literature]. / Les ruptures spléniques au cours des pancréatites chroniques. Etude de 2 cas et revue de la littérature.

The authors have observed 2 new cases of splenic rupture due to pancreatic pseudocyst and have made a review of the literature on the subject (40 cases). The clinical, diagnostic and therapeutic approaches of this rare but sometimes dramatic complication of chronic pancreatitis have been analyzed. Diagnosis should be suspected in the case of abdominal pain, mass and anemia in the male subject with previously known chronic pancreatic or alcoholic disease. Sonography completed by either angiography or CT scan if time permits are the best diagnostic methods. Ideal management should include treatment (excision or drainage) of the pseudocyst and splenectomy.

The authors report two cases of distal clinodactyly secondary to electrical burns. It is hypothesized that the etiology is a unilateral ischemic necrosis of the epiphysis. Depending on the stage in which the deformity is discovered, treatment is preventive or palliative.

Nontraumatic intramural hematoma of the duodenum. Report of 4 cases and review of the literature.

The authors report 4 cases of nontraumatic intramural hematoma of the duodenum (IMHD), 1 of pancreatic origin, 2 due to anticoagulant therapy while the 4th was an unusual complication of periarteritis nodosa. The principal anatomical, clinical, biological and radiological characteristics of IMHD are reviewed. Therapy is discussed, emphasizing nonaggressive surgery when possible.

[Needle catheter jejunostomy. An improved technique for enteral feeding (author's transl)]. / Jéjunostomie intramurale á l'aiguille. Technique a minima pour l'alimentation entérale.

The authors have perfected the method of needle catheter jejunostomy introduced in 1973 for enteral feeding. Using the Seldinger technique reduces the risks of leakage, intra-operative haematoma and fistula following removal of the catheter. The operative procedure is described and the preliminary results are reported.

The authors report one case of perforated appendix associated with an inflammatory ileo-coecal pseudotumour due to malacoplakia. Right hemicolectomy resulted in uneventful and complete recovery. This case, together with a survey of available published data, enables the authors to briefly review the pathological, pathogenic and therapeutic aspects of this rare, if not exceptional, localisation of malacoplakia.

[Lipoma of the small intestine revealed by ileocaecal intussusception (author's transl)]. / Lipome de la dernière anse grêle révélé par une invagination intestinale aiguë.

The authors report a personal case of ileocaecal intussusception in an adult, revealing a benign lipoma of the last ileal loop. A review of the available bibliography was made emphasizing the essential characteristics of this rare type of lesion.

[Inflammatory pseudo-tumors of appendiceal origin. Study of twelve cases (author's transl)]. / Pseudo-tumeurs inflammatoires d'origine appendiculaire. Etude de 12 cas.

The authors relate their experience concerning twelve cases of inflammatory pseudo-tumors of appendiceal origin treated during the last five years. Their personal acquisitions are confronted with the literature and they propose a protocole of clinical, roentgenological and intra-operatory diagnosis thus permitting rational and adapted management.
