O presente trabalho foi desenvolvido a partir do atendimento de uma paciente adulta em psicoterapia desde 2008, com freqüência de duas sessões por semana, no Ambulatório do IEPP. O objetivo deste trabalho foi compreender, através da ilustração de um caso clínico, o funcionamento e a evolução de uma paciente com aspectos psíquicos regressivos, atendida em psicoterapia de orientação psicanalítica. No estudo de caso foram abordados de forma prática e teórica aspectos presentes no desenvolvimento emocional primitivo e das primeiras relações de objeto na constituição psíquica do sujeito(AU)

This work was developed based on a clinical care of an adult patient that has been in psychotherapy since 2008, twice a week, at the IEPP Clinic. The aims of this study was to understand, through the illustration of one clinical case, the mental functioning and evolution of a patient with regressive psychic aspects, under psychoanalytic psychotherapy treatment. This case study was discussed from a practical and theoretical point of view, issues involved in the primitive emotional development and early object relations in psychic constitution of the subject(AU)

"Um espaço onde as bruxas podem ser contidas": reflexões acerca de alguns conceitos da teoria de Klein e Winnicott em um caso clínico / "A place where witches can be contained": reflections about some concepts of Klein and Winnicott's theory in a clinical case

O presente artigo foi desenvolvido a partir do atendimento clínico de um paciente de 05 anos, que ficou em psicoterapia durante 01 ano e 04 meses, com frequência de uma vez por semana em uma escola de educação infantil. O estudo buscou compreender aspectos presentes no desenvolvimento emocional primitivo e das primeiras relações de objeto. Considera-se que o setting e a relação terapêutica poderiam funcionar como um espaço de ilusão para a criança. Destaca-se ainda a importância primordial do brincar na sessão. Como forma de compreender melhor esses aspectos do caso, foram utilizados alguns conceitos da teoria de Klein e Winnicott. O encaminhamento foi realizado pela professora da escola, pois acreditava que a criança tinha problemas de aprendizagem e parecia no "mundo da lua". A psicoterapia propiciou um espaço de ressignificação das experiências primitivas através do brincar e do vínculo estabelecido com a terapeuta(AU)

This article was developed from the clinical case of a 5-year-old patient, in psychoanalytical psychotherapy for one year, once a week, in a pre-school setting. The article aimed to understand aspects of the emotional development and the early object relations. It is considered that the setting and the therapeutic relationship could work as a space of illusion for the child. It was also emphasized the major importance of playing in the session. In order to better understand these aspects of the case, the author used concepts of Klein and Winnicott’s theory. The school teacher asked for the treatment, because she believed the child had learning problems and appeared to be “disconnected” to what surrounds him. The psychotherapy favored the creation of an area of redefinition of the early experiences through play and the bond established with the therapist(AU)

Considerações sobre o fenômeno da transferência institucional / Considerations about the institutional transference’s phenomenon

A necessidade de investigar a Transferência Institucional surgiu a partir de nossa prática como psicoterapeutas no Serviço de Atendimento do Instituto de Ensino e Pesquisa em Psicoterapia (IEPP). Partimos do conceito de Transferência em Psicanálise, seguindo para os conceitos de Transferência e Contratransferência Institucional, realizando uma revisão teórica acerca do tema. A Transferência Institucional se refere à interferência de um terceiro (instituição) na relação paciente-terapeuta; em alguns casos, a instituição de tratamento em si parece mais importante que a relação desenvolvida com o terapeuta. Nesses casos, é necessário que o trabalho terapêutico busque a particularização da relação para não reforçar os aspectos patológicos do paciente, muitas vezes, associados a um funcionamento regressivo e dificuldades em estabelecer relações mais íntimas. É necessário compreender o que a busca da instituição está representando para cada paciente, uma vez que a interferência desta no trabalho da dupla é real e se faz presente em todos os atendimentos ambulatoriais(AU)

The need to investigate the Institutional Transference emerged from our practice as psychotherapists, at Instituto de Ensino e Pesquisa em Psicoterapia (IEPP). The research started with some studies of the concept of Transference in Psychoanalysis, including, after that, the concepts of Institutional Transference and Countertransference, in order to develop a theoretical review of that subject. Institutional Transference refers to the interference of a third (institution) in therapist-patient relationship; sometimes the institution, where the treatment itself occurs, seems more important for these patients than the relationship developed with the therapist. In such cases a stronger therapeutic relationship is fundamental, and must be developed, to assure that some pathological aspects of the patient, often associated with regressive functioning and difficulties in establishing intimate relationships, will not be enhanced. It is important to understand the meaning of the institution and its representation for each patient, as the institution's interference in psychotherapeutic work is real and present in all Outpatient Psychological Services(AU)

Caracterização sociodemográfica da clientela do serviço de atendimento do Instituto de Ensino e Pesquisa em Psicoterapia

O Serviço de Atendimento do IEPP, instituição que visa à promoção do ensino da psicoterapia psicanalítica a psicólogos e médicos, foi criado com o objetivo de proporcionar a prática da psicoterapia aos psicoterapeutas em formação, funcionando, desta forma, como uma clínica-escola (C-e). O objetivo deste estudo foi conhecer a população que busca atendimento psicoterapêutico no IEPP. A amostra compreendeu 1102 pacientes que buscaram atendimento na instituição no período de dezembro de 1998 a junho de 2007. A análise dos dados se deu por estatística descritiva, compreendendo freqüências e porcentagens. Exploraram-se, neste estudo, características sociodemográficas, como sexo, faixa etária, escolaridade, ocupação, estado civil, por quem são encaminhados, o motivo da busca de atendimento e o desfecho do atendimento. A partir destes resultados, podemos aprofundar nosso conhecimento sobre esta demanda populacional, estando assim, mais bem preparados para atendê-la.(AU)

The Patient’s Attendance Service of IEPP, an institution that aims at the promotion of Psychoanalytical Psychotherapy taught to psychologists and doctors, was created with the objective to provide specific practice to psychotherapists who are under their training course. So that, the clinic can be considered as a training clinic. The objective of the present study was to know the population that searches for psychotherapy attendance at IEPP. The sample was composed by 1102 patients who had come for attendance in the institution between the periods of December, 1998 and June, 2007. Data analysis had been done by descriptive statistics, comprehending frequency and percentages, and it was explored, in the current study, social-demographic characteristics such as: sex, age, education, occupation, marital state, by whom the clients were directed, the reason of the attendance’ searches and the outcome of the attendances. From the results of this study we can enhance our knowledge on the population’ demands, being, thus, better prepared to take care of it.(AU)
