RESUMO

The actuality of donorship increases continuously since donors are the only source of blood to transfuse to patients in need. The medical institutions implementing hi-tech methods of treatment present increased requirements to quality and safety of blood components. The regular analysis of functioning of blood services permits to maintain and to develop productive potential.The article presents results of analysis of statistical indicators of blood service institutions of the Republic of Tatarstan, in particular Kazan, concerning procurement and safety of donor blood and its components as exemplified by three multidisciplinary hospitals with blood transfusion departments in 2017-2020. The data concerning number and structure of donors, donations, amount of procured blood and its components, applied technologies ensuring safety and quality of blood components is presented. The achieved indicators resulted from improved forms of donor propaganda, taking of donors with low risk of hemo-transmissible infections and coordinated work of medical personnel.

RESUMO

The objective of the study is to develop and test a mechanism that allows the creation of hypotheses regarding the necessary provisions to update the medical care procedure, based on the analysis of forensic medical examinations (FME) conclusions, including issues on the assessment of the medical care quality. At present, the medical care procedure is determined by experts. This approach needs to be redesigned, but there is no formalized mechanism for doing so. The study presents the analysis of all the FME in the field of otorhinolaryngology performed in two FME bureaus of the constituent entities of the Russian Federation. Facts were revealed that provided adequate quality of medical care in specific cases and facts that did not affect the quality of medical care. These hypotheses should be considered by experts as primary for inclusion of new provisions in regulations or exclusion of those that do not affect the quality of medical care.

The procedures for the provision of medical care in otorhinolaryngology and audiology-otorhinolaryngology contain a large number of requirements. The actualization of the orders has not been carried out for a long time, which is due to the lack of methodological formalized approaches for the formation of hypotheses regarding expedient changes. The article proposes an analysis of the results of state control, which will allow the formation of specific proposals for assessment by experts in the framework of the process of updating the procedures for the provision of medical care. PURPOSE OF THE STUDY: To analyze the effectiveness of the formalized mechanism for updating the Procedures for the provision of medical care to the population in the field of otorhinolaryngology, approved by the orders of the Ministry of Health of Russia. MATERIAL AND METHODS: In this work, an analysis has been carried out, during which the provisions of the regulatory enactments of the Procedures for the provision of medical care in the field of otorhinolaryngology were investigated for the compulsory implementation and duplication. The analysis of 72 acts of inspections of state control in relation to medical organizations providing assistance (licensed) in the field of otorhinolaryngology was carried out. RESULTS: The analysis showed that only a small number of the foreseen requirements regulate the activities of medical organizations and can be subject to control. For the most part, these are requirements regarding the availability of equipment (medical devices). At the same time, the results of state control demonstrate that licensees violate the same provisions of the Procedures for the provision of medical care, including repeatedly. This circumstance should be considered as a trigger for the expert method of researching these provisions, in particular for their redundancy and a real contribution to the availability, quality and safety of medical care. FINDINGS: Today there is no formalized mechanism for updating the procedures for the provision of medical care. One of the possible triggers starting this process and guiding it is the analysis of the results of state control: 1) the absence of at least one single detection of violations of the rules and stages can alert from the point of view of the formulation of requirements - are they not recommendatory or declarative; 2) the presence of repeated violations in terms of the lack of identical positions of the equipment standards may indicate excessive requirements and should be evaluated and interpreted by experts with a decision to change the provisions of the procedure for the provision of medical care.

RESUMO

This article presents an analysis of the orders of the Ministry of Health of Russian Federation on the procedures for the provision of medical care, of which the recommended staffing standards are an integral part. Systematic erroneous provisions of regulatory documents were identified: general and typical for the normative number of employees in providing medical care to patients in outpatient and hospital settings. The authors have convincingly proved the need to revise documents on the Procedures for the provision of medical care in terms of labor standards.

The interests of society raise before the public authorities the issue of responsibility for solution of problem of unsafe Internet resources proposing health care services that can result in harm to health and life of citizen. There is a listing of information obligatory to be placed on official web-sites of medical organizations to permit users and public administration to obtain information about legal basics of medical care support. The remote trade of medications using Internet is prohibited and web-sites implementing this mode of sales are to be blocked. The Roszdravnadzor and its territorial agencies have powers concerning pretrial stopping similar web-sites functioning. At that, blocking mechanism differs the mechanism applied by the Roszdravnadzor i.e. the blocking is applied both to providers of hosting services and delegation of domain name and not to traffic filtration. The issue is discussed concerning further development of regulation of this sphere, including necessity of blocking web-resources popularizing non-acceptance of HIV and denial of AIDS therapy and also proposing abortion outside medical organization (at home) at the day of visit (without observing "silence period").
