RESUMO

INTRODUCTION: The case report describes a 29-year-old nulliparous woman that was admitted at the Department of Gynecology and Obstetrics of the Clinical Hospital Osijek complaining of mild abdominal pain without vaginal discharge. CASE OUTLINE: The patient's menstrual cycle was irregular, from 30-45 days. An ultrasound examination showed suspicion of an ectopic pregnancy with a betaHCG level of 1358 IU/L. Due to the presence of liquid in the pouch of Douglas the patient underwent emergency laparoscopy, which showed the presence of tumor mass between the right Fallopian tube and the appendix. These two structures associated with adhesions corresponded to secondary implantation after spontaneous tubal abortion which was confirmed by histopathologic analysis. CONCLUSION: Laparoscopy has emerged as the"gold standard"in the diagnosis and treatment of ectopic pregnancy, in this case the secondary abdominal pregnancy. From the diagnostic point of view, all women of reproductive age should be considered pregnant until proven otherwise, also keeping in mind that ectopic pregnancies can have different locations and many clinical features.

RESUMO

PURPOSE: The aim of this paper was to determine the incidence of high-grade squamous intraepithelial lesion (HSIL) and cancer of lower genital tract in patients with anogenital warts and their treatment. METHODS: Between 1 September 1988 and 30 June 1994, 221 consecutive women with anogenital warts were prospectively examined in the colposcopy outpatient clinic. In 2004, the same group of patients was re-examined. RESULTS: There were abnormal cytological findings in 111 (50.2%) patients; 31 patients (14.0%) had HSIL, and 6 of them (2.7%) had cancer. There were abnormal colposcopic findings in 152 (68.8%) women; 78 women (35.3%) had abnormal histological findings. In the histological findings, HSIL was found in 41 (18.5%) women and cancer in 6 (2.7%). CONCLUSION: In spite of the fact that anogenital warts are caused by HPV types of low oncogenic risk (6/11), there is an increased incidence of HSIL and cancer in these women. Therefore, women with anogenital warts belong to an increased risk group regarding the cancer of lower genital tract, especially if they are associated with HPV types of a high oncogenic risk.

RESUMO

Coexistence of laryngoceles and laryngeal carcinoma is still being debated, and there are several suggested theories about the pathophysiological relationship between these two entities. We present the case of a 66-year-old male patient with bilateral laryngomucoceles and laryngeal squamos cell carcinoma. A systematic hystological examination of whole organ sections showed that the submucosal spreading of cancer around the saccular necks on both sides caused stenosis which probably created a one-way valve mechanism allowing air to enter the saccule but not to exit. Progression of the tumor completely obstructed the laryngeal opening, leading to glandular secretion stagnation and formation of laryngomucoceles.

RESUMO

A case of persistent fetal palmar grasping of the umbilical cord in a 23-year-old primigravida is described. Palmar grasping was detected with three-dimensional ultrasound examination after cardiotocographically recorded fetal bradyarrhythmia. Because of acute fetal hypoxia, urgent Misgav-Ladach cesarean section was performed to deliver a hypotrophic female newborn, 2,120 g/43 cm, Apgar score 4/7, pH 7.29. No other pathology was found during the procedure. The postoperative course proceeded uneventfully and the early neonatal neurological status and neurosonographic findings were normal.
