RESUMO

The purpose of this study was to evaluate the ability of MRI to identify a primary site of malignancy in the breast of patients who present clinically with ipsilateral lymph nodes containing metastatic carcinoma but whose physical and mammographic examination are negative. MRI of the breast was performed on four patients using a variety of imaging parameters, all with and without gadolinium contrast. All patients had biopsy-proven adenocarcinoma of the ipsilateral axilla, with negative physical and mammographic examinations. Foci of enhancement assessed visually on precontrast and postcontrast scans (n = 1) and on subtraction studies (n = 3) were considered suspicious under the clinical circumstances defined for this study. Lesions identified on MRI were re-identified on ultrasound examination and either preoperative localization for excisional biopsy or tissue sampling was performed. Surgery was performed and histopathologic correlation was obtained in all cases. Primary sites of breast carcinoma were identified in all four patients, with multiple sites of malignancy identified in three of four patients. Breast conservation therapy was made possible for three of four patients based on the results of the MRI study showing sites of malignancy and no features of cancer elsewhere in the breast. Follow-up data of 1, 2, and 5 years of these patients show no evidence of recurrent disease. MRI of the breast is a useful technique for identifying primary sites of malignancy in patients presenting with ipsilateral lymph nodes positive for metastatic adenocarcinoma when the physical and mammographic examinations are negative.

RESUMO

A 1.5 cm focus in the breast with intense gadolinium uptake was incidentally detected during magnetic resonance (MR) imaging evaluation of a cluster of microcalcifications. Because the apparent mass was both mammographically and sonographically occult, MR imaging was used to guide a needle by "freehand technique" to the lesion, permitting successful pre-operative localization and subsequent surgical excision of the area.

Follow-up knee magnetic resonance (MR) examinations were performed on 17 patients (18 menisci) with arthroscopically proved tears of the outer third of the meniscus who were treated either conservatively (six patients) or with surgical repair (11 patients). All patients satisfied accepted clinical orthopedic criteria for meniscal healing. MR examinations obtained 3-27 months after injury revealed persistent signal intensity (grade 3), unchanged from that seen on the preoperative study, in all 15 patients in whom both pre- and postoperative studies were obtained and in three of four menisci that were proved to be healed at second-look arthroscopy. It appears that grade 3 signal from both conservatively treated and repaired menisci may persist long after the tear has become asymptomatic and has presumably healed. The presence of such signal should not be interpreted as necessarily indicating meniscal retear in these patients. Persistent signal intensity at the site of previous injuries may account for some reported cases of disagreement between MR and arthroscopic findings.

RESUMO

Focal I-131 accumulation is generally a reliable indicator of functioning thyroid tissue or a differentiated thyroid cancer metastasis. Normal accumulation of activity may be seen in areas such as the intestinal tract, liver, and salivary glands. This report describes a patient with significant accumulation of I-131 in the right upper quadrant of the abdomen. The abnormality, first thought to represent metastatic thyroid carcinoma, was subsequently proven to be accumulation within a large renal cyst.

RESUMO

Delineation of the thecal sac in CT can be improved by the presence of intrathecal metrizamide. This may be especially helpful in postoperative patients in whom the landmarks are often obscured by epidural scarring. Metrizamide-enhanced CT was performed on outpatients using 2 1/2 ml metrizamide (170 mg I/ml). Follow-up is available on 40 patients. The procedure was well tolerated with only three instances of severe headache, one case of vomiting, and no reported seizures. Adequate opacification was obtained at 92% of the levels examined with fair opacification at the remainder of the levels. This technique is considered a safe and useful outpatient procedure.

RESUMO

Colonic diverticulitis is frequently complicated by bowel obstruction, abscess, perforation and fistula formation. We report a unique case of colonic diverticulitis complicated by colovenous fistulization with Gastrografin enema examination demonstrating thrombus in the inferior mesenteric vein.
