[Infectious complications of severe trauma: achievements and problems]. / Infektsionnye oslozhneniia tiazheloi travmy: dostizheniia i problemy.

On the base of treatment results obtained in 2000 casualties the pathogenically substantiated classification of infectious complications of the injuries and traumas was proposed. The principles of classification are in conformity with modern ideas connected with the conception of systemic inflammatory response, the theory of general adaptation and the concept of traumatic disease. The classification determines the purposeful diagnostic tactics and provides the preventative therapy of traumas' infectious complications. Selection of special direction in the surgery of injuries in order to solve the problem of traumas' infectious complications in scientific, methodical and practical aspects is substantiated. First of all it is creation of specialized centers that provide the introduction into medical practice of the modern conception of traumas' infectious complications.

A present-day view on the problem of wound infection is analyzed with special reference to recent data. Active surgical approach in combination with many-component antibacterial and correcting treatment still remains important in prevention and treatment of wound infection.

[Indications, methods and results of amputations of the extremities in isolated, multiple and associated traumas]. / Pokazaniia, sposoby i rezul'taty amputatsii konechnostei pri izolirovannykh mnozhestvennykh i sochetannykh travmakh.

An analysis of 246 observations has shown that in patients with crushed extremities accompanied by shock the amputation is the most important antishock measure. The amputation performed by the method of primary surgical treatment of the wound is the method of choice. The amount of postoperative complications can be considerably reduced by using the primarily delayed suture, carbon sorbents on the stump wound, temporary immobilization of the extremity stump.

[Potentials of infusion therapy in enhancing the detoxification effect of hemosorption in patients suffering from suppurative-septic complications of mechanical injury]. / Vozmozhnosti infuzionnoi terapii v usilenii detoksikatsionnogo éffekta gemosorbtsii u postradavshikh s gnoino-septicheskimi oslozhneniiami mekhanicheskoi travmy.

Under study was the application of microvasoplegia as a method of preoperative management in fulfilling hemosorption in patients with pyo-septic complications of a severe mechanical trauma. The method of infusion and medicamentous therapy used before hemosorption is described. Laboratory examinations of the enzyme toxemia level have shown pathogenetic grounds of the complex method of treatment. Positive clinical results were obtained.

[Prevention and treatment of the suppurative complications of severe mechanical trauma]. / Profilaktika i lechenia gnoinykh oslozhnenii tiazheloi mekhanicheskoi travmy.

A curative mixture (contrical, hydrocortisone, chloramphenicol-succinate) was tested by the authors for the local or intraosseous introduction as blockades in 11 patients with severe mechanical injuries. Thirty eight patients with good outcomes of shock without hormonal drugs and 23 healthy donors were taken as control. It was established that massive doses of glucocorticosteroids during the first day after the injury exercised a regulating effect upon the dynamics of the cellular and humoral immunity.
