RESUMO

A comparative study of diagnostic potential of excretory intravenous urography, radionuclide renography and angionephroscintigraphy, renal ultrasonography and ultrasonic dopplerography of renal arteries in prehospital examination for renovascular hypertension made it possible to give preference to the above dopplerography. The latter is able to provide reliable data on renal artery stenosis and genesis of arterial hypertension.

RESUMO

The authors examined 42 patients with obstructive jaundice of various severity. Comparison of the findings of radiohepatography with intoxication values according to the medium-weight molecule test and the biochemical parameters revealed statistically significant differences of some quantitative indices depending of the severity of jaundice. The results of the study made it possible to recommend the use of the values of blood clearance, time of maximum accumulation of a radiopharmacological agent in the liver, hepatic and blood retention index, and the medium-weight molecule test for objective quantitative appraisal of the severity of obstructive jaundice. Study of the time course of changes of the above parameters showed that normalization of hepatic function in resolving jaundice begins only from the second week after decompression of the biliary tract, which must be borne in mind in choosing the optimal time for the operation.

Overall 146 females suffering from essential hypertension (borderline, stage I and II according to WHO, 1980) were examined for basal concentrations of estradiol, estriol, progesterone, testosterone, cortisol, ACTH, prolactin in plasma, LH and FSH in blood and urine. The patients and 69 control normotensive females were reproductive, premenopausal, menopausal, postmenopausal, postcastration. Latent hypertension was elicited at bicycle exercise. It was found that relationships between sex hormones and gonadotropins both in hypertensive and normotensive women did not differ noticeably in reproductive period and in menopause. Hormone defects due to primary and secondary infertility, castration, menopause did not raise the risk of arterial hypertension onset. An-Unfavorable course of hypertension was observed in premenopause, following operative castration, in climacteric syndrome. The severity of AH at reproductive age was associated with hyposecretion of progesterone, estrogens and gonadotropins LH, FSH. This relationship was absent in menopausal women. A conclusion is proposed on miner significance of sex hormones imbalance and gonadotropins in the onset of arterial hypertension in females varying by generative activity.

RESUMO

Complex study of the frequency and intensity of the duodenogastric reflux in 468 patients with peptic ulcer is analysed; 372 had duodenal ulcer and 96 had gastric ulcer. The authors give the characteristics of the diagnostic possibilities of X-ray, endoscopic, ionomanometric, and aspiration (radionuclide and biochemical) methods of examination of duodenal regurgitation. They proved the relation of the frequency and intensity of the duodenal regurgitation to the type of the ulcer and the phase of its course (exacerbation or remission), the condition of the pyloric closure function, and disorders of duodenal motor activity. The sequelae of long-term duodenogastric reflux are analysed; it is shown that patients must be examined in the preoperative period in the phase of remission so as to gain the true values of intensity of the reflux for its subsequent correction.

Platelet-vascular hemostasis and cyclic nucleotide levels as well as the effects of mexitil, ritmilen and prolecofen on those were examined in 58 coronary patients with various heart rhythm disorders. The patients showed considerably increased platelet activity, low prostacyclin level and high thromboxane and cyclic nucleotides levels, particularly in patients with high-grade ventricular extrasystoles. Mexitil did not essentially affect the parameters in question. Ritmilen produced a significant increase in 6-keto-PGF1 alpha and a decrease of platelet activity. In prolecofen-treated patients, platelet activity decreased significantly, cAMP and cGMP dropped while the cAMP/cGMP ratio went up; prostacyclin and thromboxane changes were not significant. This may be a mechanism of prolecofen's antiarrhythmic action.

RESUMO

A new method for the interpretation of data obtained during radionuclide investigation of the reticuloendothelial system (RES) was proposed. It was based on a detailed description of the mechanism of reticuloendothelial extraction of a colloid injected intravenously. A model developed for this purpose included the main stages of radioactive colloid transport. The paper also represented a comparative analysis of the results of clinical examination of the controls and patients with liver cirrhosis. A 4-fold decrease of the blood flow in portal liver cirrhosis was noted as compared with normal mean indices in a 2-fold decrease in the specific blood supply of cells of the RES and in a 2-fold decrease in its volume. An increase in phagocytosis rate in patients with liver cirrhosis needs further verification.

RESUMO

A total of 250 subjects were investigated using a diagnostic search flow-chart. Pulmonary artery thromboembolism (PATE) was diagnosed in 102 patients. The results have demonstrated the value of laboratory tests employed--markers of thrombin- and plasminemia and platelet activation--for PATE diagnosis. To diagnose PATE, levels of fibrinogen-fibrin and beta-thromboglobulin degradation products should be measured in myocardial infarction patients, and plasma soluble fibrin measurement should be added to those in patients with circulatory insufficiency. In recurrent PATE, continuous increase of plasma beta-thromboglobulin and soluble fibrin levels, antithrombin III exhaustion and reduced levels of fibrinogen-fibrin degradation products are of diagnostic value.

RESUMO

Ischaemic heart disease (IHD) in patients after amputation of the limbs runs a course with little manifest clinical symptoms, but with high risk of development of myocardial infarction with a lethal outcome. In patients after amputation of limbs there is a decrease of the blood stroke volume, increase of the total peripheral resistance and also marked disorders of the blood rheology and the function of platelets. In patients with IHD after amputation of the limbs for obliterating diseases of the vessels such changes are manifest in disorders of the microrheological properties of the blood (the rate of aggregation of red cell and the stability of red cell aggregates). Physical rehabilitation and prostheses in patients of this groups must be realized under careful cardiological control.

One hundred and sixty-two patients with acute myocardial infarction, whose ages ranged from 36 to 65, were studied. The content of thyroid hormones was found to be reduced in the acute period of myocardial infarction. The triiodothyronine level was lower on the 2nd day of the disease, the thyroxine content decreased on the 10th day of the disease but became normal on the 20-25th day. An interrelation between the clinical course of the disease and the dynamics of changes in the cholesterol content, rate of erythrocyte aggregation, the content of fibrinogen degradation products, and the level of thyroid hormones was determined. Normalization of the values was determined. Normalization of the values studied occurred earlier in the group of patients with a more favourable course of the acute period of myocardial infarction than in those with severe myocardial infarction. The data obtained show a close interrelation between thyroid activity and values studied and the clinical course of the disease.

Aggregation of erythrocytes was determined in 50 patients with angina pectoris and myocardial infarction and in 20 subjects of the control group by rheoscopy after Schmid-Schoenbein. It is shown that in patients with ischemic heart disease aggregation of erythrocytes is considerably increased and the hydrodynamic strength of the aggregates is greater. The erythrocyte aggregation curve is some patients with ischemic heart disease was of a biexponential character in distinction to that in subjects of the control group, which was of the monoexponential type. It was found that the increase in the strength of the erythrocyte aggregation and in the rate of aggregate formation was due to fibrinogen and its complex compounds with decomposition products.
