[PULMONARY AUTOGRAFT OPERATION IN PEDIATRIC PATIENTS].

Experience of performance of a pulmonary autograft operation (PAO, оperation of Ross) was analyzed, results were estimated, the ways of overcoming a procedure conduction problems were determined. Remote result was investigated in 151 patients: the aortal valve stenosis was diagnosed in 99 (65.6%) patients, insufficiency - in 42 (27.8%), combined aortal valve failure - in 10 (6.6%). Hospital lethality have constituted 3.6%, and a general one ­ 7.9%. Reoperations were performed in 52 patients. PAO constitutes one of surgical procedures for the aortal valve failure in children of different age. Improvement of surgical technique and application of a new procedures have promoted improvement of the investigation results ­ lethality lowering from 15.1 to 0.8%. Application of surgical modifications with the aortal radix strengthening have reduced the reoperations on neaortal valve risk trustworthy.

[RESULTS OF THE TOTAL CAVA-PULMONARY CONNECTION PROCEDURE WITH CORRECTION OF ATRIO-VENTRICULAR VALVES IN PATIENTS OF DIFFERENT AGE, SUFFERING SINGLE CARDIAC VENTRICULUS].

The investigation objective was to improve the patients' management, suffering a single cardiac ventriculus, in whom a total cavа-pulmonary connection (ТСРС) operation was performed together with correction of the atrio-ventricular valves (AVV) insufficiency, basing on analysis of immediate and middle-term results. From 2005 to 2015 yrs in the clinic the ТСРС operation was performed in 132 patients. Of them 24 patients were choosed , in whom moderate and pronounced insufficiency of systemic AVV was diagnosed. Good early and middle-term results were achieved in 92% patients. In 8 observations, while the AVV correcting, in every patient two and more procedures of plasty were applied. Complex approach, using combination of various procedures of plasty, has guaranteed the optimal result. Application of procedures for the fiber ring stabilization have promoted achievement of best results.

[Senning operation as a method of treatment of the main vessels transposition].

Results of performance of Senning operation for the main vessels transposition were analyzed. Analysis of complications, lethality in early postoperative period and of far-remote results was conducted. The systemic right ventricle function in patients after Senning operation was estimated. Not high level of early and late postoperative lethality was established. Clinical state of patients in far-remote postoperative period was good. A systemic right ventricle function was estimated as satisfactory.

[Pulmonary atresia with ventricular septal defect and major aorto-pulmonary collateral arteries: diagnosis and treatment].

The basic principles of diagnosis and management of pulmonary atresia with ventricular septal defect and major aorto-pulmonary collateral arteries are systematizes in this paper. The personal experience of surgical treatment of consecutive 66 patients with this disease is analyzed, including one-stage and multistage approaches.

[Strategies in surgical treatment of children with patent ductus arteriosus in their first year of life]. / Taktyka khirurhichnoho likuvannia ditei pershoho roku zhittia z vidkrytym povnym atrioventrykuliarnym kanalom.

Experience of surgical treatment of the open complete atrioventricular channel (OCACH) in 78 children of the first year of life was summarized. Congestive cardiac insufficiency and pronounced pulmonary hypertension had constituted the main indications for hospitalization. Surgical treatment of OCACH was performed in 59 (75.6%) patients, including those, operated on using artificial blood circulation--41 (69.5%) and according to the closed method--18 (30.5%). The pulmonary artery narrowing as the first stage of surgical treatment was performed in 17 patients, pulmonary biopsy--in 1. Radical anatomical correction of the failure was done in 40 patients, the correction of its unbalanced form using artificial blood circulation--in 1. Poor state of the first year of life children with OCACH was caused by congestive cardiac insufficiency already in a newborn period. That's why they need medicinal therapy right from the first days of their life, operative treatment of the failure is indicated in early age (3-6 months). Results of surgical treatment of OCACH depend upon early diagnosis of the failure in a newborn child and its timely direction to specialized institution for operative treatment.

Experience of surgical treatment of the two-barrel branching of main vessels from right ventricle in the first year of life children was summarized. There were operated 105 patients. In 53 (50.5%) infants in arms radical intervention (intraventricular tunneling, operation of Kavashima, arterial switching) was performed with general lethality 24.5%. In 52 (49.5%) patients--palliative operation was done (formation of Blelock--Taussig anastomosis, the pulmonalis artery narrowing) with general lethality 12.7%.
