Oxidized compounds in the atmosphere can occur as emitted primary compounds or as secondary products when volatile emitted precursors react with various oxidants. Due to the presence of polar functional groups, their vapor pressures decrease, and they condense onto small particles. Thereby, they have an effect on climate change by the formation of clouds and scattering solar radiation. The particles and oxidized compounds themselves can cause serious health problems when inhaled. Therefore, it is of utmost importance to study oxidized compounds in the atmosphere. Much ongoing research is focused on the discovery of new oxidized substances and on the evaluation of their sources and factors influencing their formation. Monitoring biogenic and anthropogenic primary oxidized compounds or secondary oxidized products in chamber experiments or field campaigns is common. New discoveries have been reported, including various oxidized compounds and a new group of compounds called highly oxidized organic molecules (HOMs). Analytics of HOMs are mainly focused on chromatography and high-resolution mass spectrometry employing chemical ionization for identifying and quantifying compounds at low concentrations. Oxidized compounds can also be monitored by spectrophotometric methods in which the determinations of total amounts are based on functional groups. This review highlights recent findings on oxidized organic compounds in the atmosphere and analytical methodologies used for their detection and quantification. The discussion includes gas and liquid chromatographic methods, sampling, extraction, concentration, and derivatization procedures involved, as well as mass spectrometric and spectrophotometric methods.

The adsorption and desorption behavior of volatile nitrogen-containing compounds in vapor phase by solid-phase microextraction Arrow (SPME-Arrow) and in-tube extraction (ITEX) sampling systems, were investigated experimentally using gas chromatography-mass spectrometry. Three different SPME-Arrow coating materials, DVB/PDMS, MCM-41, and MCM-41-TP and two ITEX adsorbents, TENAX-GR and MCM-41-TP were compared to clarify the selectivity of the sorbents towards nitrogen-containing compounds. In addition, saturated vapor pressures for these compounds were estimated, both experimentally and theoretically. In this study, the adsorption of nitrogen-containing compounds on various adsorbents followed the Elovich model well, while a pseudo-first-order kinetics model best described the desorption kinetics. Pore volume and pore sizes of the coating sorbents were essential parameters for the determination of the adsorption performance for the SPME-Arrow sampling system. MCM-41-TP coating with the smallest pore size gave the slowest adsorption rate compared to that of DVB/PDMS and MCM-41 in the SPME-Arrow sampling system. Both adsorbent and adsorbate properties, such as hydrophobicity and basicity, affected the adsorption and desorption kinetics in SPME-Arrow system. The adsorption and desorption rates of studied C6H15N isomers in the MCM-41 and MCM-41-TP sorbent materials of SPME-Arrow system were higher for dipropylamine and triethylamine (branched amines) than for hexylamine (linear chain amines). DVB/PDMS-SPME-Arrow gave fast adsorption rates for the aromatic-ringed pyridine and o-toluidine. All studied nitrogen-containing compounds demonstrated high desorption rates with DVB/PDMS-SPME-Arrow. Chemisorption and physisorption were the sorption mechanisms in MCM-41- and MCM-41-TP- SPME-Arrow, but additional experiments are needed to confirm this. An active sampling technique ITEX gave comparable adsorption and desorption rates on the selective MCM-41-TP and universal TENAX-GR sorbent materials for all the compounds studied. Vapor pressures of nitrogen-containing compounds were experimentally estimated by using retention index approach and these values were compared with the theoretical ones, calculated using the COnductor-like Screening MOdel for Real Solvent (COSMO-RS) model. Both values agreed well with those found in the literature proving that these methods can be successfully used in predicting VOC's vapor pressures, e.g. for the formation of secondary organic aerosols.

The World Health Organization's tumor classification guidelines are frequently updated and renewed as knowledge of cancer biology advances. For instance, in 2021, a novel lung tumor subtype named SMARCA4-deficient, undifferentiated tumor (SMARCA4-dUT, code 8044/3) was included. To date, there is no defined cell model for SMARCA4-dUT that could be used to help thoracic clinicians and researchers in the study of this newly defined tumor type. As this tumor type was recently described, it is feasible that some cell models formerly classified as lung adenocarcinoma (LUAD) could now be better classified as SMARCA4-dUT. Thus, in this work, we aimed to identify a bona fide cell model for the experimental study of SMARCA4-dUT. We compared the differential expression profiles of 36 LUAD-annotated cell lines and 38 cell lines defined as rhabdoid in repositories. These comparative results were integrated with the mutation and expression profiles of the SWI/SNF complex members, and they were surveyed for the presence of the SMARCA4-dUT markers SOX2, SALL4, and CD34, measured by RT-qPCR and western blotting. Finally, the cell line with the paradigmatic SMARCA4-dUT markers was engrafted into immunocompromised mice to assess the histological morphology of the formed tumors and compare them with those formed by a bona fide LUAD cancer cell line. NCI-H522, formerly classified as LUAD, displayed expression profiles nearer to rhabdoid tumors than LUAD tumors. Furthermore, NCI-H522 has most of the paradigmatic features of SMARCA4-dUT: hemizygous inactivating mutation of SMARCA4, severe SMARCA2 downregulation, and high-level expression of stem cell markers SOX2 and SALL4. In addition, the engrafted tumors of NCI-H522 did not display a typical differentiated glandular structure as other bona fide LUAD cell lines (A549) do but had rather a largely undifferentiated morphology, characteristic of SMARCA4-dUT. Thus, we propose the NCI-H522 as the first bona fide cell line model of SMARCA4-dUT. © 2023 The Authors. Solid phase microextraction Arrow and thermal desorption-gas chromatography-mass spectrometry allowed the collection and evaluation of volatile organic compounds (VOCs) emitted by fungal cultures from building insulation materials and in indoor air. Principal component analysis, linear discriminant analysis and supported vector machine were used for visualization and statistical assessment of differences between samples. In addition, a screening tool based on the soft independent modelling of class analogies (SIMCA) was developed for identification of fungal contamination of indoor air. Ten different fungal strains, incubated under ambient and microaerophilic conditions, were analyzed for time period ranging from 5 to 29 days after inoculation resulting in a total of 140 samples. In addition, the effect of additives on the fungal growing media was studied. The total number of compounds and concentration values were used for the evaluation of the results. Clear differences were observed for VOC profiles emitted by different fungal strains by exploiting long chain alcohols (3-octanol, 1-hexanol and 2-octen-1-ol) and sesquiterpenes (farnesene, cuprene). The analysis of glass-wool and cellulose based building insulation materials (3 samples) gave clear differences, mainly for oxygenated compounds (ethyl acetate and hexanal) and benzenoids (benzaldehyde). Moreover, the comparison of indoor air and insulation materials collected from a house with fungal indoor air problems indicated that 42% of the VOCs were found in both samples. The analysis of 52 indoor air samples demonstrated clear differences in their VOC profiles, especially for hydrocarbons, and between control (44 samples) and indoor air problem houses (8 samples). Finally, the SIMCA model enabled to recognize differences between control and fungi contaminated houses with a prediction capacity over 84%.

Raman spectroscopy together with comprehensive two-dimensional gas chromatography-time-of-flight mass spectrometry (GCxGC-TOFMS) was employed to characterize exomere- (<50 nm) and exosome-sized (50-80 nm) EVs isolated from human plasma by the novel on-line immunoaffinity chromatography - asymmetric flow field-flow fractionation method. CD9+, CD63+, and CD81+ EVs were selected to represent general EV subpopulations secreted into plasma, while CD61+ EVs represented the specific EV subset derived from platelets. Raman spectroscopy could distinguish EVs from non-EV particles, including apolipoprotein B-100-containing lipoproteins, signifying its potential in EV purity assessment. Moreover, platelet-derived (CD61+) EVs of both exomere and exosome sizes were discriminated from other EV subpopulations due to different biochemical compositions. Further investigations demonstrated composition differences between exomere- and exosome-sized EVs, confirming the applicability of Raman spectroscopy in distinguishing EVs, not only from different origins but also sizes. In addition, fatty acids that act as building blocks for lipids and membranes in EVs were studied by GCxGC-TOF-MS. The results achieved highlighted differences in EV fatty acid compositions in both esterified (membrane lipids) and non-esterified (free fatty acids) fractions, indicating possible differences in membrane structures, biological functions, and roles in cell-to-cell communications of EV subpopulations.

A wide variety of nitrogen-containing compounds are present in the environment, which contributes to air pollution and new particle formation, for example. These eventually affect human health and the climate. With all this consideration, there is a growing interest in the development of efficient and reliable methods to determine these compounds in the atmosphere. In this study, titanium hydrogen phosphate-modified Mobil Composition of Matter No. 41 was used as sorbent material for in-tube extraction (ITEX) sampling system, to selectively collect nitrogen-containing compounds from natural air samples. The effect of sampling accessories, based on adsorbent coatings (with Tenax-GR as an adsorbent material) and polytetrafluoroethylene filters, was studied to improve the selectivity of the sampling system and to remove particles. Aerial drone with miniaturized air sampling system was employed for the reliable collection of nitrogen-containing compounds in both gas phase and aerosol particles. A total of 170 air samples were collected in July 2020 at the SMEAR II station, Finland to evaluate nitrogen-containing compounds diurnal patterns and vertical profiles (0.25, 5, 50, and 150 m). More than twenty nitrogen-containing compounds, such as aliphatic amines, imines, imidazoles, and pyridines, were identified, quantified or semi-quantified. The average concentrations of detected aliphatic amines at the altitude of 50 m were up to 40.4 ng m-3 (dimethylamine) in gas phase and 128 ng m-3 (ethylamine) in aerosol particles. Among nitrogen-containing compounds detected, pyridine gave the highest average concentration of 746 ng m-3 in gas phase and 644 ng m-3 in particle phase.

Our second generation air sampling drone system, allowing the simultaneous use of four solid phase microextraction (SPME) Arrow and four in-tube extraction (ITEX) units, was employed for collection of atmospheric air samples at different spatial and temporal dimensions. SPME Arrow coated with two types of materials and ITEX with 10% polyacrylonitrile as sorbent were used to give a more comprehensive chemical characterization of the collected air samples. Before field sampling, miniaturized samplers went through quality control and assurance in terms of reproducibility (RSD ≤14.1%, N = 4), equilibrium time (≥10 min), breakthrough volume (1.8 L) and storage time (up to 48 h). 128 air samples were collected under optimal sampling conditions from July to September 2019 at the SMEAR II station and Qvidja farm, Finland. 347 VOCs were identified in the air samples either on-site or in the laboratory by thermal desorption gas chromatography - mass spectrometry, and they were quantified/semiquantified using Partial Least Squares Regression models. Individual models were developed for the different coatings and packing materials using gas phase standards obtained by an automatic permeation system. Average gas phase VOC concentrations ranged from 0.1 (toluene, the SMEAR II station) to 680 ng L-1 (acetone, Qvidja farm). Average VOC concentrations in aerosols ranged from 0.1 (1,4-cyclohexadiene, the SMEAR II station) to 2287 ng L-1 (megastigma-4,6,8-triene, Qvidja farm). Clear differences in results were seen for samples collected at the SMEAR II station and Qvidja farm, between VOC compositions in gas phase and aerosols, and between the sampling site and height.

An automated on-line isolation and fractionation system including controlling software was developed for selected nanosized biomacromolecules from human plasma by on-line coupled immunoaffinity chromatography-asymmetric flow field-flow fractionation (IAC-AsFlFFF). The on-line system was versatile, only different monoclonal antibodies, anti-apolipoprotein B-100, anti-CD9, or anti-CD61, were immobilized on monolithic disk columns for isolation of lipoproteins and extracellular vesicles (EVs). The platelet-derived CD61-positive EVs and CD9-positive EVs, isolated by IAC, were further fractionated by AsFlFFF to their size-based subpopulations (e.g., exomeres and exosomes) for further analysis. Field-emission scanning electron microscopy elucidated the morphology of the subpopulations, and 20 free amino acids and glucose in EV subpopulations were identified and quantified in the ng/mL range using hydrophilic interaction liquid chromatography-tandem mass spectrometry (HILIC-MS/MS). The study revealed that there were significant differences between EV origin and size-based subpopulations. The on-line coupled IAC-AsFlFFF system was successfully programmed for reliable execution of 10 sequential isolation and fractionation cycles (37-80 min per cycle) with minimal operator involvement, minimal sample losses, and contamination. The relative standard deviations (RSD) between the cycles for human plasma samples were 0.84-6.6%.

Several calibration approaches were evaluated for the quantitation of volatile organic compounds in air using miniaturized exhaustive and non-exhaustive sampling techniques, such as in-tube extraction (ITEX) and solid phase microextraction (SPME) Arrow. Eleven compounds, 2-ethyl-hexanol, hexanal, nonanal, toluene, ethyl-benzene, methyl isobutyl ketone, acetophenone, p-cymene, α-pinene, trimethylamine and triethylamine, all them found in the natural air samples, were selected as model analytes. Liquid injection, liquid standard addition to the sorbent bed and gas phase standards provided by an automatic permeation system, were evaluated in the case of ITEX packed with laboratory-made 10% polyacrylonitrile (PAN) material. Two different approaches, based on sampling of gas phase compounds from the permeation system and from sample vial containing gas phase standards, were evaluated for SPME Arrow with two different coatings, commercial divinylbenzene-poly(dimethylsiloxane) (DVB-PDMS) and laboratory-made mesoporous Mobil Composition of Matter No. 41 (MCM-41). In addition, interface model approach was used for the calculation of the real concentration of the target analytes in the sample from the total amount of analytes injected into the GC-MS in the case of SPME Arrow. Similar results were obtained with the different approaches used for the quantitation by ITEX and SPME Arrow. However, the use of gas phase standards with sample matrix similar to the natural samples, allowed the permeation system to provide the most reliable results for the quantitation of the target analytes. For this approach, linearity (expressed as r2 values) ranged between 0.991 and 0.999. The limit of detection ranged from 0.5 µg/m3 (trimethylamine, MCM-41) to 2.2 × 10-4 µg/m3 (methyl isobutyl ketone, MCM-41). In addition, the use of the fully automated permeation system provided good reproducibility values that were between 1.4% (acetophenone, MCM-41) and 7.8% (methyl isobutyl ketone, 10% PAN). The linear ranges were at least 3 order of magnitude for all the studied analytes with the exception of the calibration curve developed for trimethylamine with SPME Arrow (linear ranges between LOQ and 4.9 µg/m3 (DVB-PDMS) and LOQ and 9.8 µg/m3 (MCM-41)).

The applicability of an aerial drone as a carrier for new passive and active miniaturized air sampling systems, including solid phase microextration Arrow (SPME Arrow) and in-tube extraction (ITEX), was studied in this research. Thermal desorption, gas chromatography and mass spectrometry were used for the determination of volatile organic compounds (VOCs) collected by the sampling systems. The direct comparison of the profiles of VOCs, simultaneously sampled in air by SPME Arrow system including four different coatings, allowed the elucidation of their adsorption selectivity. A more complex experimental design, involving 20 samples (10 flights) and non-supervised pattern recognition techniques, was needed for the clarification of the same sampling parameters in the case of five ITEX sorbent materials. In addition, ITEX sampling accessories, such as particle, water and ozone traps, were evaluated by comparing the results obtained for air samples simultaneously collected by two ITEX systems, packed with the same sorbent and furnished or not with sampling accessories. The effect of the aerial drone horizontal displacement (HD) on the sampling efficiency was clear in the case of SPME Arrow. The number of detected compounds and their relative peak area values (RPA) revealed a clear increase (4 and 43%, respectively) in comparison with samples collected without drone HD. However, just minor differences were observed in the case of ITEX (2 compounds and 9% of the ∑RPA). In addition, the system was able to provide almost simultaneous passive (SPME Arrow) and active (ITEX) samplings at different altitudes (5 and 50 m), being a good tool for low cost vertical profiling studies (∑RPA decreased over 35% for the samples collected at 50 m). Finally, the successful simultaneous air sampling by SPME Arrow and ITEX systems in two difficult access places, such as boreal forest and wetlands, was demonstrated, resulting in 21 and 31 detected compounds in forest and wetlands by SPME Arrow, and 27 and 39 compounds by ITEX.

INTRODUCCIÓN: La intervención quirúrgica en las enterocolitis necrosantes (EN) es precisa cuando existe gangrena intestinal, hecho evidente cuando produce perforación y neumoperitoneo, siendo este la única indicación radiológica aceptada universalmente para la intervención quirúrgica. OBJETIVO: Analizar a los pacientes intervenidos de EN, saber por qué se les intervino, cómo evolucionan y si los pacientes perforados presentan neumoperitoneo. MÉTODO: Estudio retrospectivo de una cohorte de recién nacidos con EN intervenidos durante un periodo de 10 años (2006-2015). Se analizan los hallazgos radiológicos preoperatorios y se correlacionan con los quirúrgicos y con la morbimortalidad, dependiendo de la presencia de neumoperitoneo (N+) o no (N-). Se evaluó la concordancia interobservador con radiólogo pediátrico enmascarado a la clínica mediante el índice de acuerdo kappa. RESULTADOS: Se analizó a 53 pacientes. El 36% se intervino tras la visualización de neumoperitoneo; en el resto, la indicación fue deterioro clínico y metabólico, junto con hallazgos radiológicos asociados. En el 39% del grupo N- se objetivó perforación. No se encontraron diferencias significativas en ambos grupos con respecto a longitud intestinal resecada, días de intubación, día de inicio de nutrición enteral y mortalidad. La comparación entre duración de síntomas y estancia hospitalaria total en ambos grupos (N-/N+) fue significativa (7 vs. 2 días, p = 0,008; 127 vs. 79 días, p = 0,003 respectivamente), siendo más favorable en el grupo N+. Estas diferencias se mantuvieron al ajustar por peso. CONCLUSIONES: La indicación quirúrgica ha de basarse en un conjunto de datos clínicos y radiológicos, ya que el 39% de los pacientes sin neumoperitoneo presentaron perforación. En nuestro estudio la presencia de neumoperitoneo no se correlaciona con peor pronóstico


INTRODUCTION: Surgical intervention in necrotising enterocolitis (NEC) is correct when there is intestinal gangrene. This is evident when gangrene produces perforation and pneumoperitoneum, with this being the only universally accepted radiological indication for the surgical intervention of NEC. OBJECTIVE: To perform an analysis on patients with surgically managed NEC, including determining how the decision to intervene is reached, the outcomes, and if patients with perforation had a pneumoperitoneum. METHODS: Retrospective review of neonates with surgical NEC over a period of 10years (2006-2015). An analysis was made of pre-surgical x-ray findings, which were compared with surgical ones, in addition to the morbidity and mortality, depending on the presence (N+) or absence (N-) of pneumoperitoneum. An evaluation was also made of the interobserver concordance with a paediatric radiologist blinded to the clinical reason using the kappa agreement index. RESULTS: A total of 53 neonates were included in the study. Surgical treatment was indicated after observing pneumoperitoneum in 36%. In the remaining neonates, the surgical decision was made after noting a clinical and metabolic deterioration with classical x-ray findings. Intestinal perforation was observed in 39% of the N- neonates. There were no statistical differences between either group on analysing the excised intestinal length, days of intubation, starting of enteral nutrition, and the mortality rate. Comparisons in terms of duration of symptoms and total hospital stay were statistically significant (7 vs. 2 days, P = .008; 127 vs. 79 days, P = .003, respectively), with both being more favourable in the N+ group. These differences remained when the groups were adjusted by birthweight. CONCLUSIONS: Surgical indication has to be done on an ensemble of clinical and radiological evidence, as 39% of the neonates in the N- groups were perforated. In our study, the presence of a pneumoperitoneum did not correlate with a worse prognosis

This is evident when gangrene produces perforation and pneumoperitoneum, with this being the only universally accepted radiological indication for the surgical intervention of NEC. OBJECTIVE: To perform an analysis on patients with surgically managed NEC, including determining how the decision to intervene is reached, the outcomes, and if patients with perforation had a pneumoperitoneum. METHODS: Retrospective review of neonates with surgical NEC over a period of 10years (2006-2015). An analysis was made of pre-surgical x-ray findings, which were compared with surgical ones, in addition to the morbidity and mortality, depending on the presence (N+) or absence (N-) of pneumoperitoneum. An evaluation was also made of the interobserver concordance with a paediatric radiologist blinded to the clinical reason using the kappa agreement index. RESULTS: A total of 53 neonates were included in the study. Surgical treatment was indicated after observing pneumoperitoneum in 36%. In the remaining neonates, the surgical decision was made after noting a clinical and metabolic deterioration with classical x-ray findings. Intestinal perforation was observed in 39% of the N- neonates. There were no statistical differences between either group on analysing the excised intestinal length, days of intubation, starting of enteral nutrition, and the mortality rate. Comparisons in terms of duration of symptoms and total hospital stay were statistically significant (7 vs. 2 days, P=.008; 127 vs. 79 days, P=.003, respectively), with both being more favourable in the N+ group. These differences remained when the groups were adjusted by birthweight. CONCLUSIONS: Surgical indication has to be done on an ensemble of clinical and radiological evidence, as 39% of the neonates in the N- groups were perforated. In our study, the presence of a pneumoperitoneum did not correlate with a worse prognosis.

Introducción: El teratoma sacrococcígeo es el tumor neonatal sólido más frecuente. El aumento de la supervivencia de estos pacientes ha permitido diagnosticar y tratar secuelas posquirúrgicas. Evaluamos el resultado funcional y estético a largo plazo de los pacientes intervenidos en nuestro centro. Material y método: Estudio retrospectivo de los teratomas sacrococcígeos intervenidos en nuestro hospital desde 1977 hasta 2014. Se recogieron datos personales de los pacientes y se realizó encuesta telefónica sobre hábito intestinal, urinario y autoevaluación estética y social. Resultados: En el período de estudio se intervino a 14 pacientes, 11 mujeres y 3 varones, con una edad mediana en el momento de la encuesta de 17 años (8 meses-37 años). Se encuestó a 8 pacientes (57,1%). La edad mediana de esos 8 pacientes fue de 23 años (4-37 años). El 37,5% se intervino por un teratoma sacrococcígeo tipo I, 25% tipo II, 25% tipo III y 12,5% tipo IV. De los pacientes estudiados, 2 (25%) presentan estreñimiento y uno (12,5%) incontinencia fecal. Dos pacientes (25%) presentan infecciones urinarias recurrentes y 3 (37,5%) incontinencia urinaria. Cinco pacientes (62,5%) presentan alterada su percepción física con limitación en su vida social. Conclusiones: En nuestra serie, se ha encontrado un porcentaje similar a la literatura en la incidencia de estreñimiento. La incontinencia fecal está levemente disminuida respecto a series publicadas. Sin embargo, la frecuencia de infecciones y de incontinencia urinarias es mayor en nuestra serie. Cinco pacientes presentan problemas psicosociales según el cuestionario DAS-59. Los pacientes con teratoma sacrococcígeo requieren asesoramiento urológico, digestivo y psicológico, para poder adquirir un completo desarrollo funcional y emocional (AU)

RESUMO

Nephron sparing surgery (NSS) is increasingly utilized to treat patients with bilateral Wilms tumor. We present a case of NSS planning using a three-dimensional computerized and printed model of both kidneys with anatomical structures of interest (parenchyma, renal pelvis, major calyx, renal artery, renal vein, and tumor). This model allowed a better understanding of the anatomic relation between the tumor and the normal kidney on each side, improving the surgical planning and the preoperative discussion with the patient's family.

INTRODUCTION: Long-term evolution of residual hydronephrosis after successful pyeloplasty is not common. In this report, we have studied new ultrasound parameters, and have investigated the frequency of residual hydronephrosis. We highlight alternative radiation-free monitoring of pediatric pyeloplasties with ultrasound alone. PATIENTS AND METHODS: Children who had undergone successful open Anderson-Hynes pyeloplasties in the period 2001-2010 were followed up. Exclusion criteria included non-clearly obstructed renography scans (tracer clearance half-time < 20 min), crossing vessels, failed pyeloplasty, bilateral disease, other renal concomitant anomaly, absence of ultrasound measurements, and loss to follow up (<1 year). Postoperative (postoperative) ultrasound parameters were anteroposterior (AP) diameter, pelvis-cortex (P/C) ratio, a proportion that takes in account these two values, enhancing sensitivity to evaluate minimal evolutive changes), and percentage of improvement (PI) in AP diameter (which reflects in relative means the evolution of each AP diameter, being 0% no change, and 100% absence of hydronephrosis) (Figure). Echographic checks were made at 3 and 6 months postoperatively and then yearly afterwards. SPSS software (v. 17.0 IBM, College Station, TX, USA) was used. RESULTS: Out of 80 pyeloplasties performed in the above-mentioned period, 44 patients (i.e., 44 renal units) fulfilled the inclusion criteria for the main study. The median age at time of operation was 15 months (range 2 months-10.3 years). The median follow-up was 4.5 years (range 1-12 years). The mean preoperative anteroposterior diameter was 26 mm (range 16-54 mm). At the third postoperative check, the mean the PI was 29%, and rose to 53% at 6 months (p=0.027). Posterior controls showed a stable yearly PI during follow-up, without statistically significant variations (40-59% in subsequent years, p>0.5). The P/C ratio had already downgraded significantly at the third postoperative check (4.6 preoperative vs. 1.8 postoperative; p=0.03). A subgroup analysis of failed pyeloplasty (4 renal units) showed all PI < 15% at the third postoperative month (sensitivity 100%, specificity 86%). Complete resolution of hydronephrosis occurred in nine patients (20%). DISCUSSION: The P/C ratio and PI are new feasible ultrasound parameters in pyeloplasty follow-up. Early improvement in the P/C ratio can be expected and might avoid repeated ionizing scans. A PI>15% in subsequent postoperative checks might be enough for safe monitoring with ultrasound alone. Thus, renograms may be solicited only in these cases where ultrasound parameters do not improve in the first 6 postoperative months. Afterwards, ultrasound parameters often remain stable in the long term. Absence of hydronephrosis could only be documented in the long term in one out of every five patients.

Emerging organic contaminants in wastewater are usually analyzed by targeted approaches, and especially estrogens have been the focus of environmental research due to their high hormonal activity. The selection of specific target compounds means, however, that most of the sample components, including transformation products and potential new contaminants, are neglected. In this study, the fate of steroidal compounds in wastewater treatment processes was evaluated by a nontargeted approach based on comprehensive two-dimensional gas chromatography-time-of-flight mass spectrometry. The potential of the nontargeted approach to generate comprehensive information about sample constituents was demonstrated with use of statistical tools. Transformation pathways of the tentatively identified compounds with steroidal four-ring structure were proposed. The purification efficiency of the wastewater treatment plants was studied, and the distribution of the compounds of interest in the suspended solids, effluent water, and sludge was measured. The results showed that, owing to strong adsorption of hydrophobic compounds onto the solid matter, the steroids were mostly bound to the suspended solids of the effluent water and the sewage sludge at the end of the treatment process. The most abundant steroid class was androstanes in the aqueous phase and cholestanes in the solid phase. 17ß-estradiol was the most abundant estrogen in the aqueous phase, but it was only detected in the influent samples indicating efficient removal during the treatment process. In the sludge samples, however, high concentrations of an oxidation product of 17ß-estradiol, estrone, were measured.

OBJECTIVE: Vesico-vaginal reflux (VVR) is defined as the reflux of urine into the vaginal vault during voiding, occasionally seen in pre- adolescent girls. The typical history consists in post voiding leaks in the daytime, that correspond to the progressive urine discharge from the vagina, after it has been filled up during micturition. We intend to show two cases presenting with significant urocolpos. METHODS: Description of two clinical cases observed in the pediatric urology office. RESULTS: Clinical case 1.- A 10-year-old girl presented with the complaint of diurnal incontinence (in the immediate post-voiding minutes). The early voiding phase in the cystourethrogram (VCUG) demonstrated progressive gross distension of the vagina (urocolpos) due to retrograde filling as the bladder emptied. The girl was managed with behavioural modifications, and was dry afterwards. Clinical case 2.- A 9-year-old girl presented with history of incomplete voiding. In infancy, she had right-sided vesicoureteral reflux (VUR) and was endoscopically treated at the age of 2. VCUG showed VVR, and no VUR. She was then successfully treated with behavioural modifications. Clinical case 3.- A 10-year-old girl complained of diurnal urinary incontinence, described as post voiding leaks. Again, a VCUG showed VVR and a small urocolpos. After re-education of voiding habits, leaks disappeared, but recurred 6 months afterwards, when she acknowledged no adherence to the therapy. DISCUSSION: Between 12 to 15% of girls referred to Urological clinics because of urine incontinence present VVR. In the absence of a clear anatomical obstruction, reflux happens as the urine flow encounters a natural obstacle in the labia majora usually in girls that close their legs as they void. Instructions on proper voiding form a key element in the management of VVR, and if not enough, the behavioural modification consists on a reverse position during voiding.
