[The antagonistic activity of the eubiotic Maxilin towards wound infection and its effect on the antibiotic resistance of microorganisms]. / Antagonisticheskaia aktivnost' éubiotika Maksilin k ranevoi infektsii i ego vliianie na rezistentnost' mikroorganizmov k antibiotikam.

Antagonistic activity of lactic acid bacteria of strain N 2585 of "Maxilin" probiotic in respect to the strains S. aureus, P. aeruginosa, E. coli, Candida albicans and their effect on the level of sustainability of the antibiotic resistant strains S. aureus, E. coli, P. aeruginosa, which are extracted from the patients with open fractures of the mandible under various course of wound process, have been studied. The studies showed that lactic acid bacteria have suppressed growth of strains Candida most actively, followed by P. aeruginosa, S. aureus and E. coli. The use of "Maxilin" probiotic allowed reducing the epidemiological significance of pathogenetic strains already in 3 to 4 days. The sensibility studies showed high activity of the lactic acid bacteria in reducing antibiotic resistance of the strains investigated. It was revealed that "Maxilin" probiotic affects Ps. aeruginosa effectively, somewhat less St. aureus and E. coli. The extracted strains demonstrated sensibility to all investigated antibiotics already in 3 to 4 days.

[Prophylaxis of cardiovascular and respiratory complications after conduction of cholecystectomy in elderly and senile patients]. / Profilaktika serdechno-sosudistykh i dykhatel'nykh oslozhnenii posle vypolneniia kholetsistéktomii u bol'nykh pozhilogo i starcheskogo vozrasta.

Surgical treatment of calculous cholecystitis was done in 150 elderly and senile patients. Open intervention was applied in 60 patients, laparoscopic cholecystectomy (LCHE)--in 90, of them in 30--using epidural anesthesy. The LCHE performance in combination with epidural anesthesy reduces the risk of occurrence of vascular and respiratory complications in elderly and senile patients.

[Method of pancreatoduodenal resection and intraoperative prevention of acute pancreatitis]. / Tekhnika pankreatoduodenal'noi rezektsii i intraoperatsionnoi profilaktiki ostrogo pankreatita.

In the period from 1988 to 1991 the authors carried out 27 pancreatoduodenal resections with 11.1% mortality. From analysis of the results they formulated the main technical principles of this operation and suggested a complex of intraoperative measures for prevention of acute postoperative pancreatitis. The complex included application of a 5-fluorouracil-containing covering of the pancreas at the stage of exposure of the complex which will be removed; individualization of the methods for treatment of the pancreatic stump depending on the diameter of the pancreatic duct; the use of prolene in forming the pancreaticoenteroanastomosis; intraoperative administration of cephalosporins and dioxidine followed by antibacterial therapy; application of microsphere with cytostatics to the pancreas at the operation end for prolongation of pancreatic secretion suppression.

[Precision technique of pancreatoduodenal resection]. / Pretsizionnaia tekhnika pankreatoduodenal'noi rezektsii.

Precision termino-lateral pancreatico-jejunal anastomosis with or without mobilization of the pancreatic duct, with the use of inert suture materials in pancreatoduodenal resection was conducted in 21 patients, 3 of them died. Incompetence of the anastomoses was not encountered. The number of patients treated by radical operation increased almost 2.5 fold, and was 27.4%. The authors consider inclusion of the stump of the gland into digestion together with other conservative and technical measures to be most promising trend in the prevention of incompetence of the anastomoses and postoperative pancreatitis, in improvement of the results of pancreatoduodenal resection, and in expanding the scope of its use.

The information and calculating system (ICS Konkurs) for accompaniment of the work of Expert Councils of the USSR AMS at competitions of medical research works projects, developed by an IBM, is examined. The system is of high efficiency, permits one to automate a great quantity of routine operations, is easily adaptive to work conditions of any Expert council. The ICS Konkurs can be used at any competitions and in the activities of Scientific Councils of Research Institutes.

[Endocaval cannula for total vascular isolation and perfusion of the liver]. / Endokaval'naia kaniulia dlia polnoi sosudistoi izoliatsii i perfuzii pecheni.

The endocaval canule proposed by the author was tested in experiments in dogs. It was established that the canule can provide complete vascular isolation of the liver and ensure meassive bloodless resection of the liver under these conditions. When necessary, regional perfusion of the liver is possible. The investigation in corpses of adult men has confirmed the possibility of using the endocaval canule in the clinic.

[Several important questions concerning the segmental structure of dog liver]. / O nekotorykh sushchestvennykh voprosakh segmentarnogo stroeniia pecheni sobaki.

Anatomical structure of the dog liver has been studied in 82 isolated preparations. Besides the external surface of the organ, the study has been carried on in preparations of intraorganic branches of the portal vein, hepatic veins. Roentgenography of the portal vein and the hepatic artery has been performed. According to the external composition and intraorganic branching of the portal vein and the hepatic artery, two separate lobes--right and left--can be determined in the dog liver. The right lobe consists of two segments, the left one has five segments. Their classification is presented. From the left hepatic lobe, blood outflows along the hepatic veins which open into the posterior vena cava by their common orifice. The right portions of the fifth segment, segments 6, 7 are drained by veins having their own orifices. The hepatic artery has 3--4 branches: the left carrying blood to 1, 2, 3 segments, middle supplying with blood 4, 5 segments and gall bladder, one--two right branches going to 6, 7 segments.

Experimentally on 116 dogs it has been found that intravascular shunts of a big diameter or those controlled by means of extracorporal shunts ensure stable hemodynamic indices, approximately the initial ones. The isolated hypothermal perfusion protects, in its turn, the liver from an ischemic trauma and prolongs the safety period of the liver blood flow break to 40 minutes and more.
