RESUMO

The rate of the highly differentiated thyroid microcarcinoma methastasizing in central (6th) group of cervical lymph nodes was studied. In the clinic in 2013 - 2015 yrs 62 patients were operated for primary thyroid microcarcinoma. In accordance to TNM (UICC, AJCC, 2009) classification, papillary cancer in stage TlaNOMO was diagnosed in 35 (56.5%) patients, T1aN1MO - in 22 (35.5%); follicular cancer stage T1aNOMO - in 2 (3.2%); papillary cancer, follicular variant in stage T1aN1 MO - in 2 (3.2%); follicu- lar cancer, papillary variant in stage T1aNOMO - in 1 (1.6%). In 8 (12.9%) patients intraorgan multifocal metastasizing into contralateral thyroid lobe was revealed. In accordance to morphological investigation date of the excised samples, in 24 (38.7%) patients the metastases into the 6th group lymph nodes were revealed. In 4 (16.7%) patients, to whom lateral cervical lymphodissection was done, metastases in 2 - 5th groups of lymph nodes on the thyroid affection side were revealed.

RESUMO

The prognosis criteria for development of the intestinal anastomoses sutures insufficiency were elaborated, basing on analysis of the risk factors occurrence. Surgical tactic of treatment of the patients was substantiated, depending on the risk grade presence. In presence of high or moderate risk of the sutures insufficiency occurrence the methods of the deferred intestinal anastomoses formation were elaborated, excluding necessity of multistage intraabdominal operative interventions performance. Introduction of curative-diagnostic approach proposed promotes lowering of postoperative complications rate and lethality.

RESUMO

Retrospective analysis of severity of the postoperative peritonitis clinical course was conducted in 69 patients. There were identified two forms: classical and atypic. There was established, that for classical clinical form the presence of the pain syndromes characteristic as well as of the peritoneal irritation symptoms, and for the atypic one--the sepsis occurrence.

RESUMO

The retrospective analysis of treatment of 42 patients with the closed abdominal trauma, whom the relaparotomy carried out. Principal causes bringing to repeatcd operative interventions were separated, and classification of operations depending on terms of performance, are allocated. An inclination the basic directions as system and local character are allocated.

RESUMO

Experience of treatment of 52 injured persons with combined pancreatic trauma was summarized. Among combined injuries the thoracic trauma had predominated. Laparocentesis was performed in all patients in emergency. In pancreatic injury a gastroepiploica was cannulated with subsequent infusion of sandostatin and antibiotics. In 11 patients complications after the operation had occurred, mortality was 23.1% (12 patients).

RESUMO

The experience of treatment of 69 injured persons with posttraumatic retroperitoneal hematoma suffering severe multiple combined abdominal trauma was analyzed. Application of the classification proposed permits to formulate diagnosis and to choose the tactic of treatment correctly. The intraoperative tactics algorithm was elaborated. It promotes the correct analysis of intraoperative findings and reduction of the diagnostic mistakes frequency as well. In the presence of vast defect, making impossible to suture over the parietal peritoneum, extraperitonization using cerebral dura mater was done. Operative intervention was concluded by drainage with subsequent laserotherapy.
