INTRODUCTION: Adequate treatment of acute postoperative pain is one of the quality requirements in ambulatory surgery and its suboptimal management is associated with delayed discharge, unplanned admissions and late admissions after home discharge. The aim of the present study was to learn about the organizational strategy for the management of postoperative pain in ambulatory surgery units (ASU) in Spain. METHODS: A cross-sectional, multicenter study was carried out based on an electronic survey on aspects related to the management of acute postoperative pain in different ASUs in our country. RESULTS: We recruited 133 ASUs of which 85 responded to the questions on the management of postoperative pain. Of the ASUs that responded, 80% had specific protocols for pain management and 37.6% provided preoperative information on the analgesic plan. The assessment of postoperative pain is carried out in 88.2% of the ASUs in the facility and only 56.5% at home. All ASUs use multimodal analgesia protocols; however, 68.2% report the use of opioids for the treatment of moderate to severe pain. Home invasive analgesia strategies are minimally used by the surveyed ASUs. CONCLUSIONS: The DUCMA study highlights that the practice of pain treatment in day surgery remains a challenge in our country and is not always in agreement with national guidelines. The results suggest the need to establish strategies to improve clinical practice and homogenize pain management in ambulatory surgery.

Tear dysfunction syndrome, also known as dry eye disease (DED), is a multifactorial disease of the ocular surface characterized by the loss of tear film homeostasis. DED shows a significant clinical overlap with ocular allergy (OA), which alters tear film homeostasis, thus predisposing the patient to DED. Both conditions constitute the most common ocular surface disorders and have a potentially severe impact on patients' quality of life. Clinical practice guidelines recommend topical therapies as first-line treatment for OA. However, eye drop formulations may contain additional substances that can contribute to ocular surface damage and the development of DED. Therefore, physicians treating ocular allergy should be aware of problems affecting the tear film, the role of tear film disruption in OA, and topical treatment to prevent or minimize DED. The aim of this review is to present an updated overview of the topic.

RESUMO

The negative consequences of intimate partner violence against women (IPVAW) are observed not only in the victims but also in the professionals who work in this field. Self-efficacy has been observed as a significant variable in the perception of work efficiency and general well-being, and in coping with work-related stress and burnout syndrome. Thus, we performed a correlational study (N = 200) to examine the mediating role of recovery experiences and emotional exhaustion in the relationship between self-efficacy and psychological well-being in these professionals. The mediating analyses revealed that self-efficacy was related to higher levels of well-being through its effects on the increase in recovery experiences and the decrease in burnout levels. These findings emphasize the need to develop intervention programs aimed at improving self-efficacy these professionals. This is necessary to improve their employment situations, increase their health, and optimize both institutional resources and the quality of the services offered.

BACKGROUND: Infidelity is a relational process common in all types of romantic relationships and has been established as one of the main causes of relationship breakdown. However, little is known about this type of transgression in adolescent romantic relationships, although it manifests as a fairly frequent behavior involving different motivations. Even less is known about the emotional impact of infidelity on the offending person and its association with hostile behavior and psychological well-being. METHODS: Through an experimental study (N = 301 Spanish adolescents (190 female and 111 male; Mage = 15.59, SD = 0.69; range from 15 to 17), we sought to analyze the effect of manipulating two types of motivations for infidelity (sexual vs. emotional dissatisfaction) on negative affect, hostility, and psychological well-being. RESULTS: The main results revealed that committing infidelity motivated by hypothetical sexual (vs. emotional) dissatisfaction was indirectly related to lower psychological well-being through its effects on increased negative affect and hostility. CONCLUSIONS: Last but not least, we discuss these findings, highlighting the possible implications of infidelity for the psychosocial and psychosexual development of adolescents.

Instagram is a popular social networking site (SNS) among adolescents that allows them to share visual content about their lives quickly and easily, increasing social connection, acceptation, and entertainment among others. Nevertheless, SNS exposure can also lead to negative counterparts such as judgments, envy, social comparison, or cyberbullying perpetration. This research aimed to analyze the possible psychosocial factors associated with Instagram use (i.e., social comparison and envy) that could lead to the perpetration of cyberbullying towards peers. The sample consisted of 254 adolescent students aged between 15 and 18 years old (Mage = 15.77, SD = 0.74). The results indicated that high connection time to Instagram, high levels of social comparison, and malicious envy were associated with an increased tendency to carry out cyberbullying perpetration's behaviors. Likewise, the main finding showed that a high connection time to Instagram was associated with increased social comparison, which in turn was associated with malicious envy, and consequently with an increased tendency to carry out cyberbullying perpetration's behaviors. These findings contribute to a better understanding of the psychosocial processes that might precede to perpetrate cyberbullying's behaviors, as well as to promote the development of educational programs intend to encourage the responsible use of SNSs during adolescence.

El síndrome de disfunción lagrimal, también denominado enfermedad del ojo seco (EOS), es una enfermedad multifactorial de la superficie ocular caracterizada por la pérdida de la homeostasis de la película lagrimal. La EOS y la alergia ocular (AO) son patologías que comparten un abanico de signos y síntomas, y pueden aparecer simultáneamente; además, la AO altera la homeostasis de la película lagrimal, predisponiendo a la EOS. Estas dos afecciones constituyen los trastornos más frecuentes de la superficie ocular y pueden afectar notablemente la calidad de vida de los pacientes. Las guías de práctica clínica recomiendan terapias tópicas como tratamiento de primera línea para la alergia ocular. Sin embargo, las fórmulas de los colirios pueden contener aditivos y conservantes que pueden contribuir al daño de la superficie ocular y a la aparición de EOS. Por lo tanto, los facultativos que tratan la alergia ocular deben conocer las implicaciones que conlleva la alteración de la película lagrimal en la conjunctivitis alérgica, el potencial daño que pueden provocar los conservantes incluidos en los colirios empleados en el tratamiento tópico de esta patología, así como los tratamientos disponibles para manejar la EOS y la AO cuando la disfunción de la película lacrimal ya está establecida. El objetivo de esta revisión es presentar una visión general actualizada del tema. (AU)

The situation created by the COVID-19 pandemic, especially the confinement in many countries, has led to a global crisis, not only in health but also in economy and social issues. But it has also provoked a wave of solidarity and unprecedented donation behavior by many companies worldwide. Inditex, one of the main fashion multinationals, has become a referent for its reaction speed and has been ranked number one among the most significant companies for its Corporate Social Responsibility during the lockdown. Drawing from Stakeholder, Legitimacy, and Ethics of Care Theories, the aim of this paper is to analyze Inditex as a case study and reflect on the impact of its donation behavior on its corporate reputation. A desk research approach by using secondary data about the corporation, and a content analysis of press releases with ATLASti software during this time, let conclude that effective corporate donation impacts and improves the reputation of the corporation among its stakeholders.

Being responsive to a partner's sexual needs in relationships (i.e., sexual need responsiveness) is associated with higher sexual and relationship satisfaction. Previous research has focused on researcher-led definitions of sexual need responsiveness. The purpose of the present study was to develop a participant-informed definition of sexual need responsiveness. A total of 305 individuals responded to five open-ended questions concerning the definition and behaviors pertaining to sexual need responsiveness. The content analysis results showed that the most common elements in participants' definition included listening to and accommodating each other's sexual needs and wishes whenever possible, while respecting both partners' safety and boundaries. Both verbal and non-verbal communication in various forms was used to express and share needs with partners and participants often evaluated responsiveness in terms of their sexual satisfaction. The results provide a participant-informed definition of sexual need responsiveness and can be used to inform research and clinical practice.

Instagram is a social networking site (SNS) that facilitate the social-comparison and feedback-seeking (SCFS) processes, which are particularly relevant during adolescence. Likes represent numeric evaluative feedback and seem to be considered as a form of social reward. In this research we examine some psychosocial factors that could influence the Instagram usage intensity (i.e. SCFS and motivations) and analyze the moderating role of SCFS in the relationship between the number of likes on posts and adolescents' emotions. The sample consisted of 182 adolescent students aged between 13 and 18 years (M = 15.35 years, SD = 1.11). The results show that the social interaction, storage, and gossip motivations mediate the relationship between SCFS and Instagram usage intensity, and that the influence of the number of likes on emotions depended on the degree of SCFS. The discussion of the findings emphasizes that likes have a special social and affective relevance for adolescents with high SCFS, who might become more emotionally susceptible to the feedback they received from their audience on Instagram. This research could be a precedent to future research and the development of intervention programs based on the responsible use of SNSs in an educative context.

Objetivo: En la lumbalgia mecánica por síndrome facetario lumbar están indicadas como opciones terapéuticas los bloqueos periarticulares e intrarticulares de las articulaciones facetarias lumbares, así como el bloqueo nervioso del ramo medial de la raíz o ramo posterior del nervio raquídeo. El bloqueo nervioso del ramo medial puede llevarse a cabo mediante control ecográfico o radioscópico. El objetivo de este estudio es valorar si existen diferencias en la eficacia analgésica del bloqueo en función de la técnica de imagen con la que se lleva a cabo (ecografía frente a radioscopia-fluoroscopia). Pacientes y método: Se ha realizado un estudio clínico transversal durante el mes de noviembre de 2020, analizando los bloqueos facetarios realizados en la Unidad del Dolor del Hospital Universitario Clínico de Salamanca entre mayo de 2018 y septiembre de 2020. La muestra analizada cuenta con 315 pacientes, de los cuales 212 fueron sometidos a bloqueo facetario ecoguiado en consulta y 103 se realizaron bajo control radioscópico en el quirófano. Se ha evaluado la medida del dolor inicial, previo al bloqueo, mediante la escala analógica visual (EVA inicial), así como la EVA final y el porcentaje de mejoría subjetiva recogido en los 15 días posteriores a la realización de la intervención, valorando sus diferencias en función de la técnica de imagen empleada para su realización. Resultados: Los valores de EVA final de los pacientes que se realizaron el bloqueo mediante radioscopia y la de aquellos que lo realizaron ecoguiado fue similar, con un resultado final de 3,5 (mediana), así como el porcentaje de mejoría subjetiva que también fue similar en ambos grupos, alcanzando esta el 50 %. Conclusiones: No existen diferencias en la eficacia analgésica del bloqueo facetario lumbar en función de la técnica de imagen empleada para su realización: radioscopia-fluoroscopia o ecografía.(AU)


Objective: In mechanical low back pain due to lumbar facet syndrome, periarticular and intra-articular blocks of the lumbar facet joints as well as nerve blocks of the medial branch of the spinal nerve root or posterior branch of the spinal nerve are indicated as therapeutic options. The nerve block of the medial branch can be performed by ultrasound or radioscopic monitoring. The aim of this study is to assess whether there are differences in the analgesic efficacy of the block depending on the imaging technique used (ultrasound versus fluoroscopic-radioscopy). Patients and method: A cross-sectional clinical study was conducted during the month of November 2020, analysing the facet blocks performed in the Pain Unit of the Hospital Universitario Clínico de Salamanca between May 2018 and September 2020. The sample analysed consisted of 315 patients, of whom 212 underwent an echo-guided facet block in consultation and 103 were performed under radioscopic control in the operating theatre. The initial pain measurement, prior to the block, was evaluated using the visual analogue scale (initial VAS), as well as the final VAS and the percentage of subjective improvement recorded 15 days after the intervention, assessing the differences according to the imaging technique used for its performance. Results: The final VAS values of the patients who underwent radioscopic block and those who underwent echo-guided block were similar, with a final result of 3.5 (median), as was the percentage of subjective improvement, which was also similar in both groups, reaching 50 %. Conclusions: There are no differences in the analgesic efficacy of the lumbar facet block depending on the imaging technique used for its performance: fluoroscopy-radioscopy or ultrasound.(AU)

Infidelity occurs in adult romantic relationships quite often; however, little is known about this relational phenomenon in the adolescent stage, despite its being a surprisingly common behavior. Through a correlational study, we set out to examine how the various documented motivations to engage in an act of infidelity are associated with negative emotional responses, self-esteem, and psychological well-being. In a sample of Spanish adolescents (N = 346 [Mage = 15.71, SD = 1.27; range from 13 to 19]), results showed that committing an act of infidelity due to sexual or emotional dissatisfaction (vs. neglect and anger) is related to higher levels of psychological well-being by undermining negative affect, thereby increasing the levels of self-esteem. The discussion of the findings emphasizes that infidelity could favor adolescents' personal growth, because of the need to explore new sensations and feelings that arise during this period.

Summary: Objective. The association of allergic conjunctivitis (AC) with rhinitis and/or asthma is poorly understood. The objective of this study was to apply the Consensus Document for Allergic Conjunctivitis (DECA) criteria for the classification of AC to a population of patients with AC to assess the association between the severity and duration of AC and rhinitis and/or asthma. Methods. Patients with ocular symptoms of AC who participated in the 'Alergológica 2015' study were included. The demographics, classification according to the DECA criteria, etiology, and comorbidities were evaluated by age groups (less or equal than 14 and greater than 14 years). Results. A total of 2,914 patients (age range, 1-90 years) were included in the "Alergológica 2015" study. Of these, 965 patients (33.1%) were diagnosed with AC (77.5% > 14 years). AC was classified as severe, moderate, or mild in 1.8%, 46.4%, and 51.8%, respectively; and as intermittent or persistent in 51.6% and 48.4% of the patients. AC alone occurred in 4% of patients. AC was mainly associated with rhinitis (88.4%), asthma (38.2%), food allergy (8.3%) and atopic dermatitis (3.5%). In allergic respiratory disease rhinitis preceded AC and asthma developed later. The severity and duration of AC was significantly associated with severity and duration of rhinitis (p less than 0.001 for both age groups) and asthma (p less than 0.001 only in adults). Conclusions. The application of the new DECA classification for AC reveals a direct relationship between AC, rhinitis and asthma respect to severity and duration. These relationships suggest that AC should be considered an integral part of the "one airway, one disease" hypothesis.

MATERIAL AND METHODS: An approved, observational, retrospective study was conducted between March 15 to April 15, 2020. 150 adult patients aged less than 75 with Charlson comorbidity index ≤6 diagnosed with COVID-19 pneumonia were included. Seventy-five patients were randomly selected from those admitted to the critical care units (critical care group [CG]) and seventy-five hospitalized patients who did not require critical care (non-critical care group [nCG]) represent the control group. One additional cohort of hospitalized patients with COVID-19 were used to validate the score. MEASUREMENTS AND MAIN RESULTS: Multivariable regression showed increasing odds of in-hospital critical care associated with increased C-reactive protein (CRP) (odds ratio 1.052 [1.009-1.101]; P = 0.0043) and higher Sequential Organ Failure Assessment (SOFA) score (1.968 [1.389-2.590]; P < 0.0001), both at the time of hospital admission. The AUC-ROC for the combined model was 0.83 (0.76-0.90) (vs AUC-ROC SOFA P < 0.05). The AUC-ROC for the validation cohort was 0.89 (0.82-0.95) (P > 0.05 vs AUC-ROC development). CONCLUSION: Patients COVID-19 presenting at admission SOFA score ≥ 2 combined with CRP ≥ 9.1 mg/mL could be at high risk to require critical care.

This paper explores technostress and its dimensions, assessing the relationship with possible negative effects in the individual, social and professional sphere. The study uses a self-reported approach of undergraduate students in Spain (n = 337), forced to follow their academic life by using technology comprehensively because of social distancing, as a public health action necessary to reduce the spread of COVID-19. The analysis, based on the exploration of a system of archetypes of the use of social networks, presents insights into contemporary technostress management as a new approach that can suppose opportunities for the optimization of prevention plans. Pearson's correlation coefficients and structural equation modeling based on partial least squares (SEM-PLS) were the methods used for achieving the goals. The results reveal valid and reliable measures where technostress has a high impact on the individual sphere of students and there is a significant relationship between the type of user and techno-anxiety. The conclusions point to the imperative for developing a deeper understanding of technostress by archetypes, in both a higher education context (as antecedent) and the world of work, in an irreversible move towards a digital economy.
