RESUMO

Enterococci are emerging nosocomial pathogens. Their widespread distribution causes them to be food contaminants. Furthermore, Enterococci can colonize various ecological niches and diffuse into the food chain via contaminated animals and foods because of their remarkable tolerance to unfavorable environmental circumstances. Due to their potential dissemination to humans, antimicrobial-resistant Enterococci in fish are a worldwide health issue. This study characterized AMR, ARGs, VAGs, gelatinase activity, and biofilm formation in Enterococcus spp. recovered from fish and seafood and evaluated potential correlations. 54 Enterococcus spp. strains(32.73 %)were isolated from 165 samples (75 Oreochromis niloticus, 30 Argyrosomus regius, and 60 Shrimp), comprising 30 Enterococcus faecalis (55.6 %) and 24 Enterococcus faecium (44.4 %) with total 32.73 % (54/165), The maximum prevalence rate of Enterococcus spp. was observed in Nile tilapia (34/54; 63 %), followed by shrimp (14/54; 25.9 %) and Argyrosomus regius (6/54; 11.1 %). The maximum prevalence rate of E. faecalis was observed in Nile tilapia (22/30; 73.3 %), followed by shrimp (8/30; 26.7 %) with significant differences. The prevalence rate of E. faecium was observed in Nile tilapia (12/24; 50 %), followed by shrimp (6/24,25 %). E. faecium is only isolated from Argyrosomus regius (6/24,25 %). Isolates exhibited high resistance against both tetracycline (90.7 %) and erythromycin(88.9 %), followed by gentamycin (77.8 %), ciprofloxacin (74.1 %), levofloxacin (72.2 %), penicillin (44.4 %), vancomycin (37 %), and linezolid (20.4 %). 50 strains (92.6 %) exhibited resistance to more than two antibiotics, 5 strains (10 %) were XDR, and the remaining 45 strains (90 %) were classified as MDR. 92.6 % of the isolates had MARindices >0.2, indicating they originated in settings with a high risk of contamination. Additionally, ten ARGs were identified, with tet(M) 92.6 %, followed by erm(B) (88.9 %), aac(6')-Ie-aph(2″)-Ia(77.8 %), tet(K) (75.9 %), gyrA (74.1 %), blaZ (48.1 %), vanA (37 %), vanB (31.5 %), optrA (20.4 %), and catA(3.7 %). Biofilm formation and gelatinase activity were observed in 85.2 %, and 61.1 % of the isolates, respectively. A total of 11 VAGs were detected, with gelE as the most prevalent (83.3 %) followed by agg(79.6 %), pil (74.1 %), both sprE and asa1 (72.2 %), hyl (70.4 %), eps(68.5 %), EF3314 (57.4 %), ace (50 %), and cylA (35.2 %) with no detection of cylB. In conclusion, the emergence of linezolid-resistant -vancomycin-resistant enterococci recovered from Egyptian fish and shrimp, suggests that fish and seafood might participate a fundamental part in the emergence of antimicrobial resistance among humans.

INTRODUCTION: Achieving the Sustainable Development Goals to reduce maternal and neonatal mortality rates will require the expansion and strengthening of quality maternal health services. Midwife-led birth centres (MLBCs) are an alternative to hospital-based care for low-risk pregnancies where the lead professional at the time of birth is a trained midwife. These have been used in many countries to improve birth outcomes. METHODS: The cost analysis used primary data collection from four MLBCs in Bangladesh, Pakistan and Uganda (n=12 MLBC sites). Modelled cost-effectiveness analysis was conducted to compare the incremental cost-effectiveness ratio (ICER), measured as incremental cost per disability-adjusted life-year (DALY) averted, of MLBCs to standard care in each country. Results were presented in 2022 US dollars. RESULTS: Cost per birth in MLBCs varied greatly within and between countries, from US$21 per birth at site 3, Bangladesh to US$2374 at site 2, Uganda. Midwife salary and facility operation costs were the primary drivers of costs in most MLBCs. Six of the 12 MLBCs produced better health outcomes at a lower cost (dominated) compared with standard care; and three produced better health outcomes at a higher cost compared with standard care, with ICERs ranging from US$571/DALY averted to US$55 942/DALY averted. CONCLUSION: MLBCs appear to be able to produce better health outcomes at lower cost or be highly cost-effective compared with standard care. Costs do vary across sites and settings, and so further exploration of costs and cost-effectiveness as a part of implementation and establishment activities should be a priority.

BACKGROUND: Midwives are essential providers of primary health care and can play a major role in the provision of health care that can save lives and improve sexual, reproductive, maternal, newborn and adolescent health outcomes. One way for midwives to deliver care is through midwife-led birth centres (MLBCs). Most of the evidence on MLBCs is from high-income countries but the opportunity for impact of MLBCs in low- and middle-income countries (LMICs) could be significant as this is where most maternal and newborn deaths occur. The aim of this study is to explore MLBCs in four low-to-middle income countries, specifically to understand what is needed for a successful MLBC. METHODS: A descriptive case study design was employed in 4 sites in each of four countries: Bangladesh, Pakistan, South Africa and Uganda. We used an Appreciative Inquiry approach, informed by a network of care framework. Key informant interviews were conducted with 77 MLBC clients and 33 health service leaders and senior policymakers. Fifteen focus group discussions were used to collect data from 100 midwives and other MLBC staff. RESULTS: Key enablers to a successful MLBC were: (i) having an effective financing model (ii) providing quality midwifery care that is recognised by the community (iii) having interdisciplinary and interfacility collaboration, coordination and functional referral systems, and (iv) ensuring supportive and enabling leadership and governance at all levels. CONCLUSION: The findings of this study have significant implications for improving maternal and neonatal health outcomes, strengthening healthcare systems, and promoting the role of midwives in LMICs. Understanding factors for success can contribute to inform policies and decision making as well as design tailored maternal and newborn health programmes that can more effectively support midwives and respond to population needs. At an international level, it can contribute to shape guidelines and strengthen the midwifery profession in different settings.

Methicillin-resistant Staphylococcus aureus (MRSA) is a foremost human and animal pathogen with public health and veterinary significance causing hospital and community infections and contagious bovine mastitis. Due to its ability to develop multidrug resistance (MDR) and its pathogenicity, MRSA infection control is becoming a global concern. Natural antibacterial options are needed to combat MDR development and infectious dissemination. This study investigated the antimicrobial resistance and virulence genes profiling of MRSA isolates and explored the antivirulence efficacy of trans-cinnamaldehyde, thymol, and carvacrol essential oils (EOs) against multivirulent and MDR-MRSA isolates. Thirty six S. aureus isolates (25%) were retrieved, of which 34 (94.4%) were MRSA. A high prevalence of MDR (66.7%) was monitored and all 53 molecularly verified isolates possessed icaA and cna virulence genes. Moreover, 94.1% of these isolates were multivirulent with 23.5% of them carrying icaA, cna, eta, tst, and sea virulence genes. Our data proved superior in vitro antimicrobial and antivirulence activities of trans-cinnamaldehyde, thymol, and carvacrol. They inhibited the growth of multi-virulent and MDR-MRSA isolates and downregulated the transcription of examined virulence genes. Our study suggests using EOs as prospective antimicrobials with excellent antivirulence activities against MRSA isolates. We provided data regarding the eventual role of phytogenics in prevention and control of MRSA infection.

OBJECTIVES: To assess knowledge, attitude, and practice (KAP), of rural mothers in Karbala, Iraq towards prevention and home management of diarrhoeal diseases among their children, in relation to water sanitation and personal hygiene. METHODS: A cross-sectional study was conducted during the period from June to October 2016. Two hundred and five mothers of children under 5 years age living in two villages of Alhusainya district of Karbala were interviewed regarding their KAP on diarrhoea and waterborne diseases. RESULTS: The overall mean score of the participants' KAP (out of 100) was 52.74 ± 9.69 with a range of 27.00- 81.00. The majority of the respondents, (n=159 (77.6%), had low KAP, and the mean score of attitude and practice (AP) was 56.98 ± 13.82 with a range of 12.50 - 93.75. More than half 116(56.6%) had weak AP. There was a significant association between the KAP score with that of AP. The KAP score and AP score were significantly lower in young mothers and those with lower education. One hundred seventeen (57%) participants claimed that television and radio were the main sources of their health information. CONCLUSIONS: Mothers in rural areas in Karbala, especially the young and those with low education had low KAP and AP towards diarrhoea and waterborne diseases, and that there was a significant association between the two variables. Television and radio, and to a lesser extent the health centres were the main sources of knowledge.

OBJECTIVES: Single-photon emission computed tomography/computed tomography (SPECT/CT) hybrid systems have the advantage of performing various scans using the same imaging setting. Absorption and scattering of the gamma rays by the patient's body significantly affect images obtained from scintigraphy, especially in myocardial perfusion imaging. An important parameter for image quality in SPECT is image contrast which is defined as the difference in density between regions of the image corresponding to different levels of radioactive uptake in the patient. The objective of the study was to evaluate the influence of applying different correction methods on image contrast of myocardial SPECT/CT images. MATERIAL AND METHODS: A total of 114 patients, 43 females and 71 males, patient's raw data were processed and analyzed using attenuation correction (AC), scatter correction (SC), both attenuation and scatter correction together (ACSC), and no correction (NC). The short axis (coronal) slices resulted from the raw data reconstruction were chosen to perform the processing for hot and cold spheres for contrast values measurement. Statistical analysis was made for the measured contrast values for AC, SC, ACSC, and NC to determine the best image contrast. RESULTS: When applying SC alone, it yields better contrast value (0.834), compared to AC (0.677) and ACSC (0.739). Both ACSC and AC had better image contrast compared to NC (0.592). CONCLUSION: The intercomparison study between the correction conditions indicates that the counts in SPECT/ CT are highly affected by all correction methods. The image contrast has been significantly improved by using SC, AC, and ACSC when compared with the NC image. Furthermore, SC is superior in the image contrast than the other correction conditions in the reconstruction of SPECT/CT MPI.

Medication-induced pancreatitis is an overlooked cause of acute pancreatitis. We present an 81-year-old male patient with acute sharp epigastric pain radiating to his back, who was found to have lipase of more than 30,000 U/L. The patient denied current alcohol use. Abdominal ultrasound and abdominal computed tomography scan revealed no gallstones or biliary duct abnormalities. The patient had been taking sitagliptin for eight years. Supportive treatment with intravenous fluids, pain medications, and early feeding adequately treated his disease. With our case, we aim to increase awareness of sitagliptin-induced pancreatitis, regardless of the duration of use.

Preparation of amniotic membrane (AM) by air drying method followed by radiation sterilization is simple and valuable approach; sterility and quality of the final AM product are depending on the quality management system at the tissue bank. Validation and substantiation of radiation sterilization dose (RSD) for tissue allografts is an essential step for the development and validation of the standard operating procedures (SOP). Application of SOP is perfectly relying on trained staff. Skills differences among personnel involved in AM preparation could have an effect on microbiological quality of the finished product and subsequently on the RSD required. AM were processed by four different couples of the tissue bank technicians. The AM grafts were randomly selected and subjected to bioburden test to validate and substantiate the 25 kGy RSD. Bioburden test for AM grafts were also useful to evaluate the skill of the tissue bank technicians and thus, to validate the current SOP for air dried AM. Moreover, the effect of placental source on bioburden counts on AM grafts was assessed. Substantiation of the 25 kGy RSD at a sterility assurance level of 10(-1), and sample item portion = 1, was carried out using Method VD max (25) of the International Organization for Standardization, document no. 11137-2 (ISO in Sterilization of healthcare products-radiation-part 2: establishing the sterilization dose, Method VDmax-substantiation of 25 kGy or 15 kGy as the sterilization dose, International Standard Organization, 2006). The results showed that there were no significant differences in the bioburdens of the four batches (α = 1 %), this means no significant differences in the skill of the four couples of the tissue bank technicians in terms of their ability to process AM according to the air dried AM SOP. The 25 kGy RSD was validated and substantiated as a valid sterilization dose for the AM prepared with the current established SOP at the Biotechnology Research Center experimental tissue bank. The donor's type of delivery, normal or caesarean, showed no significant effect on the levels of microbial counts on the tested AMs (α = 1 %).

STUDY DESIGN: An experimental study. OBJECTIVE: To investigate the effects of different rehabilitation strategies on functional recovery of partial spinal cord of injured rats. SUMMARY OF BACKGROUND DATA: Activity-based rehabilitation is promising strategy for improving functional recovery following spinal cord injury (SCI). METHODS: Twenty-four female Sprague-Dawley rats weighing 180 to 220 g were anesthetized with chloral hydrate (450 mg kg⁻¹) by intraperitoneal injection, and laminectomy was performed at T7-T8 level, leaving the dura intact. A compression plate (2.2 × 5.0 mm) loaded with weight of 35 g was placed on the exposed cord for 5 minutes to create partial SCI. Animals were divided into 3 groups of 8 rats each. Group 1 served as control (SCI + without treadmill and swimming training). Whereas rats in Groups 2 and 3 were subjected to SCI as mentioned previously and received swimming training 5 minutes for Group 2 and treadmill training 5 minutes for Group 3 each day, which occurred beginning 14 days postsurgery and continued for 14 days. The spontaneous coordinate activity (SCA), Basso, Beattie, and Bresnahan (BBB), and Tarlov locomotor scores were used to assess functional recovery of spinal cord injured rats. RESULTS: Day 1 (baseline, 14 days after the surgery), there was no significant difference among the means for SCA, BBB, and Tarlov scores of all groups. After day 1, Groups 1, 2, and 3 showed continuous improvement in their BBB, Tarlov, and SCA scores. This improvement was maintained throughout the duration of the study with different levels for each group. By the end of the study, trained Groups 2 and 3 showed statistically significant improvement in their SCA, BBB, and Tarlov scores compared with Group 1 (P < 0.05). CONCLUSION: These results suggest that 2 weeks of treadmill locomotor training and swimming training may have positive effects on functional recovery after partial SCI.

INTRODUCTION: Reconstruction of the avulsed auricle remains a challenge to the plastic surgeon. Many surgeons suggest reattachment of the total or subtotal amputated ear as a composite graft, but this solution is unreliable. Some surgeons advocate the use of the retro auricular pocket to enhance survival of the reattached ear. However, this burying method entails multistage procedures with somewhat poor results due to the associated fibrosis. Microsurgical replantation can be performed but its applicability is limited due to difficulty with venous drainage. In this paper we advocate the use of a technique of total ear reconstruction using the cartilage of the avulsed ear in a single stage. PATIENTS: Three patients with a mean age of 36 years, all of whom were heavy smokers, were operated upon between February 2006 and January 2007. Surgery involved removal of the skin of the amputated ear and immediate reattachment. The ear cartilage was then, immediately covered with an ipsilateral temporoparietal fascia flap and a split thickness skin graft. RESULTS: Acceptable aesthetic results were obtained in all three cases. A minor complication of limited skin graft loss occurred in one case. CONCLUSION: Our technique is simple, one stage and maintains the delicate detail of the auricular cartilage.
