Blue urticaria: a previously unreported adverse event associated with isosulfan blue.

The sentinel node hypothesis is predicated on the fact that a metastasis, if it exists, will have traveled on a direct path from the primary tumor through the efferent lymphatic channels to the first draining lymph node in the regional lymphatic basin, the sentinel node. Lymphatic mapping with isosulfan blue and sentinel lymphadenectomy is being increasingly used in the management of patients with melanoma, breast cancer, and other solid tumors. This trend is exposing an increasing number of patients to isosulfan blue. Although this compound is generally safe, severe reactions have been reported. We describe 2 patients who developed "blue hives" after isosulfan blue injection.

Women's role in reproductive health decision making and vulnerability to STD and HIV/AIDS in Ekiti, Nigeria.

An exploratory study of women's role in reproductive decision making in Ekiti shows that women in the state are increasingly taking active decisions on matters affecting their daily lives. More women than ever before believed that they could take decisions on family size, when to have a baby and choice of spacing period. The cultural barrier against short postpartum abstinence appeared to have diminished and sex during lactation was not considered a major cultural and religious taboo. Knowledge of contraception has become universal in recent years, and the majority of women take decisions on the method and timing of family planning. All women who used family planning considered their decision in this regard very important. The ability of women to take decisions on these issues may not only enhance their bargaining power but also reduce their vulnerability to STDs including AIDS from diseased or high-risk partners.

Echocardiographic and Doppler evaluation of left ventricular hypertrophy and diastolic function in black and white hypertensive patients.

Echocardiography including Doppler examination has been used to assess left ventricular hypertrophy and diastolic function in 24 patients: 12 black Negro patients and 12 white patients who were matched with respect to age, heart rate and systolic and diastolic blood pressure. Significant left ventricular hypertrophy was present with normal systolic function but impaired diastolic function with a decrease in peak E velocity, an increase in peak A velocity and reversal of the E/A ratio. In the combined groups LV mass and LVPW correlated with systolic blood pressure (P less than 0.05). In the blacks systolic and diastolic blood pressure correlated with LV mass and LVPW thickness (P less than 0.05). Septal thickness correlated only with systolic blood pressure (P less than 0.01). In contrast the indices of diastolic function did not correlate with blood pressure or with left ventricular hypertrophy. No morphological differences in the left ventricle have been shown between the two groups but both had significant left ventricular hypertrophy. Whilst diastolic function is abnormal, this appeared to be independent of either blood pressure or degree of left ventricular hypertrophy. A neurohumeral mechanism has not been discounted.
