RESUMO

The influence of stretching the femur on the state of stabilized hip joint after acute hematogenic osteomyelitis of the proximal part of the femur bone was studied in 47 children aged from 9 to 18 years. The restoration of the lower extremity length was obtained by the 4-12.5 cm stretching of the femur with the help of the Ilizarov apparatus and the wire-rod apparatus. The X-ray and radionuclide (three-phase dynamic osteoscintigraphy) methods were used. Long-term results were followed-up during the period from 1 to 12 years. It was found that stretching the injured femur not only failed to destabilize the hip joint and to worsen its state but maintained and increased the effect of the stabilizing operation.

RESUMO

On the basis of examination of 58 children with diagnosed dystrophic osseous cysts there were revealed new aspects of roentgenologic characterization of dystrophic osseous cysts in children. It has been stated that high-structure roentgenography with primary magnification (apparatus Rays-D) allows to make more accurate an anatomical structure of the osseous cysts, nature of outlines as well as the state of adjacent sections of bone. Development of osseous cysts in considerable number of cases combines with the manifested symptoms of dystrophy in the surrounding osseous tissue (osteoporosis, loss of architectonics and presence of cyst-like formations).

RESUMO

203 patients with juvenile kyphosis, aged 11-16 years, have been subjected to the complex examination, including ++clinico-roentgenologic and physiologic methods. Along with the cardinal signs, common for juvenile kyphosis diagnosis and characteristic of the dystrophic process (wedge distortion, osteoporosis of vertebra bodies and fragmentation of their apophyses, dedifferentiation of the bone structure, height reduction and pathologic restructuring of intervertebral disks following the pattern of fibrosis, presence of discal hernia), the signs of bone ++ dysplasia of spine and skeleton in general should be singled out of the roentgenologic characteristic of disease which determine the variants of pathologic process course and treatment tactics. The presence of dysontogenesis signs in patients with juvenile kyphosis is suggested.

RESUMO

Contrast peridurography which was performed in 12 patients with neurofibromatosis allowed to reveal the contours of the tumour in 2 patients, peridural block in 5 patients and uneven vertebral canal in 3 patients. Only in 2 patients the vertebral canal picture was normal. It points at high information value of peridurography in the examination of the patients with neurofibromatosis with deformations of the spinal column.

RESUMO

The authors have studied the course and the efficacy of the surgical treatment of the hemimelic form of epiphyseal dysplasia in 15 children. Three roentgenologic types of epiphysis lesions have been distinguished. It has been determined that the pathologic changes are mainly localized in the posterior medial departments. In multiple severe lesions hyperfunction of the growth areas followed by their early asymmetric synostosis with the development of secondary deformations and focal disturbances in the ossification of the metaphyses have been observed. The necessity of early surgical treatment allowing to prevent the development of the pathologic changes in the growth areas and deforming arthrosis has been substantiated.

The authors have examined 43 children by the method of radiotropic angiography, 27 of whom had various diseases of the hip joint area. The blood flow in the iliac vessels and the regional blood flow in the area of the affected vessels was studied before and after the operation making use of an osteotropic radiopharmacological drug (RPD) 99mT-phosphon. Independent of the character of the degenerative and dystrophic process in the hip joints there was a reduction in the intensity of the blood flow on the affected side reaching 15-30%, which was more expressed at stage 111 of coxarthrosis with no considerable changes after the operation. In the control group this difference was +/- 5%. The regional blood flow (RBF) in the area of the operation was reduced down to 37% before the operation and increased by 17-35% 6-8 months after the operation with the exception of the cases of dystrophic lesions of the neck when no increase in the RBF was observed. The authors come to the conclusion about a relationship between the deviation from the normal conditions in the intensity of the blood flow and the degenerative and dystrophic processes in the hip joints.

RESUMO

The article is devoted to revealing of the causes of complications in children with congenital dislocation of the hip. 42 patients have undergone roentgenological, electrophysiological and neurological examinations; 32 of them had complications following surgical treatment of the dislocation of the hip. In the patients with complications disturbances in the structure of the lumbosacral section of the spine and neurological changes have been revealed. Reduced excitability of the motoneurons of the spinal cord has also been observed in these patients. In 10 children with satisfactory results the deviations in the structure of the lumbosacral section of the spine were insignificant, while gross neurological changes were not revealed. The authors have arrived at the conclusion that the main causes determining the results of surgical treatment are the initial state of the neuromuscular apparatus and dysplasia of the vertebral column structures. That is why in the patients with disturbances it is necessary to carry out adequate conservative therapy directed at improvement of the metabolism of the neuromuscular structures both before the operation and during the postoperative period, e.g. 2-4 times a year according to the expression of the neurological disturbances.

RESUMO

The investigation of roentgenograms made after a preliminary contrast study of the elbow joint has shown that traumas may be followed by pathological changes of the cartilaginous tissue, elbow bursa and filling of the fossae of the olecranon and coronary processes by scarry and osseous tissue. The pathological changes mentioned above result in disturbances of the movement of the elbow joint.
