RESUMO
From the authors' data the retained mucous membrane of the antral region at the duodenum following gastric resection for peptic ulcer results in the development of gastric ulcers. It has been found that only the removal of the retained antral mucous membrane stops the production of HCl and promotes the healing of ulcer. The reflux of the duodenal contents into the isolated pylorus does not constitute a cause of the gastrin production, the continuation of which is mainly due to the vagal denervation of the glands producing the antral hormone.

In the late period after reconstructive jejunogastroplasty such complications would arise as the resumed hydrochloric acid production and resulting peptic ulcers. Vagotomy is felt to be an effective measure for prevention and treatment of peptic ulcers of this origin. Some other complications of reconstructive jejunogastroplasty proved to be an obstruction of the interposed intestine of the functional character. The latter may necessitate surgical therapy.

Based on a comparison of the immediate and late results of treatment in patients with peptic ulcers following gastroenterostomy by means of gastric resection (24 patients) and by vagotomy, it is concluded that vagotomy in most cases results in a complete cure of patients, thus this procedure is felt to be more advantageous than gastric resection due to its technical feasibility and small operative risk.

According to the authors' findings malignant forms of ulcerogenic tumors of the pancreas in the Zollinger-Ellison syndrome clinically show a relatively favourable course in oncological aspect. As a rule, patients die from complications of peptic ulcer before a progressive tumor growth or its metastases result in mortality. Therefore, in cases when the routine radical operators prove to be unfeasible, it is recommended to perform surgery for the purpose of liquidating ulcer and conditions for its recurrence.
