RESUMO

L''Adénofibrome est l'affection la plus fréquente des pathologies bénignes du sein. L'objectif était d'identifier les aspects clinique et thérapeutique de l'adénofibrome dans le service de gynécologie obstétrique et chirurgie générale du CHU Gabriel TOURE. Patientes et Méthodes : L'étude était descriptive avec récolte rétrospective des données du 1er juillet 2018 au 31 juillet 2021. Ont été inclus, les dossiers de patientes prises en charge pour adénofibrome selon les principes éthiques garantissant l'anonymat et la confidentialité des données. Résultats : Nous avons recensé 701 cas de pathologie mammaire parmi lesquelles nous avons retenu au total 112 cas d'adénofibrome durant la période d'étude soit 15,9%. Nous avons enregistré 682 cas de tumeur mammaire, l'adénofibrome a représenté 16,42 %. Ainsi parmi toutes les 154 tumeurs bénignes du sein, l'adénofibrome (112 cas) a représenté 72,7%. L'âge moyen était de 27 ans avec un écart type de 14,8 ans (13 ans et 62 ans). La tranche d'âge de 20 à 35 ans était la plus représentée soit 44%. La fréquence de l'adénofibrome diminuait avec l'augmentation de la gestité et de la parité. L'obésité ou le surpoids ont été retrouvés chez 63 des patientes. Le motif principal de consultation était la présence d'une tuméfaction mammaire. L'atteinte unilatérale gauche prédominait dans 46% et la localisation au quadrant supéro- externe représentait 50 %. La résection chirurgicale a représenté la modalité de prise en charge la plus fréquente avec 56,2 % des cas. Conclusion : L'adénofibrome du sein est une affection bénigne assez fréquente. Son diagnostic oblige à une surveillance. Le traitement chirurgical doit être discuté selon l'âge, la taille et la présence de facteurs de risque de malignité


Fibroadenomas are the most common benign breast disorders. The aim of this study was to identify the clinical and therapeutic aspects of fibroadenoma in the obstetric gynecology department and General Surgery of Teaching hospital Gabriel TOURE in Bamako Mali. Patients and Methods: The study was descriptive with retrospective data collection from July 1, 2018 to July 31, 2021. The records of patients treated for fibroadenoma were included in accordance with ethical principles guaranteeing anonymity and confidentiality of data. Results: A total of 112 patients were selected for the study period, representing 15. 9% of all breast pathologies. We recorded 642 cases of breast mass, with fibroadenoma accounting for 16.72% of these breast tumors. Among all 154 benign breast tumors, fibroadenoma accounted for 72%. The mean age was 27 years, with a standard deviation of 14.8 years (13 years and 62 years). The 20-35 age group was the most represented, at 44%. The frequency of fibroadenoma decreased with increasing gestational age and parity. Over 60% of patients were overweight. The main reason for consultation was the presence of breast swelling. Unilateral left breast swelling was predominant in 46% of cases, and location in the upper-outer quadrant accounted for 50%. Surgical excision was the most frequent management modality, accounting for 56,2% of cases. Conclusion: fibroadenoma of the breast are a fairly common benign condition. Diagnosis requires surveillance. Surgical treatment should be discussed according to age, size and the presence of risk factors for malignancy.

Two continuous whipping devices, a rotor-stator (RS) and a narrow angular gap unit (NAGU), were used to produce aerated food with a 25% (v/v) gas fraction target. The liquid phase was a Newtonian model-solution containing 2% (w/w) of either whey proteins (WPC), sodium caseinate (SCN), or tween 20 (TW20). Strong differences emerged regarding gas incorporation and bubble size as a function of process parameters: namely, rotation speed and residence time. To improve understanding of the results obtained at pilot-scale, a second investigation consisting in the observation of the deformation and break-up of single gas bubbles has been undertaken using successively a Couette device and an impeller close to NAGU. For proteins, the observation of single bubble deformation and break-up showed that bubble break-up occurred by tip-streaming above a well-defined critical Capillary number Cac of 0.27 and 0.5 for SCN and WPC, respectively, whereas no break-up was observed with TW20 even though Ca reached 10. The poor foaming ability obtained with TW20 could be explained by a poor break-up mechanism, promoting coalescence and gas plugs at high shear instead of gas incorporation. Conversely, protein promote tip-streaming as the major break-up mechanism at low shear rate, explaining why rotation speed is not a key process parameter. Differences observed between SCN and WPC can be attributed to diffusion limitation for SCN when a much larger surface area is generated during aeration.

Introduction : La pathologie naso-sinusienne occupe une place importante en ORL. Sa prise en charge passe par une étude de données de la circonscription concernée. Ce travail avait pour but d'établir le profil épidémiologique, et clinique des pathologies naso-sinusiennes. Matériels et méthode : Il s'agissait d'une étude transversale descriptive menée en consultation sur une période de six mois allant de janvier 2018 à juin 2018 dans l'unité d'ORL-CCF du Centre de Santé de Référence de la Commune V de Bamako. Ont été inclus tout patient venu pour la première fois en consultation dans ladite unité pour pathologie naso-sinusienne. Résultats : Les pathologies naso-sinusiennes ont représenté 12,62% des consultations qui s'élevaient à 1656 patients. Nous avons recensé 60,3% de femmes et 39,7% d'hommes. La tranche d'âge 21-30 ans a constitué 25,8% des cas. L'âge moyen a été de 29 ans, avec des extrêmes de 11 jours et 80 ans. Les motifs de consultation ont été l'obstruction nasale (41,6%), l'épistaxis (19,2%), et les rhinorrhées 12,4%. Les rhinites ont représenté 59,8%, les sinusites 20,1%, les corps étrangers et les épistaxis chacun 7,6% des diagnostics. Un cas de tuberculose nasale a été observé. Conclusion : Les pathologies naso-sinusiennes sont dominées par la pathologie inflammatoire et infectieuse

BACKGROUND: In order to contribute to the fight against the pediatric HIV infection, we have assessed, through a study in which we have systematically proposed to carry out children's testing, the rate of acceptability and the feasibility of children's HIV testing during the routine activities of the department. We have also analyzed the reasons for the acceptability or the refusal of the child's HIV testing by the accompanying person. METHODS: The study took place from May to September 2015 including all the parents/legal guardians of any child aged 0 to 14 years coming for a consultation or who was hospitalized in the Pediatric Department of Souro Sanou Teaching Hospital. Counseling sessions conducted by community health workers focused on informing and proposing the principle of child testing. After obtaining the verbal and informed consent of the accompanying person, the first test was performed with Determine® by a hospital health worker. A second SD Bioline®/ImmunoCombII® test was performed if the first test was positive. With children aged less than 18 months, after a positive antibody test, we resorted to PCR for confirmation. RESULTS: A total of 848 accompanying persons, 568 of whom were female, underwent a pre-test interview during which the HIV test was offered to them. The mean age of accompanying persons was 30 (25.5 to 38) years; 747 accompanying persons (88.1%) accepted the testing of their child. We have found an influence of the accompanying person's religion (P=0.02) and the type of accompanying person on the acceptability of children's testing. Mothers were more willing to accept the test compared to other accompanying persons (P=0.002). The main reason for refusing the child's testing was the absence of one of the child's parents, mainly the father whose opinion was needed. The test was positive for HIV1 in 10 children. CONCLUSION: In health centers, getting the informed consent from parents to test their children is a big challenge. However, our study shows that this is possible, through the high rate of acceptability obtained.

OBJECTIVE: the aim of this study is to evaluate the performance of stool microscopy and culture in the diagnosis of pulmonary tuberculosis (PTB). MATERIAL AND METHOD: In resource-limited settings, PTB is mainly diagnosed in sputum by microscopy methods. In patients unable to provide sputum, invasive procedures are necessary to obtain alternative respiratory tract specimens. Because Mycobacterium tuberculosis complex (MTC) organisms are known to survive in gastric fluid, we hypothesized that swallowed MTC organisms would be detectable in stool samples. We compared the presence of MTC organisms in sputum and stool specimens collected at the same time from the same patients. RESULTS: We included samples routinely submitted to our laboratory of microbiological diagnosis from patients with suspected PTB. In addition, a stool specimen was collected within 24 h of the sputum collection or gastric aspirate. In the 57 patients included, sputum microcopy and culture confirmed respectively 9 (21%) and 15 (26%) cases. Stool samples made it possible to confirm 9 cases by microscopy and 5 cases by culture. The sensitivity of microscopy was 60% and that of culture 33%. Sputum analysis was negative for one HIV-infected patient, but the stool sample was positive and permitted the diagnosis of PTB. CONCLUSION: This study proves that despite the low sensitivity of stool cultures it can be an alternative or additional interesting sample for the diagnosis of PTB in patients who have difficulty expectorating.

AIM: This study was to determine the prevalence of missed opportunities for immunization in the pediatrics unit of the Souro Sanou University Hospital. METHODS: This was a cross-sectional descriptive study conducted from May to June 2010 involving children aged 0-11 months, accompanying persons and health personnel. Self-questionnaires were administered to health personnel in order to gage their knowledge of vaccination practices. RESULTS: 177 children including 101 girls and 49 health workers were recruited. The vaccination rate for children targeted by the Expanded Programme on Immunization (EPI) was 38% in the city of Bobo-Dioulasso and the rate of missed vaccination occasions was 50%. The low level of parental education, ignorance of EPI targeted diseases and a poor organization of the vaccination unit were noted. CONCLUSION: The overall strategy for vaccine administration should be rethought in a country with limited resources such as Burkina Faso to optimize immunization, EPI target diseases contributing to the mortality and morbidity in these regions.


BUT: L'objectif était de déterminer la fréquence des occasions manquées de vaccination dans le département de pédiatrie du Centre Hospitalier Universitaire Sourô Sanou de Bobo-Dioulasso. MÉTHODE: il s'agissait d'une étude transversale descriptive de mai à juin 2010 enrobant les enfants âgés de 0­11 mois, les personnes accompagnatrices, les agents de santé. Des auto-questionnaires ont été administrés au personnel de santé afin de récolter leur connaissance sur la pratique de la vaccination. RÉSULTATS: Avaient été inclus dans l'étude, 177 enfants dont 101 filles ainsi que leurs accompagnants et 49 agents de santé. Le taux de couverture vaccinale des enfants cibles du programme élargi de vaccination (PEV) dans la ville de Bobo-Dioulasso était de 38% et le taux des occasions manquées de vaccination était de 50%. L'enquête avait noté un faible niveau d'instruction des parents, la méconnaissance des maladies cibles du PEV ainsi qu'une mauvaise organisation de l'unité de vaccination. CONCLUSION: La stratégie globale d'administration des vaccins devra être repensée dans les pays à ressources limitées comme le Burkina Faso afin d'optimiser la vaccination, les maladies cibles du PEV contribuant à alourdir la mortalité et la morbidité dans ces contrées.

AIMS: The aim of this study was to describe the clinical and therapeutic aspects of peritonitis by typhoid ileal perforation in a tropical typhoid fever endemic area. PATIENTS AND METHODS: A descriptive retrospective study over an 8 year period (January 2000 to December 2007) was undertaken in a visceral service of the Point G teaching hospital in Bamako, Mali. Only patients diagnosed and operated for peritonitis presenting lesions or perforations of the final ileum were included in the study. RESULTS: The data from 120 patients meeting the inclusion criteria was collected. The average age was 22.7 years old. Triad abdominal pains (120 cases; 100%), vomiting (41; 62.3%), lack of matter and gas (22; 32.3%) constituted the main part of the functional signs. The physical examination showed a localised abdominal defence (7 cases, 10.3%), generalized defence (98 cases; 81.6%), and 2 unspecified cases (2.9%). A "wood belly" abdomen was palpated among 105 patients (87.5%), as well as umbilical hernias (106 cases; 88.3%). The Felix and Widal agglutination test was positive for 75 patients (62.5%) and negative for 27 patients (22.5%). During the operation, the quantity of aspirated pus was higher than 1500 ml in 2 cases and lower or equal to 500 ml in 57.4% of cases. The site of the perforation was ante mesenteric in 64 cases (53.3%). Excision-suture and abdominal cleansing were the most common surgical procedures (68 cases; 57%). Postoperatively 19 patients (15.83%) died of septic shock. The average duration of hospitalization was 20.6 days. CONCLUSION: The peritonitis by typhoid ileum perforation is still the cause of a high mortality rate in our country. The prevention by the vaccination in young populations could decrease the incidence of pathology and the serious complications which result from this.


BUT DE L'ÉTUDE: Le but de cette étude était de décrire les aspects cliniques et thérapeutiques de la péritonite par perforation iléale d'origine typhique en milieu d'endémie typhique tropicale. PATIENTS ET MÉTHODES: Une étude rétrospective descriptive sur une période de 8 ans (janvier 2000 à décembre 2007) a été menée dans un service de chirurgie viscérale du CHU du POINT G, Bamako, Mali. Les malades diagnostiqués et opérés pour péritonites présentant des lésions de perforations de l'iléon terminal ont été inclus dans l'étude. Les perforations iléales d'autres étiologies n'ont pas été retenues. RÉSULTATS: Un total de 120 malades répondant aux critères d'inclusion ont été recensés. La moyenne d'âge était de 22,7 ans. La triade douleurs abdominales (120 cas; 100%), vomissements (41; 62,3%), arrêt de matières et de gaz (22; 32,3%) a constitué l'essentiel des signes fonctionnels. A l'examen physique existait une défense abdominale localisée (7 cas, 10,3%), généralisée (98 cas; 81,6%), indéterminée 2 fois (2,9%). La contracture abdominale en « ventre de bois ¼ était palpée chez 105 malades (87,5%), ainsi qu'un cri de l'ombilic (106; 88,3%). Le test d'agglutination ou sérodiagnostic de Felix et Widal était positif pour 75 malades (62,5%) et négatif pour 27 malades (22,5%). En per opératoire la quantité de pus aspiré était supérieure à 1500 ml pour 2 cas et inférieure ou égale à 500 ml pour 57,4% de cas. Le siège de l'orifice de perforation était anté-mésentérique dans 64 cas, soit 53,3%. L'excision-suture et toilette abdominale a été la procédure chirurgicale la plus courante (68 cas soit 57%). En post opératoire 19 malades (15,83%) sont décédés de choc septique. La durée moyenne d'hospitalisation a été de 20,6 jours. CONCLUSION: La péritonite par perforation typhique iléale, est toujours et encore responsable d'une mortalité forte dans nos pays. La prévention par la vaccinationn de la population jeune pourrait diminuer l'incidence de la pathologie et les complications graves qui en découlent.
