Many young children in Ecuador suffer from high rates of malnutrition and stunting that affect their long-term growth and development. Little is known about the dietary patterns of children from the Amazon region who experience some of the highest rates of stunting (height-for-age) within Ecuador. Semi-structured interviews were conducted with 50 mothers of young children living in the Ecuadorian Amazon. In addition to descriptions of overall dietary patterns, three themes emerged from the interviews relating to strengths mothers have in feeding their children healthy diets: knowledge, autonomous and independent children, and supportive and responsive parenting. Five themes were found relating to barriers mothers have in feeding their children healthy diets. The first four themes concerned barriers (lack of knowledge of healthy foods, lack of access to healthy foods, not enough money, and child's health) related to multidimensional poverty. All these influenced the last theme found, namely, how difficult of an eater the child was. The implications of intervention efforts to reduce undernutrition and promote children's development by building on specific family and community strengths and identified barriers are also discussed in this paper.

Background: Endometrial cancer is of increasing concern in several countries, including Brazil, in part because of an ageing population, declines in fertility, and the increasing prevalence of obesity. Although endometrial tumors had lagged behind other cancer types in terms of treatment improvements, molecular characterization of these tumors is paving the way for novel therapies and an expansion of the therapeutic arsenal. We aimed to help medical oncologists who manage patients with recurrent or metastatic endometrial cancer in the Brazilian healthcare setting. Methods: The panel, composed of 20 medical oncologists, convened in November 2021 to address 50 multiple-choice questions on molecular testing and treatment choices. We classified the level of agreement among panelists as (1) consensus (≥75% choosing the same answer), (2) majority vote (50% to <75%), or (3) less than majority vote (<50%). Results: Consensus was present for 25 of the 50 questions, whereas majority vote was present for an additional 23 questions. Key recommendations include molecular testing for every patient with recurrent/metastatic endometrial cancer; choice of first-line treatment according to microsatellite instability and HER2, with the addition of programmed death ligand 1 (PD-L1) and hormone receptors (HRs) for second-line therapy; carboplatin and paclitaxel as the preferred option in first-line treatment of HER2-negative disease, with the addition of trastuzumab in HER2-positive disease; pembrolizumab plus lenvatinib as a key option in second line, regardless of HER2, PD-L1 or HRs; and various recommendations regarding treatment choice for patients with distinct comorbidities. Conclusion: Despite the existing gaps in the current literature, the vast majority of issues addressed by the panel provided a level of agreement sufficient to inform clinical practice in Brazil and in other countries with similar healthcare environments.

INTRODUCTION: Germ-cell tumors (GCTs) are the most common malignancy in young men. There is a paucity of data on GCTs in developing countries. LACOG 0515 study aimed to evaluate clinical characteristics and treatment outcomes in patients with GCTs from Brazilian cancer centers. MATERIALS AND METHODS: This is a retrospective cohort study evaluating male patients diagnosed with GCTs from 2000 to 2018 in 13 Brazilian hospitals. We described baseline characteristics, progression-free survival (PFS), and overall survival (OS). RESULTS: A total of 1232 patients were included, with a median age of 30 years. Histology was seminoma in 47.1% and non-seminoma GCT (NSGCT) in 52.9%. The primary tumor site was testis in 96.5%. At diagnosis, clinical stage I was present in 68.1% and 34.7% and clinical stages IS/II/III in 31.9% and 65.2% of patients with seminoma and NSCGT, respectively. Following orchiectomy, 55.2% of patients with clinical stage I were managed with surveillance. The 5-year disease-free survival rates among patients with stage I were 98.0% in seminoma and 92.3% in NSGCT, with 5-year OS of 99.6% and 97.6%, respectively. Among patients with advanced disease (IS, II, and III), the 5-year PFS were 88.7% in seminoma and 68.7% in NSGCT, with 5y-OS of 97.6% and 82.8%, respectively. CONCLUSION: This is the largest Brazilian cohort of GCTs. Our results show a high rate of adjuvant chemotherapy in patients with clinical stage I. Although our data demonstrate slightly inferior PFS compared with the International Germ Cell Cancer Collaborative Group and other contemporary series, the OS rates were similar.

ABSTRACT Dilated odontoma is a rare dental developmental anomaly that belongs to a class od dens in dente, being rarely described in the posterior region of the mandible. To be describe a dilated odontoma in the lower third molar region. this study had a qualitative, transversal, descriptive and documentar approach, of the case report type. Developed in the Integrated Health Clinics of a University in the Extreme South of Santa Catarina, in the Dentistry Service. This article presets a case of dilated odontoma in a 32-year-old male patient. The patient had pain and swelling in the left posterior region of the mandible. The Crown of this element was clinically within the normal range, however the radiographic examination showed barrel-shaped root alteration in element 38 and expansion of the buccal and lingual cortical bone. An incisional biopsy of the anomaly whas performed and sent to anatomopathological examination. According to the clinical, radiographic and anatomopathological characteristics, the diagnosis of dilated odontoma was reached. Due to the aberrant root anatomy, complete removal was chosen. The diagnosis of this abnormality depeds on the clinical, radiographic and anatomoathological characteristics and its therapy varies according to the invagination. In cases of deep invagination, extraction is indicated.


RESUMO O Odontoma Dilatado é uma rara anomalia de desenvolvimento dentário que pertence a uma classe de dens in dente, sendo raramente descrito na região posterior da mandíbula. O objetivo desse artigo foi descrever um Odontoma Dilatado na região de terceiro molar inferior. Materiais e métodos: este estudo teve abordagem qualitativa, transversal, descritiva e documental, do tipo relato de caso. Desenvolvido na Universidade do Extremo Sul Catarinense, no Serviço de Odontologia. Este artigo apresenta um caso de Odontoma Dilatado em um paciente do sexo masculino de 32 anos. O paciente apresentou dor e inchaço na região posterior esquerda da mandíbula. A coroa deste elemento encontrava-se, clinicamente, dentro dos padrões de normalidade, entretanto no exame radiográfico foi demonstrada a alteração radicular em forma de barril no elemento 38 e expansão de cortical óssea vestibular e lingual. Foi realizada a biópsia incisional da anomalia e enviado ao anatomopatológico. De acordo com as características clínicas, radiográficas e anatomopatológicas, chegou-se no diagnóstico de Odontoma Dilatado. Devido a anatomia aberrante optou-se pela remoção completa. O diagnóstico desta anormalidade é dependente das características clínicas, radiográficas e anatomopatológicas e sua terapêutica varia de acordo com a invaginação. Em casos de invaginação profunda a exodontia está indicada.

This study showed the effectiveness of biomedical interventions in obesity, diabetes and hypertension (NCDs), but innovative and intersectoral elements in the fight against obesity, type 2 diabetes and hypertension were rare. BACKGROUND: Is it possible to find effective and innovative actions to promote health and prevent NCDs in Brazilian municipalities? Can they be replicated? OBJECTIVE: Our objectives were to identify innovative and effective intersectoral actions for promoting and preventing NCDs in Brazilian municipalities. METHODS: This is a systematic review in an exploratory theoretical essay with a qualitative and quantitative approach. It is descriptive and analytical in terms of reporting findings and results. Inclusion and exclusion criteria favored health promotion work. Bias risk assessments was performed using the Cochrane GRADE and bias risk, with meta-analyses using RevMan and Iramuteq. RESULTS: Meta-analysis of biometric markers resulted in -4.46 [95% IC; -5.42, -3.49], p = 0.00001, indicating a reduction in NCD risk rates. The textual meta-analysis revealed P(r) ≈ 83% (Reinert), meaning low connectivity between the 'halos'. CONCLUSIONS: There is evidence of the effectiveness in interventions, but innovative and intersectoral elements to combat and prevent NCDs were barely seen. While evidence of intervention effectiveness was observed, innovative and intersectoral elements to combat and prevent NCDs were barely noticed.

This study involved evaluating the effects of rotational impeller speed agitation (N) and specific air flow rate (Фair) on bikaverin production and on the growth of Fusarium oxysporum employing 11 bench-scale bioreactor assays. The results showed that the maximum bikaverin production (close to 300 mg L-1) was achieved after 48 h of fermentation in rice medium (20 g L-1 milled rice in water) at 28 °C with a volumetric oxygen transfer coefficient (KLa) and shear stress values of approximately 20 h-1 and 17 N m-2, respectively. We reached this combination of parameters using an N of 340 rpm and Фair of 0.935 vvm. These KLa and shear stress values can be used as references when upscaling this process. Thus, this study was important to demonstrate how the main parameters in bioreactors affect bikaverin production and it presented important indications for upscaling this bioprocess.

Resumo Realizou-se pesquisa qualitativa com o objetivo de compreender as repercussões psicossociais da pandemia do SARS-CoV-2 na população atingida pelo rompimento da Barragem de Fundão, em 2015, pertencente à mineradora Samarco e suas mantenedoras - VALE e BHP Billiton - em Mariana (MG). Foram utilizadas as técnicas de observação participante e grupos focais. As narrativas de profissionais de saúde e pessoas atingidas foram analisadas através da Análise de Conteúdo, a partir dos fatores socioculturais, simbólicos e políticos encontrados nesse contexto de sobreposição de desastre da mineração e pandemia. Os resultados mostram que a vivência dos atingidos durante a pandemia apresenta singularidades, tais como a intensificação da ruptura de laços sociais e comunitários, que já era identificada desde o rompimento. O luto durante a pandemia também é potencializado pelo luto coletivo já vivenciado desde a tragédia. Há uma intensificação do sofrimento diante da falta de informação e da prorrogação da entrega do reassentamento. É possível identificar a pandemia como mais um ponto de sofrimento para essa população vulnerabilizada.


Abstract Qualitative research was developed with the aim of understanding the psychosocial repercussions of the SARS-CoV-2 pandemic on the population affected by the collapse of the Fundão Dam in Mariana (MG), in 2015, belonging to the Samarco mining company and its maintainers VALE and BHP Billiton. Participant observation techniques and focus groups were used. The narratives of health professionals and affected people were analyzed through Content Analysis, from the sociocultural, symbolic and political factors found in this context of overlapping mining disaster and pandemic. The results show that the experience of those affected during the pandemic presents singularities such as the intensification of the rupture of social and community ties that had already been identified since the disaster. Grief during the pandemic is also enhanced by the collective grief already experienced since the tragedy. There is an intensification of suffering due to the lack of information and the postponement of the resettlement delivery. It is possible to identify the pandemic as another point of suffering for this vulnerable population.

Objective: Deficient dental plaque control is common in adolescents, mainly among those wearing orthodontic apparatus. This study aimed to compare plaque control and oral hygiene habits in adolescents with and without fixed orthodontic appliances. Additionally, it was investigated whether personalized oral hygiene instruction (OHI) could be a predictor for reducing dental plaque accumulation in orthodontic patients. Material and Methods: Sixty-nine patients, aged 12 to 20 years, were evaluated in a public organization which provides dental care. A questionnaire of oral hygiene habits was applied, and the number of natural teeth and number of teeth with visible biofilm were obtained during clinical examination. A comparative analysis of sociodemographic and clinical data was performed, splitting this population into two groups: those who used fixed orthodontic appliances (n=40) and those who did not (n=29). Results: There were no differences in oral hygiene habits among groups. The Poisson regression showed that adolescents with fixed appliances were 54% more likely to have dental plaque buildup compared to those who did not. Among individuals wearing orthodontic appliance, those who had not received personalized OHI before or during orthodontic treatment were 78% more likely to have a greater number of teeth with bacterial plaque compared to instructed ones. Conclusion: Especially among the adolescents wearing fixed orthodontic appliances, those who have not received personalized OHI were almost twice as likely to have a greater number of teeth with dental biofilm accumulation. Dentists and dental hygienists play a prominent role in motivating and improving the quality of individual oral hygiene (AU)


Objetivo: O controle deficiente da placa dental é comum em adolescentes, principalmente entre aqueles que usam aparelhos ortodônticos. Este estudo teve como objetivo comparar o controle da placa e os hábitos de higiene oral em adolescentes com e sem aparelhos ortodônticos fixos. Além disso, foi investigado se a instrução de higiene oral (IHO) personalizada poderia ser um preditor para reduzir o acúmulo de placa dental em pacientes ortodônticos. Material e métodos: Sessenta e nove pacientes, de 12 a 20 anos, foram avaliados em uma organização pública que presta atendimento odontológico. Foi aplicado um questionário sobre hábitos de higiene oral, e o número de dentes naturais e o número de dentes com biofilme visível foram obtidos durante o exame clínico. Foi realizada uma análise comparativa dos dados sociodemográficos e clínicos, dividindo essa população em dois grupos: aqueles que usavam aparelhos ortodônticos fixos (n = 40) e aqueles que não o usavam (n = 29). Resultados: Não houve diferenças nos hábitos de higiene oral entre os grupos. A regressão de Poisson mostrou que os adolescentes que tinham aparelhos fixos apresentaram 54% mais chance de ter mais acúmulo de placa dental comparados aos que não tinham. Entre os indivíduos que usavam aparelhos ortodônticos, aqueles que não receberam IHO personalizada antes ou durante o tratamento ortodôntico apresentaram 78% mais chance de ter maior número de dentes com placa bacteriana em comparação com os que a receberam. Conclusão: Especialmente entre os adolescentes que usavam aparelhos ortodônticos fixos, aqueles que não receberam IHO personalizada tiveram quase duas vezes mais chances de ter um número maior de dentes com acúmulo de biofilme dental. Dentistas e higienistas dentais desempenham um papel proeminente na motivação e na melhoria da qualidade da higiene oral do indivíduo. (AU)

Microbial pigments have a distinguished potential for applications in food and pharmaceutical industries, stimulating the research in this field. The present study evaluated the ideal conditions for extracting bikaverin (red pigment) from the biomass of Fusarium oxysporum CCT7620. Among the solvents tested, ethyl acetate extraction resulted in the highest bikaverin concentration and the kinetic study revealed a saturation in bikaverin concentration from 256 min on. Based on a preliminary economic study, three sequential extractions with ethyl acetate was considered the ideal protocol to recover bikaverin. After extraction, chromatographic methods were tested to purify bikaverin. The use of silica gel or Sephadex (open column) could not successfully purify bikaverin, but the semi-preparative HPLC resulted in a bikaverin-enriched fraction with a purity degree equivalent to the commercial analytical standard. This work provides relevant information regarding the extraction and purification of bikaverin, which may be useful for other downstraming processes.

Um dos elementos essenciais para alcançar a estética do sorriso é o fenótipo e o contorno gengival que, com suas arquiteturas, in- fluenciam no tamanho das coroas dentais. O termo "fenótipo periodontal" foi padronizado no Workshop Mundial para a Classificação das Doenças e Condições Periodontais e Peri-Implantares de 2017, porém esse tema já havia sido abordado outras vezes, com outras nomenclaturas. A avaliação dos diferentes fenótipos periodontais é fundamental, pois nos dão informações relacionadas às características dos tecidos periodontais e às formas dentárias, além de tornar o tratamento mais previsível, podendo evitar problemas como: trau- ma, inflamação e outras complicações clínicas e cirúrgicas. O objetivo desta revisão de literatura é fazer uma pesquisa a respeito do tema fenótipo periodontal por meio dos artigos mais relevantes entre o período de 2017 a 2021, evidenciando sua classificação, prevalência e formas de diagnóstico. Existem várias formas de diagnosticar o fenótipo periodontal, sendo a transparência do sulco gengival por meio da sonda milimetrada a preconizada pelo Workshop Mundial. Podemos observar uma prevalência do fenótipo fino pelo gênero feminino, e o fenótipo espesso pelo gênero masculino. Ainda faltam mais evidências científicas para o correto relacionamento do fenótipo periodontal com outros fatores como idade, tabagismo, hábitos de higiene, alimentação e má oclusão.


One of the essential elements to achieving smile esthetics is the phenotype and gingival contour, which with their architecture influence the size of dental crowns. The term "periodontal phenotype" was standardized in the 2017 World Workshop on the Classification of Periodontal and Peri- Implant Diseases and Conditions. However, much has been said about the topic, with other nomenclatures. Evaluating different periodontal phenotypes is essential, as they provide us with information related to the characteristics of periodontal tissues and dental forms. In addition to making the treatment more predictable, it can avoid problems such as trauma, inflammation, and other clinical and surgical complications. The purpose of this literature review is to research the topic, periodontal phenotype, through the most relevant articles between the period 2017 to 2021, showing its classification, prevalence, and forms of diagnosis. There are several ways to diagnose the periodontal phenotype, and the one recommended by the World Workshop is the transparency of the gingival sulcus using the millimeter probe. We can observe a prevalence of the thin phenotype for the female gender, whereas the thick phenotype is prevalent for the male gender. There is still a lack of scientific evidence for the correct relationship of the periodontal phenotype with other factors such as age, smoking, hygiene habits, diet and, malocclusion

Objetivos: Avaliar os fatores que determinam o tipo e o tempo de aleitamento materno, bem como delinear sobre a situação social e nutricional de mulheres atendidas na rede de atenção primária à saúde no município de Juiz de Fora (MG). Métodos: Realizaram-se quatro avaliações: a primeira no terceiro trimestre da gestação, e as outras, nos 1o, 3o e 6o meses pós-parto. Inicialmente, aplicou-se um questionário sobre amamentação e um Recordatório 24 horas, também aplicado no 6o mês pós-parto. Posteriormente, questionou-se sobre a frequência, tipo de aleitamento e dificuldades no manejo. Resultados: As variáveis que influenciaram significativamente no aleitamento materno exclusivo nos 1°, 3° e 6° meses foram: maior consumo mediano de Vitamina C e B9, maior desempenho no teste sobre amamentação, idade da mãe entre 20 e 35 anos, consumo adequado de carboidratos e suplementação de complexo vitamínico. Os fatores protetores do aleitamento materno exclusivo ou predominante foram: apresentar alguma dificuldade para amamentar e dieta inadequada em relação aos lipídios. Já os fatores de risco foram: fumo na gestação, não saber responder sobre o correto manejo da lactação e renda familiar menor que um salário mínimo. Conclusões: Variáveis maternas, dietéticas, socioeconômicas e conhecimentos sobre o manejo da lactação influenciaram no tempo e tipo do aleitamento materno.


Objectives: To evaluate the factors determining the type and maternal breastfeeding time and report on the social and nutritional status of women at the local network of primary health care in the city of Juiz de Fora (MG). Methods: There were four evaluations: the first in the third trimester of pregnancy and the others in the first, third, and sixth months following delivery. At first, we applied a questionnaire about breastfeeding and 24-hour dietary recall, also applied in the sixth month postpartum. Subsequently, they were asked about the frequency, type of feeding, and difficulties in management. Results: The variables that significantly influenced the exclusive breastfeeding in the first, third, and sixth months were higher mean intake of vitamins C and B9, higher scores in the test on breastfeeding, mother aged between 20 and 35 years old, adequate intake of carbohydrates and supplementation of vitamin complex. Protective factors in exclusive or predominant breastfeeding were having some difficulty in breastfeeding and inadequate diet in relation to lipids. However, the risk factors were smoking during pregnancy, being unable to correctly answer about lactation management, and family income lower than one minimum wage. Conclusions: Maternal variables, diet, socioeconomic, and knowledge about lactation management influenced the time and type of breastfeeding.

OBJETIVO: identificar os Diagnósticos de Enfermagem CIPE® relacionados à necessidade humana básica de nutrição na clínica pediátrica. MÉTODO: estudo transversal com abordagem quantitativa, realizado na clínica pediátrica de um hospital público da cidade de João Pessoa, Paraíba. A amostra foi composta por 91 participantes, entre crianças e adolescentes com período de internação superior a 24 horas. RESULTADO: foi possível identificar os diagnósticos: Emagrecimento; Adesão ao Regime Dietético Eficaz; Comportamento Alimentar Infantil, Prejudicado; Apetite, Prejudicado; Obesidade; Amamentação Exclusiva; Amamentação Exclusiva Prejudicada; Problema de Peso Corporal; Sobrepeso; Adesão ao Regime Dietético Prejudicada e Condição Nutricional, Prejudicada. CONCLUSÃO: dentre os diagnósticos, destacaram-se Apetite Eficaz; Comportamento Alimentar Infantil Eficaz; Padrão de Ingestão de Alimentos ou Líquidos Eficaz; Condição Nutricional Positiva e Capacidade para Alimentar-se. A elaboração dos diagnósticos relacionados à nutrição da criança e adolescente corrobora à construção de subconjuntos terminológicos, a fim de garantir a eficácia do cuidado.


OBJECTIVE: to identify the ICNP® Nursing Diagnoses related to the basic human need for nutrition in the pediatric clinic. METHOD: a cross-sectional study with a quantitative approach, carried out in the pediatric clinic of a public hospital in the city of João Pessoa, Paraíba. The sample consisted of 91 participants, including children and adolescents with a hospital stay of more than 24 hours. RESULT: it was possible to identify the following diagnoses: Weight loss; Adherence to the Effective Dietary Regime; Infant Feeding Behavior, Impaired; Appetite, Impaired; Obesity; Exclusive Breastfeeding; Impaired Exclusive Breastfeeding; Body Weight Problem; Overweight; Impaired Adherence to Diet; and Nutritional Condition, Impaired. CONCLUSION: the following stood out among the diagnoses: Effective Appetite; Effective Infant Feeding Behavior; Effective Food or Fluid Intake Pattern; Positive Nutritional Status; and Ability to Feed. Elaboration of the diagnoses related to child and adolescent nutrition corroborates the construction of terminology subsets, in order to ensure care effectiveness.


OBJETIVO: identificar los diagnósticos de enfermería de la CIPE® relacionados con la necesidad humana básica de nutrición en la clínica pediátrica. MÉTODO: estudio transversal con abordaje cuantitativo, realizado en la clínica pediátrica de un hospital público de la ciudad de João Pessoa, Paraíba. La muestra estuvo formada por 91 participantes, niños y adolescentes con una estancia hospitalaria de más de 24 horas. RESULTADO: fue posible identificar los diagnósticos: Pérdida de Peso; Adherencia al Régimen Alimentario Efectiva; Conducta Alimentaria Infantil, Alterada; Apetito Alterado; Obesidad; Lactancia Exclusiva; Lactancia Exclusiva Comprometida; Problema dePeso Corporal; Sobrepeso; Adherencia al Régimen Alimentario Comprometida y Condición Nutricional Deteriorada. CONCLUSIÓN: entre los diagnósticos se destacó Apetito Efectivo; Conducta Alimentaria Infantil Efectiva; Patrón de Ingesta de Alimentos o LíquidosEfectivo; Estado Nutricional Positivo y Capacidad para Alimentarse. La elaboración de diagnósticos relacionados con la nutrición infantil y adolescente coincide con la construcción de subconjuntos terminológicos, a fin de asegurar la efectividad del cuidado.

Pesticides are a major tool for the intensification of agriculture, and helped to increase food, feed and biofuel production. Yet, there are persistent concerns about the negative effects of pesticides in human health and the environment, particularly in low and middle income countries (LMICs). Given the lack of information on pesticide exposure and hazard, Colombia exemplifies the need to narrow the information gap on pesticide risk in LMICs. We assessed pesticide hazard in Colombia based on the official toxicity categorization, compared it to more integral international standards, and identified main actions to narrow this information gap. Results showed that Colombia has been a relevant regional actor in pesticide production and trade, reaching almost 75 million kilogrammes and liters sold in 2016. Based on acute toxicity for humans, a quarter of the amount of pesticides sales and imports, and a third of the exports in 2016 ranged from moderately to extremenly toxic. The top-selling agrochemicals in 2016 (glyphosate with 14% of the total sales, chlorpyrifos 7.5% and mancozeb 6.9%) are also commonly used in other countries, reflecting a homogenized global industry. Compared to integral international categorizations, we found that for that year 63% of the pesticides sold with slightly acute toxicity are actually considered highly hazardous pesticides (HHP) for humans or the environment, evidencing the need to use a more integral hazard categorization in the country. Narrowing the information gap in pesticide use and associated risks demands a transparent process of knowledge creation and sharing, including funtional information and monitoring systems. This should be part of an integral assessment and regulation that better defines HHP, their production and trade to reduce pesticide risk while informing a transition towards sustainable food systems.

Prostate cancer is among the most common cancer diagnoses in men, and the best treatment for patients with metastatic disease in advanced stages is still unclear. Previously, we have demonstrated that the three 1-(3-(aryl-4,5-dihydroisoxazol-5-yl)methyl)-4-trihalomethyl-1H-pyrimidin-2- ones derivatives (8a, 8e and 9c) present important cytotoxicity and selectivity for tumoral cells. Considering that various cytotoxic drugs have been assessed in patients with prostate cancer, but few drugs show survival advantage, we decided to study these three compounds (8a, 8e and 9c) in prostate cancer cells, androgen receptor (AR)-positive 22Rv-1 and AR-negative PC-3 cells. We obtained the half maximal inhibitory concentration (IC50) of 8a, 8e and 9c in prostate cancer cells and based on high selectivity of 9c to PC-3 cells, we determined the mechanism of this compound to induce cell death through different methods. We show here that 9c compound induces cell cycle arrest in G2/M, increasing the levels of reactive oxygen species and DNA damage, and triggers DNA damage response by ataxia-telangiectasia mutated (ATM) and histone H2AX phosphorylation induction. The compound also led PC-3 to lipid peroxidation and mitochondrial depolarization which triggered the activation of intrinsic pathway, confirmed by increase of cleaved caspase-9 and 3. In this work we also show the ability of 9c in reducing vascular endothelial growth factor expression (VEGF) and inhibiting topoisomerase I enzyme, therefore indicating a potential new molecule to be further investigated for prostate cancer management.

OBJECTIVE: To determine the clinical phenotype of Guillain-Barré syndrome (GBS) after Zika virus (ZIKV) infection, the anti-glycolipid antibody signature, and the role of other circulating arthropod-borne viruses, we describe a cohort of GBS patients identified during ZIKV and chikungunya virus (CHIKV) outbreaks in Northeast Brazil. METHODS: We prospectively recruited GBS patients from a regional neurology center in Northeast Brazil between December 2014 and February 2017. Serum and CSF were tested for ZIKV, CHIKV, and dengue virus (DENV), by RT-PCR and antibodies, and serum was tested for GBS-associated antibodies to glycolipids. RESULTS: Seventy-one patients were identified. Forty-eight (68%) had laboratory evidence of a recent arbovirus infection; 25 (52%) ZIKV, 8 (17%) CHIKV, 1 (2%) DENV, and 14 (29%) ZIKV and CHIKV. Most patients with a recent arbovirus infection had motor and sensory symptoms (72%), a demyelinating electrophysiological subtype (67%) and a facial palsy (58%). Patients with a recent infection with ZIKV and CHIKV had a longer hospital admission and more frequent mechanical ventilation compared to the other patients. No specific anti-glycolipid antibody signature was identified in association with arbovirus infection, although significant antibody titres to GM1, GalC, LM1, and GalNAc-GD1a were found infrequently. CONCLUSION: A large proportion of cases had laboratory evidence of a recent infection with ZIKV or CHIKV, and recent infection with both viruses was found in almost one third of patients. Most patients with a recent arbovirus infection had a sensorimotor, demyelinating GBS. We did not find a specific anti-glycolipid antibody signature in association with arbovirus-related GBS.

In last years, the main studied microbial sources of natural blue pigments have been the eukaryotic algae, Rhodophytes and Cryptophytes, and the cyanobacterium Arthrospira (Spirulina) platensis, responsible for the production of phycocyanin, one of the most important blue compounds approved for food and cosmetic use. Recent research also includes the indigoidine pigment from the bacteria Erwinia, Streptomyces and Photorhabdus. Despite these advances, there are still few options of microbial blue pigments reported so far, but the interest in these products is high due to the lack of stable natural blue pigments in nature. Filamentous fungi are particularly attractive for their ability to produce pigments with a wide range of colors. Bikaverin is a red metabolite present mainly in species of the genus Fusarium. Although originally red, the biomass containing bikaverin changes its color to blue after heat treatment, through a mechanism still unknown. In addition to the special behavior of color change by thermal treatment, bikaverin has beneficial biological properties, such as antimicrobial and antiproliferative activities, which can expand its use for the pharmaceutical and medical sectors. The present review addresses the production natural blue pigments and focuses on the properties of bikaverin, which can be an important source of blue pigment with potential applications in the food industry and in other industrial sectors.

Un cuerpo extraño es un objeto o una estructura que se incluye accidental o intencionalmente en la intimidad de los tejidos orgánicos de un individuo. Puede desencadenar importantes procesos inflamatorios/ infecciosos, dependiendo de su naturaleza, requiriendo en la mayoría de los casos su extracción quirúrgica, con el fin de evitar daños al paciente. Este trabajo tiene como objetivo informar un caso de cuerpo extraño (fragmento de amalgama) incluido iatrogénicamente en la exodoncia de un molar inferior derecho en un paciente que, después de 8 años, optó por hacerse un implante en el área y descubrió la inclusión de este material, siendo necesaria su extracción quirúrgica para la posterior colocación del implante dental en la región. Los autores destacan la necesidad de realizar una minuciosa inspección y toilette de la caja alveolar en el acto de la exodoncia, para evitar incluir cuerpos extraños en el sitio quirúrgico y evitar así, una nueva intervención en el área (AU)


Foreign body is an object or structure included accidentally or intentionally in the intimacy of the organic tissues of individuals. They can trigger important inflammatory / infectious processes, depending on its nature, requiring its surgical removal in most cases, to prevent damage to the patient. This study aimed to report a case of a foreign body (fragment of amalgam) iatrogenically included in extraction of a right lower molar in a patient who, after 8 years, opted to have an implant in the area and discovered the inclusion of this material, requiring its surgical removal for subsequent placement of a dental implant in the region. The authors highlight the need to perform a thorough inspection and toilet of the alveolar box in the act of extraction, to avoid including foreign bodies in the surgical site, avoiding further intervention in the area (AU)

ABSTRACT OBJECTIVE: To verify the effectiveness of screening for tuberculosis (TB) on all-cause mortality and tuberculosis cases in newly diagnosed HIV-infected patients through a clinical algorithm based on recommendations of the World Health Organization. METHODS: From March 2014 to April 2016, a pragmatic randomized clinical trial was conducted with newly diagnosed and TB-free HIV-infected adults undergoing antiretroviral therapy for up to one month at a major tertiary hospital for HIV in the state of Pernambuco, Brazil. Participants were randomized into intervention and control groups using an automatically-generated random list, and followed-up for at least 6 months. The intervention group was screened for TB at hospital admission and at every follow-up visit through a series of questions addressing TB-related symptoms (cough, fever, night sweating, and weight loss). Patients presenting with any of these symptoms were referred to a pulmonologist and underwent sputum smear microscopy, sputum culture, and rapid molecular testing (GeneXpert). When at least one test result came back positive, TB treatment was initiated. In turn, if patients tested negative but presented with severe clinal symptoms, TB preventive treatment was initiated. Screening for TB was not performed systematically in the control group. The primary outcome assessed in this study was death from all causes, and secondary outcomes included sensitivity and specificity of this screening test, as well as its detection time. RESULTS: This study evaluated 581 patients, 377 in the intervention group (64.9%) and 204 in the control group (35.1%). In total, 36 patients died during the follow-up period. Of these, 26 (6.9%) were from the intervention group, reaching a cumulative mortality coefficient of 69 per 1,000 inhabitants, and 10 (4.9%) from the control group (p = 0.341), with a cumulative mortality coefficient of 49 per 1,000 inhabitants (p = 0.341).

BACKGROUND: Since 2015, the arthropod-borne viruses (arboviruses) Zika and chikungunya have spread across the Americas causing outbreaks, accompanied by increases in immune-mediated and infectious neurological disease. The spectrum of neurological manifestations linked to these viruses, and the importance of dual infection, are not known fully. We aimed to investigate whether neurological presentations differed according to the infecting arbovirus, and whether patients with dual infection had a different disease spectrum or severity. METHODS: We report a prospective observational study done during epidemics of Zika and chikungunya viruses in Recife, Pernambuco, a dengue-endemic area of Brazil. We recruited adults aged 18 years or older referred to Hospital da Restauração, a secondary-level and tertiary-level hospital, with suspected acute neurological disease and a history of suspected arboviral infection. We looked for evidence of Zika, chikungunya, or dengue infection by viral RNA or specific IgM antibodies in serum or CSF. We grouped patients according to their arbovirus laboratory diagnosis and then compared demographic and clinical characteristics. FINDINGS: Between Dec 4, 2014, and Dec 4, 2016, 1410 patients were admitted to the hospital neurology service; 201 (14%) had symptoms consistent with arbovirus infection and sufficient samples for diagnostic testing and were included in the study. The median age was 48 years (IQR 34-60), and 106 (53%) were women. 148 (74%) of 201 patients had laboratory evidence of arboviral infection. 98 (49%) of them had a single viral infection (41 [20%] had Zika, 55 [27%] had chikungunya, and two [1%] had dengue infection), whereas 50 (25%) had evidence of dual infection, mostly with Zika and chikungunya viruses (46 [23%] patients). Patients positive for arbovirus infection presented with a broad range of CNS and peripheral nervous system (PNS) disease. Chikungunya infection was more often associated with CNS disease (26 [47%] of 55 patients with chikungunya infection vs six [15%] of 41 with Zika infection; p=0·0008), especially myelitis (12 [22%] patients). Zika infection was more often associated with PNS disease (26 [63%] of 41 patients with Zika infection vs nine [16%] of 55 with chikungunya infection; p≤0·0001), particularly Guillain-Barré syndrome (25 [61%] patients). Patients with Guillain-Barré syndrome who had Zika and chikungunya dual infection had more aggressive disease, requiring intensive care support and longer hospital stays, than those with mono-infection (median 24 days [IQR 20-30] vs 17 days [10-20]; p=0·0028). Eight (17%) of 46 patients with Zika and chikungunya dual infection had a stroke or transient ischaemic attack, compared with five (6%) of 96 patients with Zika or chikungunya mono-infection (p=0·047). INTERPRETATION: There is a wide and overlapping spectrum of neurological manifestations caused by Zika or chikungunya mono-infection and by dual infections. The possible increased risk of acute cerebrovascular disease in patients with dual infection merits further investigation. Pathogens find diverse niches for survival including inside a host cell where replication occurs in a relatively protective environment. Vibrio parahaemolyticus is a facultative intracellular pathogen that uses its type 3 secretion system 2 (T3SS2) to invade and replicate inside host cells. Analysis of the T3SS2 pathogenicity island encoding the T3SS2 appeared to lack a mechanism for egress of this bacterium from the invaded host cell. Using a combination of molecular tools, we found that VPA0226, a constitutively secreted lipase, is required for escape of V. parahaemolyticus from the host cells. This lipase must be delivered into the host cytoplasm where it preferentially uses fatty acids associated with innate immune response to esterify cholesterol, weakening the plasma membrane and allowing egress of the bacteria. This study reveals the resourcefulness of microbes and the interplay between virulence systems and host cell resources to evolve an ingenious scheme for survival and escape.
