RESUMO

Alcohol intakes and dietary habits of 304 alcoholic, hospitalized patients were evaluated. There were 195 patients with hepatic cirrhosis, 40 precirrhotics, and 69 noncirrhotics. Alcohol contributed 50% to 58% of total calories. Two thirds of the patients drank excessively for more than 20 years. There were no statistically significant differences between the three groups in the duration or degree of alcohol excess. Dietary intakes were assessed for a period of at least two years before the presenting illness. Noncirrhotics had higher food caloric intakes and higher protein intakes than the cirrhotics (P less than .05). The findings suggest that dietary factors may be involved in the pathogenesis of the disease.

RESUMO

Electroencephalograms were recorded in 71 stroke-free patients suffering from mitral valve disease, complicated by atrial fibrillation in 33. The incidence of EEG abnormalities was not related either to the presence or absence of atrial fibrillation or to the type of valve lesion present, and did not differ significantly from the incidence observed in the general population. The findings suggest that occurrence of symptomatically silent emboli is not a feature of rheumatic mitral valve disease.

RESUMO

The relationship between length of stay of patients in a large general hospital and bed usage has been analyzed. Long-stay patients use beds disproportionately, and even week-end admission and discharge patterns can affect availability of beds. Transfer of long-stay patients to alternate care facilities of lower cost can have marked effects on efficient bed usage.It is argued from the analyses that since the form and type of disease causing admission are the prime factors in determining length of stay, to reduce the cost of health care the highest priority must be given, as well as to prevention, to the devising of new cures and improvements in therapy. Improving diagnostic and prognostic techniques, making available alternate care facilities and education of the public in the care of the sick should also reduce costs.
