RESUMO

The experiments on unanesthetized rats have established that the intravenous injection of verapamil in a dose of 0.6 mg/kg during 30-minute carotid occlusion with subsequent recirculation prevents postischemic hypoperfusion, normalized oxidative processes and prevents brain tissue edema. In patients with transient cerebral circulatory disorders, a single injection of verapamil in a dose of 10 mg enhances the pulse filling of cerebral vessels. During a course therapy, the drug decreases minor and middle vessel tone, increases the REG amplitude, lowers the interhemispheric asymmetric blood filling rate, and enhances the functional resistance of cerebral vessels.

Cavinton was used for 10 years in 967 patients with different cerebrovascular diseases. The highest effect was seen in patients with early forms and primarily chronic forms: vegetovascular (neurocirculatory) dystonia, initial manifestations of brain blood supply insufficiency, circulatory encephalopathy in the first and second stages. Improvement of the subjective status and a decrease of the intensity of vestibulocerebellar disorders were recorded by the end of the treatment in 75-85% of such patients. In ischemic brain stroke, regress of general cerebral and focal symptoms was more rapid and significant in the adequate reaction type of cerebral hemodynamics to cavinton administration (a rise of pulse blood content of the brain and a reduction of the vascular tone according to the REG data) and was less noticeable in the hypertonic and, in particular, in the hypotonic type. Cavinton should not be used in severe general cerebral hypertensive crises, as well as in elderly or senile patients with acute cardio-cerebral or cerebro-cardiac syndrome, postinfarction cardiosclerosis, marked disorders of heart rhythm.

RESUMO

Finoptin and sensit produced a favourable effect in the treatment of 290 patients with initial manifestations of deficient blood supply to the brain, encephalopathy attended by circulatory disturbances and ischemic brain insult. Changes in the systemic and cerebral hemodynamics were more pronounced in middle-aged and elderly patients suffering from hypertension or its combination with atherosclerosis.

Twenty-seven patients with dyscirculatory encephalopathies grades I and II and 20 acute ischemic stroke patients received finoptin (verapamil) at daily 5 to 10 mg i. v. or 120 to 240 mg oral doses. The course lasted for 2 months. Rheoencephalography (RhEG) revealed an increase in cerebral blood filling 3 to 5 min after the single finoptin administration. The effect lasted for an average of 40 to 60 min. Central hemodynamic changes were represented by lowering blood pressure and total peripheral vascular resistance, increasing stroke volume and cardiac output. The effect was observable 40 min after a single i. v. injection of the drug. Beginning from the 2nd to 3rd day of the treatment, the patients' subjective conditions improved with reduced strength of general cerebral signs of stroke, RhEG and ECG changes indicating the permanent increase in cerebral circulation and reduced left ventricular overload.

A total of 171 patients of young, middle, and old age presenting initial manifestations of cerebral circulatory insufficiency and early disordered-circulatory encephalopathy were treated with cavinton. The clinical effect of the drug mostly expressed in a considerable decrease of subjective manifestations of the disease was fairly high in most patients of different age groups. The most pronounced positive shifts (according to REG) of the cerebral hemodynamics upon cavinton therapy were observed in young and middle-aged patients. A lower efficacy in elderly patients was explained by the fact that there were grave organic changes in the cerebral vessels, predominantly in those of small and medium caliber. A comprehensive nature of the drug action, as well as the absence of any adverse effect on the cerebral hemodynamics recommend cavinton as a drug of choice in the treatment of patients with chronic forms of cerebral ischemia, including elderly ones. The drug should not be administered to patients over 60 years of age with marked disorders of cardiac activity.

RESUMO

Senzit increased volume velocity of the cerebral blood flow and oxygen delivery to the brain tissues under normal conditions at the expense of a decrease of the cerebral vessels tone. In transient cerebral ischemia the drug prevents disorders of cerebral circulation and a lowering of oxygen tension in the brain tissue. A prophylactic administration of senzit in carotid arterial occlusion prevents the development of edema and death of animals. Oral senzit produced an increase of blood supply to the brain and hypocoagulation changes in hemostasis system in patients with chronic insufficiency of cerebral circulation.
