RESUMO

Here the clinic comparison between both inhaling and oral Mucosolvin therapy is reported on. To determine the virtual viscosity of sputum has proved to be the deciding parameter to measure the effect of Mucosolvin. Both Mucosolvin inhalable and oral are equally effective. But whereas with oral therapy there were no side effects (e.g. bronchospasm) necessitating withdrawal of the drug such side effects occurred with inhaling therapy. Thus Mucosolvin allowes an effective mucolytic therapy independent of using any inhalation appliance.

RESUMO

Continuing a study on mucolytically and secretolytically effective drugs, a viscometrically energetic principle was designed for controlling both the virtual viscosity and the adhesion of sputum. The technique used is integral in character, an amount of 2 to 4 ml of inhomogeneous purulent and foam-free sputum being sufficient for the purpose. The data are shown in a curve the characteristic of which is a piecewise e-function giving separate descriptions of the virtual viscosity and the adhesive behaviour. The curves are used to record the progress of the therapy used. "Technique and device for measuring viscosity" was registered at the Amt für Erfindungs- und Patentwesen der DDR HA Rechtsschutz in 1984 (WP G 01 N/251 817 5) the authors being Scheuler, Fischer and Felgenhauer. RESUMO

Hypercrinism, dyscrinism and mucostasis are the principal symptoms accompanying diseases of the respiratory system. Both the viscosity and the adhesion of the secretion are affected by secretolytics and mucolytics. Evidence on changes in the quality of the secretion was obtained following a nine-day inhalation therapy using mucosolvin, tacholiquin, bromhexin and physiological NaCl-solution, a new viscosimetric measuring device with non-Newton liquids being applied in the investigation. The device allows for a control of the secretolytic and mucolytic treatment used.

RESUMO

Under the conditions of the thoracoscopy under anaesthesia the behaviour of the haemodynamic measuring values is investigated in 10 patients of either sex. The time of intubation and the phase of the open thorax represent the most remarkable periods of examination which are parallel to the alterations of the arterial oxygen partial pressure and the pressure parameters of the circulations within the lesser an the systemic circulatory system. The investigation results obtained from the behaviour of the oxygen partial pressure will prove to be decisive, whether treatment with thorax opening under simultaneously carried out analytic blood-gas measurements should be recommendable.

RESUMO

The investigations pointed out that responses of the pulmonary hemodynamic in patients with COLD on physical exercises are highly different from those in normal subjects. The findings of pulmonary circulation on pink puffers (type A) and blue bloaters (type B) are differing in a specific manner. The lowest capacity of load is observed in patients of type A. Patients with clinical signs of type B show under exercise conditions the ability to approach their physical load capacity rather to that of normal persons. The features of prognosis, therapy and rehabilitation in the different patterns of pulmonary heart disease are discussed.

Between sternum and spinal column, pathological changes of the mediastinum and of the adjacent areas are being not to be detected in the roentgenogram. Roentgenologically they are only to be detected in the event that there have been produced either protrusions or dilatations of the mediastinal shadow. The diagnosis of diseases of vessels, lymphnodes, tumors and inflammations, respectively, is very difficult to be obtained by roentgenogram of the chest in one or several planes. Beside endoscopic methods (bronchoscopy, thoracoscopy, mediastinoscopy), the angiography deserves to be emphasised among the various intrathoracal diagnoses. The angiography has found ever widening application with the reliable representation of processes of the heart and vessels in community with the defining of protruding processes. The angiography is to be used not only with suspicion of aneurysm and anomalies of the thoracal aorta, but with aneurysms of the heart wall, tumors of the heart, central arteriovenous fistulae of pulmonary vessels and with anomalies about the pulmonary artery, the pulmonary vein and the vena cava, respectively. Dependent upon the particular pathological changes stated, as methods of examination, the cavography and pulmonary angiography, respectively, are being applied. The statement has been supplemented by illustrating pictures.

The authors start from the fact that till now was a declining attitude in face of the thoracoscopic lung biopsy. Under local anaesthesia any air embolism were happened with it. Moreover there was no optimal instrument for thoracoscopic lung biopsy. Beginning with pressure measurements of the intrathoracic cavity systems the authors research the conditions which can result in air embolism. They found that air embolism is excluded only by unilateral ventilation of the non-biopsied lung of the patient via a channel of Carlens' double-lumen catheter. The other channel of the cathetaer which is connected with the thoracoscopically biopsied lung must be aggreed with the atmosphere in the same way as the pleural space via the open thoracoscope tube. Further it was developed the suction punch, a special instrument for thoracoscopic lung biopsy. In this instrument the proceedings of catch and detache the tissue from the parenchyma are divided contrarily to the forcepslike devices. With this new instrument 63 patients were examined. The thoracoscopic lung biopsy was performed under general anaesthesia with side-separated ventilation through Carlens' catheter. The histologically verified diagnostic yielded was 89 per cent. There aren't serious complications.

RESUMO

It is reported on first experiences (18 patients) with investigation of the breathing mechanism during ergometry for the quantification of the respiratory dyspnoea on exertion. The oesophagus-pressure-method is recommended as suitable method. The stopping of the load on account of dyspnoea in the number of patients which above all consisted of obstructive bronchitics was carried out at values for the total breathing capacity (Wtot) of 5 kpm/min and more.
